Quality of life for patients treated for penile cancer.

PURPOSE: We assessed the impact of primary surgery, including penile sparing surgery vs (partial) penectomy and lymphadenectomy, on sexuality and health related quality of life. MATERIALS AND METHODS: We invited 147 patients surgically treated for penile cancer at our institution between 2003 and 2008 to complete the IIEF-15, SF-36®, IOC (version 2) and questions on urinary function. We evaluated the impact of primary surgery type and lymphadenectomy on these outcomes. We also compared patient SF-36 scores with those of an age and gender matched normative sample from the general Dutch populat

The relief of mitral stenosis. An historic step in cardiac surgery.

Significant progress has been achieved in cardiac surgery in the last 50 years. Mitral valve surgery (especially for the relief of mitral stenosis) has paralleled the innovations and trends of cardiac surgery and often has served as the benchmark of the latest procedures and techniques. A chronological survey of mitral valve surgery is presented, with emphasis on parallels to cardiac surgery in general and with highlights of key figures and events that have conclusively altered the surgeon's approach to and success with cardiac dysfunction. A few surgeons promulgated the idea of cardiac surger

Management of mild aortic stenosis during coronary artery bypass graft surgery.

A small proportion of patients with significant coronary artery disease referred for coronary artery bypass graft (CABG) surgery have coexistent congenital or valvular disease that, if isolated, would be inadequately severe to justify surgery. While there is general agreement that CABG should be performed for obstruction of major epicardial arteries even without ischemic symptoms in patients having aortic valve replacement (AVR) for aortic stenosis (AS), there has been little or no consideration of whether "mild-to-moderate" AS should be treated by valve repair or AVR at the time of CABG. PHOTODYNAMIC THERAPY WITH INFRACYANINE GREEN (EMUNDO®) FOLLOWING SURGICAL EXCISION OF MULTIPLE CUTANEOUS MELANOCYTIC TUMORS IN A WHITE BENGAL HYBRID TIGER (PANTHERA TIGRIS CROSS).

A white Bengal hybrid tiger (Panthera tigris cross) was presented for a tumor subjacent to the right inferior eyelid. Surgical excision under general anesthesia followed by photodynamic therapy (PDT) with infracyanine green (EmunDow) was performed. Two months after surgery, a new tumor involving the concave left pinna was noted, and surgical excision followed by PDT was again performed. Healing of both surgical sites was achieved by 37 d postoperatively. Histopathologic review of both masses was consistent with cutaneous melanocytic tumors with complete surgical margins. To the authors' knowle

Awake Blue Laser Excision and Balloon Dilation of Oropharyngeal Stenosis in High-Risk Patients.

Oropharyngeal stenosis is a rare condition that can develop following head and neck cancer treatment, leading to progressive dyspnea, dysphagia, and dysphonia. Patients with symptomatic dyspnea may benefit from endoscopic surgery. However, head and neck cancer survivors can have severe trismus, limited neck extension, and altered laryngeal landmarks, which make rigid endoscopic surgery under general anesthesia high-risk or potentially not feasible without a tracheostomy tube. We describe an awake approach for the management of oropharyngeal stenosis for 2 head and neck cancer survivors with hi

Venous Air/Gas Embolism During Surgical Hysteroscopy: An Underestimated, Potentially Lethal Complication.

Hysteroscopic surgery is generally considered low risk; however, venous air/gas embolism is a potentially lethal complication with high mortality in severe cases. We summarize the pathophysiology and clinical features to support timely emergency management. A 46-year-old woman with a uterine myoma underwent hysteroscopic surgery under general anesthesia. Approximately 50 minutes after the start of the procedure, a sudden decrease in end-tidal CO₂, bradycardia, hypotension, and ST-segment elevation were observed. Transthoracic echocardiography revealed air bubbles in the right ventricle, confir

How to Do Robotic Repair of the Common Hepatic Duct: A Minimally Invasive Solution for Complex Cases.

General surgeons may encounter complex gallbladder cases throughout their careers, particularly in the context of acute or complicated gallbladder disease. The risk of bile duct injury is significantly elevated during non-elective procedures due to factors such as inflammation and anatomical variations. Over the past decade, robotic surgery has seen increasing adoption in hepatopancreatobiliary (HPB) procedures, offering enhanced visualization, improved instrument dexterity, and greater surgical precision. These advantages are especially valuable in the minimally invasive management of complex

Early detection and management of mesenteric traction syndrome using the hypotension prediction index: a case report.

Mesenteric traction syndrome (MTS) is a transient circulatory disturbance characterized by sudden hypotension, tachycardia, and facial flushing following mesenteric manipulation during abdominal surgery. Early recognition is essential; however, conventional monitoring often detects these events only after hemodynamic deterioration. We report a 68-year-old man who underwent distal pancreatectomy and splenectomy under combined general and thoracic epidural anesthesia. Sixteen minutes after the start of surgery, the hypotension prediction index (HPI)-an artificial intelligence-based monitoring sy

Diagnostic Complexities of Oral Squamous Cell Carcinoma.

Prompt diagnosis of oral cancers is critical to increase survival rates. Treatment for oral squamous cell carcinoma (OSCC) and oropharyngeal squamous cell carcinoma (OPSCC) is mainly driven by cancer stage and may include surgery alone or surgery with adjuvant or neoadjuvant radiation, chemotherapy, and/or targeted therapy. This article describes a case of a patient who was referred by his general dentist to an oral medicine clinic for assessment of an exophytic lesion on the left lateral tongue. The case report discusses the differential diagnosis and treatment, examining critical elements in

Utility of Remimazolam for Fast-Track Recovery Following Surgical Aortic Valve Replacement in an Elderly Patient With Severe Aortic Stenosis: A Case Report.

Remimazolam is an ultra-short-acting benzodiazepine that has minimal hemodynamic effects and is useful for early extubation after cardiac surgery. We present a case of an elderly patient with severe aortic stenosis (AS) who underwent surgical aortic valve replacement (AVR), was extubated in the operating room, and recovered quickly without postoperative delirium. An 87-year-old woman with severe AS underwent AVR under cardiopulmonary bypass. General anesthesia was induced with remimazolam 10 mg over one minute and fentanyl 100 µg, and maintained with remimazolam 0.4-0.7 mg/kg/hour, fentanyl, a

Anesthetic Management of Thoracoscopic Pulmonary Cystectomy in a Patient With Fontan Circulation and Disturbed Lung Inflation During Leakage Testing: A Case Report.

Congenital heart disease may require multiple cardiac surgeries during childhood. Subsequent non-cardiac surgeries increase the perioperative bleeding and hypoxia because of changes in circulation. An 18-year-old male patient with a history of multiple cardiac interventions, including Fontan surgery, underwent a thoracoscopic right lung suture and coverage for recurrent right spontaneous pneumothorax under general anesthesia with one lung ventilation (OLV). The superior dorsal and inferior lobes, which were inflatable before surgery, failed to expand during leakage testing. The trachea's condi

Ectopic Salivary Gland Found in the Vocal Cord: A Rare Case.

INTRODUCTION: Ectopic salivary gland is rarely found in the vocal cords; only two cases have been reported in the English literature. To the best of our knowledge, this is the third reported case of this anomaly. Case Presentation. A 78-year-old man with hoarseness two years ago visited our department. There were no other symptoms such as cough or sore throat. He had no history of smoking. Laryngeal endoscopic examination revealed a smooth mass in the anterior right vocal cord. We performed surgery under general anesthesia to remove the polyps and improve hoarseness. The histopathological spec
