Evaluating parental genetic knowledge of hearing loss with and without pre-test genetic counseling in a pediatric otolaryngology clinic.

This study evaluated parental knowledge of genetics of sensorineural hearing loss (SNHL) and satisfaction following pre-test consult with and without genetic counseling (GC). A survey evaluating parents' knowledge of genetics for SNHL with and without GC was administered to parents of children with SNHL who were offered genetic testing. The survey also inquired about satisfaction, and decision to pursue genetic testing. Statistical tests included Fisher-Freeman-Halton test and Mann-Whitney test. Forty-nine participants completed the survey and were seen by both otolaryngology and GC intern (EN

A Review of the Conversational Pragmatic Skills of Children With Cochlear Implants.

Many children who use cochlear implants (CI) have strong skills in many aspects of spoken language; however, limited information is available about their mastery of the pragmatic skills required to participate in conversation. This study reviewed published literature describing the pragmatic skills of children who use CIs in conversational contexts. Twenty-five studies met the inclusion criteria, and data were extracted describing participant characteristics, methodology, data type, outcomes, and factors associated with outcomes. Pragmatic skills were described in three broad categories: speec

Youth with hearing loss: Emotional and behavioral problems and quality of life.

OBJECTIVES: To compare parent- and self-reported emotional and behavioral problems and quality of life (QoL) among youth with hearing loss (HL) to norms, and to investigate possible associations between emotional and behavioral problems and QoL among youth with HL. METHODS: We used the Strengths and Difficulties questionnaire (SDQ) and the Inventory of Life Quality in Children and Adolescents (ILC) to measure emotional and behavioral problems and QoL in youth with HL (n = 317, ages 6-18), where 78% had bilateral HL, 22% unilateral HL, 16% had cochlear implants, and 59% conventional hearing ai

Children with hearing loss can predict during sentence processing.

Listeners readily anticipate upcoming sentence constituents, however little is known about prediction when the input is suboptimal, such as for children with hearing loss (HL). Here we examined whether children with hearing aids and/or cochlear implants use semantic context to predict upcoming spoken sentence completions. We expected reduced prediction among children with HL, but found they were able to predict similarly to children with normal hearing. This suggests prediction is robust even when input quality is chronically suboptimal, and is compatible with the idea that recent advances in

Trends in COSI responses associated with age and degree of hearing loss.

OBJECTIVE: To investigate the relationships between age, gender, hearing loss and hearing asymmetry with reported hearing difficulties and perceived benefit from hearing aids. DESIGN: A retrospective service evaluation of patients' COSI responses. STUDY SAMPLE: A non-sampled population (n = 995) attending audiology over two months. RESULTS: Clear trends were observed between hearing difficulties, age and hearing loss. A direct logistic regression model suggested that increasing age was the predominant cause of the most common hearing problems. Gender and hearing asymmetry had no significant

Neural correlates of visual stimulus encoding and verbal working memory differ between cochlear implant users and normal-hearing controls.

A common concern for individuals with severe-to-profound hearing loss fitted with cochlear implants (CIs) is difficulty following conversations in noisy environments. Recent work has suggested that these difficulties are related to individual differences in brain function, including verbal working memory and the degree of cross-modal reorganization of auditory areas for visual processing. However, the neural basis for these relationships is not fully understood. Here, we investigated neural correlates of visual verbal working memory and sensory plasticity in 14 CI users and age-matched normal-

Free-Field Cortical Steady-State Evoked Potentials in Cochlear Implant Users.

Auditory steady-state evoked potentials (SS-EPs) are phase-locked neural responses to periodic stimuli, believed to reflect specific neural generators. As an objective measure, steady-state responses have been used in different clinical settings, including measuring hearing thresholds of normal and hearing-impaired subjects. Recent studies are in favor of recording these responses as a part of the cochlear implant (CI) device-fitting procedure. Considering these potential benefits, the goals of the present study were to assess the feasibility of recording free-field SS-EPs in CI users and to c

Ketogenic Diet for KARS-Related Mitochondrial Dysfunction and Progressive Leukodystrophy.

KARS encodes lysyl-tRNA synthetase, which is essential for protein translation. KARS mutations sometimes cause impairment of cytoplasmic and mitochondrial protein synthesis, and sometimes lead to progressive leukodystrophies with mitochondrial signature and psychomotor regression, and follow a rapid regressive course to premature death. There has been no disease-modifying therapy beyond supportive treatment. We present a 5-year-old male patient with an asymmetrical leukodystrophy who showed overt evidence of mitochondrial dysfunction, including elevation of lactate on brain MR spectroscopy and

Genetic etiology of non-syndromic hearing loss in Latin America.

Latin America comprises all countries from South and Central America, in addition to Mexico. It is characterized by a complex mosaic of regions with heterogeneous genetic profiles regarding the geographical origin of the ancestors and proportions of admixture between the Native American, European and African components. In the first years following the findings of the role of the GJB2/GJB6 genes in the etiology of hearing loss, most scientific investigations about the genetics of hearing loss in Latin America focused on assessing the frequencies of pathogenic variants in these genes. More rece

Genetics of Childhood Hearing Loss.

Compelling evidence indicates that some newborns harboring genetic variants associated with hearing loss might not be identified by current physiologic newborn hearing screening (NBHS) rendering current NBHS protocols suboptimal. Incorporating genomic sequencing into NBHS would improve clinical diagnosis and decrease time to early intervention efforts.

Association of vitiligo with high-frequency sensorineural hearing loss: a systematic review and meta-analysis.

The association between vitiligo and hearing loss has been noted but the specific frequencies and degrees of hearing impairment remain unclear. The objective of this systematic review was to investigate the relationship between vitiligo and hearing thresholds at various specific frequencies. We followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. We searched MEDLINE and Embase for relevant studies from inception to 10th April 2021. Case-control studies, cross-sectional, or cohort studies that compared the frequency-specific hearing thresholds bet

Osteogenesis Imperfecta and hearing loss in the paediatric population.

BACKGROUND: Osteogenesis Imperfecta is a genetic disorder affecting the synthesis of collagen in the body. It is also known as 'Brittle Bone Disease'. It is heterogenous in its clinical presentation. The commonest presentation is a history of frequent fractures, joint deformities and blue sclera. Secondary deformities of the extremities, spine, skull as well short stature observed frequently. Hearing loss has been well documented to occur in Osteogenesis Imperfecta. It is most commonly seen in types I, II and III. Hearing loss forms part of the diagnostic criteria for these types. Depending on

Rapidly progressive hearing loss occurring over weeks: review of differential diagnoses.

BACKGROUND: Sudden hearing loss, or progressive hearing loss occurring over months to years, are well-established presentations. However, little is described in the medical literature on how to approach patients presenting with a rapidly progressive hearing loss occurring over weeks. This study aimed to evaluate the clinical significance of patients presenting with rapidly progressive hearing loss. METHODS: A case of rapidly progressive hearing loss occurring over 12 weeks is presented. A PubMed literature review was performed to determine the evidence-based differential diagnoses for rapidly

A behavioral method to estimate charge integration efficiency in cochlear implant users.

BACKGROUND: In cochlear implants, pulse amplitude (PA) or pulse phase duration (PPD) can be used to increase loudness. Loudness grows more slowly with increasing PPD, resulting in a larger dynamic range (DR), possibly reflecting "leaky" charge integration associated with neural degeneration due to hearing loss. Here, we propose a method to estimate charge integration efficiency for CI users. NEW METHOD: The DR was measured with increasing PA or PPD, relative to a common threshold anchor with a short PPD (25μs/ph); DRs were converted to the common unit of charge (nC). Charge integration effici

A new training package (3Cs: Connect, Communicate and Collaborate) for improving family responsive service delivery in early intervention for children with hearing loss: A proof of concept study.

BACKGROUND: An objective of early intervention for children with hearing loss is to enhance family engagement in therapy to maximise a child's speech and language potential. However, requiring a service provider to work collaboratively with a child's family can create problems in developing and underdeveloped countries, where skilled service providers and services for children with hearing loss are lacking and where an expert model of intervention prevails. OBJECTIVES: To determine the preliminary effectiveness of a new training package Connect, Communicate and Collaborate (3Cs), in improving
