[Cervical spine manipulation: risks--benefit--assessment].

Cervical manipulation is a widely used method indicated in non-specific mechanical neck pain and cervicogenic headache. Cervical manipulation can cause severe neurologic complications which are both rare and generally unpredictable, which can be compared with the accidents occurring with other treatments (nonsteroidal anti-inflammatory drugs...). To decrease this risk, we propose five recommendations developed by consensus: --unwanted effects, however minor (e.g. nausea or vertigo), of prior manipulation should be searched for routinely and taken as contraindications for future spinal manipula

[Rational diagnostic approach in hypertension].

The list of causes of hypertension is quite long. However, the diagnostic procedure for detection of such forms should take in account the fact that the cause of nearly 95% of the cases with high blood pressure is unknown, i.e. primary or essential. The most often form of secondary hypertension is seen in patients with chronic renal disease, whereas renovascular, endocrine, cardiovascular or neurologic disorders all show a prevalence of less than 1%. Careful anamnestic evaluation and clinical examination combined with few simple laboratory tests allow the detection of most cases with secondary

Clinical and neurological abnormalities in adult celiac disease.

We assessed the occurrence of neurological signs and symptoms in adult patients with celiac disease and evaluated the correlation between neurological features and diet. A total of 176 patients and 52 age-matched controls underwent a semistructural interview and a neurologic examination. The effect of gluten-free diet was evaluated by comparing the prevalence of signs and symptoms among patients adhering to a gluten-free diet and patients on an unrestricted diet. The occurrence of headache, dysthymia and signs of peripheral neuropathy was significantly higher in patients with celiac disease th

Hereditary motor and sensory neuropathy type VI with optic atrophy.

PURPOSE: To present the detailed clinical findings of a large family with hereditary motor and sensory neuropathy type VI (HMSN VI), a syndrome featuring optic atrophy. DESIGN: Observational case series. METHODS: A detailed history was obtained and physical examination was made of the extended family of the proband for evidence of neurologic dysfunction. The OPA1 gene was screened for mutations by direct DNA sequencing. RESULTS: Twelve of 97 family members examined are affected with signs of HMSN VI. Three other members have either optic atrophy or peripheral neuropathy, thus allowing an ap

Acute motor axonal neuropathy with selective involvement of the lower limbs.

We report an unusual variant of acute motor axonal neuropathy completely restricted to the lower limbs and responding rapidly to intravenous immunoglobulin (IVIG). A 31-year-old man presented with 4 weeks of progressive weakness in the lower limbs. Magnetic resonance images of the spine had been normal. On neurologic examination he had symmetric weakness in the lower limbs, but muscle strength in the arms was normal. Tendon reflexes were absent. Cranial nerves and sensation were normal. Electromyography (EMG) revealed an axonal involvement with changes restricted to the lower limbs. A lumbar p

The newborn examination: part II. Emergencies and common abnormalities involving the abdomen, pelvis, extremities, genitalia, and spine.

Careful examination of the neonate at delivery can detect anomalies, birth injuries, and disorders that may compromise successful adaptation to extrauterine life. A newborn with one anatomic malformation should be evaluated for associated anomalies. If a newborn is found to have an abdominal wall defect, management includes the application of a warm, moist, and sterile dressing over the defect, decompression of the gastrointestinal tract, aggressive fluid resuscitation, antibiotic therapy, and prompt surgical consultation. Hydroceles are managed conservatively, but inguinal hernias require sur

Neuropathology associated with stillbirth.

Neuropathologic findings in stillbirths oftentimes provide insight into the specific mechanisms leading to death. Examination of the brains of stillborn infants may also identify pathophysiologic processes that result in prenatal brain injury in liveborn as well as stillborn infants and that lead to neurologic disorders in liveborn infants, such as cerebral palsy or the sudden infant death syndrome (SIDS). A variety of abnormalities are found in the brains of stillborns, the most common including cerebral white matter necrosis (periventricular leukomalacia) or gliosis, germinal matrix or intra

[Prophylaxis of neurological manifestations of lumbar osteochondrosis: intermediate results, unsolved problems and some methodologic aspects].

Issues of prophylaxis and rehabilitation are considered as regards neurologic manifestations of spinal osteochondrosis (NMSO). NMSO primary prophylaxis requires investigation and quantitative evaluation of the disease risk factors, epidemiologic determination of both incidence and prevalence in different groups of population in various periods of time. The standard risk group of NMSO in Byelorussian consists of population of 40-49-year-olds. Some professions may be risk factors for both onset and relapse of NMSO. Secondary prophylaxis of NMSO concerns elevated relapse risk, screening for indiv

A case of human rabies in the urban area of Ribeirã Preto, SP, Brazil.

A 39-year old male patient was admitted to the University Hospital of the Faculty of Medicine of Ribeirão, Preto with signs and symptoms of sudden dyspnea, generalized myalgia and behavioral disorders. The initial suspicion was alcohol abstinence syndrome and the patient was referred for psychiatric and neurologic care. The evolution of the patient with a worsening of signs and symptoms, presence of crises of tachypnea, agitation, difficulty to swallow, irritability and hydrophobia, and his report of having been bitten by a suspected dog raised the hypothesis of rabies. The diagnosis was confi

Carbamazepine induced bradycardia--a problem in general or only in susceptible patients? A 24-h long-term electrocardiogram study.

Carbamazepine is a first line drug in the treatment of epilepsy and trigeminal neuralgia, but may exert negative chronotropic and dromotropic effects on the cardiac conduction system. Bradyarrhythmias of different types and severity have been described, especially in the elderly, but the prevalence of arrhythmias in a larger group of carbamazepine treated patients is unknown. Forty-eight patients, 40 years of older, on continuous carbamazepine treatment because of various neurologic disorders were investigated by interview, physical examination, 12-lead surface electrocardiogram, and 24-h long

Cramp-fasciculation syndrome: a treatable hyperexcitable peripheral nerve disorder.

We report nine patients with muscle aching, cramps, stiffness, exercise intolerance, and peripheral nerve hyperexcitability. Neurologic examination showed calf fasciculations in seven, quadriceps myokymia in two, and deltoid myokymia in one patient. Two patients had mild increase in serum creatine kinase. Muscle biopsy showed either no abnormality (three patients) or mild neurogenic changes (four patients). Fasciculations were the only abnormality on routine electrodiagnostic studies. Supramaximal stimulation of the median, ulnar, peroneal, and posterior tibial nerves at frequencies of 0.5, 1,

Developmental Gerstmann's syndrome.

The tetrad of finger agnosia, dysgraphia, dyscalculia, and right-left disorientation make up Gerstmann's syndrome. The tetrad has been infrequently described in children with learning disability and has been called developmental Gerstmann's syndrome (DGS). Developmental Gerstmann's syndrome may occur in brain-damaged and apparently normal children. Five children in whom DGS occurred in association with brain abnormalities underwent long-term observation, which indicated persistence of the deficits. The identification of these cases suggests that DGS may not be as rare as previously thought and

The adult diplomyelia syndrome.

Diplomyelia is an uncommon variant of diastematomyelia in which the spinal cord remains divided caudal to the diastematomyelia spur. Presented here are three adults who presented with a sign-and-symptom complex that is likely to occur only when such an anatomic aberration is found. Each patient complained of pain and paresis in one leg. Neurologic examination demonstrated dysfunction of not only both long tracts but also lower motor modalities in the symptomatic leg and no abnormalities in the opposite leg. These findings, therefore, would strongly suggest that each leg was supplied by one of

Electrocochleography recorded non-invasively from the external ear.

A method is described to record cochlear evoked potentials in humans (ECochG) with a low impedance electrode on the external ear near the opening of the external auditory canal. A piezoelectric click stimulus was delivered to the ear through a polyethylene tube creating diminution of stimulus artifact and a signal delay. Wave forms resembling the cochlear microphonic (CM), summating potential (SP) and the eighth nerve action potential (N1) were differentially accentuated by changes in stimulation and recording parameters. The addition or subtraction of condensation and rarefaction click wave f

Pneumocystis carinii and Toxoplasma gondii infections in patients with AIDS.

Pneumocystis carinii and Toxoplasma gondii are the commonest protozoans causing infections in patients with acquired immunodeficiency syndrome (AIDS). P. carinii is almost exclusively a pulmonary pathogen and caused the commonest serious infection experienced by AIDS patients. The clinical findings are those of progressive pneumonia. Diagnosis requires microscopic examination of lower respiratory secretions or lung tissue. Pentamidine or combinations of trimethoprim and sulfamethoxazole are equally effective (85% recovery), but about one-half of patients thus treated experience severe toxicity

The effects of piracetam in children with dyslexia.

Following previous research which suggests that piracetam improves performance on tasks associated with the left hemisphere, a 12-week, double-blind, placebo-controlled study of developmental dyslexics was conducted. Six study sites treated 257 dyslexic boys between the ages of 8 and 13 years who were significantly below their potential in reading performance. Children were of at least normal intelligence, had normal findings on audiologic, ophthalmologic, neurologic, and physical examination, and were neither educationally deprived nor emotionally disturbed. Piracetam was found to be well tol

The cerebrovascular effects of intraarterial CO2 in quantities required for diagnostic imaging.

The recent development of digital subtraction angiography (DSA) has renewed interest in the use of gases, especially carbon dioxide, as contrast agents. Both early physiologic studies and recent clinical reports suggest that arterial CO2-DSA is a safe and useful diagnostic tool in the examination of extremity circulation. The present study examined the neurologic sequelae, pathologic changes, and effects on the blood-brain barrier to intravenous Evans-Blue and horseradish peroxidase of intracarotid CO2, in albino rats. As with other forms of cerebral gas embolization, CO2 produced multifocal i

Motor disorders of voice and speech in Reye's syndrome survivors.

Disorders of voice and speech were studied in 43 survivors of Reye's syndrome (RS). During hospital convalescence 26 (60%) of 43 were aphonic, hoarse, or had other alterations of speech production. These disorders occurred in those patients with the worst severity of RS. Four patients (9% of survivors) had a persistent motor voice or speech disorder at follow-up examination 1 1/2 to five years after recovery. All of the patients have breathy, low-intensity voice quality, whereas three of the four exhibit rapid and slurred speech. These four children have no difficulty with the symbolic aspects
