Oral Lichen Planus in a Pediatric Patient: A Novel Therapeutic Approach.

Lichen planus is a mucocutaneous disease, predominantly affecting the middle-aged individuals and may be associated with a plethora of signs and symptoms related to the skin, scalp, nails and mucous membranes. The definitive etiology of lichen planus is not yet known and no therapeutic modality has yet been universally accepted. Lichen planus in pediatric patients is a rare phenomenon and its presence in the oral mucosa is even rarer. The aim of this article is to present a rare case of a symptomatic oral lichen planus (OLP) occurring in a 12-year old child that was managed successfully with a…

Extreme aplasia cutis congenita involving the skull.

Aplasia cutis congenita (ACC) is a rare congenital malformation of primarily the skin; it is most commonly seen on the scalp but can occur anywhere on the body. The exact etiology is still unclear but there are many suggested causes. Classification systems have been proposed to help categorize patients and assist with treatment. Treatment options are controversial and range from conservative to surgical interventions. We report an extreme case of ACC that included a significant part of the skull. We discuss this case and review salient literature. Although such cases of ACC with bony involveme…

Chronic orofacial pain.

The issues specific to trigeminal pain include the complexity of the region, the problematic impact on daily function and significant psychological impact (J Dent, 43, 2015, 1203). By nature of the geography of the pain (affecting the face, eyes, scalp, nose, mouth), it may interfere with just about every social function we take for granted and enjoy (J Orofac Pain, 25, 2011, 333). The trigeminal nerve is the largest sensory nerve in the body, protecting the essential organs that underpin our very existence (brain, eyes, nose, mouth). It is no wonder that pain within the trigeminal system in t…

Current and Emerging Therapies for Itch Management in Psoriasis.

Pruritus is a common and significant symptom among patients with psoriasis. Pruritus is often present beyond the borders of psoriatic plaques, and frequently affects the scalp and genital regions. Psoriatic itch may be severe and can profoundly affect quality of life and sleep, even in the context of mild-to-moderate disease. These features often make the treatment of psoriatic pruritus challenging. However, there are a variety of effective topical and systemic treatment modalities available to address this symptom. While there remains a need for treatments that specifically target psoriatic i…

Encephalocraniocutaneous lipomatosis.

Encephalocraniocutaneous lipomatosis (ECCL) is an unusual condition marked by characteristic dermatologic and neurologic findings presenting in a mosaic fashion. These are now being found to be due to specific genetic mutations. Traditionally, the diagnostic features include ocular dermoids, scalp changes, and spinal lipomas. While there are other similar diagnostic considerations, ECCL, is sufficiently distinct clinically to allow differentiation. Such information is of use when considering pathogenesis and in counseling. Consideration of possible associated clinical findings is key for corre…

A case of trichotillomania with recently defined trichoscopic findings.

Trichotillomania (TTM) is an impulse control disorder characterized by repetitive behavior of hair pulling resulting in secondary alopecia. It is among the psychocutaneous diseases known to be associated with psychiatric comorbidity, social, and functional impairment. Although most of the time, an experienced dermatologist easily recognizes the key features of TTM, a history and physical examination alone might not be enough to make a definitive diagnosis. As an effective noninvasive technique for the evaluation of scalp and hair diseases, trichoscopy also has proven to be a valuable tool in t…

Pitfalls and pearls in the diagnosis of monilethrix.

A 4-year-old girl presented with sparse, brittle hair on her entire scalp and keratosis pilaris on the nape of her neck. Subtle microscopic and macroscopic diagnostic features presented a challenge for physicians. Only repeated, optimized light microscopy revealed the diagnosis of monilethrix, a rare genetic hair shaft disorder with a variable phenotypic expression and inheritance pattern. We provide a short overview of methods that maximize the diagnostic yield in a clinical setting and of light microscopy to reach a rapid and accurate diagnosis in difficult cases. We conclude with essential …

Eccrine poroma on the postauricular area: a rare presentation.

An eccrine poroma is a benign neoplasm that originates from the intraepidermal ductal portion of the eccrine sweat duct. Although eccrine poromas are most commonly found on the sole or side of the foot, eccrine poromas have been observed on other areas of the skin, such as the scalp, neck, and chest. We report an interesting case of an eccrine poroma, which presented as a 1×1 cm protruding dome-shaped, skin-colored-to-black nodule on the right postauricular area. The patient denied a previous history of trauma to this area. The histopathologic diagnosis was consistent with an eccrine poroma. T…

Calculation of fluence and absorbed dose in head tissues due to different photon energies.

Calculations of fluence and absorbed dose in head tissues due to different photon energies were carried out using the MCNPX code, to simulate two models of a patient's head: one spherical and another more realistic ellipsoidal. Both head models had concentric shells to describe the scalp skin, the cranium and the brain. The tumor was located at the center of the head and it was a 1 cm-radius sphere. The MCNPX code was run for different energies. Results showed that the fluence decreases as the photons pass through the different head tissues. It can be observed that, although the fluence into t…

Jaw angiosarcoma. First case, with massive intraosseous localization, described in Italian literature.

Angiosarcoma (AS) is a rare non-epithelial malignant neoplasm arising from neoplastic vascular degeneration of endothelial cells. It usually occurs in soft tissue and skin. The incidence, according to American authors, is 1% of all soft tissue sarcomas. About 50% of AS is localized in head and neck region (scalp and face skin) and represents less than 1% of all malignancies of this district; the primitive intra- oral localization is rare, even rarer intraosseous development of AS in jaw bones. The Authors report a case of a mandibular intraosseus angiosarcoma with different peculiarities: the …

Castleman's Disease with Cutaneous Involvement Manifestating as Multiple Violaceous Plaques on Entire Body.

Castleman's disease (CD) is an uncommon B-cell lymphoproliferative disorder characterized by lymph node hyperplasia with vascular proliferation. Cutaneous involvement in CD is rare. A 65-year-old man presented with a 7-year history of gradually developing multiple reddish to violaceous indurated plaques on the scalp, trunk, and legs. On physical examination, there were palpable enlarged cervical, axillary, and inguinal lymph nodes. Laboratory examination revealed anemia, thrombocytosis, hyperproteinemia, hypoalbuminemia, and polyclonal hypergammaglobulinemia. An inguinal lymph node biopsy and …

[Seborrhoeic dermatitis: clinical manifestations and management].

Seborrhoeic dermatitis is a frequent chronic inflammatory dermatosis characterized by erythematous patches surmounted by fatty and yellowish scales, affecting particularly the scalp, the naso-labial folds and the eyebrows. Its etio-pathogeny is still not clear, but Malassezia type yeast appear to play a very important role in its development. Several conditions can be associated with high prevalency of seborrhoeic dermatitis (HIV, Parkinson's disease, Down's Syndrome). No curative treatment is available yet, but nevertheless the symptomatology can be controlled, mainly with topical treatments,…

Cutaneous metastases as the first clinical sign of metastatic gastric carcinoma.

Cutaneous metastases from gastric cancer are uncommon with a frequency of 7 % but can rarely be the presenting sign. A 54-year-old man complained of multiple pea-sized scalp nodules which had been present for four months. Histology showed a metastatic adenocarcinoma. Initial evaluation revealed liver metastases and gastroscopy then identified a tumor involving the distal esophagus and gastric cardia that was diagnosed as a gastric tubular carcinoma. The patient had a good response to polychemotherapy. While gastric carcinoma generally metastasizes to the abdominal wall or lymph nodes, our pati…

[Clinical and electroneurophysiological characteristics of polysynaptic reflex excitability in headache of tension].

To study polysynaptic reflex excitability in different clinical forms of headache of tension, 187 patients with episodic (135) and chronic (52) forms of the disease were examined. The Visual-Analogue Scale, clinical neurological examination, neuropsychological testing, interference EMC of scalp and cervical muscles, electroneurophysiological study of polysynaptic blinking reflex were used. The increase of polysynaptic reflex excitability, the level of which was correlated with disease severity, was found in patients. This finding characterizes the insufficiency of central inhibition and ineffi…

Birth injury of the cranium and central nervous system.

Birth injury of the scalp, skull and central nervous system (CNS) is a well-recognized complication of a difficult delivery. The rate of birth trauma has dropped precipitously and now accounts for less than 2% of neonatal deaths. Despite this dramatic decrease in birth-trauma mortality significant injuries still occur. A variety of risk factors clearly predispose certain infants to birth-related injury. Recent neuroradiology studies indicate that intracranial hemorrhage, even in asymptomatic infants, is not rare. Pathologists' (neuropathologists and forensic pathologists) appreciation of the s…

Delayed hypersensitivity reaction from black henna tattoo manifesting as severe facial swelling.

We report on a 14-year-old boy who was presented to the emergency department with an acute swelling of the face and scalp 3 days after using a new hair dye. The patient had applied a black henna tattoo 1 year earlier. Patch testing revealed an allergy to the potent skin sensitizer paraphenylenediamine, a common ingredient of hair dyes and also found in black henna tattoo. It is important for emergency physicians to be aware of the possibility of a delayed type-IV hypersensitivity reaction from black henna tattoos manifesting as an acute contact dermatitis. These patients may have gross facial …

Contribution of surgical evacuation of a large subgaleal hematoma to the resolution of severe conjugated hyperbilirubinemia in a neonate. Case report.

Subgaleal hematomas (SGHs) in neonates may result from trauma to the scalp sustained during delivery. In the majority of cases, these lesions will resolve on their own without serious or long-term consequences. The authors report on a case of SGH resulting in hypotension, anemia, coagulopathy, and eventually direct hyperbilirubinemia in a neonate. After several weeks of medical management failed to resolve the hyperbilirubinemia, surgical evacuation of the clot was undertaken and yielded favorable results. The direct bilirubinemia in this case was believed to be the result of an overload of ir…

Metastatic angiosarcoma of the kidney: a case report with treatment approach and review of the literature.

Angiosarcomas are rare soft tissue malignancies. Typically they originate from the skin of the scalp or face, whereas visceral sarcomas are very rare. We report the case of a 67-year-old man affected by a large angiosarcoma of the kidney. After surgical removal, a rapid peritoneal, visceral and cutaneous diffusion developed. Palliative chemotherapy, based on anthracycline and ifosfamide, which are normally used to treat all other high-grade spindle cell sarcomas, was totally inactive. On the basis of these results and of the biological characteristics of these rare neoplasms it is mandatory to…

Hybrid cyst: case reports and review of 15 cases in Japan.

Hybrid cyst is a rare cystic lesion that includes more than two components of the pilosebaceous units. To clarify the clinical and pathological features of hybrid cysts, we report two cases and review 15 cases of hybrid cyst in Japan. On the whole, the age range was 12-73 years with a 2.95:1 female predominance and predilection for the scalp and face (46.7%). Most of the tumours presented as a solitary lesion and the size range was 2-45 mm. The most frequent histological type was the combination of infundibular and trichilemmal cysts (60.0%). Studying the clinicopathological features of hybrid…

Bilateral retro-auricular dermal sinus tracts with intradural extension. Case report.

The authors describe a previously unreported malformation involving paired, bilaterally symmetrical dermal sinus tracts in the retro-auricular area, both of which passed through the asterion and posterior fossa dura mater to end intracranially. Cranial dermal sinus tracts are congenital lesions that virtually always originate from the midline scalp posteriorly at the external occipital protuberance, anteriorly at the nasion or along the nasal dorsum, or in the posterior parietal midline. A lateral origin is extremely rare, and intracranial extension of a lateral dermal sinus tract has not been…
