Does neck dissection affect post-operative outcomes in parotidectomy? A national study.

OBJECTIVE: To characterize post-operative complications in parotidectomy with neck dissection. METHODS: Patients age ≥ 18 receiving a parotidectomy or parotidectomy with neck dissection between 2005 and 2017 were eligible for inclusion. Patients with unknown demographic variables were excluded. Univariate and multivariable logistic regression analyses were performed. RESULTS: A total of 13,609 parotidectomy patients were analyzed, 11,243 (82.6%) without neck dissection and 2366 (17.4%) with neck dissection. Both length of surgery (mean minutes ± standard deviation [SD] = 335.9 ± 189.2 vs. 15

Completion Gastrectomy with Esophagojejunostomy for Management of Complications of Benign Foregut Surgery.

BACKGROUND: With the worldwide epidemic of obesity, an increasing number of bariatric operations and antireflux fundoplications are being performed. Despite low morbidity of the primary foregut surgery, completion gastrectomy may be necessary as a definitive procedure for complications of prior foregut surgery; however, the literature evaluating outcomes after completion gastrectomy with esophagojejunostomy (EJ) for benign diseases is limited. We present our experience of completion gastrectomy with Roux-en-Y EJ in the setting of benign disease at a single tertiary center. METHODS AND PROCEDU

Cancer antigen-125 and outcomes in acute heart failure: a systematic review and meta-analysis.

BACKGROUND: Carbohydrate antigen-125 (CA125) is an ovarian cancer marker, but recent work has examined its role in risk stratification in heart failure. A recent meta-analysis examined its prognostic value in heart failure generally. However, there has been no systematic evaluation of its role specifically in acute heart failure (AHF). METHODS: PubMed and EMBASE databases were searched until 11 May 2018 for studies that evaluated the prognostic value of CA125 in AHF. RESULTS: A total of 129 and 179 entries were retrieved from PubMed and EMBASE. Sixteen studies (15 cohort studies, 1 randomise

Variation in Diagnostic Test Use and Associated Outcomes in Staphylococcal Scalded Skin Syndrome at Children's Hospitals.

OBJECTIVES: The incidence of staphylococcal scalded skin syndrome (SSSS) is rising, but current practice variation in diagnostic test use is not well described. Our aim was to describe the variation in diagnostic test use in children hospitalized with SSSS and to determine associations with patient outcomes. METHODS: We performed a retrospective (2011-2016) cohort study of children aged 0 to 18 years from 35 children's hospitals in the Pediatric Health Information System database. Tests included blood culture, complete blood count, erythrocyte sedimentation rate, C-reactive protein level, ser

Predicting and managing donor-site wound complications in abdominally based free flap breast reconstruction: improved outcomes with early reoperative closure.

BACKGROUND: The purpose of this study was to (1) determine risk factors predictive of delayed abdominal healing; (2) determine characteristics that perpetuate progression to chronic abdominal wounds and describe the resultant morbidity; and (3) identify outcomes and cost following two treatment strategies-conservative wound care and early reoperative primary closure. METHODS: Patients were identified from a database of abdominally based free flaps performed from January of 2005 through July of 2012. RESULTS: One thousand two hundred eighteen abdominal donor sites were reviewed, and 167 cases

A novel clinical grading scale to guide the management of crusted scabies.

BACKGROUND: Crusted scabies, or hyperinfestation with Sarcoptes scabiei, occurs in people with an inadequate immune response to the mite. In recent decades, data have emerged suggesting that treatment of crusted scabies with oral ivermectin combined with topical agents leads to lower mortality, but there are no generally accepted tools for describing disease severity. Here, we describe a clinical grading scale for crusted scabies and its utility in real world practice. METHODOLOGY/PRINCIPAL FINDINGS: In 2002, Royal Darwin Hospital (RDH), a hospital in tropical Australia developed and began us

Transplantation for type I diabetes: comparison of vascularized whole-organ pancreas with isolated pancreatic islets.

OBJECTIVE: We sought to compare the efficacy, risks, and costs of whole-organ pancreas transplantation (WOP) with the costs of isolated islet transplantation (IIT) in the treatment of patients with type I diabetes mellitus. SUMMARY BACKGROUND DATA: A striking improvement has taken place in the results of IIT with regard to attaining normoglycemia and insulin independence of type I diabetic recipients. Theoretically, this minimally invasive therapy should replace WOP because its risks and expense should be less. To date, however, no systematic comparisons of these 2 options have been reported.

Patient compliance with postoperative analgesia after day case surgery: a multisite observational study of patients in North East London.

BACKGROUND: Pain is the commonest reason for delayed discharge and readmission post day surgery with up to 45% of patients reported to suffer moderate-to-severe post-surgical pain 24 hours after discharge. The importance of post-surgical pain management extends beyond the acute phase when one considers that all chronic post-surgical pain was once acute. Although much focus is given to perioperative analgesia, a patient's pain management once discharged can be overlooked, whilst at this time the patient's pain management is within their own hands. METHODS: We conducted this multisite observati

Novel immune-modulating formula versus standard formula for gastrointestinal cancer patients at risk of malnutrition undergoing surgery: A pilot randomized controlled trial.

BACKGROUND & AIMS: Malnutrition is prevalent among gastrointestinal cancer patients undergoing surgery. The European Society for Clinical Nutrition and Metabolism (ESPEN) guidelines recommend 10-14 days of preoperative nutrition therapy for patients at nutritional risk, and perioperative immunonutrition to improve outcomes. However, benefits of preoperative immunonutrients versus standard formulas remain unclear. This study evaluated a novel curcumin-containing immunonutrient formula compared to a standard formula administered for 10-14 days preoperatively in gastrointestinal cancer patients a

Outcomes of modern total joint arthroplasty in patients with dementia: a systematic review of challenges and considerations for perioperative care.

BACKGROUND: As the global prevalence of dementia rises, an increasing number of patients with cognitive impairment require Total Joint Arthroplasty (TJA). However, the specific impact of dementia on postoperative outcomes remains underreported. This systematic review evaluates the complications, mortality, and healthcare utilization associated with dementia in patients undergoing Total Hip (THA) and Total Knee Arthroplasty (TKA). METHODS: A systematic search of PubMed, Scopus, Web of Science, Embase, and the Cochrane Library was conducted following PRISMA guidelines. Eligible studies included

Impact of Systemic Lupus Erythematosus on Short-Term and Long-Term Outcomes in Total Knee Arthroplasty: A National Database Propensity-Matched Cohort Study.

BACKGROUND: Systemic lupus erythematosus (SLE) is a chronic autoimmune condition that elevates perioperative risk in patients undergoing total knee arthroplasty (TKA), including thromboembolic, renal, and infectious complications. Previous studies have been limited by small sample sizes and inadequate cohort matching. This study aims to evaluate 90-day and 2-year complications in SLE patients undergoing TKA using a large, propensity-matched national database. METHODS: A retrospective cohort analysis was conducted using the TriNetX database. A total of 920 SLE patients undergoing primary TKA w

Untreated mild-moderate mitral regurgitation after isolated aortic valve replacement for aortic stenosis.

BACKGROUND: Postoperative persistent/worsening mitral regurgitation (MR) is reported to be a prognostic marker in patients undergoing isolated aortic valve replacement (AVR). However, there is little evidence regarding the surgical indication for coexisting mild-to-moderate MR in patients undergoing isolated surgical AVR. This study evaluates the relationship between untreated mild-to-moderate MR and postoperative significant MR after surgical AVR. METHODS: Between 2016 and 2021, 430 patients with severe aortic stenosis underwent isolated surgical AVR with untreated mild-to-moderate MR. The p

Complication profiles after total hip arthroplasty in non-cirrhotic NAFLD versus cirrhotic patients: A national matched cohort comparison.

INTRODUCTION: Rising obesity has led to a surge in non-alcoholic fatty liver disease (NAFLD), now the most common chronic liver disease in the U.S. As obesity is also a major risk factor for osteoarthritis, orthopedic surgeons increasingly encounter patients with hepatic dysfunction undergoing joint replacement. While the surgical risks of cirrhosis are well-documented, direct comparisons between NAFLD and non-alcoholic cirrhosis in the context of total hip arthroplasty (THA) remain limited. This study compares postoperative outcomes following THA in matched cohorts with NAFLD versus cirrhosis

Impact of Pacemaker Implantation on Long-Term Survival After Mitral Valve Surgery.

BACKGROUND: Conduction disorders requiring permanent pacemaker implantation (PPM) after aortic valve surgery have been associated with reduced long-term survival. However, data regarding PPM after mitral valve surgery (MVS) have been limited. We evaluated the impact of new PPM on long-term survival after MVS. METHODS: All patients who underwent MVS from 2000-2022 at a single institution were stratified by need for a new postoperative PPM. The primary outcome was long-term survival. To evaluate resource use, secondary outcomes included intensive care unit (ICU) length of stay and 30-day hospit

Bilateral Thoracic Cytoreductive Surgery for Pseudomyxoma Peritonei of Appendiceal Origin.

PURPOSE: The 2022-2023 Italian diabetes guidelines emphasize individualized therapy and structured discharge planning for hospitalized patients with type 2 diabetes mellitus (T2DM). Despite the associate risks, traditional T2DM treatment regimens often include sulfonylureas and insulin. This study evaluated the impact of the Diagnostic and Therapeutic Care Pathway (PDTA) - a guideline-based program prioritizing GLP-1 receptor agonists (GLP-1RA) and SGLT2 inhibitors (SGLT2i), along with structured outpatient follow-up - on glycemic control in non-critically ill hospitalized T2DM patients.
