[Two cases of conjunctival malignant melanoma treated with topical interferon alpha-2b drop as an adjuvant therapy].

BACKGROUND: Topical interferon alpha-2b (IFNalpha-2b) eye drops suppress tumor growth through the activation of host immunity on the ocular surface. Conjunctival malignant melanoma, a rare malignant tumor in Japan, develops local recurrence and lymph node metastasis. We herein report two cases with conjunctival melanoma treated with topical IFNalpha-2b eye drops. CASES: Case 1, a 77-year-old woman presented with temporal conjunctival pigmented nodules, simulating primary acquired melanosis (PAM) on the cornea and around the conjunctiva OD. Case 2, an 84-year-old woman, presented with nasal co

Association between acute motor axonal neuropathy and ophthalmoplegia during pregnancy.

Different subtypes producing the clinical picture of Guillain-Barré syndrome (GBS) and overlapping forms of GBS subtypes have been described. GBS as a complication in pregnancy has been reported rarely. The present report describes the clinical, electrophysiological and prognostic features of a pregnant woman with overlapping forms of GBS subtypes, acute motor axonal neuropathy and ophthalmoplegia.

Systemic drugs in patients with skin diseases.

Systemic drugs play an important role in the modern practice of dermatology. The purpose of this article is to review systemic therapies in the following categories: antifungals, immunosuppressants, retinoids, and biologic immunomodulators. We provide a historical perspective, summarize current clinical indications, and discuss novel mechanisms of action for each drug. Suggestions for dosing regimens and laboratory monitoring are given. To help clinicians safely implement these therapies in their practice, we review major adverse reactions and clinically important drug interactions of these sy

Progesterone and autoimmune disease.

Sexual dimorphism in human immune systems is most apparent in the female predominance of certain autoimmune diseases (ADs) like systemic lupus erythematosus (SLE). Epidemiologic, observational and experimental evidence strongly suggest sex steroids are important modulators of genetic risk in human AD. In this regard, the roles of progesterone (Pg), an immunomodulatory female sex steroid, are poorly understood. Several lines of investigation indicate Pg and synthetic progestins impact risk of AD and immune-mediated injury in different ways depending on their concentrations and their engagement

Invasive aspergillosis in cystic fibrosis: a fatal case in an adolescent and review of the literature.

Invasive aspergillosis is a rare complication of cystic fibrosis. In this article, we describe a case of an adolescent with cystic fibrosis, which was well-controlled previously, colonized with Aspergillus fumigatus. The patient developed fatal disseminated aspergillosis in the absence of any preexisting risk factors after a short course of intravenous corticosteroid treatment.

Pharmacovigilance and PML in the oncology setting.

Methods developed by the Southern Network on Adverse Reactions project, the only state-funded pharmacovigilance program in the nation, are invaluable in identifying rare and serious drug events and in disseminating related safety reports quickly throughout the medical community. An important discovery was identification and reporting of an association of rituximab and progressive multifocal leukoencephalopathy (PML) in patients without human immunodeficiency virus (HIV). A recent investigation identified 57 patients with rituximab-associated PML, including bone marrow samples, brain biopsies,

[Severe Wegener's granulomatosis successfully treated with rituximab].

The paper discusses the problems of the diagnosis and treatment of Wegener's granulomatosis, the most common anti-neutrophil cytoplasmic antibody-associated systemic vasculitis. It describes a clinical case in which the administration of rituximab could achieve remission of severe Wegener's granulomatosis in a young man having numerous adverse reactions associated with the long-term use of high- and average-dose glucocorticoids and nonselective immunosuppressive agents. A place of rituximab in current drug therapy for Wegener's granulomatosis is shown.

Quality of life in multiple sclerosis: effects of current treatment options.

Multiple sclerosis is the most common non-traumatic neurodegenerative disease in adults. Most of the patients present with both physical and mental deficits which reflect the dissemination of the lesions in the central nervous system, produced by the inflammatory process. The incomplete recovery after relapses, the accumulation of new deficits and the progressive nature of the condition interfere with daily activities of individuals and have a negative impact on their well-being. Indeed, studies show that quality of life measurements are constantly lower in patients with multiple sclerosis. Es

A case of explosive progression of hepatocellular carcinoma in a patient with common variable immunodeficiency (CVID).

OBJECTIVE: While it is well known that patients with common variable immunodeficiency (CVID) are predisposed to various malignancies, primarily non-Hodgkin's lymphoma and gastric carcinomas, to our knowledge no cases of hepatocellular carcinoma have been reported in the absence of preexisting liver disease. METHOD AND RESULTS: We report a 50-year-old male patient with CVID with a B- and T-cell deficiency. The patient was on prophylactic intravenous gammaglobulin and had received several years earlier a course of rituximab for an autoimmune disorder. He had no history of hepatitis. The patient

Use of cytokine therapy in primary immunodeficiency.

Of the six cytokine therapies approved by the US Food and Drug Administration, five of them have been used in patients with primary immunodeficiency (PID). In some applications, clear benefits have been demonstrated, while in others, effects have been more marginal. The most compelling current applications of cytokine therapy in PID are those of granulocyte colony stimulating factor in severe congenital neutropenia and interferon gamma in chronic granulomatous disease. Despite encouraging results with interleukin-2 in common variable immunodeficiency and select other indications, its use in PI

Liver transplantation in children: update 2010.

Pediatric liver transplant recipients represent an important target population for primary care health professionals as well as transplant practitioners. With improving patient and graft survival, new concerns now face health care professionals caring for the transplant community, namely the long-term complications of immunosuppressive therapy and the potential for withdrawal of immunosuppression, transplant recipients' quality of life, and the persistent shortage of donor organs leading to morbidity and mortality on the waiting list. These issues require constant collaboration between pediatr

[Vaccination and multiple sclerosis].

Vaccinations to prevent communicable diseases are, like in other chronic diseases, of special importance in patients with multiple sclerosis (MS). Various bacterial and viral infections have been shown to induce relapses of MS. Reports of possible adverse effects of vaccinations on the course of multiple sclerosis have led patients and treating physicians to exercise caution in the use of vaccines. A number of vaccines have been studied with respect to the risk in MS patients. Some vaccines, for example against yellow fever, are not indicated in MS due to the risk of MS exacerbation. In contra

Successful treatment of refractory adult-onset Still's disease with anti-CD20 monoclonal antibody.

BACKGROUND: Tobacco use is a leading cause of premature mortality among people living with HIV (PLWH), especially in Botswana, where high smoking rates coincide with successful HIV control. There is a need for evidence-based interventions to support smoking cessation among PLWH, but research on smoking cessation in Botswana is limited. Therefore, to address this gap, a qualitative study was conducted in Botswana to understand the experiences of PLWH who smoke and identify barriers and facilitators to quitting in preparation for a larger implementation trial. METHODS: In 2024, 36 interviews we

The immunologic basis for gastrointestinal food allergy.

PURPOSE OF REVIEW: Food allergies are common in children, although rare in adults. They can be life threatening via anaphylaxis, especially to peanut, and cause distress to children and parents because of the disease itself, and then through the difficulty of pursuing a strict elimination diet. In parents/carers and adults, this has significant impact on quality of life. The purpose of this review is to determine the therapeutic potential and current treatments for these conditions by understanding and manipulating the immune response. RECENT FINDINGS: The single largest change in attitude to

Intravenous immunoglobulin treatment in laryngeal pemphigoid.

We report a 29-year-old white woman who presented with oral involvement and subsequently developed severe and extensive laryngeal involvement with mucous membrane pemphigoid. She could not tolerate systemic corticosteroids due to side-effects. Dapsone improved only the oral disease, but the laryngeal disease remained active and progressive. She refused immunosuppressive agents, because she wanted to conceive. The acute severe widespread laryngeal pemphigoid was accompanied by severe relentless coughing and dysphonia. The patient was treated with intravenous immunoglobulin (IVIg) as monotherapy
