[Massive alveolar haemorrhage in Wegener's granulomatosis].

Wegener's granulomatosis is a systemic vasculitis with involvement of primary granulomatous upper and lower respiratory tract, glomerulonephritis and vasculitis of small vessels. The lung disease ranges from asymptomatic pulmonary nodules to pulmonary infiltrates and fulminant alveolar haemorrhage. The prognosis is poor due to kidney and respiratory failure, although the data are changing due to new treatments with glucocorticoids and cyclophosphamide. We report a case with severe lung disease, which after appropriate anamnesis, multiple tests, and optimal sequential action, the patient was di

[Diagnostic value of antineutophil cytoplasmic antibodies].

Antineutrophil cytoplasmic antibodies (ANCA) constitute a family of auto-antibodies directed against various components of the neutrophil cytoplasm. The indirect immunofluorecence assays detected three fluorescent staining patterns: cANCA--cytoplasmic; pANCA--perinuclear and aANCA--atypical. Occurence ANCA is mainly associated with Wegener's granulomatosis and vasculitis, but they are also detected in autoimmune diseases (eg. in systemic lupus erythematosus, in rheumathoid arthritis, Sjögren's syndrome, in dermatomyositis) and in inflamatory bowel diseases (Crohn disease, colitis ulcerosa). Atypical eosinophilic angiocentric fibrosis on nasal septum.

Eosinophilic angiocentric fibrosis (EAF) is a rare benign condition of unknown aetiology and is most commonly found in the nasal septum and sinus mucosa. We report a case of EAF and present a review of the available literature. A 51-year-old man with progressive nasal obstruction was referred to our hospital. CT and MRI scans revealed a mass on the nasal septum; this was surgically excised. Histological analysis of the resected tumour showed an inflammatory infiltrate with a predominance of eosinophils present in fibrous matrix and absence of eosinophilic vasculitis and onion skinning. Histolo

[Comparison between immunofluorescence techniques and ELISA, using whole neutrophil extract and primary granules, for the detection of antineutrophil cytoplasma antibodies (ANCA)].

The detection of antineutrophil cytoplasmic antibodies is a very important tool for the diagnosis of systemic vasculitis. The specificity and sensitivity of these antibodies depends on the assay utilized for their detection. Therefore we have compared the immunofluorescence test (IF) with the ELISA using two different antigens: total neutrophil extract and isolated primary granules. Two patterns of fluorescence were detected by IF: the classic pattern was highly specific for Wegener's granulomatosis. In contrast the perinuclear staining correlated with renal vasculitis but was also observed in

Systemic corticosteroids are important in the treatment of Fournier's gangrene: a case report.

Fournier's gangrene represents an acute severe necrotizing inflammatory process affecting the scrotum and penis. It has an associated mortality of 30-50%. In most cases, aetiological factors can be identified, such as diabetes mellitus, chronic alcoholism and perianal, perirectal or periurethral infection. The disease is characterized by a polybacterial infection, and the classic treatment includes surgical removal of the necrotic tissue and the use of broad-spectrum antibiotics. We report a case of Fournier's gangrene, histologically characterized by a necrotizing vasculitis, in which surgica

Influence of immunoadsorption on the removal of immunoglobulin G autoantibodies in crescentic glomerulonephritis.

We report the treatment of 5 patients with crescentic glomerulonephritis by immunoadsorption using a protein A column. Two had systemic vasculitis, 2 antiglomerular basement membrane disease and 1 systemic lupus erythematosus (SLE). In the patients with systemic vasculitis and SLE, there was successful removal of autoantibodies and rapid control of disease; remission was maintained over a mean follow-up of 2 years. Clinical improvement was not seen in 2 patients with anti-glomerular basement membrane (GBM) disease who presented with dialysis-dependent renal failure. There were no apparent clin

Methotrexate therapy in connective tissue diseases: a review of the literature.

Low dose methotrexate (MTX) therapy is now widely used with considerable success in refractory rheumatoid arthritis. Experience with the drug in other connective tissue diseases and vasculitis has been limited. This article reviews hypotheses about the mode of action of MTX, pharmacology and pharmacokinetics, reasons to use cytotoxic agents in the different diseases, and results of MTX therapy. Risk factors, adverse reactions and drug interactions of MTX with other drugs are mentioned. Although based on a small number of patients, data about remarkable clinical improvement in polymyositis, sys

Burned-Out Autoimmune Disease: A Multicenter Case Series Revealing a Distinct Clinical Phase and Its Implications for Immunosuppressive Therapy.

Background and objective Distinguishing active inflammation from irreversible organ damage in autoimmune diseases remains a major clinical challenge and may result in the inappropriate continuation of immunosuppressive therapy. The concept of "burned-out" autoimmune disease, in which inflammatory activity subsides following irreversible tissue damage, remains underrecognized. This study aimed to describe the burned-out phase across multiple autoimmune diseases and to propose a cross-disease clinical framework to differentiate active inflammation from irreversible damage. Methods We conducted a

Residential altitude, aerosol optical depth, and diagnostic composition of biopsy-confirmed renal diseases: a single-center renal biopsy study from Yunnan, China.

BACKGROUND: Residential altitude may influence human physiology and reflect broader environmental gradients, but its relationship with the diagnostic composition of biopsy-confirmed renal diseases remains unclear. METHODS: We retrospectively studied patients undergoing native kidney biopsy at The First People's Hospital of Yunnan Province, China, between February 2023 and February 2026. Residential altitude was derived from geocoded permanent addresses and analyzed using altitude quartiles and continuous models per 500-m increase. Restricted cubic spline analyses were performed as exploratory

A rare case report of polyangiitis overlap syndrome: granulomatosis with polyangiitis and eosinophilic granulomatosis with polyangiitis.

BACKGROUND: Granulomatosis with polyangiitis (GPA) is a systemic ANCA-associated vasculitis characterized by necrotizing granulomatous inflammation and a predilection for the upper and lower respiratory tract. Eosinophilic granulomatosis with polyangiitis (EGPA) is also a systemic ANCA-associated vasculitis, but EGPA is characterized by eosinophilic as well as granulomatous inflammation and is more commonly associated with asthma and eosinophilia. Polyangiitis overlap syndrome is defined as systemic vasculitis that does not fit precisely into a single category of classical vasculitis classific

A clinical, electrophysiological, and pathological study of neuropathy in rheumatoid arthritis.

Neuropathy in rheumatoid arthritis (RA) may result secondary to entrapment, vasculitis, and drug toxicity. We aimed to study clinical and electrophysiological neuropathy and pathological changes in sural nerve in patients with RA. One hundred eight patients of RA, fulfilling American College of Rheumatology 1987 criteria (mean age, 45.83 years; M/F 1:3, 80.3% seropositive) were examined clinically and electrophysiologically for evidence of peripheral neuropathy. Sural nerve biopsies were performed in the involved cases. In all RA patient medications, disease activity, results of blood tests, a

Identification and characterization of the carboxy-terminal region of Sip-1, a novel autoantigen in Behçet's disease.

Given the lack of a serological test specific for Behçet's disease, its diagnosis rests upon clinical criteria. The clinical diagnosis is nevertheless difficult because the disease manifestations vary widely, especially at the onset of disease. The aim of this study was to identify molecules specifically recognized by serum autoantibodies in patients with Behçet's disease and to evaluate their diagnostic value. We screened a cDNA library from human microvascular endothelial cells with serum IgG from two patients with Behçet's disease and isolated a reactive clone specific to the carboxy-termin

Cutaneous histopathology of Rocky Mountain spotted fever.

The dermatologic diagnosis of Rocky Mountain spotted fever (RMSF) is often presumptive; the clinical presentation includes skin rash and febrile illness with or without a clear history of tick bite. The characteristic cutaneous manifestations include a generalized skin eruption with purpuric, blanching or non-blanching macules and papules usually involving the extremities. Although skin biopsies are often performed to confirm the diagnosis, the spectrum of cutaneous histopathology in RMSF has not been well described. We studied a series of 26 cases of RMSF, of which 10 were surgical specimens

A Case Report of Childhood Polyarteritis Nodosa: A Rare Vasculitic Disorder with Multisystem Involvement.

BACKGROUND: Childhood polyarteritis nodosa (cPAN) is a rare systemic necrotizing vasculitis affecting small- and medium-sized arteries. Its heterogeneous clinical manifestations often delay diagnosis, yet early recognition is essential to prevent organ damage. According to the EULAR/PRINTO/PRES criteria, diagnosis requires evidence of necrotizing vasculitis in medium- or small-sized arteries or characteristic clinical features such as skin involvement, myalgia, hypertension, or peripheral neuropathy. This case adds to the limited literature by presenting a severe multisystem form of cPAN in a
