[Pedophilic Disorder and the Use of Child Sexual Abuse Material: The Need for a Multimodal Treatment Approach].

In DSM-5 and ICD-11, pedophilic and/or hebephilic disorder is defined by either psychological distress and/or acting on one's sexual preference-such as through the use of child sexual abuse material (CSAM). It is assumed that most CSAM users remain unknown to law enforcement. Studies on undetected CSAM offenders indicate a higher relapse rate compared to forensic samples. This presents unique challenges for treatment, as the goal is not only to alleviate psychological distress but also to achieve complete behavioral control. This study outlines the multimodal treatment approach for CSAM users

Measles Outbreak in Texas - Urgent Need for Attention.

Measles (rubeola) caused by measles virus is highly contagious and can be transmitted via respiratory droplets or can spread via sneezing or coughing of an infected person. In January 2025, two cases of measles associated with international travel seen in unvaccinated individuals of Harris County were reported by the Houston Health Department. This disease which was once declared eradicated from United States (US) in the year 2000, unfortunately has affected a total of 607 cases since January 2025, across the US, with highest number of cases recorded in Texas. Majority of the cases are witness

Trichomonas vaginalis in neonates.

BACKGROUND: Trichomonas vaginalis is one of the most prevalent pathogens responsible for non-viral sexually transmitted infections. The infection mainly affects women of childbearing age, with the potential for complications during and after pregnancy. Although perinatal transmission is rare, it carries the risk of resulting in neonatal infections. To date, only a dozen documented cases have been reported in the scientific literature. Since modern PCR techniques is more sensitive than conventional methods, it can be anticipated that this microbe will be detected more frequently, even in newbor

Asking difficult questions about fetal alcohol spectrum disorder in the context of the child, the mother, and the systems in which they live.

Alcohol is a known teratogen and prenatal alcohol exposure remains a major ongoing public health concern. Fetal alcohol spectrum disorder has become the diagnosis for describing individuals who have been affected by prenatal alcohol exposure. In this Viewpoint, we raise major concerns about its continued use as a diagnostic term in how it perpetuates a misleading and outdated narrative about child development and maternal health. We argue that the term fetal alcohol spectrum disorder has contributed to a culture of racism and discrimination for many who are diagnosed with it. The term fetal al

Chronic Inflammation Offers Hints About Viable Therapeutic Targets for Preeclampsia and Potentially Related Offspring Sequelae.

The combination of hypertension with systemic inflammation during pregnancy is a hallmark of preeclampsia, but both processes also convey dynamic information about its antecedents and correlates (e.g., fetal growth restriction) and potentially related offspring sequelae. Causal inferences are further complicated by the increasingly frequent overlap of preeclampsia, fetal growth restriction, and multiple indicators of acute and chronic inflammation, with decreased gestational length and its correlates (e.g., social vulnerability). This complexity prompted our group to summarize information from

Successful vaginal delivery in a patient with reconstructed bladder exstrophy and vaginoplasty after cesarean section, a case report from Syria.

INTRODUCTION AND IMPORTANCE: Bladder exstrophy (BE) is a rare congenital anomaly in which the anterior wall of the bladder is absent. In females, sexuality and fertility may be negatively affected with the association with pubic diastasis, dysplasia of the pelvic floor muscles, clitoris bifurcated, short vagina or narrow introitus. PRESENTATION OF CASE: We present a case of 26-year-old woman with treated BE, vaginoplasty, and bicornuate uterus that came at week 36 of gestation. An elective cesarean section was made by paramedian section. Five years later, she came back with placental abruptio

Cerebral cavernous malformation with associated Streptococcus pyogenes abscess in a child: a case report and literature review.

Cerebral cavernous malformations (CCMs) are angiographically occult vascular lesions that present with a variety of neurological symptoms, including seizures, features of raised intracranial pressure and focal neurological deficits. In extremely rare circumstances, CCMs have presented with concomitant brain abscess formation. To date, five cases have previously been reported, the majority of which have affected patients aged 16 years or older. We present a case of a 7-year-old child with recent chickenpox infection, who presented with headache and left hemiparesis and was diagnosed with a larg

Recovering the Lost Soul: Older Women's Reflections on Past Intrafamilial Child Sexual Abuse.

Intrafamilial child sexual abuse (IFCSA) is a social problem with longstanding effects on victims' lives. While scholarly literature has focused on the adverse aftermath of sexual abuse, only a few studies have acknowledged older women's perspectives on their experience of IFCSA and their journey of healing and recovery. The aim of the present study was to explore how older survivors of IFCSA construct and shape their experience of healing in later life and the meaning they assign to this process. Narrative inquiry was selected to explore the narratives of 11 older women survivors of IFCSA. Pa

Facilitating Children's Informal Disclosures of Sexual Abuse: The Role of Online Counsellors at a National Children's Helpline.

The literature on disclosures of child sexual abuse makes clear that in the absence of physical indicators of abuse, children need to articulate their abuse in order to prevent further harm. However, many struggle to verbalize their abuse. Previous research has recognized the interactional nature of disclosures but has focussed only on what the children say, rather than considering the role of the other interlocutor. To provide a novel perspective on disclosures, this research addresses the question 'To what extent do counselors facilitate children's disclosures of sexual abuse?' Online text-b

Mutations in GHR and IGF1R Genes as a Potential Reason for the Lack of Catch-Up Growth in SGA Children.

The aim of this review was to describe all of the mutations in the growth hormone receptor (GHR) and insulin-like growth factor-1 receptor (IGF1R) genes that have been discovered so far, and their possible impact on final body height, as well as their relationship with catch-up growth in children born small for gestational age (SGA). Mutations in the GHR gene were found to cause a body height below -2 SD, from the mean for sex and age, whereas the mutations in the IGF1R gene were associated with low body height and intrauterine growth restriction (IUGR), and with being born SGA. After birth, w

GIST presenting as refractory iron-deficiency anaemia in paediatric patient.

Gastrointestinal stromal tumours (GISTs) are very rare gastrointestinal (GI) mesenchymal tumours affecting only 0.02 children/million/year below the age of 14 years. We reported a 9-year-old girl presented to emergency department with pallor and haemoglobin of 50 g/L. Extensive workup for anaemia suggested iron-deficiency anaemia secondary to GI loss. Ultimately after blood transfusion of packed cells, she was discharged with a haemoglobin of 92 g/L with iron supplementation. Upper endoscopy showed incidental antral nodularity with biopsy proven helicobacter gastritis and an isolate 3-4 cm sus

Limited Prosocial Emotions in a Clinical Population of Children and Adolescents: Proposal for Core and Ancillary Characteristics.

OBJECTIVE: Limited prosocial emotions (LPE) has been recently incorporated into international classifications as a specifier for conduct disorder in the DSM-5 and for all disruptive behavioural disorders in the ICD-11. The aims of the current work were to determine (a) the accuracy of each of the characteristics used to assess the LPE specifier and (b) whether the manner in which symptoms group together supports the idea of LPE having core characteristics. METHOD: Trained clinicians conducted interviews and determined LPE characteristics using responses from 74 parent/guardian and child/adole

A rare case of pediatric primary central nervous system differentiating neuroblastoma: an unusual and rare intracranial primitive neuroectodermal tumor (a case report).

Neuroblastoma represents the most common solid extracranial tumor in children under 5, accounting for 8% to 10% of all childhood cancers. Primary central nervous system (CNS) neuroblastomas are a very rare location and only few cases are available in the literature. It was first described in 1973 by Hart and Earl as supratentorial primitive neuroectodermal tumors. Clinical presentation is highly variable and depends on the initial location of the tumor. Regarding imaging, primary brain neuroblastoma shows no pathognomonic appearance on brain computed tomography (CT) whether or not enhanced or

Sexual Abuse vs. Sexual Freedom? A Legal Approach to the Age of Sexual Consent in Adolescents in Spanish-Speaking Countries.

Child and adolescent sexual abuse (CSA) is an international public health problem. Despite the importance of CSA, there is no consensus definition, and the lack of consensus is related to difficulties in conducting prevalence studies as well as research in other areas. To establish a consensual definition, legal aspects such as the age of sexual consent and the difference in age or power between victim and aggressor as well as aspects related to sexual freedom and sexual indemnity must be considered. Therefore, the main goal of this research was to analyze the age of sexual consent in the lega

Microcephaly primary hereditary (MCPH): Report of novel ASPM variants and prenatal diagnosis in a Vietnamese family.

OBJECTIVE: MCPH (microcephaly primary hereditary) is a group of autosomal recessive developmental disorders with microcephaly present at birth and intellectual disability. Since a second trimester ultrasound is not able to detect subtypes with minimal prenatal presentations, only prenatal diagnosis by genetic testing can confirm these cases and allow for effective genetic counseling, especially a family with a previously affected child. CASE REPORT: A 37-year-old women was pregnant for the third time and had two prior children with profound microcephaly and mental retardation. Targeted panel

Pre-surgical risk assessment in patients with cirrhosis.

Over the last decades, significant improvements in the clini- cal management of patients with cirrhosis have increased their life expectancy. Thus, indications for surgical procedures other than liver transplantation are becoming more frequent. However, patients with advanced liver disease are at high risk of perioperative morbidity and mortality. This is the consequence of multiple factors that include the presence of portal hypertension, alterations on hemostasis and coagulation, the immune dysfunction that entails an increased risk of infections, and the impaired synthesis of proteins that

Safer Roads to School.

Any and all advances made by medical science cannot solve the problem of road traffic injuries (RTIs) in school-going children, especially if the only concerned people are those of the medical fraternity. Children are a vulnerable part of the traveling population and thus have been persistent due to the callous nature of the citizens and policy makers toward road safety and injury prevention. In our multicultural and multilingual country, there is a need for multistakeholder initiative with a countrywide presence if we are to stem the rise of mortality and morbidity due to these accidents.
