Management of gout: a 57-year-old man with a history of podagra, hyperuricemia, and mild renal insufficiency.

Gout is an ancient disease. Despite significant advances in the understanding of its risk factors, etiology, pathogenesis, prevention, and treatment, millions of people with gout experience repeated attacks of acute arthritis and other complications. The incidence of gout is increasing, most likely reflecting increasing rates of obesity and other lifestyle factors, including diet. Comorbid conditions that often accompany gout, including chronic kidney disease and diabetes mellitus, present challenges for the management of gout. Using the case of Mr R, a 57-year-old man with a history of podagr

Longitudinal Assessment of Mental Health Disorders and Comorbidities Across 4 Decades Among Participants in the Dunedin Birth Cohort Study.

IMPORTANCE: Mental health professionals typically encounter patients at 1 point in patients' lives. This cross-sectional window understandably fosters focus on the current presenting diagnosis. Research programs, treatment protocols, specialist clinics, and specialist journals are oriented to presenting diagnoses, on the assumption that diagnosis informs about causes and prognosis. This study tests an alternative hypothesis: people with mental disorders experience many different kinds of disorders across diagnostic families, when followed for 4 decades. OBJECTIVE: To describe mental disorder

RSPO2 inhibits BMP signaling to promote self-renewal in acute myeloid leukemia.

Acute myeloid leukemia (AML) is a rapidly progressing cancer, for which chemotherapy remains standard treatment and additional therapeutic targets are requisite. Here, we show that AML cells secrete the stem cell growth factor R-spondin 2 (RSPO2) to promote their self-renewal and prevent cell differentiation. Although RSPO2 is a well-known WNT agonist, we reveal that it maintains AML self-renewal WNT independently, by inhibiting BMP receptor signaling. Autocrine RSPO2 signaling is also required to prevent differentiation and to promote self-renewal in normal hematopoietic stem cells as well as

Outcome after extended follow-up in a prospective study of operable breast cancer: key factors and a prognostic index.

In 1990, 215 patients with operable breast cancer were entered into a prospective study of the prognostic significance of five biochemical markers and 15 other factors (pathological/chronological/patient). After a median follow-up of 6.6 years, there were 77 recurrences and 77 deaths (59 breast cancer-related). By univariate analysis, patient outcome related significantly to 13 factors. By multivariate analysis, the most important of nine independent factors were: number of nodes involved, steroid receptors (for oestrogen or progestogen), age, clinical or pathological tumour size and grade. Re…

Restricted and repetitive behaviors in young children with autism spectrum disorders.

Restricted and repetitive behaviors (RRBs) on the Autism Diagnostic Interview- Revised (ADI-R: Lord, Rutter, & Le Couteur (1994) were examined in 165 children with Autism Spectrum Disorders (ASD), 49 children with non-spectrum developmental disorders (DD), and 65 children with typical development (TD) at approximately 2 years of age. A factor analysis found evidence for a repetitive sensorimotor (RSM) factor and an insistence on sameness (IS) factor. Behaviors that loaded on the RSM factor were prevalent in children with ASD and significantly more common and severe than in children with DD or

The Development and Validation of the Sexual and Relationship Distress Scale.

BACKGROUND: Sexual distress is an important factor in the etiology, maintenance, and treatment of sexual difficulties, and as such, there is a need for validated measures. A limitation in the research and treatment of distressing sexual difficulties has been the lack of validated measures, and in particular, existing measures are unable to measure the impacts at the relationship level and currently focus on intra-personal distress. AIM: This study sought to develop and psychometrically evaluate a new measure of distress associated with sexual difficulties. METHODS: An initial pool of 73 item

Social determinants of health, genetic biomarkers, and cognitive function in Chinese older adults with subjective cognitive decline and mild cognitive impairment: A cross-sectional analysis.

BACKGROUND: Social isolation (SI) is an established risk factor for Alzheimer's disease (AD) and cognitive decline. However, its stage-specific effects across the AD continuum, particularly at subjective cognitive decline (SCD) and mild cognitive impairment (MCI) stages, remain unquantified in Chinese populations. The sex-specific effects of SI on cognitive decline remain incompletely characterized. The apolipoprotein E (APOE) genotype is a driver of cognitive decline and dementia. OBJECTIVE: To investigate social connection characteristics and gene distribution in individuals with SCD or MCI

Polatuzumab vedotin-containing regimens as bridge to CART: analysis from the CART-SIE study.

High tumor burden negatively affects responses to anti-CD19 chimeric antigen receptor T-cell (CART) therapy in large B-cell lymphoma (LBCL). Therefore, bridging therapy (BT) is crucial for disease control before infusion. Here, we retrospectively compared polatuzumab vedotin (PV) combined with rituximab (PV-R) ± bendamustine (PV-BR) in a cohort of 200 patients with LBCL enrolled in the prospective, multicenter, observational CART-Società Italiana di Ematologia (SIE) study. Commercial CARTs were infused between July 2020 and January 2025. Patients received BT with either PV-BR (n = 122) or PV-R

Establishing the clinical utility of integrated quasistatic acoustic tweezing thromboelastometry for blood coagulation analysis.

BACKGROUND: Rapid and accurate coagulation analysis is essential for managing critical care and surgical patients, patients with coagulation disorders, and patients with cardiovascular disease on anticoagulant therapy. Traditional assays often require large sample volumes, posing iatrogenic risks in vulnerable populations, including pediatric and elderly patients. Integrated quasistatic acoustic tweezing thromboelastometry (i-QATT) offers a comprehensive, noncontact assessment of coagulation using a 4- to 6-μL drop of blood that minimizes diagnostic errors associated with sample-container surf

NLRP6 is essential for TREM2-alleviated cardiac ischemia/reperfusion injury via affecting PANoptosis.

Ischemia/reperfusion (I/R) injury is characterized by the activation of multiple regulated cell death pathways, including pyroptosis, apoptosis, and necroptosis-collectively known as PANoptosis. Nevertheless, the molecular determinants driving myocardial PANoptosis remain poorly defined. Triggering receptor expressed on myeloid cells 2 (TREM2), an innate immune modulator, has been implicated in diverse pathological contexts, but its role in cardiac I/R injury is unclear. In parallel, NOD-like receptor family pyrin domain-containing 6 (NLRP6) has recently been recognized as a regulator of infla

High-Dose Melphalan Followed by Busulfan and Fludarabine Conditioning Prior to Allogeneic Stem Cell Transplantation in Elderly Patients with Active Acute Myeloid Leukemia-A Retrospective Single-Center Study.

In patients with relapsed or refractory (r/r) acute myeloid leukemia (AML), long-term survival in most cases can only be achieved with allogeneic hematopoietic stem cell transplantation. The introduction of sequential conditioning regimens has contributed to the steadily improving prognosis of r/r AML patients. As most studies mainly included younger patients, feasibility of sequential conditioning in elderly or comorbid patients remains subject of debate. We retrospectively investigated outcomes of 103 patients aged ≥55 years with active r/r AML receiving sequential melphalan-based conditioni

Association between the central sensitization inventory score and health-related quality of life in community-dwelling middle-aged and older adults.

BACKGROUND: Central sensitization is an important factor associated with impaired health-related quality of life in patients with musculoskeletal disorders and community-dwelling older adults. However, health-related quality-of-life domains strongly associated with central sensitization in the general population remain unclear. This study aimed to examine the association between the Central Sensitization Inventory Part A scores and health-related quality of life using community health checkup data. METHODS: A total of 419 middle-aged and older adults (mean age, 64.4 ± 11.2 years; 59.4% female

Extracellular RNA drives TNF-α/TNF-receptor-1 mediated cardiac ischemia/reperfusion injury: Mechanistic insights and therapeutic potential of RNase1.

Myocardial ischemia/reperfusion (I/R) injury causes cardiomyocyte death and exacerbates inflammation. Emerging evidence implicates extracellular RNA (eRNA) and tumor necrosis factor-α (TNF-α) as key mediators. We hypothesize that eRNA released from ischemic cardiomyocytes amplifies I/R injury via TNF-α/TNF-receptor-1 (TNF-R1) signaling, and that hydrolysis of eRNA by RNase1 can attenuate I/R injury by disrupting this pathway. Here, we investigated the mechanistic role of eRNA and its interplay with TNF-α signaling in cardiac I/R injury, and evaluated the therapeutic potential of RNase1 and cyc

Complex association of body mass index and outcomes in patients with relapsed and refractory multiple myeloma treated with CAR-T cell immunotherapy.

BACKGROUND AIMS: Chimeric antigen receptor-T (CAR-T) cells have exhibited remarkable efficacy in treating refractory or relapsed multiple myeloma (R/R MM). Although obesity has a favorable value in enhancing the response to immunotherapy, less is known about its predictive value regarding the efficacy and prognosis of CAR-T cell immunotherapy. METHODS: We conducted a retrospective study of 111 patients with R/R MM who underwent CAR-T cell treatment. Using the body mass index (BMI) classification, the patients were divided into a normal-weight group (73/111) and an overweight group (38/111). W

MSC Promotes the Secretion of Exosomal lncRNA KLF3-AS1 to Regulate Sphk1 Through YY1-Musashi-1 Axis and Improve Cerebral Ischemia-Reperfusion Injury.

Stroke remains the 3rd leading cause of long-term disability globally. Over the past decade, mesenchymal stem cell (MSC) transplantation has been proven as an effective therapy for ischemic stroke. However, the mechanism of MSC-derived exosomal lncRNAs during cerebral ischemia/reperfusion (I/R) remains ambiguous. The oxygen-glucose deprivation/reoxygenation (OGD/R) and middle cerebral artery occlusion (MCAO) rat model were generated. MSCs were isolated and characterized by flow cytometry and histochemical staining, and MSC exosomes were purified and characterized by transmission electron micro
