We report five cases of intertrochanteric fractures that needed open reduction after failed closed reduction techniques. In all cases the shaft fragment included the lesser trochanter, and there was a long spike on the head-neck fragment. This was evident clinically as the proximal shaft, pulled by the iliopsoas tendon, produced a swelling in front of the hip joint. Radiographically, the fracture was minimally comminuted. Hemiarthroplasty of the hip is a commonly performed procedure for subcapital neck of femur fractures. Dislocation of the hemiarthroplasty is a recognised but uncommon complication. Dislocations can be complicated by the uncoupling or dissociation of the femoral head component from the femoral stem. Dissociations are rare and require open reduction. We present a case of a patient with a hip hemiarthroplasty dislocation and dissociation subsequent to a fall.

This paper presents the short term results of an ongoing prospective randomized trial comparing a cemented unipolar with a cemented bipolar hemiarthroplasty for the treatment of displaced femoral neck fractures in the elderly. Forty-seven patients with an average age of 77 years completed 6-month followup. Outcomes at 6 weeks, 3 months and 6 months were assessed by completion of a patient oriented hip outcome instrument and by functional tests of walking speed and endurance.
