Chronic graft-versus-host disease (cGVHD) is one of the most serious long-term complications of allogeneic haematopoietic stem cell transplantation (allo-HSCT), substantially impairing quality of life and increasing non-relapse mortality. This narrative review summarises current evidence on cellular therapies for cGVHD, focusing on mesenchymal stromal cells (MSCs) and regulatory T cells (Tregs), with emphasis on graft source differences, GVHD prevention and immune reconstitution, cell source, manufacturing, dosing, concomitant immunosuppression, and endpoint definition, alongside emerging appr

Donor cell-derived leukemia (DCL) is a rare complication of allogeneic hematopoietic cell transplantation (allo-HCT), previously described in patients receiving allo-HCT for hematologic malignancies. Allo-HCT has also been investigated as an immunotherapy platform for solid tumors including clear cell renal cell carcinoma (RCC). Herein, we describe a unique patient case treated by allo-HCT for RCC who subsequently developed DCL. A 40-year-old man was diagnosed with metastatic clear cell RCC treated surgically by left nephrectomy plus metastasectomy of a rib lesion. He was referred to the Fred

BACKGROUND: Liver transplantation from previously transplanted donors is an emerging yet rarely reported strategy to address global organ shortages. This case describes the clinical course and outcome of a liver transplant recipient who received an organ from a multiorgan donor recently submitted to pancreas-kidney transplantation.
CASE PRESENTATION: A 40-year-old woman with Budd-Chiari syndrome underwent orthotopic liver transplantation. The donor was a 24-year-old male with type 1 diabetes mellitus and end-stage renal disease who had received a pancreas-kidney transplant 10 days before suff

Organ transplantation is the last-resort option to treat organ failure. However, less than 10% of patients benefit from this only option due to lack of major histocompatibility complex (MHC)-matched donor organs and 25%-80% of donated organs could not find MHC-matched recipients. T cell allorecognition is the principal mechanism for allogeneic graft rejection. We herein present a "donor MHC-specific thymus vaccination" (DMTV) strategy to induce T cell tolerance to both autologous and allogeneic donor MHC. Allogeneic MHC molecules were expressed in the recipient thymus through adeno-associated

BACKGROUND: Autologous bone grafting is the standard treatment for the surgical management of atrophic nonunion of long bones. Other solutions, such as bone marrow mesenchymal stem cells (BM-MSC) combined with phospho-calcium material, have also been used. Here we evaluate the safety and early efficacy of a novel procedure using autologous or allogenic adipose tissue mesenchymal stromal cells (AT-MSC) seeded in a patented tricalcium phosphate-based biomaterial for the treatment of bone regeneration in cases of atrophic nonunion.
METHODS: This was a prospective, multicentric, open-label, phase

BACKGROUND: Allografts donated after cardiac death (DCD) are the fastest growing organ source worldwide. Unfortunately, information is lacking on how to judge these organs' viability. Here, we analyzed the effects of donor characteristics, including age and BMI, on outcomes of DCD simultaneous-pancreas-kidney transplantation (SPK).
METHODS: We evaluated UNOS DCD-SPK transplants from 1988 to 2012. Effects of donor characteristics on graft and recipient survival were evaluated using Cox Regression and the Kaplan-Meier method, and compared to predictions from the pancreas and kidney donor risk i

INTRODUCTION: Prior to transplantation, the transplant team is responsible for transplant education and posttransplant expectations. The majority of outcomes research focuses on 1- and 3-year graft survival, with a lack of literature focused upon whether patients have a realistic understanding of how many years deceased donor kidneys can be expected to function after transplant.
OBJECTIVE: To determine whether potential kidney transplant patients' expectations for how long a deceased donor kidney will function after transplantation differs from transplant surgeons, using quantitative analysis

Traditionally transplant nephrectomy was required as a separate procedure prior to retransplantation in simultaneous pancreas kidney transplant patients. An alternative approach combining both procedures is described, during which the rejected kidney is removed and replaced orthotopically by the new allograft.

BACKGROUND: Shortage of donor organs is one of the major problems for liver transplant programmes. Living liver donation is a possible alternative, which could increase the amount of donor organs available in the short term.
OBJECTIVE: To assess the attitude towards living organ donation in the general population to have an overview of the overall attitude within Germany.
METHODS: A representative quota of people was evaluated by a mail questionnaire (n = 250). This questionnaire had 24 questions assessing the willingness to be a living liver donor for different potential recipients. Factors

Specific immunomodulatory strategies are required to eliminate the need for lifelong dependence on debilitating immunosuppressants. One proposed strategy is to simultaneously transplant the kidney and infuse donor-specific bone marrow cells. We prospectively studied the effect of unmodified donor-specific bone marrow infusion (DSBMI) on rejection, infection, graft-versus-host disease (GvHD), and graft survival. We performed 57 kidney transplants in mixed lymphocyte culture (MLC)-reactive, outbred pigs. The groups of recipient pigs differed according to the use of (1) indefinite versus short-te

We carried out a retrospective analysis of 117 patients with severe combined immunodeficiency who were examined in a single center between Jan. 1, 1970, and Jan. 1, 1992, for the purpose of evaluating disease onset, progression, and outcome. The frequency of case referral increased from 8 from 1970 to 1975 to 56 from 1986 to 1991. The most frequent phenotype was T-/B+ (absence of T lymphocytes and presence of B lymphocytes) (n = 51); there were 36 cases of alymphocytosis, 16 of adenosine deaminase deficiency, 13 of Omenn syndrome, and 1 of reticular dysgenesis. The survival of semiallogeneic or fully allogeneic rat kidney allografts was prolonged in some strain combinations with immune complexes formed with hapten-conjugated alloantigens (TNP-Ag) of donor origin and a mouse monoclonal antihapten antibody (anti-TNP). Complexes were administered i.v. at the time of operation--and, in some cases, on subsequent days. Complexes using TNP-modified alloantigen in various forms--donor whole spleen lymphocytes, cellular membrane sonicate, or papain-solubilized-were all found to be effective in enhancing graft survival in specific donor-recipient combinations.

Despite current successes in modern cardiology and heart surgery, the problem of restoring and maintaining cardiac function in cases of sudden arrest or ventricular insufficiency is still urgent. Different methods of mechanically assisting the heart and replacing its pumping function exist, and direct mechanical cardiac massage (DMCM) is one of the more important. Experimental results with dogs and calves on 2- to 4-h DMCM, cardiac massage lasting for up to 48 h, subsequent restoration of cardiac activity, and cardiac massage during experimentally induced myocardial infarction permitted clinic

Aortic dissection (AD) is a life-threatening vascular disorder for which current mechanical interventions provide limited biological repair of the injured aortic wall. Here, we evaluated the short-term reparative potential of systemic endothelial progenitor cell (EPC) therapy in AD. We observed increased accumulation of CD34 + VEGFR2+ EPC-like cells in human AD tissues, particularly in regions of intimal injury, although these findings should be interpreted as an injury-associated tissue response rather than definitive evidence of endogenous protection. In a murine model of AD, transplanted EP

BACKGROUND: Mucormycosis associated with COVID-19 has been linked with higher mortality. Patients with risk factors such as immunosuppression, especially transplant recipients, are at heightened risk for these fungal related complications and poor clinical outcomes. Species like Rhizopus, Mucor, Rhizomucor and Lichtheimia are characterized by tissue necrosis, requiring prompt diagnosis and treatment to mitigate associated morbidity.

Immunodeficient large animals that allow engraftment of human tissues and cells are highly valuable for translational studies. Recently, a strain of immunodeficient pigs (termed RGD pigs) was developed through the genetic inactivation of RAG1, IL2RG, and CD47. These RGD pigs lack functional T, B, and NK cells and do not exhibit CD47-SIRPα incompatibility-triggered macrophage xenoresponses. Transplantation of human hematopoietic stem/progenitor cells (HSPCs) into RGD pigs results in high levels of durable human hematopoietic engraftment and systemic repopulation with multilineage human lymphohe

The aim of this study is to report a case in which a patient with nephrotic syndrome underwent surgery to remove fibrous gum tissue (ulectomy). An 8-year-old patient, diagnosed with early onset nephrotic syndrome due to a mutation in the NUP107 gene, had received a kidney transplant and was therefore taking various medications, including immunosuppressants. On oral examination, the patient was found to have a fibrous gingiva that was preventing the eruption of the upper permanent central incisors. A ulectomy was performed and the gingival tissue was sent for histopathological analysis, which s

BACKGROUND: Complex injuries of the hand frequently result in loss of essential functional features. Common reconstructive procedures for soft tissue defects of the thumb or phalanges are locoregional flaps like Moberg-, Foucher-, Cross-Finger- or Littler flaps. Microneurovascular toe (joint-) transfers complete the arsenal of operative reconstructive procedures and allow for most detailed reconstructions. Our experiences with free toe transplants are reported and diversely discussed regarding contending procedures.
METHODS: From 2010 until 2019 14 patients who received emergency or elective

This review focuses on the role of metalloproteinases in the pathogenesis of myocardial injury in various disease entities. It reveals how the expression and serum levels of metalloproteinases and their inhibitors change in many disease states. At the same time, the study offers a review of the impact of immunosuppressive treatment on this relationship. Modern immunosuppressive treatment is based mainly on the use of calcineurin inhibitors, including cyclosporine A and tacrolimus. The use of these drugs may carry a number of side effects, specifically to the cardiovascular system. The scale an

Detrusor hypocontractility (DH) is a disease without a gold standard treatment in traditional medicine. Therefore, there is a need to develop innovative therapies. The present report presents the case of a patient with DH who was transplanted with 2 x 106 adipose tissue-derived mesenchymal stem cells twice and achieved significant improvements in their quality of life. The results showed that cell therapy reduced the voiding residue from 1,800 mL to 800 mL, the maximum cystometric capacity from 800 to 550 mL, and bladder compliance from 77 to 36.6 mL/cmH2O. Cell therapy also increased the maxi
