Fucoidan therapy decreases the proviral load in patients with human T-lymphotropic virus type-1-associated neurological disease.

BACKGROUND: Human T-lymphotropic virus type-1 (HTLV-1) is a human retrovirus that causes HTLV-1-associated myelopathy/tropical spastic paraparesis (HAM/TSP) and adult T-cell leukaemia (ATL). A higher viral load in individuals with HTLV-1 infection increases their risk of developing HAM/TSP and ATL. Moreover, the high proviral load is associated with the clinical progression of HAM/TSP. Reduction of the number of HTLV-1-infected cells is therefore crucial for preventing and treating HTLV-1-associated diseases. Recently, fucoidan, a complex sulphated polysaccharide derived from marine seaweed, h

T-cell epitopes on the 70-kDa protein of the (U1)RNP complex in autoimmune rheumatologic disorders.

High-titre IgG antibodies against the immunodominant 70-kDa protein of the (U1)ribonucleoprotein (RNP) complex are present in virtually 100% of patients with mixed connective tissue disease (MCTD), and less commonly in a variety of other autoimmune rheumatic diseases. As T-cell 'help' is assumed to be required for this potentially pathogenic form of immune response, investigations to define T-cell epitopes on the 70-kDa protein were undertaken. In prior studies we expressed the 70-kDa protein and a number of its fragments, spanning most of the molecule, as recombinant fusion proteins using the

Modeling sporadic Alzheimer's disease using brain organoids: Emerging trends and translational opportunities.

Sporadic form of Alzheimer's disease (sAD), remains a complex neurodegenerative disorder with limited translational models. Brain organoids derived from human induced pluripotent stem cells (hiPSCs) have emerged as promising tools to recapitulate aspects of human brain development and pathology. Recent advances have introduced vascularized, immune-competent organoids capable of modeling hallmark features of sAD, including amyloid-β accumulation, tau pathology, and neuroinflammation. New strategies to enhance organoid maturation, cellular diversity, and aging phenotypes are pushing the boundari

Dermatologic Research in Displaced Populations: Importance, Challenges, and Proposed Solutions.

Displaced populations face complex dermatologic challenges. Contributing factors include low immunization rates, poor sanitation, crowded living conditions, and physical abuse. Chronic inflammatory conditions and infectious diseases, including fungal infections and scabies, are prevalent. Research is crucial to reduce the spread of disease, improve care in these populations, and develop sustainable frameworks for long-term dermatologic health care in crisis settings. The paucity of dermatologist support in this field exacerbates the issue. Ethical considerations include nonmaleficence and cult

Cytokines in Thyroid-Associated Ophthalmopathy.

Thyroid-associated ophthalmopathy (TAO), also known as thyroid eye disease (TED) or Graves' orbitopathy (GO), is a complex autoimmune condition causing visual impairment, disfigurement, and harm to patients' physical and mental health. The pathogenesis of TAO has not been fully elucidated, and the mainstream view is that coantigens shared by the thyroid and orbit trigger remodeling of extraocular muscles and orbital connective tissues through an inflammatory response. In recent years, cytokines and the immune responses they mediate have been crucial in disease progression, and currently, commo

Interactions of host-associated multispecies bacterial communities.

The oral microbiome comprises microbial communities colonizing biotic (epithelia, mucosa) and abiotic (enamel) surfaces. Different communities are associated with health (eg, immune development, pathogen resistance) and disease (eg, tooth loss and periodontal disease). Like any other host-associated microbiome, colonization and persistence of both beneficial and dysbiotic oral microbiomes are dictated by successful utilization of available nutrients and defense against host and competitor assaults. This chapter will explore these general features of microbe-host interactions through the lens o

Glucocorticoid-sensitive Paroxysmal Atrial Fibrillation, Sick Sinus Syndrome, and Mitral Regurgitation in a Patient with Malignant Rheumatoid Vasculitis.

An 80-year-old woman with rheumatoid arthritis presented with chest pain. Clinical examination revealed new-onset paroxysmal atrial fibrillation with symptomatic sinus pauses and worsening mitral regurgitation, which were both resistant to conventional therapies. Based on her skin lesions, an increase in pleural and pericardial effusion, possible myocardial involvement, and a positive finding for immune complex testing, rheumatoid vasculitis was diagnosed. Subsequent glucocorticoid therapy suppressed systemic inflammation, resulting in structural, functional, and electrical reverse remodeling

Targeting Syk in Autoimmune Rheumatic Diseases.

Spleen tyrosine kinase (Syk) is a member of the Src family of non-receptor tyrosine kinases, which associates directly with surface receptors, including B-cell receptor and Fcγ receptor, and is involved in a variety of signal transduction pathways. Rheumatoid arthritis (RA) and systemic lupus erythematosus are autoimmune diseases in which autoantibodies, immune complexes, and autoreactive T cells account for the expression of tissue inflammation and damage. Syk inhibitors efficiently suppress RA in patients albeit in the expression of unwanted side effects, including gastrointestinal effects,

Capsule endoscopy in Crohn's disease: are we seeing any better?

Crohn's disease (CD) is a complex, immune-mediated disorder that often requires a multi-modality approach for optimal diagnosis and management. While traditional methods include ileocolonoscopy and radiologic modalities, increasingly, capsule endoscopy (CE) has been incorporated into the algorithm for both the diagnosis and monitoring of CD. Multiple studies have examined the utility of this emerging technology in the management of CD, and have compared it to other available modalities. CE offers a noninvasive approach to evaluate areas of the small bowel that are difficult to reach with tradi

[The cytokine profile in the acute period of tick-borne neuroinfections in children].

Clinical peculiarities and blood levels of several cytokines were studied in 199 children, aged from 1 to 15 years, in the acute period of different tick-borne infections. The mixed infection manifested itself by the more severe progression of general clinical presentations compared to monoinfections. Etiologically unspecific changes of blood concentrations of cytokines in the acute period of the disease were found. These data were associated with different clinical forms of tick-borne infections. The decrease in the interferon gamma production was associated with the most severe disease progr

[Vasculitis and liver disease].

Extrahepatic manifestations of liver diseases especially the hepatitis B virus (HBV)-infection and hepatitis C virus (HCV)-infection may occur during acute and/or chronic viral hepatitis. Besides a serum like illness with fever, arthralgia and urticaria, haematological disorders with transient bone marrow suppression and cryoglobulinemia have been described. Vasculitis is a rare complication of viral hepatitis. However, HCV can trigger a cryoglobulinemic vasculitis and may clinically present with purpura, arthritis, neuropathy, glomerulonephritis and fatigue. Panarteritis nodosa is frequently

Leprosy-associated renal disease: case report and review of the literature.

Leprosy is an infectious disease the principal clinical manifestations of which are anesthetic skin lesions and the development of peripheral neuropathy. The most common renal manifestation in leprosy patients is glomerulonephritis. Both immunofluorescent and electron microscopic studies suggest that the varied glomerular lesions found in these patients are immune complex mediated. Other renal lesions that have been described include amyloidosis, tubulointerstitial disease, acute renal failure, and functional defects in the absence of identifiable histologic abnormalities. In this report, a pa

The Gut Microbiota-Immunity Axis in Colorectal Cancer: Implications for Immunotherapy and Clinical Translation.

Colorectal cancer (CRC) arises within a complex microbial ecosystem that may influence carcinogenesis, tumor immunity, and therapeutic response. This is particularly relevant in metastatic disease, where most tumors are microsatellite-stable/mismatch repair-proficient (MSS/pMMR) and derive little meaningful benefit from immune checkpoint inhibitor (ICI) monotherapy. Specific pathobionts, including Fusobacterium nucleatum, enterotoxigenic Bacteroides fragilis, and colibactin-producing Escherichia coli, have been implicated in genotoxic, inflammatory, and checkpoint-related mechanisms that may c

Adjuvants orchestrate cross-organ induction of mucosal CD8+ T cell immune responses in respiratory tracts.

Mucosal immunity is paramount for combating respiratory infections, but conventional vaccination methods, such as intramuscular injection, often struggle to induce it. Although direct mucosal vaccination is effective, it carries an increased risk of respiratory adverse effects, making it particularly challenging for individuals with chronic respiratory diseases. Consequently, developing a strategy to induce sufficient mucosal immune responses without modifying conventional vaccination methods would be highly desirable. The potential of vaccine adjuvants to facilitate such cross-organ induction

Chemo-metallo nanodrivers for spatiotemporally controlled in situ vaccination.

In situ cancer vaccines exhibit promising therapeutic potential; however, their efficacy is constrained by limited antigen release, suboptimal spatiotemporal immune coordination, and adaptive immune resistance. In this study, we identify zinc ions (Zn2+) as synergistic agents with chemotherapeutics in modulating PANoptosis and immune signaling pathways. Capitalizing on this concept, a chemo-metallic nanodriver was developed for effective in situ cancer vaccination. This system used zinc-aluminum layered double hydroxides as the structural base and efficiently loaded cisplatin (DDP) and indocya

The Role of Immunotherapy in Lung Cancer Treatment: Current Strategies, Future Directions, and Insights into Metastasis and Immune Microenvironment.

Lung cancer is a leading cause of mortality worldwide. Immunotherapy has emerged as a potentially effective strategy, as traditional medicines have shown minimal success. This review investigates the current state of immunotherapy for lung cancer treatment, focusing on its mechanisms, clinical applications, strategies, and future directions. This study focuses on the different characteristics of non-small and small-cell lung cancer to emphasize the need for targeted treatment strategies. In non-small cell lung cancer, immune checkpoint inhibitors that target PD-1, PDL1, and CTLA-4 have shown a

Targeting GDF15 to enhance immunotherapy efficacy in glioblastoma through tumor microenvironment-responsive CRISPR-Cas9 nanoparticles.

Despite the outstanding clinical success of immunotherapy, its therapeutic efficacy in glioblastoma (GBM) is still limited. To identify critical regulators of GBM immunity, we constructed a mouse single-guide RNA (sgRNA) library corresponding to all disease-related immune genes, and performed an in vivo CRISPR knockout (KO) screen in syngeneic GBM mouse models. We demonstrated that the deletion of GDF15 in GBM cells ameliorated the immunosuppressive tumor microenvironment (TME) and enhanced the antitumor efficacy of immune checkpoint blockade (ICB) response. Moreover, we designed unique nanopa

Mast Cell Response to Parasites: from Recognition and Activation to Host Defense Modulation.

Parasites represent a diverse and widely distributed group of pathogens that cause diseases with significant global health implications. The interaction between parasite and host is characterized by a high degree of complexity, with both parties continuously adapting to changes in the other. The successful host invasion is largely attributable to the evasion strategies employed by parasites to ensure their survival in immunocompetent individuals. In turn, the host's defense mechanisms utilize a variety of structures and processes, ranging from primary barriers to the most sophisticated ones, t

Functional and phenotypic characterization of peripheral blood mononuclear cells from tuberculosis patients in Southern Thailand.

INTRODUCTION: Tuberculosis (TB) remains a global health challenge, with active TB disease (ATB) and latent TB infection (LTBI) representing distinct immunological states. Understanding immune responses in these groups is critical for developing effective interventions. The complex nature of immune responses to Mycobacterium tuberculosis (M.tb) within and between different stages of TB, host evasion mechanisms of the bacterium, variable protection conferred by the BCG vaccine in adults, and lack of validated immune correlates of protection are among the key challenges to the successful control
