Storage of Corneas With Predissected Descemet Membrane in an Active Storage Machine.

PURPOSE: We previously demonstrated the superiority of an active storage machine (ASM; which restores a physiologic intraocular pressure [IOP]) over conventional passive storage for whole corneas. To reduce surgical time, corneas are often predissected in eye banks, 2 to 4 days before surgery for endothelial keratoplasty. The aim is here to demonstrate the possibility of also storing predissected Descemet membrane endothelial keratoplasty (DMEK) in ASM so that their use is not limited to penetrating keratoplasty and to extend the storage life of these endothelial grafts for much more flexibili

Bruch's Membrane Contributes to the Structural Integrity of the Normal Human Eye.

PURPOSE: To investigate the contribution of the Bruch's membrane and sclera tissues to the overall structural integrity of the ocular wall. METHODS: Twenty-three human globes were subjected to biomechanical testing. A piece of sclera measuring 5 × 5 mm was carefully removed at the nasal region, 2 mm away from the optic nerve head. The IOP was increased at approximately 1 mm Hg/s until Bruch's membrane-uvea-retina tissue layer (BMUR) ruptured. Next, strips of sclera and Bruch's membrane-choriocapillaris (BMC) complex were isolated from the superior fundus region. Uniaxial tension tests were pe

Navigating Misfolded Terrain: ER-Associated Degradation of Membrane Proteins.

Integral membrane proteins face unique folding challenges in the endoplasmic reticulum (ER) due to their hydrophobic transmembrane domains and complex topologies. Membrane protein misfolding disrupts ER homeostasis, contributing to diseases such as neurodegeneration, metabolic disorders, cystic fibrosis, and cancers. ER-associated degradation (ERAD) is a protein quality control pathway that detects, ubiquitinates, and retrotranslocates terminally misfolded proteins for proteasomal degradation in the cytosol. Recognition of membrane substrates involves multiple determinants, including topologic

Defective membrane dynamics in Huntington's disease: A role of Rab11 dysfunction.

Huntington's disease (HD) is a life-threatening neurodegenerative disease caused by a mutation in Huntingtin (HTT). Neuropathology in HD is marked by a progressive loss of neurons in the caudate putamen and the deposition of mutant HTT aggregates as cytoplasmic and nuclear inclusions. At present, no disease-modifying treatment for HD exists. Vesicular trafficking is a fundamental way to target proteins to their functional sites and thus essential for the maintenance of cellular homeostasis. HTT associates with vesicles and interacts with molecular motor proteins driving vesicle movement along

Extracorporeal membrane oxygenation in trauma patients with major pulmonary contusion.

BACKGROUND: Patients with pulmonary contusion (PC) may require extracorporeal membrane oxygenation (ECMO) due to respiratory failure refractory to conventional management. We aimed to compare outcomes in patients with major PC who received ECMO with those who received conventional management. METHODS: We performed a retrospective cohort study using the Trauma Quality Improvement Program Database (2017-2020). We identified adult patients with major PC who underwent veno-arterial or veno-venous ECMO or had respiratory failure and received mechanical ventilation. The primary outcome was survival

Time-varying voriconazole clearance during extracorporeal membrane oxygenation.

Invasive aspergillosis causes high mortality in critically ill patients, particularly those with viral pneumonitis receiving extracorporeal membrane oxygenation. Achieving effective voriconazole exposure in this setting is difficult, and existing data on drug concentrations during extracorporeal support are inconsistent. To characterize voriconazole pharmacokinetics in adults receiving extracorporeal membrane oxygenation and evaluate the influence of CYP2C19 genotype on drug exposure. This single-center prospective observational study included adults treated with intravenous voriconazole for s

The lipid-podocyte axis: emerging clues in membranous nephropathy pathogenesis.

Membranous nephropathy (MN) is an immune-mediated glomerular disease and the most common cause of nephrotic syndrome in adults. Classical paradigms concentrate on the binding of circulating autoantibodies (e.g., anti-PLA2R, anti-THSD7A) to podocytes, resulting in subepithelial immune deposits, complement activation, and podocyte damage. Nonetheless, mounting evidence suggests that lipid metabolism in podocytes is a crucial regulator of MN pathophysiology. Podocyte slit diaphragms are situated within specialized cholesterol-enriched lipid rafts that orchestrate essential structural and signalin

The Molecular Interplay Between Lipids, Cholesterol and Membrane Hydration Layer.

Understanding membrane structure, including its hydration, is vital for unraveling the chemistry of life. The nanoscopic layer of water directly hydrating biological membranes plays a critical role in maintaining cell structure, regulating biochemical processes, and mediating intermolecular interactions at the membrane interface. Cholesterol-rich lipid rafts, on the other hand, facilitate membrane fusion-a process essential for neurotransmitter release, fertilization, and viral entry-with localized, transient membrane dehydration serving as a key step in this process. This chapter summarizes t…

Solute carrier membrane transporters: emerging targets in CNS disorders.

Prominent members of the solute carrier (SLC) superfamily of membrane transporters are established targets of central nervous system (CNS) drugs, including the blockbuster neurotransmitter reuptake inhibitors. Numerous other SLC transporters exhibit dysregulated expression in the CNS relative to other tissues, many of which have been genetically or functionally linked to neurological disorders and may therefore represent tractable therapeutic targets. As key regulators of metabolite and ion fluxes, SLCs shape developmental programmes and cellular states associated with epilepsy, neurodegenerat

Extracapillary Proliferation Superimposed on Primary Membranous Glomerulonephritis: A Case Report.

We report the case of a 34-year-old patient with no significant medical history who was followed for primary membranous glomerulonephritis confirmed by renal biopsy, initially revealed by nephrotic syndrome and preserved renal function. After nine months of conservative treatment, the patient presented with progressive deterioration of renal function and increased proteinuria, leading to the initiation of immunosuppressive therapy. Persistent renal deterioration prompted a second biopsy, which revealed extracapillary proliferation in addition to membranous nephropathy. The clinical course was

Thromboembolic complications unmasking membranous nephropathy: diagnostic and therapeutic challenges.

Nephrotic syndrome is associated with a hypercoagulable state and can lead to serious thromboembolic complications. In patients presenting with pulmonary embolism without an identifiable provoking factor, hypoalbuminaemia and significant proteinuria should prompt consideration of underlying nephrotic syndrome. Awareness of this link is important to avoid delay in diagnosis.A woman in her 60s presented with exertional dyspnoea and pleuritic chest pain. CT pulmonary angiography confirmed bilateral pulmonary emboli and she was treated with apixaban. After discontinuing anticoagulation prematurely

Tartrazine clearing of the tympanic membrane for improved visualization of the middle ear.

Biological tissues intrinsically scatter light, which limits the depth of penetration of optical imaging. Optical coherence tomography (OCT) is a non-invasive, label-free imaging modality that generates real-time 2- and 3- dimensional in vivo images with high spatial resolution (<10 µm). While OCT has been used in nascent otologic studies to noninvasively diagnose otitis media, distinguish cholesteatoma from normal middle ear mucosa, and identify vascular tumors of the middle ear, the intrinsic light scattering of the tympanic membrane (TM) limits OCT penetration depth. Optical clearing agents

Extracorporeal membrane oxygenation support for tropical infections: a scoping review.

BACKGROUND: Extracorporeal membrane oxygenation (ECMO) for infectious causes of refractory cardiopulmonary failure is established as appropriate therapy in high-income countries. Its use in low- and middle-income nations for tropical infections is not well-studied, however, perhaps because most of these countries have not been historically able to offer ECMO support. Tropical infections remain an important cause of global morbidity and mortality, but the role of ECMO is poorly described. METHODS: We identified a list of viral, bacterial, fungal, and parasitic infections that qualified as trop
