Hypospadias repairs are performed in a unique environment with unique tissue. The potential for edema is tremendous. Given the difficult conditions, there will probably never be an operation for hypospadias that will be uniformly and totally successful. Nevertheless, the authors provide a categorization of complications and some useful guidelines for management.

Two cases of gallbladder carcinoma occurring in intramural diverticulosis (adenomyomatosis) are reported. The first was adenocarcinoma associated with cholesterolosis without gallstone formation. The second was squamous cell carcinoma. Four other carcinomas developing on diverticulosis were reported in the literature, none of them being squamous cell carcinoma. Our cases and those of the literature show that diverticulosis, a benign condition, can be associated with carcinoma which can make the pathological diagnosis difficult.

Diverticula are thought to result from high intraluminal pressures in closed segments of the sigmoid colon, causing herniation of mucosa and submucosa through muscle along the paths of the nutrient vessels. Mild diverticular disease usually can be managed with bed rest and a controlled diet, plus administration of antibiotics. Acute diverticulitis, especially recurrent bouts, may call for colonic resection.

The clinical presentation and subsequent treatment of 160 consecutive patients with proven diverticular disease of the colon from the Auckland Hospital, New Zealand were compared with 182 consecutive patients admitted to St. Bartholomew's Hospital, London. There were significant differences in sex incidence, in the ages of the male patients at presentation, patient symptoms, extent of disease and methods of surgical treatment. These probably reflect differences in the types of National Health Services of the two countries, rather than differences in the natural history of diverticular disease

39 patients with sigmoid lesions have been examined with angiografphy, using epinephrine and propranolol. 11 had carcinoma, all were diagnosed angiographically. Fibrosis may appear similar on phamacoangiography whereas the inflammatory reaction is different.

A case of progressive systemic sclerosis associated with colonic involvement is described. Barium-enema examination revealed segmental involvement. Colonoscopic examination of the whole range of the involved colon revealed wide-mouthed diverticula with interspersed rigid areas and numerous ulcerations. These colonoscopic findings appeared to be distinct from those associated with tuberculosis and Crohn's disease.

32 patients suffering from diverticulitis of the sigmoid colon underwent resection with end-to-end-anastomosis. In 24 cases the operation was performed electively as a one step procedure, the rest of the patients were operated on by two or three steps. One patient with ileus and peritonitis came to death. 28 from 31 surviving patients live without trouble. Resection of the inflammatory part of intestine is recommended instead of permanent colostomy.

A technique for the double contrast method was described which was used in our radiology institute on 2400 patients. It takes into account economic factors (amount of film used) as well as the lowest possible radiation exposure (mean screening time: 6.4 minutes). Typical lesions from malignant processes in the colon were illustrated. Today, radiographic examination of the colon should be conducted only with the double contrast method. Diverticular disease of the appendix involves about 1 per cent of all appendices removed. Considering the large number, the subject appears to have been neglected in medical literature. Since the symptomatology is similar to that of appendicits and diverticula are frequently very small, they could go unnoticed. In this paper is presented the case of a 27 year old woman in whom a congenital duplication of the distal ileum led to a massive hemorrhage. Heterotopic gastric mucosa was found in the ileal lesion and ulcer formation histologically proven. This case shows that congenital lesions of this sort may not come to the attention of the pediatrician or pediatric surgeon but become the concern of the surgeon in adult life.
