Development of polycystic kidney disease in juvenile cystic kidney mice: insights into pathogenesis, ciliary abnormalities, and common features with human disease.

Significant progress in understanding the molecular mechanisms of polycystic kidney disease (PKD) has been made in recent years. Translating this understanding into effective therapeutics will require testing in animal models that closely resemble human PKD by multiple parameters. Similar to autosomal dominant PKD, juvenile cystic kidney (jck) mice develop cysts in multiple nephron segments, including cortical collecting ducts, distal tubules, and loop of Henle. The jck mice display gender dimorphism in kidney disease progression with more aggressive disease in male mice. Gonadectomy experimen…

Pig kidney: anatomical relationships between the intrarenal arteries and the kidney collecting system. Applied study for urological research and surgical training.

PURPOSE: We present a systematic study of the anatomical relationships between intrarenal arteries and the kidney collecting system in pigs. MATERIALS AND METHODS: The intrarenal anatomy (collecting system and arteries) was studied in 91, 3-dimensional endocasts of the kidney collecting system together with the intrarenal arteries. RESULTS: Some anatomical details that have importance to help research and surgical training in urology when using the pig as an animal model were observed and described. It was found that there was only 1 artery per kidney. This artery divided into cranial and ca…

Prevalence of chronic kidney disease and decreased kidney function in the adult US population: Third National Health and Nutrition Examination Survey.

BACKGROUND: Recently developed clinical practice guidelines and calibration of the Third National Health and Nutrition Examination Survey (NHANES III) serum creatinine assay provide a basis for estimating the prevalence and distribution of chronic kidney disease (CKD) in the United States using standardized criteria based on estimated glomerular filtration rate (GFR) and persistent albuminuria. METHODS: A nationally representative sample of 15,625 noninstitutionalized adults aged 20 years and older from the NHANES III was analyzed. Kidney function (GFR), kidney damage (albuminuria), and stage…

Kidney stone disease: to drink or not to drink-that is the question.

Kidney stone prevention presents a paradox: an effective, safe, and inexpensive intervention-hydration-is widely recommended, yet difficult to implement in practice. The recent Prevention of Urinary Stones With Hydration (PUSH) trial by Desai et al. (n = 1658 patients) evaluated a behavioral strategy to increase fluid intake and reduce recurrence over 2 years but found no significant benefit compared with standard of care, despite modest increases in urinary volume. These results contrast with earlier trials, notably that of Borghi et al. in 1996, in which sustained urine output above 2 L/d si…

The landscape of allele-specific expression in human kidneys.

Allele-specific expression (ASE), the preferential expression of one gene copy, is a key mechanism of genomic regulation. However, its role in human kidney disease remains poorly understood. In this study, we generated a high-quality, genome-wide ASE map using paired whole-genome sequencing and RNA sequencing from microdissected glomerular and tubulointerstitial compartments of patients with proteinuric kidney disease. We showed that the majority of common ASE events were deterministic and sequence-mediated. We also found that diseased kidneys exhibited significantly more ASE in glomeruli (GLO…

[The floating kidney: a rare cause of abdominal pain].

A 25-year-old woman presented with a mobile mass in the lower right abdomen, accompanied by pain and nausea. Ultrasound and renogram imaging revealed nephroptosis, a rare condition typically seen in young, slender women. The patient underwent a robot-assisted laparoscopic nephropexy, after which the nephroptosis resolved.

Kidney transplantation in the transgender population: Lights and shadows.

Transgender individuals are gradually increasing in worldwide. Hormonal therapies may impact cardiovascular mortality, kidney function and transplant stability, the inverse is also true: immunosuppressive agents, particularly high-dose glucocorticoids used to treat acute rejection episodes, may adversely affect mental health, potentially complicating gender-affirming treatment. We aim to provide a comprehensive summary of the status of kidney transplantation in transgender individuals. A comprehensive search was conducted using MeSH keywords, including: "transgender," "kidney function," "hormo…

The many roles of the calcium-sensing receptor in the kidney.

The calcium-sensing receptor (CaSR) is a central regulator of renal mineral homeostasis and tubular function. Since the discovery of its expression in the kidney, numerous studies have demonstrated that its actions are finely distributed along the nephron. Through these segment-specific mechanisms, the CaSR orchestrates diverse physiological processes: contributes to the integrity of the filtration barrier, regulates proximal and distal tubular handling of essential ions such as calcium, magnesium, and sodium, and influences water transport and acid-base balance, integrating mineral metabolism…

Little kidneys amid large global inequities.

Chronic kidney disease (CKD) is a common non-communicable disease in children, and kidney dysfunction is the leading metabolic risk factor for death. Despite this, awareness of the CKD burden remains limited, and significant inequities exist in access to diagnosis and care worldwide. Kidney disease risk in children begins in utero and is dependent on the mother's health and wellbeing. This is further impacted each day by poverty, nutrition, education, infection, and safety. Greater community awareness is needed, especially in lower resource settings, where children present late and may have no…

The APOL1 gene and kidney transplantation: a review article.

INTRODUCTION: Chronic kidney disease (CKD) is a global public health problem and kidney transplantation is the treatment of choice for patients with stage 5 (G5) CKD. Kidney transplant survival at the end of the first year exceeds 95%, and at 5 years, it approaches 90%. However, the decline in the estimated glomerular filtration rate (eGFR) over the years remains a challenge. The rate of eGFR decline is attributed to several factors and has recently been attributed to the presence of APOL1 gene variants. DISCUSSION: The risk variants G1 and G2 of the APOL1 gene are related to CKD. Evidence su…

The Role of Natural Killer Cells in Kidney Transplantation.

Natural killer (NK) cells are increasingly recognized as central effectors in kidney allograft injury, extending beyond their traditional role in innate immunity. This narrative review summarizes current evidence linking NK cells to microvascular inflammation, with a focus on both antibody-dependent and antibody-independent rejection. In antibody-mediated rejection, NK cells may be activated through Fc gamma receptor IIIa (CD16a) engagement with endothelium-bound donor-specific antibodies, leading to antibody-dependent cellular cytotoxicity and proinflammatory cytokine release. In parallel, em…

Impact of kidney function on the metabolome in the general population.

BACKGROUND: A reduction in kidney function cause metabolites that normally are cleared by the kidney to accumulate. These accumulated metabolites could potentially link kidney function to an increased risk of cardiovascular disease. AIM: To assess the relationships between creatinine-based estimated glomerular filtration rate (eGFR) and the metabolome (791 endogenous metabolites) in the general population, as well as the cortisol to cortisone ratio. PATIENTS AND METHODS: Three population-based cohorts, Epihealth, PIVUS and POEM (total n = 3,444), were used with a discovery/validation approac…

Advances in the diagnosis and detection of chronic kidney disease.

Chronic kidney disease affects 788-844 million adults worldwide and is projected to become the fifth leading cause of death by 2040. Global burden estimates remain limited by ascertainment bias and inadequate access to testing, particularly in low-income and middle-income countries. Advances in detection include improved estimation of glomerular filtration rate (GFR) using cystatin C and the recognition of albuminuria as a key marker for screening and risk stratification. Kidney biopsy is improving diagnostic accuracy and prognostic prediction, and multiomics approaches are advancing our under…

Are Renal Cysts a Barrier to Donation and Kidney Transplant?

OBJECTIVES: We evaluated the long-term outcomes of kidney transplants performed using grafts that contain simple renal cysts, to determine whether these cysts constitute a barrier to kidney donation or transplant. MATERIALS AND METHODS: All kidney transplants performed by our team from February 2012 to May 2025 were retrospectively reviewed. Cases in which the graft contained at least 1 simple renal cyst were included. Demographic characteristics, cyst size, perioperative complications, postoperative renal function, and cyst size during follow-up ultrasonography were recorded. Postoperative s…

To be, or not to be, on insulin for simultaneous kidney-pancreas transplant: Is that the question?

The prevalence of type 2 diabetes mellitus (T2DM) among patients with kidney failure is nearly 40%. The therapeutic paradigm for the management of T2DM in the setting of chronic kidney disease (CKD) has undergone a dramatic evolution in recent years. Progressive CKD decreases the need for exogenous insulin, allowing noninsulin treatments to be more effective in controlling serum glucose levels. Although diet, exercise, weight loss, and behavioral therapy are important factors in diabetes management, several noninsulin alternatives have risen to the forefront, including glucagon-like peptide 1 …
