Most vesical neck contractures occur after resection of adenomas weighing less than 20 gm. This complication is believed to be secondary to excessive resection or fulguration of an undilated bladder neck. Prophylactic bladder neck incision was performed in conjunction with transurethral resection of the prostate on 114 patients with prostatic adenomas weighing less than 20 gm. In an 11-year period, 49 patients developed bladder neck contracture after prostatectomy--an incidence of 0.86%. This complication was found to be more common after resection of small fibrous hyperplastic prostates. The best results for treatment of the contracture were obtained after bladder neck incision, which gave a 9% incidence of recurrence; after transurethral resection of the bladder neck contracture the recurrence rate was 46%, and after bladder neck dilatation it rose to 100%.

Sixty-five patients had a cystectomy for a solitary bladder tumour (28) or multifocal lesions (37). The histological extent of the disease within the bladder was correlated with mucosal abnormalities within the urethra. The presence of multifocal or solitary bladder disease was not an accurate predictor of the presence or absence of urethral dysplasia. Details of mortality and morbidity associated with one-stage cysto-urethrectomy are presented and it is concluded that irrespective of whether the bladder contains a solitary or multifocal tumour, complete cysto-urethrectomy should be considered

Thirty-two male patients with mechanical or functional bladder outlet obstruction were treated by bilateral endoscopic bladder neck incision. In three cases a repeat incision was necessary. Subjective cure or improvement was obtained in 81%. Objective evaluation, including uroflowmetry and residual urine measurements, showed a similar improvement. During transurethral electroresection in the posterolateral bladder neck, trigone and posterior urethra, unintentional contractions of the thigh-adductor muscles may occur due to irritation of the obturator nerve. The sudden displacement of the bladder wall against the cutting loop may cause a perforation of the bladder. The authors describe the topographic relation of the bladder wall to the passage of the obturator nerve in the minor pelvis. The technique of obturator nerve block by local anaesthesia is described and its efficacy is demonstrated in 21 patients.

High flow infravesical obstruction in male patients with severe prostatism is defined by a maximum flow rate of more than 15 ml. per second and a vesical pressure exceeding 100 cm. water at maximum flow rate. During a 9-month period urodynamic screening of 225 patients referred to our hospital with prostatism revealed high flow infravesical obstruction in 16 (7 per cent). Transurethral intravesical scanning is useful in staging bladder tumors. A new ultrasonic scanner was developed to obtain a real-time image. Diameter of the probe sheath was reduced to 24F. The resolution of the tomogram was improved by using 7 MHz, ultrasound, which clearly displayed the ureteral penetration of the bladder wall. It was demonstrated that the real-time scanner was effective not only in staging bladder cancer but also in the followup of tumor volume, in monitoring transurethral resection of the tumor and in the observation of tumor movement effected by the finger.

The authors describe nine cases of pseudo-ureterocele secondary to various obstructive lesions: bladder tumors, stones in the ureteral meatus, tuberculosis of the bladder. Certain radiological signs, such as asymmetrical swelling of the terminal ureter and the thick halo which surrounds the dilatation, can sometimes distinguish pseudo-ureteroceles from idiopathic ureteroceles, and associated radiological anomalies in the bladder are also of greater value for differentiation.

An assessment of 139 cases of outlet obstruction following spinal cord injury, in a consecutive series of 471 patients, is presented. The figures in relation to the level and extent of the spinal lesion and the type of operation (transurethral resection and external sphincterotomy), are reviewed. The incidence of repeat operation is recorded and assessed. It is concluded that transurethral resection and external sphincterotomy, either alone or combined, will relieve outlet obstruction in most cases of neuropathic bladder following spinal cord injury.

Adjuvant therapy for vesical neck contractures with triamcinolone injection has given good results with long-term followup but similar results have been obtained with transurethral resection alone in our current re-evaluation.
