BACKGROUND: In contrast to dual-energy x-ray absorptiometry (DXA), the D3-creatine (D3Cr) dilution method provides a direct measure of skeletal muscle mass and in a cohort of older men has been strongly associated with health-related outcomes. However, sensitivity to detect changes in D3Cr-derived muscle mass due to an intervention is limited.
METHODS: Twenty-one older adults (≥70 years) with low-to-moderate physical function were randomized to a 15-week high-intensity strength training (ST) or a health education (HE) group. Full-body progressive intensity ST was performed 3 days per week.

BACKGROUND: Age-related loss of skeletal muscle mass and function begins in early middle age, yet research to date has focused on older individuals, limiting our understanding of interventions earlier in the lifespan. To date, no high-intensity interval training studies have been conducted in middle-aged adults with low relative lean soft tissue mass.
METHODS: Eighty-two middle-aged adults (40-50 years of age) with low appendicular lean soft tissue mass index confirmed with dual energy x-ray absorptiometry (DXA) were randomly allocated (1:1) to group-based, 20-week, three times a week, high-i

BACKGROUND: Cardiac structure and function change with age. The higher prevalence of left ventricular hypertrophy (LVH) with concentric remodeling is indicative of a typical geometric pattern of aging associated with a higher cardiovascular (CV) risk and diseases. The recent associations found between low left ventricular and skeletal mass in older patients with frailty and sarcopenia have raised great interest in investigating cardiac characteristics and determinants of left ventricular mass (LVM) in this population.
DESIGN: Cross-sectional study.
METHODS: We evaluated 100 sarcopenic and ph

Cancer patients and survivors are at risk of poor clinical outcomes due to poor nutritional intake following cancer diagnosis. During cancer treatment, treatment toxicities can affect eating patterns and can lead to malnutrition resulting in loss of lean body mass and excessive weight loss. Following treatment and throughout survivorship, patients are at risk of not meeting national nutrition guidelines for cancer survivors, which can affect recurrence and survival. Obesity, which is highly prevalent in cancer patients and survivors, can affect clinical outcomes during treatment by masking mal

BACKGROUND: Sarcopenia is a state of degenerative skeletal muscle wasting induced by cancer cachexia.
OBJECTIVE: To evaluate the prognostic impact of changes in skeletal muscle mass (SMM) during first-line sunitinib therapy on oncological outcomes in metastatic renal cell carcinoma (mRCC).
PATIENTS AND METHODS: Sixty-nine patients were evaluated retrospectively. The skeletal muscle index (SMI) was calculated based on computed tomography images obtained before the initiation (pre-treatment SMI) and after two cycles of sunitinib treatment (post-treatment SMI). The change in SMM was evaluated b

BACKGROUND: Although muscle mass declines with testosterone deficiency in men, previous studies of muscle function have not demonstrated consistent deficits, likely due to relatively insensitive methodology. Our objective was to determine the effects of testosterone deprivation on the biomechanical function of individual lower-limb muscles.
METHODS: We conducted a 12-month prospective, observational case-control study of 34 men newly commencing androgen deprivation treatment (ADT) for prostate cancer and 29 age-matched prostate cancer controls. Participants were assessed at 0, 6, and 12 month

BACKGROUND: Due to the worldwide demographic transition healthcare systems are facing new demands and are increasingly confronted with an older population with specific medical needs related to multiple chronic disorders. The majority of older persons have an increased risk of frailty. In addition to pharmaceutical interventions another beneficial approach for counteracting frailty might be exercise or physical activity intervention.
OBJECTIVE: The first goal was to narratively investigate the state of the art effective exercise interventions for frail older persons and briefly discuss the im

Growth hormone (GH) and/or ghrelin mimetics represent potential treatment and/or prevention options for musculoskeletal impairment associated with aging. Use of improvement in muscle function as an outcome in studies of GH and ghrelin mimetics is complicated by the lack of a standardized definition for clinically meaningful efficacy of this end point. Based on preliminary study results, the use of ghrelin mimetics may be more suitable for use in this age group than GH itself. There are still several unanswered questions related to the use of ghrelin mimetics in the elderly, which prevents reco

Background: Overweight and obesity are burgeoning health epidemics. Diets including alternate-day fasting (ADF) promote weight loss but risk significant muscle loss, posing health implications to seniors at risk of sarcopenia. Leucine, the key muscle protein synthesis (MPS) driver, effectively stimulates MPS with lower calorie intake than protein. However, the effects of body-mass-scaled protein and leucine intakes are unclear. Objectives: To evaluate body-mass-scaled leucine or protein supplementation on body composition, sarcopenic, and cardiometabolic markers in older men during ADF. Method

To study serum creatinine (Scr) levels and bone mineral density (BMD), we used Mendelian randomization (MR) to investigate the causal relationship between them. The genetic association data of Scr levels and total body BMD across age groups were retrieved from the genome-wide association studies, and the causal relationship between Scr levels and BMD was analyzed using the inverse-variance weighting method, MR-Egger regression method, weighted median estimation method, simple mode, and weighted mode. The results of the inverse-variance weighted method indicated that the creatinine level might

Sarcopenic obesity (SO) is a complex condition involving increased fat accumulation along with reduced muscle mass and impaired muscle function; however, there are currently no approved pharmacological interventions targeting these pathological features. Akt and AMPK are key signaling pathways regulating muscle homeostasis and energy metabolism. Natural compounds that modulate these pathways may offer a promising multi-targeted therapeutic strategy. This study investigated the effects of Fucoidan P on SO and its underlying molecular pathways, while assessing translational relevance using clini

BACKGROUND: Sarcopenic obesity (SO) is prevalent in older adults and is associated with metabolic dysfunction and adverse health outcomes. Lifestyle factors including diet may influence SO development and associated metabolic perturbations. This study examined associations between MedDiet adherence and body composition and metabolic outcomes in community-dwelling older adults at risk of SO and explored links between individual food groups and these outcomes.
METHODS: This cross-sectional study included adults aged ≥ 60 years at risk of SO (BMI ≥ 30 kg/m2, SARC-F score ≥ 2). Diet was assessed

Background: Incretin-based therapies (IBTs), including glucagon-like peptide-1 receptor agonists (GLP-1 RAs) and dual glucose-dependent insulinotropic polypeptide/glucagon-like peptide-1 receptor agonists (GIP/GLP-1 RAs), have transformed the management of obesity and type 2 diabetes mellitus (T2DM). Although these agents provide substantial metabolic and cardiometabolic benefits, their effects on skeletal muscle, particularly in older adults at increased risk of sarcopenia and functional decline, remain incompletely understood. Methods: This narrative review synthesizes evidence from randomiz

Chronic obstructive pulmonary disease (COPD) is a complex and heterogeneous lung condition characterized by persistent airflow obstruction, associated with cardiovascular, metabolic and musculoskeletal comorbidities. In 2022, approximately 480 million people were affected by this disease, making it the third leading cause of mortality worldwide. Projections indicate that by 2050, this number could rise to 600 million. Nutrition status is a fundamental component in managing COPD patients, as it is a critical prognostic factor for morbidity and mortality. Patients with COPD may display different
