Effects of gonadal steroid hormone treatments on opioid antinociception in ovariectomized rhesus monkeys.

RATIONALE: Gonadal steroid hormones altered opioid antinociception under some conditions in rodents, and we reported previously that chronic estradiol enhanced kappa but not mu opioid antinociception in ovariectomized rhesus monkeys. Sex differences have also been observed in the antinociceptive effects of opioid agonists. These findings suggest that gonadal hormones may modulate opioid antinociception. OBJECTIVES: To extend our previous studies of estradiol by examining the effects of progesterone alone, estradiol in combination with progesterone, and testosterone alone on opioid antinocicep

Prehospital use of analgesia for suspected extremity fractures.

INTRODUCTION: Pain and its control have been studied extensively in the emergency department. Numerous studies indicate that inadequate treatment of pain is common, despite the availability of myriad analgesics. It has been suggested that oligoanesthesia is also a common practice in the prehospital setting. OBJECTIVE: To assess the use of prehospital analgesia in patients with suspected extremity fracture. METHODS: Emergency medical services (EMS) call reports were reviewed for all patients with suspected extremity fractures treated from June 1997 to July 1998 in a midwestern community with

Postoperative analgesia with "3-in-1" femoral nerve block after prosthetic hip surgery.

PURPOSE: To evaluate the efficacy of a single shot "3-in-1" femoral nerve block for prosthetic hip surgery in association with general anaesthesia on post-operative analgesia. METHODS: Forty patients, ASA 1 to 3, received sham block or "3-in-1" femoral nerve block, following Winnie's landmarks with a nerve stimulator, and 40 ml bupivacaine 0.5% with epinephrine were injected after induction of anaesthesia. Vecuronium, 0.1 mg.kg-1, was added after performing the block and anaesthesia was maintained with isoflurane, oxygen 40% and nitrous oxide 60%. Fentanyl, 1.5 microgram.kg-1, was administere

Modification of some lymphocyte functions in vitro by opioid receptor agonists and antagonist in chronic uremic patients and healthy subjects.

In ten chronic uremic patients on regular hemodialysis treatment in vitro experiments revealed that stimulation of opioid receptors with morphine did not significantly change the mitogen-induced proliferative response of peripheral blood lymphocytes and interleukin-2 (IL-2) receptor expression on PHA-stimulated lymphocytes, while it appreciably decreased surface transferrin (Trf) receptor expression on PHA-stimulated lymphocytes. However, metenkephalin inhibited mitogen-induced proliferation and surface Trf receptor expression on uremic lymphocytes without affecting IL-2 receptor expression on

Responses of medullary dorsal horn neurons to corneal stimulation by CO(2) pulses in the rat.

Corneal-responsive neurons were recorded extracellularly in two regions of the spinal trigeminal nucleus, subnucleus interpolaris/caudalis (Vi/Vc) and subnucleus caudalis/upper cervical cord (Vc/C1) transition regions, from methohexital-anesthetized male rats. Thirty-nine Vi/Vc and 26 Vc/C1 neurons that responded to mechanical and electrical stimulation of the cornea were examined for convergent cutaneous receptive fields, responses to natural stimulation of the corneal surface by CO(2) pulses (0, 30, 60, 80, and 95%), effects of morphine, and projections to the contralateral thalamus. Effects of mu-opioid receptor agonists on circulatory responses to simulated haemorrhage in conscious rabbits.

1. Cardiac output, arterial pressure, heart rate, systemic vascular conductance, respiratory rate and arterial blood PO2 and PCO2 were measured in unanaesthetized rabbits. Haemorrhage was simulated by inflating a cuff placed around the inferior vena cava so that cardiac output fell at a constant rate of about 8% of its resting value per min. 2. The effects of drug treatments on resting haemodynamic and respiratory variables, and on the haemodynamic response to simulated haemorrhage, were tested. The treatments were; 4th ventricular (-)-naloxone HCl (10-100 nmol), 4th ventricular H-Tyr-D-Ala-Gl

Management of acute cancer-related pain in a patient with opioid use disorder: a case report.

BACKGROUND: Managing cancer-related pain in patients with opioid use disorder (OUD) poses significant challenges. We present this case of an OUD patient with severe intractable pain from metastatic testicular cancer to illustrate one approach to effective pain management. CASE PRESENTATION: A 31-year-old non-Hispanic White man with OUD experienced acute intractable cancer-related pain that was ultimately controlled with > 4700 daily oral morphine equivalents of patient-controlled fentanyl analgesia. He successfully weaned to oral methadone with oxycodone for breakthrough pain. CONCLUSIONS: E

Cross-titrating opioid agonists with buprenorphine for chronic pain in sickle cell disease.

Chronic opioid therapy for pain in sickle cell disease (SCD) carries risks of tolerance, hyperalgesia, and morbidity. Buprenorphine offers a safer alternative, yet optimal induction strategies for SCD patients dependent on full agonists remain unclear and traditionally require mild withdrawal prior to initiation. Three adult SCD patients transitioned from full opioid agonists (morphine equivalent daily dose 90-410 mg) to buprenorphine using a cross-titration strategy stratified by baseline opioid use to avoid withdrawal and maintain analgesia. These cases support an individualized buprenorphin

Ultrasound-Guided Biceps Femoris Short Head Block for Posterolateral Knee Analgesia After Total Knee Arthroplasty: A Case Report.

The biceps femoris short head (BiFeS) block is a novel, ultrasound-guided, motor-sparing regional anesthesia technique targeting the posterolateral sensory innervation of the knee. We present two unilateral total knee arthroplasty (TKA) cases in which BiFeS was combined with adductor canal block (ACB) under spinal anesthesia. Blocks were administered postoperatively, before the onset of pain. Both patients had low pain scores, preserved quadriceps and ankle motor function, minimal morphine consumption (6 mg and 5 mg), and high Quality of Recovery-15 (QoR-15) scores (140 and 137). BiFeS may be

Pain Management in Brugada Syndrome: A Case Report and Review.

Brugada syndrome is a rare condition that increases the risk of life-threatening arrhythmias. Although there are existing anesthesia recommendations for patients with Brugada syndrome, guidance on pain management is limited. We present a novel approach to pain management in these patients, illustrated by the case of a young woman with Brugada syndrome who underwent ropivacaine peripheral nerve infusion and intravenous ketamine infusion for acute-on-chronic left upper limb pain. She received perioperative multimodal analgesia, which included neuropathic agents (pregabalin, duloxetine), and opio

[Abdominal pain out of proportion by a young, healthy patient].

We report on a 29-year-old patient with highly intense, acute abdominal pain of idiopathic origin (out of proportion), admitted to the emergency department with suspected appendicitis. An ultrasound revealed signs of bowel obstruction, and due to persistent pain despite intravenous morphine analgesia, a computed tomography was performed, revealing a closed-loop obstruction. The patient underwent laparoscopy, where an omental adhesion was identified and surgically resolved. The postoperative course was uneventful. This potentially dangerous diagnosis should be considered even in healthy patient

Peripheral Nerve Stimulation for Treating Acute Pain Following Traumatic Fracture: A Case Report of Rapid-Onset Analgesia Without Motor Blockade.

Analgesia following acute traumatic fracture remains a clinical challenge. Pain relief via peripheral nerve stimulation (PNS) is a promising treatment modality due to its opioid-sparing effects and rapid, reversible sensory blockade without motor blockade. We present the case of a patient who suffered a traumatic tibial plateau fracture. A popliteal sciatic PNS device was placed on postoperative day 1 following inadequate pain control. The patient reported marked pain relief, a significant reduction in morphine milligram equivalent (MME) utilization, and improved early functional recovery. The

[Tapering of opioid-based pain treatment for chronic pain patients in an outpatient setting].

Caring for chronic pain patients under opioid therapy is challenging. Opioid treatments above 50 milligrams morphine equivalents (MME) per day are associated with an increased risk of morbidity and mortality. A tapering or a discontinuation should be discussed. Shared decision-making with individualized goals and motivational interviewing principles should be used. Tapering should be slow, with initial rate based on the duration of opioid use and with regular monitoring of patients. Inability to taper may require further reassessment of opioid dependence. Temporary increases in pain may occur

Celiac plexus block for chronic flank pain: a case series.

Non-obstructive, chronic flank pain in urologic patients can be a challenging problem to manage. In this series, we examined the efficacy of celiac plexus blockade in providing pain relief and reducing opiate use in 14 adult urology patients with non-obstructive flank pain for > 1 year. Demographic, clinical, and procedural variables were collected from the medical record for retrospective analysis. Subjective improvement in pain occurred in 11 individuals (79%), and 5 (50%) were able to reduce their daily morphine equivalent dose (MED). Celiac plexus blockade is a viable option for symptomati

Dyspnea.

Dyspnea is a subjective experience of breathing discomfort that can only be known through a patient's report. Numeric rating or visual analog scales allow assessment of intensity when the patient can self-report. The Respiratory Distress Observation Scale is a valid, reliable tool for estimating distress when self-report cannot be elicited. Treating dyspnea begins with managing the underlying condition. Other dyspnea-specific evidence-based interventions include morphine and fentanyl, upright positioning, oxygen, invasive and noninvasive ventilation, and balancing rest with activity. Effective

Methadone-induced bilateral severe sensorineural hearing loss.

Methadone, a long-acting opiate agonist, and naltrexone, an opiate receptor antagonist, are both commonly used to treat patients with morphine and heroin addiction. We present a rare case of methadone-induced persistent bilateral sensorineural hearing loss (SNHL) after chronic naltrexone use and review opioid-induced hearing loss in the literature. Methadone-induced hearing loss has been described previously described in the literature with all reported cases recovering functional hearing. This is the first description of persistent bilateral severe SNHL following methadone ingestion. We propo

Potential uses of topical opioids in palliative care--report of 6 cases.

Opioids used topically may exercise several useful clinical effects. Opioids may cause immediate local analgesia and also may work indirectly through decreasing the inflammation process. In this article we describe six patients treated with topical opioids because of cutaneous pain due to tumor infiltration. skin ulcers of malignant and non-malignant origin, severe oral mucositis, pain due to knee arthrosis and severe tenesmoid pain. In all but one case, topical morphine provided rapid relief which lasted usually for 7-8 h. The side effects of topical opioids were none or minimal. Possible mec

Mechanisms in IgE-dependent anaphylaxis to anesthetic drugs.

The discovery of IgE antibodies to neuromuscular blocking drugs, to thiopentone and narcotics, particularly morphine, reinforced earlier beliefs formed from skin test and other clinical findings that many "anaphylactoid" reactions to drugs were true type 1 immediate hypersensitivity reactions. Immunochemical studies established the fine structural specificities of the drug-reactive IgE antibodies and provided an explanation in molecular terms for a number of observed clinical cross-reactions. Subtleties in interpreting relationships between skin tests and IgE radioimmunoassays are pointed out

[The psychopharmacology of narcotics: a preclinical study].

Simple and economic tests for estimating addictive potential of drugs using conditioned place preference and intravenous self-administration paradigms have been developed. Various groups of substances were revealed among 12 opioids which differed by their potencies in analgesics and reinforcing tests. Index of "narcological safety" and "addictive index" were proposed for comparative evaluation of therapeutic and adverse (addictive) effects. Using the self-administration procedure a group of addictive psychostimulants were investigated and quantitative criteria for the reinforcing action of the
