Sexually Transmitted Infections in the U.S. Military: A Sexual Health Paradigm to Address Risk Behaviors, Unintended Pregnancy, Alcohol Use, and Sexual Trauma.

To address the ongoing epidemic of sexually transmitted infections (STIs) in the United States, the National Academies of Sciences, Engineering, and Medicine (National Academies) conducted a consensus study on STI control and prevention in the United States to provide recommendations to the Centers for Disease Control and Prevention and the National Association of County and City Health Officials. The culminating report identified military personnel as one of the priority groups that require special consideration given the high prevalence of STIs and their associated behaviors (e.g., concurren

Chronic sinusitis pathophysiology: the role of allergy.

INTRODUCTION: In 2005, a train derailment and subsequent chlorine spill ravaged the rural town of Graniteville in South Carolina, resulting in one of the worst chlorine gas exposures in US. HISTORY: Significant health and economic challenges persist in the community more than a decade later. Healthcare providers offered healthcare services to community members in the immediate aftermath of the disaster, and many still live in the community and continue to provide healthcare services. As such, healthcare professionals provide a unique and important perspective to help understand the impact of

Scoping Review on the Impact of Outbreaks on Sexual and Reproductive Health Services: Proposed Frameworks for Pre-, Intra-, and Postoutbreak Situations.

INTRODUCTION: Recent experiences from global outbreaks have highlighted the severe disruptions in sexual and reproductive health services that expose women and girls to preventable health risks. Yet, to date, there is no review studying the possible impact of outbreaks on sexual and reproductive health (SRH). Methodology. Studies reporting outbreaks impacting sexual and reproductive health and pregnancy outcomes were identified using MEDLINE, Embase, and ISI-WoS. Reported impacts were reviewed at systems, community, and legislative levels. RESULTS: The initial run listed 4423 studies; the 37

From Access to Outcomes: A Practical Approach to Strengthen Rural Health Systems.

Durable improvements in rural population health require stronger enabling conditions, including governance, workforce, payment, and data infrastructure, rather than a narrow focus on access to health care. We synthesized empirical evidence and health systems research published through 2025, emphasizing US studies of rural health outcomes, primary care, prevention, workforce, and payment models. Rural health systems are heterogeneous, and single interventions do not produce equal gains across communities. Evidence supports integrated approaches that combine community-wide prevention, team-based

Implementing a COVID-19 vaccination outreach service for people experiencing homelessness.

ISSUE ADDRESSED: In 2021, the Australian government implemented a population wide COVID-19 vaccination program. People experiencing homelessness faced challenges accessing vaccines and many were not being reached. By reorienting vaccination services to include assertive outreach strategies, a Brisbane-based non-profit health care team successfully administered 2065 COVID-19 vaccinations to homeless and precariously housed people. This study examines insights from stakeholders delivering the service and perspectives of clients who received a vaccine. METHODS: Semi-structured interviews with fi

Reaching Adolescent Girls and Young Women With HIV Self-Testing and Contraception at Girl-Friendly Drug Shops: A Randomized Trial in Tanzania.

PURPOSE: We hypothesized that an intervention designed to create girl-friendly drug shops would increase access to sexual and reproductive health products and services among adolescent girls and young women (AGYW) (ages 15-24 years) in Tanzania. METHODS: We conducted a four-month randomized trial at 20 drug shops in Shinyanga, Tanzania from August-December 2019 to determine if the Malkia Klabu ("Queen Club") intervention increased AGYW patronage and the provision of HIV self-testing (HIVST), contraception, and health facility referrals to AGYW (primary outcomes). Drug shops were randomized 1:

The Impact of COVID-19 Confinement on Cognition and Mental Health and Technology Use Among Socially Vulnerable Older People: Retrospective Cohort Study.

BACKGROUND: COVID-19 forced the implementation of restrictive measures in Spain, such as lockdown, home confinement, social distancing, and isolation. It is necessary to study whether limited access to basic services and decreased family and social support could have deleterious effects on cognition, quality of life, and mental health in vulnerable older people. OBJECTIVE: This study aims to explore the impact of the COVID-19 outbreak on cognition in older adults with mild cognitive impairment or dementia as the main outcome and the quality of life, perceived health status, and depression as

Implementation of the World Health Organization's Package of Essential Noncommunicable Disease Interventions: an overview of reviews.

OBJECTIVES: To synthesize the evidence on the implementation of the World Health Organization's Package of Essential Noncommunicable Disease Interventions (WHO PEN). METHODS: This narrative overview of reviews draws on evidence from systematic reviews and other review articles. RESULTS: The findings reveal substantial gaps and challenges in public primary health facilities across many low- and middle-income countries, particularly in workforce capacity and the availability of essential medicines, diagnostics, and equipment. Several delivery strategies-such as high-risk population screening,

Global synergistic actions to improve brain health for human development.

The global burden of neurological disorders is substantial and increasing, especially in low-resource settings. The current increased global interest in brain health and its impact on population wellbeing and economic growth, highlighted in the World Health Organization's new Intersectoral Global Action Plan on Epilepsy and other Neurological Disorders 2022-2031, presents an opportunity to rethink the delivery of neurological services. In this Perspective, we highlight the global burden of neurological disorders and propose pragmatic solutions to enhance neurological health, with an emphasis o

The health needs of adults with learning disabilities and the Health of the Nation strategy.

Studies of mortality and morbidity relating to people with learning disabilities are reviewed within the context of the five Health of the Nation Key Areas. It is concluded that the Key Areas have become increasingly relevant as a result of increased longevity and changing lifestyles. For example, both cardiovascular disease and cancers appear to be more common than before, and a high proportion of people with learning disabilities carry one or more risk factors associated with morbidity and mortality from these causes. However, there are a range of other health needs in this population which

Barriers and Facilitators of Responding to Problem Gambling: Perspectives from Australian Mental Health Services.

Despite high rates of comorbidity between problem gambling and mental health disorders, few studies have examined barriers or facilitators to the implementation of screening for problem gambling in mental health services. This exploratory qualitative study identified key themes associated with screening in mental health services. Semi-structured interviews were undertaken with 30 clinicians and managers from 11 mental health services in Victoria, Australia. Major themes and subthemes were identified using qualitative content analysis. Six themes emerged including competing priorities, importan

Socioeconomic marginality and health services utilization among Central Harlem substance users.

The article examines whether decrements in socioeconomic measures in a poor, substance using population predict changes in health services utilization. The sample consisted of 658 "hard drug" (crack, powder cocaine, and heroin) users drawn from Central Harlem in New York City during 1998 and 1999. Chain referral and social networking were used in order to gain access to hidden users. The sample was stratified according to operational measures indicating socioeconomic marginality, one calculated using indices of income, education, and employment and another designed to measure lived homelessnes

How Did Women From South Asian Backgrounds and People Seeking Asylum and Refugees Look After Their Health and the Health of Others During the COVID-19 Pandemic? A Community Research Approach.

BACKGROUND: The COVID-19 pandemic had a profound impact on people's lives globally and affected access to, utilisation and the delivery of healthcare. Many communities were disproportionally affected by the pandemic. Community-based research approaches may help address potential healthcare inequities by identifying and understanding people's experiences and needs relating to healthcare access and utilisation. METHODS: For this qualitative study, members of community organisations were trained and supported to interview women from South Asian backgrounds and people seeking asylum and refuge ab

[Observing migration and deficiencies in health care - STD center at the Cologne Municipal Health Department].

BACKGROUND: In spite of the compulsory health insurance in Germany, many people only have limited access to medical services. This has serious consequences, especially in the field of sexual health. The affected people are not only undocumented migrants, but also many people from the new European Union countries who are temporarily living in Germany. Many of these people, especially in larger cities, frequent STD counselling centers. METHOD: Since 2002, in addition to basic socio-demographic data, other anonymous data have been recorded for all consultations in the STD offices of the Cologne

Acceptability of an mHealth App That Provides Harm Reduction Services Among People Who Inject Drugs: Survey Study.

BACKGROUND: Harm reduction services reduce the negative consequences of drug injection and are often embedded within syringe service programs (SSPs). However, people who inject drugs (PWID) suboptimally engage with such services because of stigma, fear, transportation restrictions, and limited hours of operation. Mobile health (mHealth) apps may provide an opportunity to overcome these barriers and extend the reach of SSPs beyond that of the traditional brick-and-mortar models. OBJECTIVE: This study aims to assess the prevalence of smartphone ownership, the level of comfort in providing the p

Supporting the Health and Well-Being of Indigenous Communities: A Position Paper From the American College of Physicians.

Indigenous peoples in the United States experience many health disparities and barriers to accessing health care services. In addition, Indigenous communities experience poor social drivers of health, including disproportionately high rates of food insecurity, violence, and poverty, among others. These challenges are unsurprising, given historical societal discrimination toward Indigenous peoples and government policies of violence, forced relocation with loss of ancestral home, and erasure of cultures and traditions. Indigenous peoples have displayed resilience that has sustained their commun

Problem Gambling Messages on College Counseling Center Websites: An Over-Time and Cross-Country Comparison.

Gambling problems remain more prevalent among college students than any other adult population. Among the different informational services available to students struggling with this issue, the convenience, anonymity, and accessibility of the college counseling center website (CCW) make this a particularly appealing resource. This updated assessment explored the nature and frequency of problem gambling messages on U.S. CCWs. and assessed whether differences existed between U.S. and U.K. sites. Results showed that problem gambling remains disproportionately underrepresented on U.S. CCWs compared

Satisfaction with health care and cancer screening practices among women in a largely rural region of West Texas.

BACKGROUND: Satisfaction with medical care has been shown to influence patient behavior, but its effect on the use of preventive services is largely unstudied. This study examined whether women's satisfaction with the accessibility and quality of care was associated with the odds of receiving an annual clinical breast examination, conducting a monthly self-breast examination, or receiving an annual Pap smear. METHODS: A telephone survey was conducted among 675 women in West Texas, an area with a relatively high proportion of rural residents and Hispanics. Multiple logistic regression analyses
