Drug use among inner-city African American women: the process of managing loss.

The grounded theory study described in this article investigated illicit drug use in the lives of 32 drug-using women living in two inner-city neighborhoods of a large metropolitan U.S. city. The underlying purpose was to describe the process of how life situations and events influenced the onset of drug use and changes in drug-using behaviors. Analysis of in-depth interviews revealed several themes. The basic social process, managing loss, was identified. Painful feelings of loss resulted from the separation of someone or something from the lives of participants and included death or desertio

Fracture-dislocation of the shoulder in a 32-month-old child.

A case of fracture-separation and dislocation of the proximal humeral epiphysis, a rare injury, is reported. The patient, a 32-month-old girl suspected of being an abused child, was reported to have fallen from her crib 2 days earlier. She had a painful, swollen, slightly ecchymotic left shoulder, which she held in the neutral position and refused to move. Radiographs showed a healing, nondisplaced radial fracture and fracture-separation of the proximal humeral epiphysis, which was dislocated into an anterior subglenoid location. Surgery under general anesthesia showed the humeral head to lie

Developmental transitions in presentations of externalizing problems among boys and girls at risk for child maltreatment.

The present study examined the impact of children's maltreatment experiences on the emergence of externalizing problem presentations among children during different developmental periods. The sample included 788 youth and their caregivers who participated in a multisite, prospective study of youth at-risk for maltreatment. Externalizing problems were assessed at ages 4, 8, and 12, and symptoms and diagnoses of attention-deficit/hyperactivity disorder, oppositional defiant disorder, and conduct disorder were assessed at age 14, during interviews with youth and caregivers. Information about malt

The association between childhood maltreatment and neural functioning during a working memory task in adults with internalizing disorders.

Childhood maltreatment is pervasive – with an estimated 1 in 4 children experiencing abuse or neglect at some point in their lifetime. Exposure to childhood maltreatment has been associated with detrimental neurodevelopmental changes to brain structure and function in stress-sensitive regions and regions with extended developmental periods. Such regions include the frontoparietal regions (e.g., middle frontal gyrus and dorsolateral prefrontal cortex), which are vital for cognitive processes, including working memory; however, few studies have investigated how childhood maltreatment impacts the

Excess costs of post-traumatic stress disorder related to child maltreatment in Germany.

BACKGROUND: Childhood maltreatment (CM) significantly increases the risk of developing post-traumatic stress disorder (PTSD) for which the prevalence in Europe is higher than initially assumed. While the high economic burden of PTSD is well-documented, little is known about the health care cost differences between individuals with PTSD-CM and those without PTSD in Germany. This study aimed to determine the excess health care and absenteeism costs associated with PTSD-CM in Germany. METHODS: Baseline data from a multi-center randomized controlled trial on individuals with PTSD-CM (n = 361) wer

Brain network controllability in genetic risk, childhood abuse, and adult anxiety.

Background: Network control theory can quantify controllability to evaluate how altered transitions between brain states contribute to cognitive, emotional, and behavioural challenges. Childhood abuse, influenced by genetics, is associated with disrupted network function, though the exact control processes are not yet understood. Objective: This study aims to investigate the association between brain network controllability and childhood abuse experiences, and to elucidate its potential mediating role in the relationship among polygenic risk scores (PRS) for childhood abuse, childhood abuse exp

Beliefs about inaccessible memories of childhood sexual abuse: a survey of laypeople and professionals.

Evidence indicates that perceived event plausibility influences the likelihood of false memory implantation. Accordingly, it is central to map out the beliefs that people hold regarding the plausibility of different events. In three studies, we examined beliefs concerning the plausibility of being unable to remember childhood sexual abuse by use of two vignettes. This was investigated in Danish (Study 1; N = 94) and American laypeople (Study 2; N = 303) and Danish professionals across six different groups of potential relevance to the legal system (Study 3; N = 335). Most participants in Studi

Psychosocial Correlates of PTSD and Problem Drinking in Sexual Assault Survivors and Their Informal Supports.

The current study examined predictors PTSD and problem drinking using survey data (N = 432 matched pairs) from sexual assault survivors and informal support providers (SPs, e.g. family, friend, partner) to whom they disclosed. Perceptions of helpfulness, relationship satisfaction, social reactions to disclosure of sexual assault, coping strategies, and child sexual abuse (CSA) severity were examined in separate survivor and SP regression models. In the survivor model, White-identified and less educated survivors had more PTSD symptoms than those of other races and with more education, respecti

Inflammation following childhood maltreatment is associated with episodic memory decline in older adults.

Childhood maltreatment is recognized as a risk factor for cognitive decline in adulthood. However, the mechanisms underlying this association, particularly the role of systemic inflammation, remain understudied. To address this gap, this study investigated the indirect effects of inflammation on the associations between childhood maltreatment and both episodic memory (EM) and executive functioning (EF) performance 10 years after inflammatory measurement in older adults. We selected 590 participants (Mage = 65.5 years) from the Midlife in the United States Study based on available childhood mal

Correlates of resilience after childhood sexual abuse among men who have sex with men.

Men who have sex with men (MSM) are at high risk for physical and mental health conditions and are often discussed in the health literature as "at risk" versus "at promise". However, there is an ongoing need to examine factors that place MSM "at promise" for optimal well-being. This manuscript examines correlates of resilience, the ability to "bounce back" and function adaptively after adversities, among MSM. One hundred and five MSM with a history of childhood sexual abuse, who were enrolled in a randomized control trial were recruited for a supplemental study assessing resilience and other p

"Neglected moms" - The implications of emotional neglect in childhood for the transition to motherhood.

BACKGROUND: The transition to motherhood involves many challenges that require adjustment; included among them are adapting to body changes, forging a maternal identity, and attaching to the baby. Although these tasks may not be easy for any women, those who experienced emotional neglect during childhood may find them especially difficult. OBJECTIVE: The aim of the current study was to examine a model illuminating the mechanism underlying the association between childhood emotional neglect and women's adjustment during pregnancy and the postpartum period. The protective role of mentalizing: Reflective functioning as a mediator between child maltreatment, psychopathology and parental attitude in expecting parents.

BACKGROUND: Childhood maltreatment impacts parenting and has intergenerational consequences. It is therefore crucial to identify clinically responsive resilience-promoting factors in pregnant women and expecting men with history of childhood maltreatment. Mentalization, or reflective functioning, appears as a promising concept to understand risk and resilience in the face of childhood maltreatment. OBJECTIVE: This study evaluated the multivariate relationship between exposure to childhood maltreatment, reflective functioning, psychological symptoms and parental attitude in expecting parents.

Childhood emotional and sexual maltreatment moderate the relation of the serotonin transporter gene to stress generation.

Emerging evidence suggests that the tendency to generate stressful life events may be, at least in part, genetically determined. However, the role of the early environment in shaping responses to later stressors is crucial to fully specifying biogenetic models of stress generation. The current study examined the moderating role of childhood emotional, physical, and sexual maltreatment on the relation of the serotonin-transporter-linked promoter region (5-HTTLPR) polymorphism of the serotonin transporter gene to proximal independent, dependent, and dependent-interpersonal life events. This ques

Intimate partner violence.

Intimate partner violence (IPV) is a common problem, affecting large numbers of women, men, and children who present to primary care practices. It takes on many forms, including psychologic/emotional, physical, and sexual abuse, and its effects on the health of victims and their children are varied. Although many primary care physicians may be uncomfortable inquiring about IPV, a knowledge of patients' IPV victimization may help physicians develop a better understanding of patients' presenting symptoms and health risks, form more effective therapeutic relationships, and work toward reducing th

Alexithymia and childhood abuse among patients attending primary and psychiatric care: results of the RADEP Study.

BACKGROUND: Some authors have suggested that the background of alexithymia is related to affective development during early childhood. OBJECTIVE: Authors studied the associations between alexithymia and childhood emotional neglect and sexual and physical abuse, also taking into account the significance of concomitant psychopathology. METHOD: Associations between childhood difficulties and adulthood alexithymia were studied with samples of primary-care (N=1,033) and mental-health center (N=243) outpatients assessed by The Toronto Alexithymia Scale and questionnaires for depressive, manic, and

Impact of a history of physical and sexual abuse in eating disordered and asymptomatic subjects.

The present study aimed to explore the impact of sexual and/or physical abuse among eating disordered patients (ED) and asymptomatic subjects. A total of 86 patients with anorexia nervosa, 69 patients with bulimia nervosa and 81 asymptomatic subjects were assessed. Among ED, we did not find a significant association between abuse experiences and the severity of the eating disorder, or between abuse and dissociative symptoms. Among ED, self-destructive behaviour appears to be the most important predictor of a history of sexual and/or physical abuse. In contrast, in the asymptomatic group, the s

History of trauma and attempted suicide among women in a primary care setting.

The results of past research have demonstrated apparent links between suicidality and a history of sexual abuse or physical abuse. However, the relative predictive power of such abuse histories in explaining sucidality remains unknown, as does the potential relationship between sucidality and emotional abuse, physical neglect, and witnessing violence. In the current study, 151 women who presented for nonemergent medical care indicated whether they had experienced each of five types of abuse and whether they had ever attempted suicide. Similar to past research, increased rates of having attempt
