Parental reports of symptoms of childhood disorders in Dutch children.

The number of children displaying childhood disorders in the Netherlands is estimated through a questionnaire referencing DSM-IV symptoms filled out by the parents of 2,563 4- 18-year-old Dutch children randomly taken from the general Dutch Youth population in 2004. The number of impaired children was estimated by applying the DSM-IV criteria for impairment by symptoms. It was found that 14% of all Dutch children display developmental difficulties, with 8% at risk of moderate behavioral and emotional difficulties and 6% at risk of a childhood psychiatric disorder. These prevalence figures base

Dissociation predicts later attention problems in sexually abused children.

OBJECTIVE: The goals of this research are to develop and test a prospective model of attention problems in sexually abused children that includes fixed variables (e.g., gender), trauma, and disclosure-related pathways. METHODS: At Time 1, fixed variables, trauma variables, and stress reactions upon disclosure were assessed in 156 children aged 8-13 years. At the Time 2 follow-up (8-36 months following the initial interview), 56 of the children were assessed for attention problems. RESULTS: A path analysis involving a series of hierarchically nested, ordinary least squares multiple regression

Environmental stressors and violence: lead and polychlorinated biphenyls.

Many environmental risk factors for antisocial and violent behavior have been described. In recent years, this topic has become widely researched in the fields of environmental health, psychology, sociology, and many other disciplines. The results from a myriad of studies have shown that the etiologies of violent and aggressive behavior range from definitive biological environmental stressors like lead or polychlorinated biphenyls to various socio-cultural environmental stressors, such as social, economic, and racial factors. The aim of this paper is (a) to provide an overview of the specific

Depressed preschoolers with bipolar family history: a group at high risk for later switching to mania?

Earlier age of onset of an episode of depression and family history of bipolar disorder (FHBPD) are well known to be associated with increased rates of switching to mania in childhood major depressive disorder (MDD). These findings suggest that the youngest samples of depressed children who have FHBPD might be at very high risk for switching. The finding of a valid depressive syndrome in preschool children has raised the question of whether mania could also manifest at this early stage. We investigated FHBPD among three preschool study groups: a depressed group and two nondepressed comparison

The disruptive behavioral disorder in children and adolescents: comorbidity and clinical course.

Comorbidity, time to recovery, rate of chronicity, and probability of recurrence following recovery were studied in 51 children diagnosed with attention deficit disorder, conduct disorder, and oppositional defiant disorder. Thirty-three percent of the children had two of the above diagnoses, and one child had all three diagnoses. The mean duration of attention deficit disorder was 8 years up to the time of the interview; the mean duration of oppositional defiant disorder was 4.5 years, and the mean duration of conduct disorder was 3 years. Life-table estimates showed that 14% of the children w

Focal refractory epilepsy associated to 15q13.3 microduplication.

A young man in his early 20s, with a maternal history of epilepsy and psychiatric disorders, presents with drug-resistant epilepsy (DRE). His seizures began at age 5 following mild head trauma and have since progressed to frequent generalised and focal seizures. Despite treatment with carbamazepine, levetiracetam and gabapentin, seizure control remains elusive.Genetic testing reveals a 440.5 Kb microduplication at 15q13.3 involving the CHRNA7 gene. Brain MRI shows non-specific gliosis in the left frontal subcortical region. This finding aligns with previous associations of 15q13.3 copy number

Neurodevelopmental disabilities in children with intermediate and premutation range fragile X cytosine-guanine-guanine expansions.

To determine the range of neurodevelopmental diagnoses associated with intermediate (45-54 repeats) and premutation (55-200 repeats) range cytosine-guanine-guanine fragile X expansions, the medical records of children with intermediate or premutation range expansions were retrospectively reviewed, and all neurodevelopmental diagnoses were abstracted. Twenty-nine children (9 female, 20 male; age, 13 months to 17 years) with intermediate (n = 25) or premutation (n = 4) range expansions were identified with neurodevelopmental diagnoses, including global developmental delay/intellectual disability

The effects of methylphenidate on the classroom behavior of elementary school-age children with cerebral palsy: a preliminary observational analysis.

High- and low-dose methylphenidate administration was evaluated prospectively for 3 elementary school-age children with cerebral palsy, cognitive impairments, and attention-deficit hyperactivity disorder (ADHD) symptoms using single-case, randomized, double-blind, placebo-controlled designs. An observational time sampling protocol was used to directly measure and quantify classroom behavior. Summary level analysis showed that (1) low-dose (0.3 mg/kg/dose) administration was associated with clinically significant (>50%) reductions in stereotyped and disruptive behavior relative to baseline and

Predicting anxiety diagnoses with the Youth Self-Report.

Empirical studies that assess which items of the Youth Self-Report (YSR) are the best predictors of anxiety disorders in adolescents are lacking, whereas several attempts have been made to construct an anxiety scale for the YSR. It is important to gap the bridge between existing YSR and DSM-IV diagnostic constructs. My objective in this study was to investigate which YSR items or scales can be used best to predict anxiety disorders in adolescents. In a referred 183-subject sample of 11- to 18-year-olds, we examined which YSR items and scales predicted DSM-IV anxiety diagnoses assessed with the

Systemic hydroa vacciniforme lymphoproliferative disorder in a patient with chronic active EBV infection.

Hydroa vacciniforme lymphoproliferative disorders (HVLPD) fall within the clinical spectrum of chronic active epstein barr virus (EBV) disease (CAEBVD), ranging from localised and/or indolent forms (classic HVLPD) to systemic disease with fever, hepatosplenomegaly and lymphadenopathy (systemic HVLPD). A preadolescent male with 47XYY, multicystic dysplastic kidney, autism spectrum disorder and Attention-deficit/hyperactivity disorder (ADHD) presented with photodistributed non-pruritic, non-painful necrotic papulovesicles accompanied by non-febrile intermittent fatigue and lymphadenopathy. Increased adrenal androgen functioning in children with oppositional defiant disorder: a comparison with psychiatric and normal controls.

OBJECTIVE: To examine the relationship between adrenal androgens and aggression in children with oppositional and antisocial behavior and to compare their levels with those of psychiatric and normal controls. METHOD: Dehydroepiandrosterone sulfate (DHEAS) was measured in 24 children with oppositional defiant disorder (ODD), 42 psychiatric controls (including 20 children with attention-deficit/hyperactivity disorder [ADHD]), and 30 normal controls. The children's parents filled out the Child Behavior Checklist (CBCL). RESULTS: Children with ODD had higher DHEAS levels than either the psychiat

Assessing Condition-Specific Adverse Event Profiles of Modafinil for Labelled and Off-Label Uses: A Systematic Review and Meta-Analysis.

Modafinil is approved for excessive daytime sleepiness in narcolepsy, obstructive sleep apnoea (OSA), and shift work sleep disorder (SWSD), but its widespread off-label use raises safety concerns. We evaluated the risk of adverse events (AEs) associated with both labelled and off-label use of modafinil. A systematic search of PubMed, Embase, and Cochrane identified 54 studies that met the inclusion criteria. In labelled uses, narcolepsy patients had significantly elevated risks of diarrhoea (risk ratio [RR]: 2.16, 95% confidence interval [CI]: 1.06-4.41) and nausea compared to those with place

Tics in autism spectrum and in intellectual disability.

Tourette syndrome (TS) is a neuropsychiatric and a neurodevelopmental disorder often presenting alongside other neurodevelopmental conditions such as attention deficit/hyperactivity disorder (ADHD), autism spectrum disorder (ASD), learning difficulties, and dyspraxia. TS shares genetic and neurobiologic features with ASD and ADHD, suggesting common underlying mechanisms. Recent diagnostic frameworks such as the DSM-5 (Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition, APA, 2013) allow for multiple co-occurring diagnoses, reflecting the complex overlap among neurodevelopmenta

Lidocaine Ineffectiveness Suggests New Psychopharmacology Drug Target.

OBJECTIVES: The mechanism of many neuropsychiatric disorders remains unknown, but the ineffectiveness of the sodium channel blocker lidocaine has been suggested to be a biomarker for Attention Deficit Hyperactivity Disorder (ADHD) and a severe form of Premenstrual Syndrome (PMS) that is considered psychiatric. We conducted single-arm double-blind clinical trials to test whether lidocaine ineffectiveness can be used as a biomarker to identify people with these conditions and provide a clue as to the molecular mechanism and potential psychopharmacological intervention. EXPERIMENTAL DESIGN: We d
