Impulsivity in youth predicts early age-related cognitive deficits in rats.

Impulsivity is a feature of psychiatric disorders such as mania, addictive behaviors or attention deficit-hyperactivity disorder (ADHD), which has recently been related to complaints of forgetfulness in adults. We investigated whether impulsiveness exerts a long-term influence on cognitive function in rats in a longitudinal study. Impulsivity, assessed by the ability to complete a sequence of presses to obtain food (conditioning box), spatial working memory (8-arm radial maze) assessed with varying degree of attentional load and recognition memory (Y-maze) were tested at different ages. Marked

[Cardiac arrest due to fever-induced ventricular fibrillation in a 73-year-old patient with hidden Brugada syndrome: an exceptional case of late onset].

Brugada syndrome is an inherited cardiac channelopathy associated with sudden cardiac death, typically diagnosed in middle-aged individuals. Fever is a well-known trigger for malignant ventricular arrhythmias in predisposed patients. We report the case of a 73-year-old man without cardiovascular risk factors or previous syncope who experienced ventricular fibrillation-related cardiac arrest triggered by fever as the first clinical manifestation of Brugada syndrome. This case highlights the pivotal role of fever in unmasking high-risk ECG patterns and challenges the assumption of age-related pr

Annular Elastolytic Giant Cell Granuloma in a Post-COVID-19 Patient: A Case Report.

Annular elastolytic giant cell granuloma (AEGCG) is a rare, benign granulomatous disease that most commonly affects middle-aged women with a history of extensive sun exposure. It typically presents as asymptomatic annular plaques with raised erythematous borders and hypopigmented, atrophic centers. Histologically, it is characterized by granulomatous infiltration with elastolysis and elastophagocytosis by multinucleated giant cells. Here, we present a case of a 69-year-old woman with a unique and widespread presentation of AEGCG to the trunk, legs, and arms following a recent history of COVID-

Clostridium innocuum Bacteremia: Case Report and Systematic Review.

Clostridium innocuum is an anaerobic gram-positive spore-forming bacteria, a commensal member of the human gut microbiome. We encountered a case of bacteremia with this organism as a complication of a foot ulcer in a middle-aged patient with diabetes, end-stage renal disease, and malnutrition. A systematic review of the limited number of previously reported cases of C. innocuum bacteremia is presented, along with a brief narrative review the organism's association with other diseases, ranging from diarrhea and local infections to modulating inflammatory bowel disease and kidney disease.

Ovarian fibrothecoma in a 15-year-old girl: A rare case report.

Ovarian fibrothecomas are rare benign tumors typically found in middle-aged women. This report presents a case of a 15-year-old girl with an irregular menstrual cycle and a left lateral uterine mass, ultimately diagnosed as an ovarian fibrothecoma. This case highlights the diagnostic challenges posed by fibrothecomas in younger patients, as they may mimic malignant ovarian tumors on imaging. Magnetic resonance imaging (MRI) findings, surgical intervention, and pathological examination confirmed the diagnosis. This case emphasizes the importance of accurate imaging and multidisciplinary approac

Potential Biomarkers and Treatment of Neuroinflammation in Parkinson's Disease.

Parkinson's disease (PD) is a degenerative disease of the central nervous system primarily affecting middle-aged and elderly individuals, significantly compromising their quality of life. Neuroinflammation is now recognized as a key feature in the pathogenesis of PD. This study reviews recent advances in the identification of potential biomarkers associated with neuroinflammation in PD and their significance for therapeutic strategies. These findings suggest that inflammatory factors play a pivotal role in PD treatment, and interventions involving anti-inflammatory drugs, physical exercise, an

Esophageal Impaction of a Large Chicken Bone in a Patient with Schizophrenia.

Esophageal foreign body impaction is a common clinical issue, particularly among individuals with psychiatric disorders. Although most cases are managed successfully with endoscopic intervention, sharp or large foreign bodies can lead to serious complications. We present the case of a middle-aged man with schizophrenia who accidentally ingested a large chicken bone, resulting in esophageal impaction that required extraction under general anesthesia. This case underscores the importance of heightened clinical awareness regarding altered pain perception and disordered eating behaviors in patient

Autologous Microfragmented Adipose Tissue Injection Combined With Core Decompression for Treatment of Early-Stage Osteonecrosis of the Femoral Head.

Osteonecrosis of the femoral head is a disease that occurs frequently in the young and middle-aged population. Because of its high disability rate, it affects the ability to work, so early treatment of this disease is particularly important. Microfragmented adipose tissue contains high levels of adipose-derived stem cells, which show osteoprotective and anti-inflammatory qualities. This Technical Note aims to introduce in detail the harvesting and lipogems processing of autologous microfragmented adipose tissue, combined with core decompression surgery in treating early-stage osteonecrosis of

Pulmonary mixed squamous cell and glandular papilloma in the elderly: a case report and literature review.

Mixed squamous cell and glandular papilloma (MSCGP) is a rare benign lung tumor, which mostly occurs in the central airway and primarily affects middle-aged and elderly individuals. Due to the limited understanding of the clinical characteristics of elderly patients, clinicians often lack a comprehensive grasp of MSCGP, leading to misdiagnosis. Therefore, in this article, we report a case of an elderly patient with central MSCGP and provide a comprehensive review of the relevant literature on elderly patients with MSCGP, aiming to improve the diagnostic rate of this disease.

An Elusive Diagnosis Hiding in Plain Sight.

This case report highlights the diagnostic and therapeutic challenges faced in the management of a middle-aged female patient presenting with persistent left gluteal pain and a history of remote pulmonary tuberculosis. Despite multiple hospital admissions and diagnostic procedures, the cause of her debilitating symptoms remained elusive. The diagnosis was clinched when a repeat investigation, including bone biopsy and bone marrow aspirate, revealed the growth of Salmonella Typhi (S. Typhi). The patient's symptoms markedly improved following treatment with appropriate antibiotics, underscoring

Chorea-acanthocytosis.

A middle-aged Asian man had gait difficulty progressing over several years. His speech had gradually become slurred with involuntary tongue biting. He was the product of a consanguineous marriage with no other relevant family history. MR scan of brain showed bilateral caudate atrophy. Nerve conduction studies showed a predominantly sensory peripheral neuropathy. Serum creatine kinase was slightly elevated but electromyography showed no evidence of myopathy. Three consecutive peripheral blood films were negative for acanthocytes. Whole-genome sequencing confirmed a mutation in VPS13A gene, cons

Unilateral Papilledema with Bilateral Optic Nerve Sheath Distension: A Case Report.

Bilateral edematous optic disc swelling from papilledema is caused by elevated intracranial pressure (ICP). Idiopathic intracranial hypertension (IIH), a clinical syndrome with elevated ICP of unclear etiology, is a frequent cause of this condition. IIH typically affects obese middle-aged females. Papilledema usually has a fairly symmetrical bilateral pattern. Unilateral papilledema is a rare disorder that must be detected early to avoid optic nerve damage. However, the etiology of unilateral papilledema remains unclear. Based on bilateral optic nerve sheath diameter measurements, we aimed to

Anesthetic Management of a Neurosurgical Patient With Amyotrophic Lateral Sclerosis: A Case Report.

Amyotrophic lateral sclerosis (ALS) is a progressive form of neurological disorder that affects both the upper and lower motor neurons. Anesthesia management in these patients is always challenging as they can develop respiratory complications because of pre-existing muscle involvement. We report a middle-aged male with ALS posted for chronic subdural hematoma evacuation (CSDH) surgery. Surgery was done under scalp block with monitored anesthesia care. The choice of anesthesia in these patients should be one that interferes the least with the disease pattern while still providing optimal condi

GDF11 (Protein of Juvenility) and GDF15 (Protein of Senility) in the Plasma of Patients with Sleep Apnea Syndrome: a Pilot Study.

We performed a matched-pair analysis of the content of GDF11 and GDF15 proteins in the plasma of patients (56 middle-aged men) with obstructive sleep apnea syndrome (OSAS) and healthy volunteers (27 men with no complaints of sleep disorders). The groups were comparable in terms of age and presence of chronic diseases. No statistically significant differences in GDF11 content in the studied groups were revealed, while the content of GDF15 in the OSAS group was 1.3 times higher. These results require further research from the viewpoint of geriatric somnology and molecular biology.

An Abysmal Quadrad: A Rare Case of Ankylosing Spondylitis, Ulcerative Colitis, and Takayasu Aortoarteritis Complicated by Secondary Renal Amyloidosis.

Ankylosing Spondylitis (AS) is a chronic inflammatory arthritis that typically manifests in young males and may present with extra-articular manifestations. Takayasu aortoarteritis (TA) is a large vessel vasculitis that predominantly affects young and middle-aged females. Despite the limited number of studies examining the potential association between these two diseases, we report a unique case of an individual with ankylosing spondylitis and ulcerative colitis who subsequently developed Takayasu aortoarteritis. This progression ultimately led to the development of secondary renal amyloidosis

Giant calyceal diverticulum with an extra-renal component: A case report.

Calyceal diverticula are urine-filled non-secretory cavities that result from outpouchings of the kidney's calyx or pelvis. These cavities lie in the renal parenchyma and are linked to the kidney's collecting system through a narrow channel. They are generally small in size and present without symptoms. Here, we report the case of a middle-aged patient who was diagnosed, following imaging exams, with a giant calyceal diverticulum presenting with an extra-renal component, a remarkably rare finding. The patient's condition was successfully treated through excision by laparoscopic surgery.

A rare case of a peripheral Ewing sarcoma primitive neuroectodermal tumor of pelvic origin.

Primitive neuroectodermal tumors (PNET) represent malignant neuroectodermal tumors composed of small round cells. They can be differentiated between originating from the peripheral nervous system or the central nervous system. Peripheral PNET (pPNET) can be further subclassified as one of the Ewing family tumors (EFT). Although rare, EFT can originate in the female genital tract and pelvic region. Here, we present a case of a middle-aged female with PNET masses in her uterus, abdomen, and hepatic lobes. We discuss the diagnostic modalities, including immunohistochemistry, histopathology, and i
