Cutaneous Malignancies Metastatic to the Female Genital Tract and Pelvic Lymph Nodes: Analysis of Metastatic Patterns and Pathogenesis.

Background/Objectives: Metastases from cutaneous malignancies to the female genital tract and pelvic lymph nodes are rare clinical entities that frequently masquerade as primary gynecologic tumors, leading to significant diagnostic challenges. The distinction between primary and metastatic disease is critical, yet complex, given the varying patterns of spread exhibited by different skin cancers. This study aims to provide a tumor-specific overview of these metastatic patterns to guide diagnosis and therapy. Methods: We conducted a narrative review informed by a systematic literature search of

Indigenous oral traditions and psychological well-being under modernization: from external valuation to relational selfhood.

Across many Indigenous communities, modernization, urbanization, market integration, formal schooling, language shift, and cultural commodification have reshaped the conditions under which psychological well-being is understood and sustained. These pressures influence how Indigenous identities, languages, knowledges, and cultural practices are valued within dominant social, educational, economic, and clinical frameworks. This article conceptualizes Indigenous oral traditions as living psychosocial knowledge embedded in community-held epistemic and relational infrastructures and examines their

Aligning the stars: quality metrics must evolve from screening rates to diagnostic resolution.

Cancer screening in the US is at a critical juncture, facing an urgent need to shift from a focus on initial test uptake toward a mandate for complete diagnostic resolution. Historically, quality metrics have prioritized the singular event of a recommended test over the necessity of diagnostic resolution. Financial barriers, administrative blind spots in quality metrics, and the "Medicare 5-star paradox" have created a screening purgatory in which patients with abnormal findings are frequently left without a definitive diagnosis. Although the Affordable Care Act successfully increased access t

Machine Learning Prediction of Local Failure in GI Brain Metastases on the Day of Gamma Knife Radiosurgery.

BACKGROUND: Time to local failure after stereotactic radiosurgery (SRS), including Gamma Knife radiosurgery (GKRS), for gastrointestinal (GI) brain metastases is difficult to predict and may vary substantially across tumors and patients. We developed and internally evaluated a survival machine learning framework to estimate tumor-specific time to local failure using pre-SRS features. METHODS: We performed a retrospective study of GI brain metastases treated with GKRS. Local failure was modeled as a time-to-event outcome under right censoring. A Random Survival Forest survival model was traine

The evolution of RTOG and NRG Oncology CNS tumors clinical trials.

PURPOSE: Radiation Therapy Oncology Group (RTOG) and NRG Oncology have played a key role in shaping evidence-based management of central nervous system (CNS) tumors. These have defined standards for radiotherapy delivery, integrated systemic therapies for multimodal approaches, and advanced trial methodology across a range of CNS malignancies through large, multi-institutional studies. This is a comprehensive narrative review of trials involving adult CNS tumors, focused on gliomas, metastases, meningioma, and primary CNS lymphoma (PCNSL). METHODS: Trials were identified through protocol arch

Stage I Non-Seminomatous Testicular Cancer: Long-Term Follow-Up with Surveillance Approach Post-Orchiectomy.

INTRODUCTION: Adjuvant treatment for patients with stage I non-seminomatous germ cell tumors (NSGCTs) could be active surveillance (AS), chemotherapy, or retroperitoneal lymph node dissection. AS is the preferred option in most cases. The aim of this study was to evaluate long-term survival and prognostic factors in our population with AS approach. METHODS: We collected information from patients with stage I NSGCT of the testis in medical records from 1995 to 2016. Patients had negative serum tumor markers and imaging of the chest, abdomen, and pelvis with no evidence of metastasis. At relaps

Potentially preventable hospitalizations among American Indian and Alaska Native adults, 2016-2021.

OBJECTIVE: Potentially preventable hospitalizations are inpatient admissions for a standard set of selected acute illnesses and chronic conditions that might have been avoided with preventive care or outpatient management. During 2010-2012, Alaska Native adults had higher rates of potentially preventable hospitalizations compared to other adults in Alaska. We evaluated potentially preventable hospitalizations among American Indian/Alaska Native (AI/AN) adults in the United States during 2016-2021. METHODS: We used hospital discharge data from the Indian Health Service National Patient Informa

Mechanistic classification of isolated severe aortic regurgitation in a contemporary cohort of patients.

AIMS: Aortic regurgitation (AR) arises from leaflet disease and/or dilatation of the functional aortic annulus complex. Understanding the mechanisms of AR informs surgical planning of valve and aorta repair. This study investigates the mechanisms, aetiologies, and outcomes of isolated native severe AR in a consecutive cohort of patients. METHODS AND RESULTS: Patients with moderate-to-severe (3+)/severe (4+) native valve AR, identified from our institutional echocardiography database (2014-2018), were included. Exclusions were significant concomitant valve disease, endocarditis, or aortic diss

The impact of household physical distancing and its timing on the transmission of SARS-CoV-2: Insights from a household transmission evaluation study.

BACKGROUND: Studies on SARS-CoV-2 household transmission often assume random mixing, overlooking detailed contact patterns and the timing of physical distancing. METHODS: To address this, we examined interactions within 280 households, including 280 index cases and 544 members, enrolled from April 2020 to April 2021 in Nashville, Tennessee, and central Wisconsin. Eligible households were enrolled within 7 days of index case symptom onset if at least one member was initially asymptomatic. Participants were monitored for 14 days, with symptoms and respiratory specimens collected daily, and cont

Posttraumatic stress disorder factor structure in hurricane-affected Puerto Ricans: A PTSD Checklist for DSM-5 comparison with non-Latiné White individuals.

Due to Puerto Rico's location, there is heightened vulnerability to the consequences of natural disasters, contributing to an elevated risk of posttraumatic stress disorder (PTSD). Given PTSD's heterogeneous nature, this study examined whether PTSD factor structure, based on DSM-5 criteria and measured using the PTSD Checklist for DSM-5 (PCL-5), was equivalent across hurricane-exposed Puerto Ricans (n = 596) and non-Latiné White (NLW) individuals (n = 459). Confirmatory factor analysis (CFA) indicated the seven-factor hybrid model of PTSD was the best-fitting structure, χ2(N = 897, 298) = 685.

Estimating the long-term health impact and program cost-effectiveness of providing direct screening services to low-income, medically underserved patients through the Colorectal Cancer Control Program (CRCCP).

PURPOSE: The Colorectal Cancer Control Program (CRCCP) aimed to increase colorectal cancer (CRC) screening among U.S. medically underserved populations through promotion and provision of CRC screening. We used simulation modeling to estimate the lifelong health impact and program cost-effectiveness of direct screening services, typically a single cycle of routine screening/follow-up testing provided through the CRCCP ("intervention"). METHODS: Data for this study were from CDC's Colorectal Clinical Data Elements (CCDE), which captured screening and follow-up services received from CRCCP betwe

Navigating family dynamics and ethical considerations in genetic diagnosis of pulmonary arterial hypertension: insights from in-depth semi-structured interviews.

BACKGROUND: Genetic diagnosis and precision medicine are rapidly advancing, driven by innovations in next-generation sequencing and omic methods. The UK's collaboration between national research initiatives and the National Health Service facilitates translation of research into clinical practice. This rapid transition impacts family dynamics and family planning, and raises ethical concerns, compounded by limited public and practitioner awareness of the long-term consequences of genetic diagnosis. Our objective is to explore the impact of genetic diagnosis on family dynamics and the ethical co

Multi-modal sentiment recognition with residual gating network and emotion intensity attention.

Multimodal emotion recognition focuses on the prediction of emotions using text, visual and acoustic modalities, and some results have been generated in this field. Previous approaches fall short in two aspects, one is the processing of complementary information among modalities, the other is how to avoid the long-term dependency and select the most important joint modal features. In this paper, we propose a new multimodal emotion recognition framework MSRG, which consists of feature extraction (FE), emotional intensity attention (EIA), time-step level fusion (TLF), utterance level fusion (ULF

A computational workflow for assessing drug effects on temporal signaling dynamics reveals robustness in stimulus-specific NFκB signaling.

Single-cell studies of signal transduction have revealed complex temporal dynamics that determine downstream biological function. For example, the stimulus-specific dynamics of the transcription factor NFκB specify stimulus-specific gene expression programs, and loss of specificity leads to disease. Thus, it is intriguing to consider drugs that may restore signaling specificity in disease contexts, or reduce activity but maintain signaling specificity to avoid unwanted side effects. However, while steady-state dose-response relationships have been the focus of pharmacological studies, there ar

Stratification-based instrumental variable analysis framework for nonlinear effect analysis.

Nonlinear causal effects are prevalent in many research scenarios involving continuous exposures, and instrumental variables (IVs) can be employed to investigate such effects, particularly in the presence of unmeasured confounders. However, common IV methods for nonlinear effect analysis, such as IV regression or the control-function method, have inherent limitations, leading to either low statistical power or potentially misleading conclusions. In this work, we propose an alternative IV framework for nonlinear effect analysis, which has recently emerged in genetic epidemiology and addresses m
