Management of hyponatraemia in pre-eclampsia with severe features.

Pre-eclampsia is a common pregnancy complication with many associated maternal and fetal risks, yet its pathophysiology remains poorly understood. Hyponatraemia is a rarely described finding in pre-eclampsia that has been associated with both maternal and fetal complications and medically indicated delivery. We present a case of hyponatraemia in a patient admitted for induction of labour for gestational hypertension, which developed into pre-eclampsia with severe features requiring magnesium sulfate therapy for seizure prophylaxis. The patient's hyponatraemia resolved with delivery, fluid rest

Hypermagnesaemia causing mesenteric ischaemia and small bowel infarction.

We present a case of mesenteric ischaemia caused by hypermagnesaemia after ingestion of a large oral dose of magnesium citrate, which resulted in smooth muscle relaxation, hypotension and bowel infarction. The patient had a history of chronic bowel dysmotility and renal impairment. On operative exploration, the bowel was noted to have a distinct pattern of ischaemia along its antimesenteric border. Small bowel resection was performed, and the patient was left in discontinuity, with definitive repair and anastomosis performed 24 hours later. The patient's magnesium level was 8.39 mg/dL, which w

Severe Preeclampsia in the Setting of Myasthenia Gravis.

Myasthenia gravis (MG) is a rare autoimmune disease that leads to progressive muscle weakness and is common during female reproductive years. The myasthenic mother and her newborn must be observed carefully, as complications during all stages of pregnancy and the puerperium may arise suddenly. Preeclampsia is a common obstetrical condition for which magnesium sulfate is used for seizure prophylaxis. However, magnesium sulfate is strongly contraindicated in MG as it impairs already slowed nerve-muscle connections. Similarly, many first-line antihypertensive medications, including calcium channe

Recurarization in a successfully managed case of posterior reversible encephalopathy syndrome (PRES) for emergency caesarean section.

Posterior reversible encephalopathy syndrome (PRES) is a clinicoradiologic syndrome of headache, visual changes, altered mental status and seizures with radiologic findings of posterior cerebral white matter edema. It is seen in hypertensive encephalopathy, renal failure, and autoimmune disorders or in patients on immunosuppressants. We report a case of 24-year-old primigravida who presented at term with sudden onset hypertension, neurological deficits, and an episode of the visual blackout. Magnetic resonance imaging showed features suggestive of PRES. She was posted for emergency lower segme

Nasal irrigation: From empiricism to evidence-based medicine. A review.

Nasal irrigation plays a non-negligible role in the treatment of numerous sinonasal pathologies and postoperative care. There is, however, a wide variety of protocols. The present review of the evidence-based literature sought objective arguments for optimization and efficacy. It emerged that large-volume low-pressure nasal douche optimizes the distribution and cleansing power of the irrigation solution in the nasal cavity. Ionic composition and pH also influence mucociliary clearance and epithelium trophicity. Seawater is less rich in sodium ions and richer in bicarbonates, potassium, calcium

Induction of triacylglycerol production in Chlamydomonas reinhardtii: comparative analysis of different element regimes.

In this study, impacts of different element absence (nitrogen, sulfur, phosphorus and magnesium) and supplementation (nitrogen and zinc) on element uptake and triacylglycerol production was followed in wild type Chlamydomonas reinhardtii CC-124 strain. Macro- and microelement composition of C. reinhardtii greatly differed under element regimes studied. In particular, heavy metal quotas of the microalgae increased strikingly under zinc supplementation. Growth was suppressed, cell biovolume, carbohydrate, total neutral lipid and triacylglycerol levels increased when microalgae were incubated und

Severe impaired deambulation in a patient with vitamin D and mineral deficiency due to exocrine pancreatic insufficiency.

CONTEXT: Skeletal muscle weakness and impaired gait function are common risk factors for disease and even death. Therefore, identification of the modifiable causes of skeletal muscle weakness should have high priority. Knowledge regarding optimal vitamin D treatment in cases of pancreatic insufficiency is scarce. CASE REPORT: We report a case of a slow decrease in ability to walk distances more than 100 m during the previous 6 months. Low exocrine pancreatic function resulting in phosphorus, magnesium and vitamin D deficiency was found. Medical treatment with peroral pancreatic enzymes, phosp

Characterization of elemental and structural composition of corrosion scales and deposits formed in drinking water distribution systems.

Corrosion scales and deposits formed within drinking water distribution systems (DWDSs) have the potential to retain inorganic contaminants. The objective of this study was to characterize the elemental and structural composition of extracted pipe solids and hydraulically-mobile deposits originating from representative DWDSs. Goethite (alpha-FeOOH), magnetite (Fe(3)O(4)) and siderite (FeCO(3)) were the primary crystalline phases identified in most of the selected samples. Among the major constituent elements of the deposits, iron was most prevalent followed, in the order of decreasing prevalen

[Chronic use of proton-pump inhibitors associated with giardiasis: A rare cause of hypomagnesemic hypoparathyroidism?].

Hypomagnesemia is a rare cause of hypoparathyroidism that can have a very serious clinical presentation. We report the case of a 62-year-old woman hospitalized for exploration of acute tetraparesis with vomiting and swallowing disorders associated with a severe hypocalcemia. Biological explorations revealed hypoparathyroidism (PTH=16ng/L) related to low plasma and erythrocyte magnesium (0.32 and 1.32mmol/L, respectively) as well as hypocalciuria and hypomagnesuria linked to gastrointestinal malabsorption. Etiologic investigations led to the discovery of Giardiasis lamblia on duodenal biopsies

The emerging structural understanding of transglutaminase 3.

Transglutaminases (TGase; protein-glutamine: amine gamma-glutamyl-transferase) are a family of calcium-dependent acyl-transfer enzymes ubiquitously expressed in mammalian cells and responsible for catalyzing covalent cross-links between proteins or peptides. A series of recent crystal structures have revealed the overall architecture of TGase enzymes, and provided a deep look at their active site, calcium and magnesium ions, and the manner by which guanine nucleotides interact with this enzyme. These structures, backed with extensive biochemical studies, are providing new insights as to how ac

Delay of natural bone loss by higher intakes of specific minerals and vitamins.

For early prevention or inhibition of postmenopausal and age-related bone loss, nutritional interventions might be a first choice. For some vitamins and minerals an important role in bone metabolism is known or suggested. Calcium and vitamin D support bone mineral density and are basic components in most preventive strategies. Magnesium is involved in a number of activities supporting bone strength, preservation, and remodeling. Fluorine and strontium have bone-forming effects. However, high amounts of both elements may reduce bone strength. Boron is especially effective in case of vitamin D,

The effect of urease inhibitors on the encrustation of urethral catheters.

Encrustation and blockage of indwelling urethral catheters is primarily brought about by infection of the urinary tract by Proteus mirabilis or other urease-producing species. The bacteria colonise the catheter forming a biofilm community within a polysaccharide matrix. The activity of the urease drives up the urinary pH and causes the crystallisation of calcium and magnesium phosphates in the biofilm. We have used a simple physical model of the catheterised bladder to investigate the ability of urease inhibitors to control encrustation. It was observed that acetohydroxamic acid (1.0 mg/ml) an

The effects of alcohol on the heart.

We have discussed in this review many features and possible mechanisms responsible for the development of alcoholic cardiomyopathy. The evidence suggests that defects in myofibrillar protein turnover occur in both acute and chronic alcohol studies. Possible mechanisms to explain poor contractile function include alterations in cellular calcium, magnesium or phosphate homeostasis. The toxic effects of acetaldehyde or the formation of fatty acid ethyl esters may cause impairment of mitochondrial oxidative phosphorylation. Alternatively, reduced amounts of heat shock proteins may result in poor a

Antioxidant defence system in a patient with cerebrovascular accident.

The increased production of reactive oxygen metabolites in the central nervous system may result in cellular damage and vascular-parenchymal injury. Vascular injury is also a cause of the vasoregulation deficiency. In this case report, a 60-year-old woman was admitted to the department of neurology, with a diagnosis of cerebrovascular accident. During 18 days of follow-up, the status of the antioxidant system was determined by measuring red cell superoxide dismutase and catalase activity. Red cell and plasma copper, zinc and magnesium concentrations were also measured. Red cell superoxide dism

Comparison of in vitro predictive tests for irritation induced by anionic surfactants.

Skin compatibility of anionic surfactants may often but not always be predicted by in vitro tests. For instance, the correlation between in vivo and in vitro data is classically hampered in the presence of magnesium. This ion is known to interfere with in vitro skin irritation predictive tests based on protein denaturation. This study was conducted to compare a recently introduced assay, corneosurfametry, with other in vitro tests including the pH-rise of bovine serum albumin, collagen swelling, and zein solubilization tests. Corneosurfametry entails collection of cyanoacrylate skin surface st

Torsade de pointes and sudden death associated with diabetic autonomic diarrhoea--a case report.

A 68-year-old diabetic and hypertensive woman presented with chronic autonomic diarrhoea, syncope and palpitations which were associated with QT prolongation and recurrent episodes of torsade de pointes. She was on glibenclamide, indapamide and probucol (for type V hyperlipidaemia). Despite intravenous infusions of potassium, lignocaine and amiodarone, the unstable rhythm persisted. However, intravenous magnesium sulphate with small doses of intravenous propranolol terminated the torsade de pointes. She was stabilised but following discharge she relapsed, and upon re-admission, succumbed to in

[Effects of ascorbic acid on trace element metabolism in the choroid-retina of streptozotocin-induced diabetic guinea pigs].

In order to investigate the effect of ascorbic acid on trace element metabolism in the choroid-retina, experimental diabetes mellitus was induced in Hartley guinea pigs with intraperitoneal injection of streptozotocin. After 4 weeks, ascorbic acid-deficient feed was given for 3 weeks. Trace elements of the choroid-retina were measured with inductively-coupled plasma emission spectrometry. The activity of superoxide dismutase and lipid peroxide content also were measured. The results indicated that zinc and magnesium increased in diabetes mellitus. However, with ascorbic acid deficiency in diab

Failure of Brucella abortus lipopolysaccharide (LPS) to activate the alternative pathway of complement.

Bovine erythrocytes (E) coated with either crude or purified preparations of Brucella abortus LPS were not lysed by human complement (C) in the presence of the chelating agent ethyleneglycol-bis-N, N'-tetraacetic acid (EGTA). On the other hand, bovine red cells coated with Salmonella typhimurium LPS were lysed by human C in EGTA. B. abortus LPS preparations did not cause fluid phase human C consumption in the presence of calcium and magnesium ions. However, as expected, S. typhimurium LPS consumed C from human serum in a dose-dependent fashion. The results of these experiments indicate that B.

Model for parathyroid hormone secretion and metabolism in calves.

A model for parathyroid gland response to differing calcium and magnesium concentrations is proposed based on direct determinations of parathyroid hormone (PTH) secretion rate. A proportional control seems adequate for physiological variations of calcium and magnesium levels. At extreme hypocalcemia, nonlinearities are observed: a model involving the depletion of a storage compartment is proposed, the size of which is calculated. PTH distribution was studied in another group of animals; sampling was made from the saphenous artery and the thoracic duct. Concentrations of plasma and lymph-intact
