Transitional cell carcinoma of the renal pelvis or ureter: patterns of failure.

OBJECTIVES: To identify recurrence patterns and possible indications for adjuvant treatment. METHODS: Ninety-four patients with transitional cell carcinoma of the renal pelvis or ureter were reviewed to determine their pattern of failure. Factors including gender and age, tumor stage and grade, and extent of surgical procedure and adjuvant radiation therapy (RT) were analyzed with respect to local and distant recurrence and survival. RESULTS: Seventy-seven patients had resections without residual. On multivariate analysis, grade (P = 0.01) and adjuvant RT (P = 0.02) had significant effects o

[Gemcitabine and carboplatin chemotherapy in advanced transitional cell carcinoma in regard to patients with impaired renal function].

PURPOSE: The aim of this analysis is the evaluation of the activity and toxicity of gemcitabine and carboplatin in patients with advanced urothelial transitional carcinoma (TCC) with special regard to patients with impaired renal function. PATIENTS AND METHODS: 30 consecutive patients with metastatic TCC [mean age: 68 (range: 47 - 82) years, median ECOG-PS:1] were treated with gemcitabine (1000 mg/m (2) on days 1 and 8 of a 21-day schedule) and carboplatin (AUC 4.5 day 1). In 15 patients (considered as renal unfit) a creatinine clearance of less than 60 mL/min (range: 31 - 59 mL/min) was seen

Involvement of multiple epigenetic mechanisms by altered DNA methylation from the early stage of renal carcinogenesis before proliferative lesion formation upon repeated administration of ochratoxin A.

Ochratoxin A (OTA) is a rat renal carcinogen that induces karyomegaly and micronuclei in proximal tubular epithelial cells (PTECs). We previously performed comprehensive gene profiling of alterations in promoter-region methylation and gene expression in PTECs of rats treated with OTA for 13 weeks. The OTA-specific gene profile was obtained by excluding genes showing expression changes similar to those upon treatment with 3-chloro-1,2-propanediol, a renal carcinogen not inducing karyomegaly. In this study, we validated the candidate genes using methylated DNA enrichment PCR and real-time RT-PCR

Malignancy Prevalence in the Dialyzed Population and in Waitlisted Potential Kidney Transplant Recipients.

Malignancy is the second cause of death in the dialyzed population. However, data on the prevalence of cancer are very scarce. Kidney transplantation improves quality of life, prolongs survival, and is cost-effective but bears some serious complications including malignancy. Therefore, active screening for cancer is of utmost importance. The aim of this study was to assess the prevalence of malignancy in dialyzed patients in relation to status on the on the waiting list and type of dialysis. This cross-sectional study was conducted in 108 hemodialyzed patients (mean age 65 years, 47 women) and

[Functional state of a solitary kidney after nephrectomy for renal cancer].

INTRODUCTION: The outcome of surgical treatment of renal cancer depends not only on cancer-specific survival, but also on the degree of loss of renal function, which often develops after surgery, especially radical nephrectomy. AIM: To study the features of functional changes in a solitary kidney as a compensation mechanism after radical nephrectomy for renal cancer. MATERIALS AND METHODS: The functional state of a solitary kidney in 36 patients with renal cancer who undergone to radical nephrectomy was evaluated. There were 20 and 16 women. The mean age was 59.0+/-10.8 years (from 39 to 76

Can a Modified Bosniak Classification System Risk Stratify Pediatric Cystic Renal Masses?

PURPOSE: We characterize and apply the modified Bosniak classification system to a cohort of children with cystic renal lesions and known surgical pathology. MATERIALS AND METHODS: We identified all patients at our institution with cystic renal masses who also underwent surgery for these lesions. Patients without available preoperative imaging or pathology were excluded. All radiological imaging was independently reviewed by a pediatric radiologist blinded to pathological findings. Imaging characteristics (size, border, septations, calcifications, solid components, vascularity) were recorded

Native nephrectomy in transplant patients with autosomal dominant polycystic kidney disease.

INTRODUCTION: This study examined the clinical indications and timing for native nephrectomy (NN), together with the associated pathological findings in transplant patients with autosomal dominant polycystic kidney disease (ADPKD) at our institute over a period of 20 years. METHODS: A retrospective review was performed of ADPKD patients who had undergone both kidney transplantation and NN. Patients were identified from the kidney transplant database between 1988 and 2008 at Guy's and St Thomas' Hospital and the notes reviewed. All NN specimens were re-reviewed and reported according to curren

Sarcomatoid acquired cystic disease-associated renal cell carcinoma.

In this article, we report a rare case of hitherto undescribed acquired cystic disease (ACD)-associated renal cell carcinoma (RCC) with sarcomatoid change. A 78-year-old woman had been receiving hemodialysis for fourteen years at the time when a renal tumor was encountered on the follow-up examination of the kidney. Microscopically, oncocytic cuboidal cells proliferated with tubular, cribriform or papillary growth patterns, and atypical columnar cells with abundant cytoplasm proliferated with papillary configuration. Oxalate crystal deposition was observed in the stroma and the tumor focally r

Randomized trial of Alemtuzumab for prevention of graft rejection and preservation of renal function after kidney transplantation.

BACKGROUND: A randomized, multicenter, controlled trial was undertaken to evaluate the safety and efficacy of Alemtuzumab, a powerful lytic agent for both T and B lymphocytes, in the prophylaxis of rejection in renal transplantation (RTx). METHODS: Thirty patients were randomized to receive Alemtuzumab together with low-dose cyclosporine (CsA) monotherapy (CAMPATH, n = 20) or to full doses of CsA with azathioprine and corticosteroids (Standard, n = 10). CsA was administered at doses to achieve whole-blood trough CsA levels of 90 to 110 ng/mL and 180 to 225 ng/mL in CAMPATH and Standard groups

Association of renal adenocarcinoma and BK virus nephropathy post transplantation.

While most BK virus infections are asymptomatic, immunosuppression has been associated with BK virus reactivation and impaired graft function or ureteric ulceration in renal transplant patients and hemorrhagic cystitis in bone marrow transplant patients. Oncogenicity is also postulated and this is the first report of a child with a carcinoma of the donor renal pelvis following BK virus allograft nephropathy. Removal of the primary tumor and cessation of immunosuppression led to regression of secondary tumors and a return to health.

A histomorphologic and ultrastructural study of the malignant tumours of the renal pelvis.

The study was carried at two different centres. Only 9 cases of primary malignant tumours of the renal pelvis could be collected during the period of 7 years (1984-1990). Renal pelvis malignancies constituted 0.21% of all the malignancies and 12.16% of all the malignant growths of the kidney (9 out of 74 cases). The age of these patients ranged from 24 to 70 years; the mean being 41.7 years. Male/female ratio was 8:1. Common triad of complaints (pain, haematuria and lump) was noticed in 22.2% of patients. Individually they were noticed in 77.8%, 66.7% and 44.4% of patients respectively. Transi

Beyond the stone: nephrolithiasis as a metabolic sentinel in upper tract urothelial carcinoma and papillary renal cell carcinoma: toward a risk-stratified management framework.

CONTEXT: Nephrolithiasis is increasingly recognized as a recurrent disorder that often coexists with broader metabolic vulnerability. Observational studies have also raised the possibility that stone disease may identify patients with a higher subsequent risk of upper tract urothelial carcinoma (UTUC) and, less consistently, renal cell carcinoma (RCC), particularly papillary RCC (pRCC). OBJECTIVE: To critically review the epidemiologic and mechanistic literature linking nephrolithiasis to UTUC and RCC, distinguish association from causation, and outline a clinically practical risk-stratified

Case report: Renal clear cell carcinoma presenting with metastatic nodules in the male breast: a case and literature review.

Metastasis of renal cell carcinoma (RCC) most frequently occurs in the lungs, bones, liver, and brain. Breast metastasis from RCC is exceedingly rare, particularly in male patients presenting with a breast mass as the initial manifestation. We report a 54-year-old man who presented with a breast nodule that, upon excision, was pathologically confirmed as metastatic Clear cell RCC. Abdominal computed tomography (CT) revealed a primary lesion in the left kidney. One month and nine months after laparoscopic partial nephrectomy, recurrent metastatic lesions were detected in the breast. After two s

Predictive model for clear cell renal cell carcinoma: a novel model integrating sunitinib resistance and prognosis related genes and clinical factors.

PURPOSE: Sunitinib serves as the primary treatment for clear cell renal cell carcinoma (ccRCC). However, patients frequently develop resistance to it over time. Early prediction of sunitinib resistance and ccRCC prognosis is beneficial for optimizing clinical treatment. Therefore, we aimed to identify key genes associated with ccRCC sunitinib resistance and prognosis and develop a Clinical Gene Prognostic Nomogram Model (CGPNM) to assess drug sensitivity, treatment response, and survival outcomes in patient groups stratified by CGPNM nomogram scores. METHODS: Key genes associated with sunitin
