Sleep Studies in Children.

Sleep-related breathing disorders (SRBD), also referred to as sleep-disordered breathing (SDB), are common sleep disorders in children. They can be broadly divided between central and obstructive sleep-disordered breathing with or without associated hypoventilation. In most cases, SRBD are associated with adenotonsillar hypertrophy (obstructive SDB) which are classified as simple. SRBD can co-exist with an underlying condition like obesity, genetic syndromes or neuromuscular disorders which are classified as complex. Polysomnography (PSG) is the gold standard for diagnosing sleep disorders. Ho

Sleep Disorders and Coronary Artery Disease.

A sufficient amount and quality sleep is vital components for maintaining optimal physical wellbeing, cognition, and cardiovascular health. Normal sleep provides us with a low physiological stress that promotes overall cardiovascular health. Sleep disorders are common, and often goes undiagnosed. Patients with sleep disorders, commonly suffer from poor quality of life due to the poor quality of their sleep. They are associated with adverse effects on cardiovascular health, such as coronary artery disease. Sleep disorders discussed in this review article include sleep-disordered breathing (obst

Management of paediatric sleep-disordered breathing.

Paediatric sleep-disordered breathing is a common condition which varies in severity from snoring to obstructive sleep apnoea. Paediatric sleep-disordered breathing is usually diagnosed clinically, with investigations such as polysomnography reserved for more complex cases. Management can involve watching and waiting, medical or adjunct treatments and adenotonsillectomy. National working groups have sought to standardise the pathway for surgery and improve the management of surgical and anaesthetic complications. Current guidelines use age, weight and comorbidities to stratify risk for these s

Potential ultrasonic anatomical markers of obstructive sleep apnoea-hypopnoea syndrome.

AIM: To investigate the potential value of ultrasonography in evaluating the pathophysiology of obstructive sleep apnoea-hypopnoea syndrome (OSAHS) by assessing the correlation of critical ultrasonic anatomical characteristics of the oropharynx with the severity of OSAHS. MATERIALS AND METHODS: One hundred and seventy-one patients with suspected OSAHS underwent oropharyngeal sonographic examination and overnight polysomnography. Ultrasonic measurement was compared with the apnoea-hypopnoea index (AHI) and other parameters. An ordinal logistic regression model was used to identify potential ul

Long-term intermittent hypoxia in mice induces inflammatory pathways implicated in sleep apnea and steatohepatitis in humans.

Obstructive sleep apnea (OSA) induces intermittent hypoxia (IH), an independent risk factor for non-alcoholic fatty liver disease (NAFLD). While the molecular links between IH and NAFLD progression are unclear, immune cell-driven inflammation plays a crucial role in NAFLD pathogenesis. Using lean mice exposed to long-term IH and a cohort of lean OSA patients (n = 71), we conducted comprehensive hepatic transcriptomics, lipidomics, and targeted serum proteomics. Significantly, we demonstrated that long-term IH alone can induce NASH molecular signatures found in human steatohepatitis transcripto

Sleep apnea and sleep-related breathing disorders in patients with craniofacial synostosis.

Seventeen patients with craniofacial synostosis (CFS) have been treated at the Department of Plastic and Reconstructive Surgery at Osaka Medical College during the past 10 years. Six patients were thoroughly evaluated at the Department of Otolaryngology by polysomnography (PSG), cephalometric X-ray and nasopharyngoscopy during sleep. In 4 of the patients PSG showed obstructive sleep apnea syndromes (OSAS). Heavy snoring without apnea and paradoxical respiration were noted in the other 2 patients. Thus, all of the patients had sleep-related breathing disorders (SRBD). It is assumed that the inc

Nightmares and oxygen desaturations: is sleep apnea related to heightened nightmare frequency?

In the 19th century, several authors held the view that nightmares are caused by oxygen shortage. The present study was designed to study nightmare frequency in patients with obstructive sleep apnea syndrome and its relationship to respiratory parameters. A brief questionnaire was administered to 323 patients with sleep apnea syndrome before their first laboratory night. The reduction in nightmare frequency in the sleep apnea group was explained by the reduced dream recall frequency. Despite some illustrative examples of a correlation between oxygen desaturation and dream content, the respirat

Trajectories of Emergent Central Sleep Apnea During CPAP Therapy.

BACKGROUND: The emergence of central sleep apnea (CSA) during positive airway pressure (PAP) therapy has been observed clinically in approximately 10% of obstructive sleep apnea titration studies. This study assessed a PAP database to investigate trajectories of treatment-emergent CSA during continuous PAP (CPAP) therapy. METHODS: U.S. telemonitoring device data were analyzed for the presence/absence of emergent CSA at baseline (week 1) and week 13. Defined groups were as follows: obstructive sleep apnea (average central apnea index [CAI] < 5/h in week 1, < 5/h in week 13); transient CSA (CAI

Acute right heart failure due to adenotonsillar hypertrophy.

A case is presented in which a child with underlying chronic lung disease, developed cor pulmonale and severe pulmonary hypertension as a result of adenotonsillar hypertrophy. His cardiac function acutely decompensated with an upper respiratory infection which exacerbated his obstructive sleep symptoms. Pre and postoperative documentation of cardiopulmonary function was critical in the peri-operative management of this patient. His severe pulmonary hypertension was stabilized using a nasopharyngeal airway and medications pre-operatively, in order to minimize his risk of anesthesia. He continue

Observations on sleep-disordered breathing in idiopathic Parkinson's disease.

BACKGROUND: This study has two main goals: 1.) to determine the potential influence of dopaminergic drugs on sleep-disordered breathing (SDB) in Parkinson's disease (PD) and 2.) to elucidate whether NREM and REM sleep differentially impact SDB severity in PD. METHODS: Retrospective clinical and polysomnographic study of 119 consecutive PD patients and comparison with age-, sex- and apnea-hypopnea-index-matched controls. RESULTS: SDB was diagnosed in 57 PD patients (48%). Apnea-hypopnea index was significantly higher in PD patients with central SDB predominance (n = 7; 39.3±16.7/h) than obstr

Midsagittal jaw movements as a sleep/wake marker.

The seriousness of the Obstructive Sleep Apnea/Hypopnea Syndrome is measured by the apnea-hypopnea index (AHI), the number of sleep apneas and hypopneas over the total sleep time (TST). Cardiorespiratory signals are used to detect respiratory events while the TST is usually assessed by the analysis of electroencephalogram traces in polysomnography (PSG) or wrist actigraphy trace in portable monitoring. This paper presents a sleep/wake automatic detector that relies on a wavelet-based complexity measure of the midsagittal jaw movement signal and multilayer perceptrons. In all, 63 recordings wer

Portable prescreening system for sleep apnea.

Obstructive sleep apnea (OSA) occurs in more than 4 % of the adult population. Diagnoses for OSA in sleep clinics are costly and more than half of those submitted to a sleep clinic do not have OSA. A simple, easy, and portable homebased monitoring system to evaluate who are in high-or low risk of suffering from OSA would be beneficial. The system must be able to identify individuals with a high pre-test reliability regarding OSA with the aim of referral and further investigation. We aimed to develop a portable, smartphone, and homebased monitoring system to classify whether a patient screened

Sleep apnea in REM sleep as an amplifier of vascular contributions to Alzheimer's disease.

Obstructive sleep apnea (OSA), a common though underdiagnosed disorder in older adults, represents a potential upstream vascular stressor linked to increased Alzheimer's disease (AD) risk. In this Perspective article, we propose a stage-informed framework in which rapid eye movement (REM) sleep-related respiratory events amplify vascular contributions to AD. This framework is unified across four linked lines of evidence suggesting that (1) REM sleep physiology magnifies the hypoxic and hemodynamic burden of OSA, (2) the cardiometabolic consequences of sleep apnea are heightened in REM sleep, (
