Late complications in blood and marrow transplant survivors.

Hematopoietic stem-cell transplantation is a potentially curative therapy for many high-risk hematologic malignancies and other life-threatening disorders. Advances in transplantation over the last four decades have resulted in an increasing number of long-term survivors who may be at risk for developing late infections, noninfectious complications, and secondary cancers. Awareness on the part of primary care physicians of the risk factors and screening recommendations for late complications can lead to their early diagnosis and treatment.

Sirolimus for calcineurin inhibitors in organ transplantation: contra.

Sirolimus (SRL) is an antiproliferative agent inhibiting the mammalian target of rapamycin (mTOR) proposed as a non-nephrotoxic alternative to calcineurin inhibitors for the prevention of acute rejection in renal transplantation. Despite initial encouraging results, enthusiasm faded with large trials showing an increased risk of acute rejection with this molecule that did not provide superior graft function over cyclosporin or tacrolimus. Recent data showed that SRL, along with an immunosuppressive activity on CD4+ T cells, exerts a paradoxical stimulatory effect on innate immunity, which may

[Liver disease recurrence after liver transplantation].

Liver transplantation has become a standard option in the management of patients with end-stage liver disease. It is now evident that the most common etiology of long-term graft dysfunction is the recurrence of the primary liver disease. Autoimmune liver disorders such as autoimmune hepatitis, primary biliary cirrhosis, and primary sclerosing cholangitis recur between 15 to 30% of the graft recipients. The clinical expression of this recurrence tends to be milder; the diagnosis is only established in many patients by findings in the liver biopsy. This milder clinical expression may be due to t

Characterization and management of hypercalcemia following transplantation for osteopetrosis.

Autosomal recessive osteopetrosis (OP) is characterized by insufficient osteoclast activity resulting in defective bone resorption and marked increase in skeletal mass and density. OP has been successfully treated with hematopoietic cell transplantation (HCT), secondary to engraftment of donor-derived functioning osteoclasts resulting in remodeling of bone and establishment of normal hematopoiesis. Although hypercalcemia is a common presenting feature of OP, it may be observed following HCT due to engraftment of osteoclasts differentiated from the hematopoietic precursors. To characterize hype

ABO-incompatible renal transplantation in Epstein syndrome.

Epstein syndrome (ES) is an autosomal dominant hereditary disease characterized by hereditary nephritis, sensory deafness, and thrombocytopenia. We herein report the case of a 20-yr-old man with ES who underwent ABO blood type-incompatible living-donor kidney transplantation from his mother. He was given platelet transfusion, and his pre-operative number of platelets were 108 x 10(3)/microL. After transplantation, urine output and the decrease in serum creatinine (sCr) were within the acceptable ranges. On the seventh post-operative day (POD), sCr had risen and urine output decreased. Anti-typ

Outcome of pediatric renal transplantation in north India.

We report our experience and long-term outcome of pediatric renal transplantation at a referral center in New Delhi. During 1995-2008, 45 transplants were performed in 43 patients at a mean age of 13.3 ± 4.0 (range 3.8-18) yr. The chief causes for ESRD were reflux nephropathy, obstructive uropathy, vasculitis, renal dysplasia, and focal segmental glomerulosclerosis. Most (91.1%) donors were living related. Post-transplant immunosuppression comprised prednisolone, a calcineurin inhibitor and azathioprine or MMF. AR and CR were seen in 14 (31.1%) and 12 (26.7%) allografts, respectively. Predicto…

Cardiac transplantation in adults with congenital heart disease.

BACKGROUND: Due to increasing success with repair or palliation in childhood, there is a rapidly growing population of adult patients with complex congenital heart disease who may require transplantation. There remains little data on outcomes of cardiac transplantation in this group. METHODS: 38 orthotopic cardiac transplants were performed in 37 patients (18 men) > or =18 years of age with congenital heart disease (CHD) from 1988 to 2009 in our institution. Outcomes were reviewed using medical records and transplant databases. RESULTS: 15 patients (41%) had univentricular and 22 (59%) biven…

Update on extracorporeal photopheresis in heart and lung transplantation.

Transplant rejection of solid organs remains a threat to thousands of patients despite modern immunosuppressive regimens. The currently avai … ects. Extracorporeal photopheresis (ECP) has been shown to benefit especially patients with cardiac transplants, but also those who received lung allografts. ECP likely modulates the recipient's antigen-specific immune responses and inflammation in transplantation by in vivo generation of apoptotic leukocytes. This review will highlight the need for ECP, how …

[Amniotic membrane transplantation in herpetic corneal infections].

Severe infectious corneal ulceration such as herpetic stromal keratitis commonly causes loss of vision and may lead to blindness. Treatment depending on the underlying disease includes antimicrobial medication and the development of surgical strategies to restore the integrity of the corneal ocular surface. Ulcerative herpetic stromal keratitis and/or neurotrophic keratopathy with the risk of corneal perforation are still clinically challenging conditions in ophthalmic surgery of the ocular surface. Since the introduction of newly developed preservation methods, amniotic membrane (AM) function

Deflux injections for vesicoureteral reflux in transplanted kidneys.

INTRODUCTION: Vesicoureteral reflux (VUR), a complication after kidney transplantation, may be caused by recurrent urinary tract infections evaluating in life-threatening pyelonephritis and urosepsis. Open surgical correction is the standard treatment despite its morbidity. However, minimally invasive approaches are available. MATERIALS AND METHODS: Our study group describes seven patients with functioning kidney grafts and a diagnosis of VUR associated with recurrent urinary tract infections. The procedure was performed under antibiotic prophylaxis and spinal anesthesia. An endoscopic inject

Thrombotic microangiopathy in haematopoietic cell transplantation: an update.

Allogeneic hematopoietic cell transplantation (HCT) represents a vital procedure for patients with various hematologic conditions. Despite advances in the field, HCT carries significant morbidity and mortality. A rare but potentially devastating complication is transplantation-associated thrombotic microangiopathy (TA-TMA). In contrast to idiopathic TTP, whose etiology is attributed to deficient activity of ADAMTS13, (a member of the A Disintegrin And Metalloprotease with Thrombospondin 1 repeats family of metalloproteases), patients with TA-TMA have > 5% ADAMTS13 activity. Pathophysiologic me

[Bone quality evaluation in allograft hand transplant].

Five bilateral forearms allograft have been performed between January 2000 and July 2009 in Lyon (France). The first four patients (three males, one female) have been the subject of an assessment of the bone quality of those allografts. The techniques selected for this study were: radioclinical analysis, bone scintigraphy, MRI, bone densitometry and High Resolution peripheral Quantitative Computed Tomography (HR-PQCT). Histology has been performed only on the first patient unilaterally grafted in 1998 who did not take part in this clinical research protocol, after amputation of his rejected gr

Orthotopic heart transplantation in a child with hereditary spherocytosis.

Hereditary spherocytosis (HS) is a genetic, frequently familial hemolytic blood disease that presents with varying degrees of hemolytic anemia, splenomegaly, and jaundice. The disease arises as a result of defects in any of a number of proteins responsible for maintaining the shape and flexibility of the red blood cell, resulting in an osmotically fragile and characteristically spherical red blood cell. Theoretically, cardiopulmonary bypass can exacerbate hemolysis and subsequent renal dysfunction. There are few reports of open heart surgery for patients with HS and none for orthotopic heart tra

[Chronic respiratory insufficiency: the role of lung transplantation].

Lung transplantation has been established as an appropriate ultimate treatment strategy in end-stage lung disease, when all conventional therapeutic options have been exhausted. Acute and potentially reversible lung diseases (ARDS, pneumonia) and malignancy (broncho-alveolar carcinoma) are not established indications. Retrospective analyses on the natural course of the disease and new medical treatment options have led to fewer transplantations in recipients with emphysema and idiopathic pulmonary hypertension. In end-stage cystic fibrosis and patients with idiopathic pulmonary fibrosis, alter

Hepatitis B virus infection and liver transplantation.

PURPOSE OF REVIEW: Infection with hepatitis B virus (HBV) continues to be a major global cause of acute and chronic liver disease despite the existence of successful vaccine and antiviral therapies. As such, HBV-related liver disease remains one of the most common indications for liver transplantation. An understanding of the pathophysiology of hepatitis B viral infection including the potential sequelae of acute-liver failure, cirrhosis, and hepatocellular carcinoma (HCC) is important to the successful treatment of the patient. RECENT FINDINGS: A better understanding of the differential immu
