We have extended automated sequential trace enrichment of dialysates and high performance liquid chromatography to include the simultaneous assay of lamotrigine with other anticonvulsant drugs: phenobarbitone, carbamazepine and phenytoin. The procedure is reliable and precise (between-batch coefficients of variation < 2.8%) with no interference from other commonly prescribed drugs. Patients on lamotrigine presented with serum concentrations up to 67 mumol/L with no adverse effects, suggesting that the therapeutic range may need re-appraisal.

A method for measuring tryptophan hydroxylase activity by assaying the product 5-HTP using high-performance liquid chromatography (HPLC) with electrochemical detection is described. A nocturnal elevation (80%) in rat pineal gland tryptophan hydroxylase activity was detected. Experiments on isolated rat pinealocytes with the protein synthesis inhibitor cycloheximide indicate that tryptophan hydroxylase turns over rapidly in these cells. This method will be valuable in studies of the adrenergic mechanisms regulating pineal tryptophan hydroxylase activity.

The amniotic fluid lecithin/sphingomyelin (L/S) ratio from 128 cases of high risk pregnancies were measured by using high performance liquid chromatography (HPLC). The L/S ratio was found to be increased in patients with pre-eclampsia, hypertension, intrauterine growth retardation (IUGR), prolonged rupture of membranes (PROM), and twin gestation, but decreased in patients with diabetes from gestational diabetes to class C. In patients with placenta previa, the L/S ratio was not affected.

Culture of a bronchial aspirate from an immunosuppressed patient with severe pneumonia yielded a growth of filamentous poorly staining gram-negative rods. Fluorescence with Legionella pneumophila direct fluorescein-isothiocyanate conjugate was equivocal. Gas-liquid chromatography (GLC) and GLC/mass-spectrometry (GLC-MS) of the cellular fatty acids of the isolate confirmed that the organism was L. pneumophila. GLC and GLC-MS constitute a rapid and definitive method for identification of L. pneumophila isolates.
