Thalidomide in Refractory Tuberculomas and Pseudoabscesses.

Paradoxical immune reconstitution inflammatory syndrome is a well-described entity even in immunocompetent children, principally in association with Mycobacterium tuberculosis infections. Central nervous system involvement is a potential life-threatening form, sometimes refractory to standard treatment. We report the case of an HIV-negative refugee teenager, who presented with brain tuberculomas and pseudoabscesses responsive only to thalidomide.

At home with hostility: How do pathogenic bacteria evade mammalian immune surveillance to establish persistent infection?

Bacterial persistence is of major concern as persistent bacterial infections involving bacteria such as Helicobacter pylori, Salmonella enterica serotype Typhi, and Mycobacterium tuberculosis pose significant public health problems worldwide. This report discusses the recent advances in understanding the strategies used by bacteria during persistent infection that allow them to colonize specific sites in the host and evade immune surveillance.

Short course of clarithromycin in an immunocompetent patient with BCG-induced regional complications.

Bacillus Calmette Guerin (BCG) has been used to vaccinate against tuberculosis since 1921. Persistant skin and lymph node lesions yielding Mycobacterium bovis are a rare complication for which there is not standardized treatment. We report an 11 month old child with a progressive cutaneous nodule and lymphadenopathy after BCG vaccination. These rapidly cleared with administration of clarithromycin.

Mixed infection of an atypical Mycobacterium and Aspergillus following a cryopreserved fat graft to a face.

We report a chronic infection of a patient who received a cryopreserved fat graft on her face. A 22-year-old female patient presented with multiple abscesses of her face. Four months previously, she received a second fat graft with the fat harvested at a previous surgery which was cryopreserved for 2 months. On examination, she had tender erythematous nodules on both cheeks. A computed tomography of her neck showed multiple peripheral enhancing nodular lesions. In an open pus fungus culture, Aspergillus fumigatus growth was observed. On the Mycobacterium Other Than Tuberculosis identification

The correlation between immune profiles and pathological changes in pulmonary tuberculosis granulomas revealed by bioinformatic analysis and experimental validation.

Most of Mycobacterium tuberculosis(Mtb) infection result in the formation of granulomas, which are often rich in immune cells, with subsequent clinical symptoms. However, the role of the immune system in the formation of tuberculosis granuloma structures has not been fully revealed. Here we first analyzed single-cell transcriptome and microenvironment spatial characteristics to reveal the contribution of immune cells to granuloma expansion with validation by immunofluorescence. We then integrated published peripheral blood transcriptome data for Mtb-infected patients and healthy controls. Immu…

Contacts of tuberculosis patients in high-incidence countries.

The risk of acquiring infection with Mycobacterium tuberculosis correlates with duration of exposure to an infectious source of tuberculosis. Contact identification is therefore a comparatively high-yield activity. However, in resource-poor settings tuberculin is rarely available, and even where it is available, non-specific cross-reactions to tuberculin resulting from BCG vaccination complicate the interpretation of tuberculin test results. The identification of a putative infection with M. tuberculosis in a contact must result in intervention. Excluding active tuberculosis is mandatory befor…

Impact of tuberculosis on HIV-1 replication, diversity, and disease progression.

HIV and Mycobacterium tuberculosis not only co-circulate throughout the developing world but each has contributed to prevalence and mortality caused by the other. Several reports have described how HIV-1 increases the incidence of new M. tuberculosis infections, exacerbates the severity of tuberculosis (TB), and re-activates latent M. tuberculosis. However, the converse relationship is more difficult to understand considering TB can emerge in asymptomatic individuals and as an opportunistic infection during AIDS. Development of TB in HIV infected individuals with higher CD4 cell counts (> 200/…

Involvement of Mycobacterium smegmatis small noncoding RNA B11 in triacylglycerol accumulation and altered cell wall permeability.

BACKGROUND: Pathways involving triacylglycerol (TAG) accumulation are thought to play a crucial regulatory role in bacterial growth and metabolism. Despite this understanding, little is known about the biological functions and regulatory mechanisms of small RNAs in Mycobacterium. Mycobacterium smegmatis (M. smegmatis), a type of Mycobacterium, serves as a model organism to investigate the molecular, physiological, and drug resistance features of M. tuberculosis. RESULTS: In this study, we demonstrated that overexpression of B11 significantly affects bacterial growth and colony morphology, inc

Development and evaluation of a formaldehyde-stabilized tuberculin as a safe and potent alternative to phenol-based purified protein derivative for the diagnosis of animal tuberculosis.

BACKGROUND AND AIM: Bovine tuberculosis (bTB), caused by Mycobacterium bovis, remains a significant zoonotic threat to livestock and public health, resulting in major economic losses. The diagnostic accuracy of purified protein derivative (PPD) tuberculin, the cornerstone of in vivo screening, is influenced by the allergen's composition and stabilizer. Conventional phenol-stabilized tuberculin raises toxicity and safety concerns, prompting calls for safer alternatives. This study aimed to enhance the diagnostic value of tuberculin by replacing phenol with formaldehyde as a preservative and sta

Possible correlation between Mycobacterium abscessus subsp. massiliense spondylodiscitis and spinal anesthesia: Diagnostic and therapeutic challenges.

Spinal infections caused by Mycobacterium abscessus are rare and difficult to treat. We report the first case of M. abscessus subsp. massiliense spondylodiscitis temporally associated with spinal anesthesia. A 57-year-old man developed back pain after undergoing spinal anesthesia for prostate surgery. Six months later, decompressive laminectomy was performed; results for bacterial and fungal culture and molecular testing for Mycobacterium tuberculosis were negative. Two years later, a mycobacterial culture of material from a paravertebral abscess revealed the presence of M. abscessus, later id…

Approaches to detecting tuberculosis in children and youth.

This practice point provides a framework for initiating investigation in children suspected of having infection with Mycobacterium tuberculosis (Mtb). Some areas in Canada have a high burden of tubercular disease, with Indigenous populations being most at risk. Tuberculosis (TB) can present either as an acute or subacute illness and both primary or reactivation infection can cause pulmonary or multisystem disease. Tuberculin skin tests and interferon-γ release assays can be used to support a suspected diagnosis. TB elimination in Canada is possible but requires improving social determinants of…
