Management of iatrogenic injury to the spinal accessory nerve.

Spinal accessory nerve injury results in a debilitating shoulder dysfunction. The trapezius is a major suspensory muscle of the shoulder girdle, and paralysis results in chronic pain and debility from disruption of synchronous scapulohumeral rhythm. This injury usually follows a simple posterior triangle lymph node biopsy and is treatable if appropriate measures are taken in a timely fashion. A major pitfall in early management is either failure to recognize or acknowledge the injury or hoping that it will resolve with conservative treatment. Variation of innervation of the trapezius alters cl

Occipital condyle fractures.

OBJECTIVE: Occipital condyle fractures (OCFs) are infrequently recognized. Three recent cases of OCF in our center prompted a review of the incidence, clinical presentation, diagnosis, and treatment of this entity. METHODS: A retrospective review of medical records and radiographic results was performed for 93 of 316 consecutive patients who were victims of trauma, who presented at the Toronto Hospital during a 13-month period, and who had undergone computed tomography of the occiput. RESULTS: A review of the literature regarding OCF revealed that cranial nerve deficits occurred in 31% of th

Systemic C-reactive protein and vitamin D are not associated with development of chronic musculoskeletal pain over a 2-7 year follow-up period.

Prior studies report association between elevated C-reactive protein (CRP), low vitamin D and development of chronic musculoskeletal pain. However, these small studies did not consider relevant and potentially influential biopsychosocial factors. This longitudinal study using UK Biobank data explored associations between baseline (2006-2010; T1) systemic (blood) levels of CRP and vitamin D, biopsychosocial and lifestyle factors, and later presence of chronic musculoskeletal pain (2012-2013; T2; neck/shoulder, back, hip, knee, and 'any pain' aggregation). All participants were free of pain at T

Addressing Pain in Oral Mucositis: Narrative Review of Current Practices and Emerging Treatments.

OBJECTIVE: Oral mucositis (OM) is a debilitating complication of cancer therapies, affecting up to 85% of patients undergoing bone marrow transplantation and nearly all receiving head and neck radiotherapy. Characterized by mucosal inflammation, ulceration, and severe pain, OM significantly impairs oral intake, speech, and quality of life. These disruptions, compounded by complications such as infection, bleeding, and increased healthcare costs, often necessitate treatment delays or modifications, negatively impacting cancer prognosis. Recent insights into nociceptive and neuropathic mechanism

Restless leg syndrome presenting as chronic severe limb pain in a dialysis patient.

"Chronic pain" is a commonly reported symptom among hemodialysis patients. Despite its high prevalence and the poor health-related quality of life associated with it, chronic pain remains an ineffectively assessed and managed entity in dialysis patients. We report a case of a 55-year-old gentleman on maintenance hemodialysis who presented with 3 months history of "excruciating flitting and fleeting type" of pain largely involving both lower limbs and occasionally neck, shoulder, chest, and upper limbs. The pain was so intolerable that it even triggered suicidal intentions in the patient. Commo

Osteopathic Manipulation in the Management of Chronic Pain: Current Perspectives.

Chronic pain is a common condition that often interferes with work or other activities. Guidelines support the use of non-pharmacological treatments, such as spinal manipulation, in patients with chronic pain. Osteopathic physicians in the United States are uniquely positioned to manage chronic pain because their professional philosophy embraces the biopsychosocial model and they are trained in the use of osteopathic manipulative treatment (OMT) to complement conventional medical care. This narrative review provides current perspectives on the osteopathic approach to chronic pain management, i

An exploratory, large-scale study of pain and quality of life outcomes in cancer patients with moderate or severe pain, and variables predicting improvement.

BACKGROUND: There have been few large-scale, real world studies in Spain to assess change in pain and quality of life (QOL) outcomes in cancer patients with moderate to severe pain. This study aimed to assess changes on both outcomes after 3 months of usual care and to investigate factors associated with change in QoL. PATIENTS AND METHODS: Large, multi-centre, observational study in patients with lung, head and neck, colorectal or breast cancer experiencing a first episode of moderate to severe pain while attending one of the participating centres. QoL was assessed using the EuroQol-5D quest

Effectiveness of motor imagery techniques in the treatment of chronic musculoskeletal pain: A scoping review.

Recent neurophysiological studies provide evidence that motor imagery techniques, as a non-pharmacological intervention, reduce the perception of pain intensity at the cortical level and can therefore be used in clinical practice in patients with chronic musculoskeletal pain (CMP). The aim of this scoping review was to evaluate recent randomised controlled trials on the effect of motor imagery techniques for the relief of CMP. Four databases (PubMed, Scopus, Proquest, Cochrane Clinical Trials) were searched to identify randomised controlled trials on the effect of motor imagery techniques on t

Comparative main effects, mediators, and moderators of cognitive behavioral therapy, acceptance and commitment therapy, and emotional awareness and expression therapy for chronic spinal pain: Randomized controlled trial rationale and protocol.

BACKGROUND: Chronic spinal (back/neck) pain is common and costly. Psychosocial treatments are available but have modest effects. Knowledge of treatment mechanisms (mediators and moderators) can be used to enhance efficacy. Trials that directly compare different treatments are needed to determine which mechanisms are treatment-specific, which are shared across treatments, and which contribute the most to outcomes. METHODS: We will conduct a 4-arm randomized, controlled clinical trial to compare the main effects, mediators, and moderators of three pain therapies: Cognitive-Behavioral Therapy, A

The role of graded nerve root compression on axonal damage, neuropeptide changes, and pain-related behaviors.

Rapid neck motions can load cervical nerve roots and produce persistent pain. This study investigated the cellular basis of radicular pain and mechanical implications of tissue loading rate. A range of peak loads was applied in an in vivo rat model of dorsal root compression, and mechanical allodynia (i.e. pain) was measured. Axonal damage and nociceptive mediators were assessed in the axons and cell bodies of compressed dorsal roots in separate groups of rats at days 1 and 7 after injury. In the day 7 group, damage in the compressed axons, evaluated by decreased heavy chain neurofilament immu

Evaluation of Cost-Utility of Thoracic Interlaminar Epidural Injections.

PURPOSE OF REVIEW: Chronic thoracic pain, even though not as prevalent as low back and neck pain, appears in approximately 30% of the general population. The severity of thoracic pain and degree of disability seems to be similar to other painful conditions. Despite this severity, interventions in managing chronic thoracic pain are less frequent, and there is a paucity of literature regarding epidural injections and facet joint interventions. RECENT FINDINGS: As with lumbar and cervical spine, a multitude of interventions are offered in managing chronic thoracic pain, including interventional

Association Between Extreme Climate Exposures and Bodily Pain: Evidence from the Chinese Health and Retirement Longitudinal Study.

BACKGROUND AND AIMS: Extreme climate events may be associated with increased bodily pain, particularly among aging populations. However, individual-level evidence linking specific climate exposures to pain outcomes remains limited. This study examined the association between extreme climate exposure and pain in different body regions among middle-aged and older Chinese adults. METHODS: This study used data from the Chinese Health and Retirement Longitudinal Study (CHARLS, 2011-2020), which included 65,693 person-wave observations. Pain outcomes across the 15 body regions were self-reported. C

Prospective, randomized, and controlled trial on ketamine infusion during bilateral axillo-breast approach (BABA) robotic or endoscopic thyroidectomy: Effects on postoperative pain and recovery profiles: A consort compliant article.

BACKGROUND: Robotic or endoscopic thyroidectomy using bilateral axillo-breast approach (BABA) is frequently performed for excellent cosmesis. However, postoperative pain is remained as concerns due to the extent tissue dissection and tension during the operation. Ketamine is a noncompetitive N-methyl-D-aspartate (NMDA) receptor antagonist that reduces acute postoperative pain. We evaluated the effects of intraoperative ketamine infusion on postoperative pain control and recovery profiles following BABA robotic or endoscopic thyroidectomy. METHODS: Fifty-eight adult patients scheduled for BABA

Contrast dispersion pattern and efficacy of computed tomography-guided cervical transforaminal epidural steroid injection.

BACKGROUND: The causes of upper extremity radicular pain or neck pain are varied, often involving disc herniation, spinal stenosis, or spondylosis. Cervical transformaminal epidural steroid injection (C-TFESI) is a common treatment for such pain. However, its efficacy conceivably may depend on needle-tip placement, linking the degree of pain reduction achieved to the pattern of contrast dispersion. OBJECTIVE: The current study explores this relationship, evaluating contrast dispersion patterns of C-TFESI in conjunction with short-term patient outcomes. STUDY DESIGN: This study was conducted
