Slow pre-movement cortical potentials do not reflect individual response to therapy in writer's cramp.

OBJECTIVE: To investigate whether movement-related cortical potentials (MRCP) provide a physiological correlate that indicates the response to treatment in patients with writer's cramp. METHODS: In 21 patients with writer's cramp, who underwent 4 weeks of limb immobilization followed by re-training for 8 weeks, we recorded MRCPs preceding a self-initiated brisk finger abduction movement. MRCP measurements of pre-movement activity were performed at baseline, after the end of immobilization and four and 8 weeks of re-training. We examined 12 controls, who received no intervention, twice 4 weeks

Short-term continuous infusion of apomorphine hydrochloride for treatment of Huntington's chorea: A double blind, randomized cross-over trial.

We evaluated tolerability and the efficacy of continuous infusion of apomorphine hydrochloride on involuntary movements and mood disorder in Huntington's disease (HD) patients in a pilot, single center, double-blind, randomized, crossover, and controlled versus placebo study. Nine patients with a molecular diagnosis of HD were screened for response to acute apomorphine injection. Four of them, not ameliorating at the acute test, were discontinued. Five patients, responding to acute apomorphine, received continuous infusion of either apomorphine or placebo for 5 days. After 2 days of washout, t

Blepharoclonus, pseudoasterixis, and restless feet.

OBJECTIVE: To report a syndrome in 30 patients characterized by headache, myoclonic tremors of the eyelids (blepharoclonus), action tremors … port supports the notion that in certain cases, an association exists between headache, involuntary movement disorders, and peripheral neuropathy. Additional studies are needed to elucidate the pathogenesis of this syndrome.

Sleep features in Tourette's syndrome, neuroacanthocytosis and Huntington's chorea.

Twenty-one patients affected by extrapyramidal disorders were polygraphically recorded during spontaneous nocturnal sleep for two consecutive nights to assess their sleep and movement patterns. The patients (pts) sample included: Gilles de La Tourette syndrome (TS, nine pts), neuroacanthocytosis (NA, six pts) and Hungtington's chorea (HC, six pts). Sleep recording included C3/A2, 01/A2, ROC/LOC, submental EMG, EKG, nasal airflow thoracoabdominal respirogram, bilateral anterior tibialis and other EMGs, in relation to the individual distribution of the abnormal movements. According to our observ

Behavior modification of abnormal gait and chronic pain secondary to somatization disorder.

An interdisciplinary team, which included a physiatrist, psychologist, physical therapist, occupational therapist, social worker, and nursing staff, undertook the treatment of a 33-year-old woman with a 16-year history of gait problems and multiple somatic complaints. Previously, she had been followed by a number of physicians and had undergone both invasive and noninvasive diagnostic procedures as well as several surgical procedures. After limited response to such treatment, she was referred to the outpatient PM&R clinic for evaluation. Physical and psychologic study led to a primary diagnosi

The clinical picture in occult hydrocephalus.

From a study of 16 successfully shunted cases of symptomatic normal pressure hydrocephalus, the typical symptomatology has been characterized. It was found that gait disturbance preceded impaired mentation in 12 of the 16 cases, it began at the same time in three cases and in only one case did impairment of mentation precede it. Of 11 cases of shunt failure, dementia came first in nine and the disturbance of locomotion was relatively less severe or was absent. The various ways in which hydrocephalus may present have been outlined.

[Hiccups, not always unremarkable].

Hiccups are most often benign and of short duration. However, they may also be persistent (superior to 48h) or even refractory (superior to 1 month). In such cases, they markedly alter the quality of life and can lead to severe impairment of health. We here review hiccups pathophysiology, etiologies, work up and treatment. We suggest that hiccups should be considered as a non-epileptic myoclonic movement disorder.

Paroxysmal non-kinesigenic dyskinesia due to a PNKD recurrent mutation: report of two Southern European families.

Paroxysmal non-kinesigenic dyskinesia (PNKD) is an autosomal dominant disorder characterized by attacks of dystonic or choreathetotic movements precipitated by stress, fatigue, coffee, alcohol or menstruation. In this report we present two families with PNKD of Southern European origin carrying a PNKD recurrent mutation. Incomplete penetrance and intrafamilial variability was detected in both families. Treatment with valproic acid and levetiracetam provided favorable response.

The RFID technology for neurosciences: feasibility of limbs' monitoring in sleep diseases.

This contribution investigates the feasibility of the passive UHF RF identification technology for the wireless monitoring of human body movements in some common sleep disorders by means of passive tags equipped with inertial switches. Electromagnetic and mechanical models as well as preliminary experimentations are introduced to analyze all the significant issues concerning the required power, the tag antenna design, the read distance, and the expected biosignals collected by the interrogation device.

Extraocular muscle dystonia due to acquired (non-Wilsonian) hepatocerebral degeneration.

We present a video report of a patient with advanced non-Wilsonian cirrhotic liver disease who developed extraocular muscle dystonia (oculogyric crisis) and severe orofaciolingual dyskinesias. Acquired hepatocerebral degeneration causes choreic movements, especially of cranial muscles, but dystonic ocular spasm is an infrequent manifestation of this disorder. This case illustrates that AHD should be considered in the differential diagnosis of extraocular muscle dystonia.

Hypocretin-1 and secondary signs in Huntington's disease.

Huntington's disease is characterised by unwanted movements, psychiatric disturbances and cognitive decline. Less well recognised symptoms and signs are weight loss, autonomic dysfunction and sleep disorders. In this study we focus on hypocretin-1 and hypothalamus functions. We found a reduction by about 30% in hypocretin signalling in patients with HD. However, it remains unclear whether this moderate decrease in hypocretin signalling contributes to clinical symptoms.

REM behavior disorder and excessive daytime somnolence in Machado-Joseph disease (SCA-3).

We reported previously that behavior suggestive of rapid eye movement sleep behavior disorder (RBD) was markedly increased in a small population of SCA-3 patients. We, therefore, asked patients and nonpatient attendees at an SCA-3 annual clinic to complete a questionnaire soliciting RBD-like behavior. Our results support the previous observation that RBD-like behaviors are significantly increased in SCA-3.

[Periodic laryngeal dyspnea in bi-opercular syndrome].

Periodic bouts of laryngeal dyspnea in a 77-year-old man were observed early in the course of a syndrome due to bilateral opercular infarcts. Laryngoscopy showed alternate slow opening and closure of the glottis superimposed on respiratory vocal cord movements. The disorder could result from bilateral removal of cortical control of bulbar centers, its periodicity suggesting analogies with Cheyne-Stokes' dyspnea.

The clinical symptoms of Parkinson's disease.

In this review, the clinical features of Parkinson's disease, both motor and non-motor, are described in the context of the progression of the disease. Also briefly discussed are the major treatment strategies and their complications. Parkinson's disease is a slowly progressing neurodegenerative disorder, causing impaired motor function with slow movements, tremor and gait and balance disturbances. A variety of non-motor symptoms are common in Parkinson's disease. They include disturbed autonomic function with orthostatic hypotension, constipation and urinary disturbances, a variety of sleep d

Restless Legs Syndrome/Willis-Ekbom Disease and Periodic Limb Movements in Sleep in the Elderly with and without Dementia.

There is great interest in the study of sleep in healthy and cognitively impaired elderly. Sleep disorders have been related to quality of aging. Sleep-related movements are a frequent cause of disordered sleep and daytime sleepiness. Restless legs syndrome/Willis-Ekbom disease (RLS/WED) is often unrecognized in the elderly. This review explores RLS/WED in the elderly population. The elderly population may be subdivided into 3 groups: healthy, dependent, and frail. The RLS/WED could be a predictor for lower physical function; its burden on quality of life and health care-related costs, in the
