We report a patient with a syndrome resembling frontotemporal dementia (FTD); however, on further diagnostic testing, the diagnosis was Whipple's disease. Because Whipple's disease is treatable, it should be considered in the workup of patients with a FTD-like behavioural and cognitive syndrome.

Human faces look more similar to each other when they are presented upside down, leading to an increase in error rate and response time during individual face discrimination tasks. This face inversion effect (FIE) is one of the most robust findings in the face processing literature. Recent neuroimaging studies using adaptation to face identity have shown that the "fusiform face area" was the primary neural source of the behavioral FIE. However, the time course of the FIE, that is, when inversion affects the coding of facial identity in the human brain, remains unclear. Here, we addressed this

The connectivity between functionally distinct areas in the human brain is unknown because of the limitations posed by current postmortem anatomical labeling techniques. Diffusion tensor imaging (DTI) has previously been used to define large white matter tracts based on well-known anatomical landmarks in the living human brain. In the present study, we used DTI coupled with functional magnetic resonance imaging (fMRI) to assess neuronal connections between human striate and functionally defined extrastriate ventral cortical areas. Functional areas were identified with conventional fMRI mapping

There have been no systematic efforts to manage and treat patients with frontotemporal dementia (FTD), but Perry described pharmacologic interventions for some behavioral syndromes in 2001. In Perry's report, selective serotonin reuptake inhibitors (SSRI) were recommended as first choice drugs because they were well tolerated and might have an effect on some symptoms such as compulsive symptoms and eating abnormalities. Some reports were presented concerning Japanese FTD patients which showed the effect of SSRI on stereotyped behaviors and eating abnormalities by Nishikawa, et al. (2001), Iked

Recent models of visual recognition have suggested that perceptual analysis may start with a parallel extraction of different spatial frequencies (SF), using a preferential coarse-to-fine (low-to-high SF) sequence of processing. A rapid extraction of low spatial frequency (LSF) information may thus provide an initial and crude parsing of the visual scene, subsequently refined by slow but more detailed high spatial frequency (HSF) information. However, the sequence of SF analysis could be flexible, a high-to-low (HtL) being sometimes preferred to a low-to-high (LtH) SF sequence depending on tas

Congenital prosopagnosia is a severe impairment in face identification manifested from early childhood in the absence of any evident brain lesion. In this study, we used fMRI to compare the brain activity elicited by faces in a congenital prosopagnosic subject (YT) relative to a control group of 12 subjects in an attempt to shed more light on the nature of the brain mechanisms subserving face identification. The face-related activation pattern of YT in the ventral occipito-temporal cortex was similar to that observed in the control group on several parameters: anatomical location, activation p

OBJECTIVE: To analyze the neural bases of perception and the recognition of faces. First of all we consider the concept of functional hemisphere specialization; then we look at the results obtained with regard to the neuroanatomy of processing faces and finally refer to the disorders of recognition of faces in humans. For this we review the clinical evidence obtained from the neuropsychological studies of prosopagnostic patients and the data derived from psychophysiological experiments done using intracranial recordings of nonhuman primates.
DEVELOPMENT: The agreement between the results anal

Benign rolandic epilepsy (BRE) and childhood epilepsy with occipital paroxysms (CEOP) are overlapping in age range of presentation. Some children have been reported to manifest occipital and rolandic seizures, as distinct events. However, the presence during the same seizure of rolandic and occipital symptoms is exceptional. We present a 7-year-old boy with BRE in whom we video-EEG recorded two seizures at 4 years of age. The first episode was a classic rolandic seizure during sleep, while the second was prolonged with initial rolandic and late occipital involvement. It is possible that a rola

A brain-damaged patient is described whose pattern of performance provides insight into both the functional mechanisms and the neural structures involved in visual mental imagery. The patient became severely agnosic, alexic, achromatopsic and prosopagnosic following bilateral brain lesions in the temporo-occipital cortex. However, her mental imagery for the same visual entities that she could not perceive was perfectly preserved. This clear-cut dissociation held across all the major domains of high-level vision: object recognition, reading, colour and face processing. Our findings, together wi

OBJECTIVE AND IMPORTANCE: Chronic encapsulated intracerebral hematoma is a fairly rare clinical entity. This unique lesion is peculiar for its clinical onset, formation of a capsule, and progressive enlargement. The mechanism for capsule formation and self-perpetuating expansion still remains uncertain, as well as the causal relationship with vascular malformations. Because of their pseudotumoral course and misleading neuroradiological features, these lesions can be misdiagnosed preoperatively as cerebral neoplasms.
CLINICAL PRESENTATIONS: A 62-year-old man was referred with a 2-month history

Monkeys with inferior temporal cortex lesions cannot discriminate between different shapes (e.g., + vs. O) but can discriminate between shapes that differ only in orientation (e.g., 6 vs 9). Lesions of the parietal cortex, on the other hand, impair the discrimination of rotated shapes but spare the ability to discriminate between different shapes. A similar dissociation is found between some visual agnosics who can match but not identify rotated views of objects and other patients who can identify and discriminate objects only if the view is conventional; any change in orientation disrupts per

Previous independent EEG and PET studies suggest that administration of intravenous procaine hydrochloride selectively activates limbic brain structures. To further elucidate procaine's effects and explore the relationship between quantitative EEG (qEEG) and regional cerebral blood flow (rCBF), we simultaneously recorded qEEG and sampled rCBF using O-15 water PET in 20 healthy volunteers during single-blind injections of saline (baseline condition) followed by intravenous procaine (1.84 mg/kg). After thorough screening of EEG records, a subgroup of 7 subjects with EEG data relatively free of b

Residual or implicit knowledge has been observed in patients with object agnosia, optic aphasia and pure alexia. Previous investigators have considered implicit knowledge in these patients to be dissociated from awareness on the basis of intact semantic capabilities that are consistent with right hemisphere processing. The absence of explicit verbal identification is presumably dependent upon damaged left hemisphere systems. We describe a 72-year-old woman with a left occipital infarction, object agnosia and pure alexia who was unable to explicitly identify visual stimuli (objects and words),

While unilateral electroconvulsive therapy (U-ECT) is generally considered to induce negligible disturbances of memory as compared with bilateral bitemporal ECT, its relative antidepressant efficacy has been questioned. The aim of the present study was to compare, in a double-blind design, clinicians' ratings of global clinical impression of the antidepressant efficacy of the two treatment modalities. The treatment technique included avoidance of benzodiazepines, prolonged hyperventilation with oxygen, intermittent unidirectional pulses distributed in a long pulse train, permitting individuali

Two cases of pleomorphic xanthoastrocytoma (PXA) involving the temporal lobe are reported, both occurring in adolescents. This neoplasma is a recently described, rare variant of astrocytoma affecting the superficial cerebral cortex and meninges of children and young adults. The tumor is considered to arise from the subpial astrocytes of the superficial cortex. Electron microscopic examination and immunoperoxidase stains for glial fibrillary acidic protein (GFAP) are helpful in making a definitive histologic diagnosis. Despite its pleomorphic appearance, this type of supratentorial glioma appea…

A connectionist model of visual search is presented, in which search is determined by patterns of grouping between distractors and between the target and the distractors. Grouping is based on conjunctive relations between simple form elements (corners, line-end terminators), and uses principles of similarity and spatial proximity. In its normal, 'unlesioned' state, the model simulates the search data generated by human subjects when they search for simple form conjunctions amongst either homogeneous or heterogeneous distractors (Humphreys & Muller, in press). In this paper, the performance of

The authors present two cases of a rare coexistence of cerebral meningioma and glioma. In one case meningioma was situated closely to malignant glioma. In the second case in a patient operated on for cerebral glioma a meningioma was removed 20 years later. The meningioma was situated far from the first tumour.

Fifteen patients have been followed for more than one year following callosotomy having presented with long standing epilepsy, no well defined focus amenable to radical excision, and severely incapacitating atonic seizures that were refractory to anticonvulsant therapy. Atonic fits have been reduced by more than 80% in thirteen patients, with two patients suffering long term sequelae (slight dysarthria in one, and dyslexia with mild visuo-spatial disturbances in another). Anticonvulsant therapy was still required post-operatively.

A case of intermittent alexia, sometimes accompanied by severe dysgraphia and sometimes by mild dysgraphia, which had a probable migrainous origin, is described. On some occasions the patient could write to dictation, although with many errors, while unable to read words or letters. On other occasions the patient's writing to dictation was seriously disordered in terms of content and the letters were produced clumsily. Reading of numbers, colour vision and colour naming were normal although impairments on right-left orientation and visual short term memory were present and a mild finger agnosi

On the basis of the characteristic symptoms or the result of a speech examination, 127 right-handed cases with various types of aphasia were subdivided into two groups within each aphasic syndrome. Using a microcomputer, the locus and extent of the lesions, as demonstrated by computed tomography for each group were superimposed onto standardized matrices. The relationship between the focus and the extent of the lesions and the various symptoms was investigated. Broca aphasics: More than 80% of the group with obvious anarthric components had lesions of the third frontal gyrus involving Broca's
