OBJECTIVES: The Contegra bioprosthesis (valved heterologous bovine jugular vein) is used for reconstruction of the right ventricular outflow tract (RVOT) in congenital heart malformations and pulmonary valve replacement in different settings. Compared to pulmonary homografts, the Contegra conduit is readily available 'on the shelf'. So far, its use was mainly described in children. The aim of this study is to evaluate the feasibility and the outcome of Contegra graft implantation in the adult.
METHODS: Between November 1999 and December 2007, a total of 32 Contegra grafts were implanted in 31

A 45-year-old woman presented with triple valve infective endocarditis and ventricular septal defect. There were vegetations on the tricuspid valve, pulmonary valve, and aortic valve. She had multiple complications such as nephrotic syndrome, severe anemia, congestive heart failure, and convulsion. Her general condition was extremely poor. Intensive medical therapy, such as blood transfusion, mechanical ventilation, and continuous venovenous hemofiltration, allowed her to tolerate surgery. Triple valve replacement and ventricular septal defect closure was successfully performed without major c

Anatomic and functional features of the normal and abnormal mitral valve are reviewed. Of 1,010 personally studied necropsy patients with severe (functional class III or IV, New York Heart Association) cardiac dysfunction from primary valvular heart disease, 434 (43%) had mitral stenosis (MS) with or without mitral regurgitation (MR): unassociated with aortic valve stenosis or regurgitation or with tricuspid valve stenosis in 189 (44%) patients, and associated with aortic stenosis in 152 (35%), with pure (no element of stenosis) aortic regurgitation in 65 (15%) patients, and with tricuspid val

This report describes the case of a 13-year-old boy with endocarditis of a Contegra® pulmonary valved conduit used for the correction of tetralogy of Fallot. He had severe pulmonary stenosis after endocarditis resolution. We performed percutaneous pulmonary valve implantation with resolution of the conduit dysfunction. The procedure was well tolerated by our patient and offered many advantages.

BACKGROUND: Surgical treatment of secondary mitral regurgitation (SMR) is controversial.
AIM: To analyse outcome after undersizing annuloplasty (UA) and mitral valve replacement (MVR).
METHODS: Consecutive patients operated on for severe SMR, with left ventricular ejection fraction (LVEF)<40% and refractory CHF, were included. Endpoints were in-hospital mortality, mid-term cardiovascular (CV) mortality, evolution of LV variables and recurrence of mitral regurgitation (MR).
RESULTS: 59 patients were included (mean age 65±10 years, preoperative LVEF 36±6%; effective regurgitant orifice [ERO]

BACKGROUND: Although the Ross operation requires double-valve replacement for aortic valve disease, it has been shown to provide excellent hemodynamic results and is associated with low morbidity and mortality rates. We reviewed our long-term experience after completion of the Ross procedure.
METHODS: Between October 1994 and February 2009, 58 consecutive patients underwent a Ross procedure at our institution. The right ventricular outñow tract was repaired with a cryopreserved pulmonary homograft. All patients were scheduled for a yearly study thereafter that ended at the time of death or at

BACKGROUND: Limited data exist regarding the impact of mitral annular calcification (MAC) on outcomes of transcatheter edge-to-edge repair for mitral regurgitation (MR).
METHODS: We retrospectively analyzed 968 individuals (median age, 79 [interquartile range, 70-86] years; 60.0% males; 51.8% with functional MR) who underwent an isolated, first-time intervention. Stratified by MAC extent per baseline transthoracic echocardiogram, the cohort was assessed for residual MR, functional status, all-cause mortality, heart failure hospitalizations, and mitral reinterventions post-procedure.
RESULTS:

In the heart, the most common sequelae after electrical injury are myocardial contusion and arrhythmias. A case is presented of segmental ventricular dysfunction and severe aortic regurgitation due to laceration of the right coronary cusp of the aortic valve caused by electrical injury. To the authors' knowledge, this is the first reported case of valvular rupture due to electrical injury.

The authors report a case of double-orifice mitral valve (DOMV) which showed mitral stenosis and mild insufficiency. An associated anomaly was secundum atrial septal defect. DOMV is an unusual congenital heart defect. The occurrence of this anomaly with or without secundum atrial septal defect is very rare. More often it is associated with other congenital malformations arising from atrioventricular canal defects. There may be no haemodynamic consequences but mitral insufficiency and/or stenosis may complicate this malformation. Treatment can be summarised as abstention, surgical repair or val

From April, 1984 to April, 1989, 104 heart valve replacements were performed in 103 patients. The atrio-ventricular valves (97 mitral and 7 tricuspid valves) were replace by bioprosthetic valves mounted on the flexible stent made of Elgiloy. They included 91 porcine aortic valves and 13 bovine pericardial valves. Only 74 patients recovered and discharged from the hospital because of high operative mortality of re-replacement and double valve replacement (19 and 30 patients, respectively). These patients were followed-up for 8 to 68 months with a total follow-up period of 297 patient-years. Two

Left atrial thrombus after mitral valve surgery is believed to be relatively rare. However, we previously reported on cases of left atrial thrombus (LAT) with spontaneous regression in the early postoperative period. Forty two patients who underwent mitral valve replacement were studied using computerized tomography (CT). Early postoperative LAT was found in 11 patients out of 42 (26.2%); all of whom had an uneventful postoperative course. The mean preoperative atrial diameter in the LAT group (81.4 mm) was greater than that in the non-LAT group (57.0 mm). This was the most decisive factor sig

During a 20-year period 303 young subjects between 9 and 20 years of age (mean, 16.2 +/- 2.72 years) with rapid and relentlessly progressive valvular disease from rheumatic fever underwent valve replacements. The Starr-Edwards ball valve prosthesis remains the device of choice, although other valves have been implanted. The overall hospital mortality rate was 9.6% in the mitral valve, 3.5% in the aortic valve, and 4.2% in the double valve replacement groups. Actuarial survival at 10, 15, and 20 years was 78.4% (+/- 3.3%), 70.0% (+/- 5.8%), and 59.3% (+/- 11.1%), respectively, for patients with

During the year 1978, the decision to perform cardiac catheterization in patients with valvular heart disease was delayed until clinical and noninvasive assessment have been completed. As a result, 184 patient underwent operation without invasive studies, and 59 had elective catheterization. Another 62 patients were referred during the same period for valve replacement after routine catheterization had been performed elsewhere. Age, sex distribution, symptoms, and cause of valve disease were similar in all three groups, although we managed emergencies and second operations more frequently with

The cases of 6 patients who had ventricular wall rupture after isolated mitral valve replacement and were seen in our service are reviewed. In the first 2, the main lesion was mitral stenosis and calcification was severe. Injury to the ventricular myocardium during removal of the valve was the causative factor in 1 and the most likely explanation in the other. In the other 4 patients, the dominant lesion was insufficiency. Calcium was absent, and fibrosis of the valves was minimal. Defects of technique were not obvious. All perforations were beneath the annulus. The first of these 4 latter pat

BACKGROUND: Due to potential thromboembolic complications, mechanical valves within the native heart are often considered contraindications to ventricular assist device (VAD) support.
METHODS: A retrospective review of VAD cases between June 1982 and March 1998 showed 8 patients with mechanical valves who were supported with Thoratec (Pleasanton, CA) VADs.
RESULTS: There were 6 males and 2 females ranging in age from 20 to 69 years (mean 49.8+/-5.6). Four patients were supported when they could not be weaned from cardiopulmonary bypass after reparative procedures and were thought to have rev

The special pathology of reconstructable or only prosthetically correctable congenital malformations of the mitral valve is described on the basis of the following examples taken from our own operative and autopsy material of the last 5 years: 1. Congenital isolated mitral stenosis in female twins (7 month old infant and 33 month old child). 2. Congenital isolated mitral insufficiency in a 7 1/2 year old boy. 3. Combined mixed mitral valve malformations with a parachute valve-like mitral valve anomaly, combined with hypoplasia of the ascending and descending aortas, in a 6 1/2 year old girl. 4

A 49-year-old male with a history of left ventricular systolic function dilated cardiomyopathy and severe symptomatic bicuspid aortic stenosis recently diagnosed (syncope, chest pain and dyspnea) was admitted to the coronary care unit. During hospitalization, he developed cardiogenic shock requiring double inotropic support. High surgical risk and an elevated chance of graft rejection contraindicated surgical replacement or heart transplant. We performed a transcatheter aortic valve replacement with a favorable evolution.

Heart valve disease is often characterized by a prolonged asymptomatic period that lasts for years and presents primary care physicians with an opportunity to detect disease before irreversible heart failure or other cardiac complications develop. Acute valvular disease can masquerade as respiratory illness or present with nonspecific systemic symptoms, and an astute examination by a primary care physician can direct appropriate care. Therefore, an understanding of the common pathologies and presentations of valvular heart disease is critical. This review focuses on the 2 most common valve les
