This article describes a rare clinical case of a left-sided oblique inguinal extraperitoneal ureteral hernia, incidentally detected during follow-up contrast-enhanced computed tomography of the abdomen, retroperitoneum, and pelvis performed four months after robotic-assisted left partial nephrectomy. Identification of this urinary tract anomaly may substantially influence the choice of optimal management in the presence of obstructive uropathy and, in asymptomatic cases, helps prevent potential iatrogenic injury to the involved ureter.

Cleidocranial dysplasia is a rare autosomal dominant disorder of bones mainly affecting bones formed by intra-membranous and endochondral ossification. It presents clinically as brachycephalic skull, frontal bossing, depressed nasal bridge, hypertelorism, delayed closure of fontanelles, delayed eruption of permanent teeth, missing clavicles and wide pelvis. It runs in families; sporadic cases are rarer. Just over than 1000 cases have been reported. We are presenting here 2 cases of cleidocranial dysplasia in siblings.

Flexible nephroscopy while using carbon dioxide to insufflate the collecting system has been described as a "good trick" to remove caliceal stones during laparoscopic pyelolithotomy. We found this true only for those calices with undisturbed urine drain into the renal pelvis, while it provided suboptimal view of stones that were located in dependent lower pole calices. We describe how we dealt successfully with a group of lower pole caliceal stones during laparoscopic pyeloplasty.

Ultrasound-guided percutaneous pyelography has previously been used in dogs to diagnose ectopic ureter. To the authors' knowledge its use has never been reported in the horse. This paper reports the technique used in a three-week-old filly with a suspected ectopic ureter. Pyelography can be performed under sedation and is a relatively simple, safe procedure. The technique enables assessment of individual renal function through collection and analysis of urine from each renal pelvis.

A 40-year-old woman presented with asymptomatic gross hematuria caused by the nutcracker phenomenon. Despite treatment with hemostatic agents and injection of silver nitrate into the renal pelvis, the hematuria had continued, and severe anemia (hematocrit 17%) had developed. We performed expandable metallic stent (EMS) placement across the left renal vein. Although mild hematuria continued, the anemia resolved after this interventional radiotherapy. EMS placement is a minimally invasive therapy for the nutcracker phenomenon.

OBJECTIVE: To describe a case of giant renal calculus.
METHODS/RESULTS: A 57-year-old female with a previous history of left renal calculus 30 years earlier, consulted for episodes of gross hematuria. The imaging studies showed a giant, spherical calculus, 9 cm in diameter, in the left renal pelvis with hydronephrosis and remaining renal parenchyma functioning. The patient refused surgery.
CONCLUSION: Giant renal calculi may progress slowly, without symptoms and without abolishing renal function.

Urinary oxygen tension may be an index of renal medullary blood flow. The effect of i.v. furosemide on urinary oxygen tension was studied in four patients with indwelling nephrostomy tubes. An intravascular oxygen sensor (Paratrend 7, Biomedical Sensors Ltd, UK) was inserted into the renal pelvis via the nephrostomy and urine oxygen tension measured. In all cases, furosemide 20 mg i.v. produced a decrease in pelvic urinary oxygen. The possible mechanisms and implications are discussed.

A primigravida presented at 33 weeks gestation with varicella pneumonia. Cardiotocography demonstrated repeated late decelerations of fetal heart rate. Cesarean section was indicated for contracted maternal pelvis and breech presentation, but was decided against because of the risks of anesthesia. Maternal and fetal improvement followed intrauterine resuscitation and suppression of preterm labor. Cesarean section was performed 9 days later after spontaneous rupture of membranes with excellent outcome. The same approach is suggested for similar cases.

Thirteen patients with a variety of renal pelvic filling defects were evaluated using computed tomography (CT). Nonopaque calculi and some blood clots can be more accurately differentiated with this modality than has heretofore been possible. In transitional-cell carcinoma of the renal pelvis, the main value of CT appears to be in tumor staging. CT may also be helpful where urography cannot discriminate between extrinsic compression and true intraluminal renal pelvic defects.

Accessory spleen is a rare anomaly occurring in approximately 10% of people. Most often, accessory spleen can be located in spleen gate. However, localization in parenchymal abdominal organs, retroperitoneal space, pelvis and chest is also possible. Congenital accessory spleen is extremely rare, and only several cases are described in the literature. Diagnosis causes some difficulties. CT-based differentiation with malignant tumor is difficult. We present a 72-year-old patient with congenital intrathoracic accessory spleen.

A 54-year-old man presented with a 1-month history of pain and numbness in the right lower limb. Lumbar spine MRI revealed bone metastases. 18 F-FDG PET/CT showed a soft tissue mass with increased 18 F-FDG uptake in the rectum, accompanied by multiple hypermetabolic lesions in the bilateral ribs, spine, pelvis, and upper right femur. A colonoscopy biopsy of the submucosal rectal mass confirmed the diagnosis of epithelioid angiosarcoma.

A 77-year-old man with back pain and abdominal distention underwent F-NaF PET/CT examination to evaluate potential bone lesions. Multiple foci of increased activity were noted in the chest, which corresponded to bilateral ribs and suggested possible occult fractures. Surprisingly, increased F-NaF uptake was observed in the region of pelvis, which corresponded to calcification along the colon wall, a sequela of intestinal Japanese schistosomiasis that the patient experienced 30 years ago.

A 59-year-old woman with back pain underwent an F-NaF PET/CT bone scan. Unexpectedly, multiple foci of increased tracer uptake were present in the abdomen and pelvis, which corresponded to the calcified soft tissue masses. The subsequent F-FDG PET/CT not only confirmed increased FDG activity in these partially calcified mass but also revealed abnormal activity in noncalcified lesions. The pathological examination demonstrated that the patient had ovarian cancer.

Metastatic disease involving the spine and pelvis is common, often resulting in significant pain and disability. Several percutaneous interventions have been described, including osteoplasty, ablation, and screw fixation, that when used alone or in combination can significantly reduce pain and disability from metastatic bone disease. Although it is possible to make a significant impact in patient care with basic principles and techniques, certain advanced techniques can extend the application of percutaneous interventions while minimizing morbidity.

Ultrasound examination of the peritoneal cavity, small pelvis, iliac lymph nodes and femoral blood vessels (downstream of the blood flow) was performed in 346 patients who had undergone pelvic lymphadenectomy. Post-operative lymphocele developed in 302 (7.2%). However, treatment was required in 98 (28.3%) only. Our strategy of follow-up after pelvic lymphadenectomy facilitated diagnosis and effective excision of lymphocele without resorting to laparotomy due to an improved procedure of ultrasound monitoring.

We report a case of a large inguino-scrotal bladder hernia presenting with obstructive lower urinary symptoms and causing massive unilateral uretero-hydronephrosis with ipsilateral renal function deterioration. Careful anamnesis and computer tomography (CT) scan of the abdomen and pelvis were important for the diagnosis. Open surgery with mesh hernia repair was successful. The obstructive lower tract symptoms subsided following surgery while the deteriorated renal unit regained only part of its function.

A 65-year-old man with small cell lung cancer treated with two courses of chemotherapy manifested appetite loss and abdominal distention 10 days before admission. Helical CT scanning of the abdomen and pelvis disclosed marked hepatomegaly without any visible nodular lesion in the hepatic parenchyma. He died of severe liver dysfunction with multiorgan failure on the 20th hospital day. Autopsy revealed diffuse invasion of tumor cells into the sinusoid throughout the liver.

A prospective study of detecting vesicoureteric reflux (VUR) by ultrasound is to be presented. In case of reflux, gas bubbles can be seen in the renal pelvis during the filling phase of a gas cystogram and during micturition, utilizing ultrasound as the imaging modality. In children under two years 52 sonographic reflux studies were performed and compared with radiographic voiding cystourethrograms (VCUG). All higher grade VUR (grades II-IV) have been detected by our method.
