The chyloesophageal fistula. A new approach to thoracic duct drainage.

The main reason for the virtual abandonment of external thoracic duct drainage as an immunosuppressive measure is not its lack of efficacy, … the thoracic duct drains and anastomosing this conduit to the cervical esophagus a chyloesophageal fistula was created. A mean patency of 19 days was demonstrated radiologically and there was a reproducible peripheral blood lymphopenia of over 50% of preoperative values at 4 weeks. Although plasma albumin levels fell from 37 g/L to 29 g/L at 1 week, they …

Spontaneous occlusion of traumatic carotid cavernous fistula after orbital venography.

A 50-year-old man was admitted to our hospital with a diagnosis of traumatic carotid cavernous fistula. The fistula was observed to disappear spontaneously just after orbital venography. This was confirmed by clinical examinations and subsequent carotid angiogram. The possible mechanisms responsible for the spontaneous occlusion of carotid cavernous fistula are discussed.

Anastomotic leak and recurrent fistula following operation of oesophageal atresia.

Experiences from 14 disruptions of oesophageal sutures and with 8 cases of recurrent fistula from 1973 to 1982 are described. Healing of an early oesophageal insufficiency could be achieved by conservative means in 3 newborns. Conservative therapy seems to be possible if life-threatening clinical conditions are reduced effectively. 6 oesophago-pulmonary and recurrent oesophago-tracheal fistulas were closed operatively, two cases closed spontaneously.

[Treatment of enterocutaneous small intestine fistula using parenteral nutrition therapy].

A retrospective and prospective analysis was carried out of 48 patients with 54 enterocutaneous small bowel fistulae. 31 patients received total parenteral nutrition (TPN) by 17 patients only the daily losses of fluids and electrolytes were covered. In 19 fistulae (39,5%) closed spontaneously (48,5% with TPN and 14,3% without TPN). 10 patients underwent surgery; 5 fistulae closed operatively. The efficacy of treatment of enterocutaneous fistulae was influenced by the location of fistula, by the daily output, by the septic complications and by the use of TPN. Total parenteral nutrition can be t…

Spontaneous closure of a traumatic middle meningeal arterio-venous fistula.

A case is reported of a traumatic middle meningeal arteriovenous fistula, which was formed on the contralateral side to the head injury without a skull fracture and was no longer demonstrable on the repeated angiogram after 35 days without any surgical intervention. Possible mechanisms of fistula formation and its spontaneous closure are discussed.

Treatment of traumatic carotid-cavernous fistula using detachable balloon catheters.

The goal of therapy in patients with traumatic carotid-cavernous fistulas is to occlude the fistula, preferably while maintaining the carotid blood flow. Surgical techniques that treat the fistula remote from the cavernous sinus often cannot maintain carotid patency. Various interventional techniques using detachable balloons have been developed. The most common technique uses the endarterial route, introducing the balloon catheter in the neck or the groin. If the balloon is detached in the cavernous sinus, the carotid blood flow will be preserved. A second approach uses the venous retrograde …

High intermuscular anal abscess and fistula: analysis of 25 cases.

Although the majority of anal abscesses and fistulas are of the simple or low variety (intersphincteric or transsphincteric in Parks' classification), some of the simple, but high intermuscular, type are not recognized clinically and are not properly treated because they do not present the usual visible signs. Characteristically, there is no external swelling, induration or opening and there is high extension with a palpable mass or induration above the levator ani. Out of 350 patients with anal abscesses and fistulas treated by the first author, 25 (7%) patients had a high intermuscular absce…

The operative treatment of Crohn's enteritis complicated by ileosigmoid fistula.

An ileosigmoid fistula complicating Crohn's disease of the small bowel has been considered a perplexing entity, but in practice does not pose a serious operative problem. The presence of such a fistula does not signify that the sigmoid colon is a primary site of Crohn's disease. The operative treatment of this entity is simple and usually does not require resection of the sigmoid colon. Rather the sigmoid colon defect may easily be treated by simple closure, or by minimal partial resection and closure. Temporary diversion of the fecal stream is occasionally indicated because of local inflammat…

Uretero-neocystostomy in the treatment of postoperative uretero-vaginal fistula.

Nine patients with unilateral uretero-vaginal fistula afer hysterectomy were seen over a 9-year period. In 5 patients radical pelvic surgery combined with radiotherapy had been carried out because of carcinoma of the cervix. In one patient the fistula developed following total hysterectomy 18 years after radiotherapy because of a cervical carcinoma. In 3 patients hysterectomy had been performed for benign disease, uterine fibroids or fibroids combined with endometriosis. In one patient the fistula was successfully treated with an indwelling ureteral catheter for 7 days. In the other 8 patients…

Gastrocolic fistula. Report on two cases healed by medical treatment.

Two cases of gastrocolic fistula secondary to benign gastric ulcer disease are described in two patients without any previous gastric surgery. Both cases were successfully treated medically with cimetidine. Implications of the successful outcome of the medical treatment and the possible causes of the symptom of diarrhea are discussed.
