Hemifacial microsomia. A report of two cases.

Hemifacial microsomia (HFM) is an asymmetrical congenital deformity of the head and face caused by anomalous development of the structures derived from the first and second branchial arches. Oral and maxillofacial malformations present diagnostic and treatment challenges unique to the dental profession. The etiology, a report of two cases and a brief description of treatment modalities are discussed in this article.

The biomaterial influences the ossification after sinus floor elevation using tissue-engineered bone grafts.

Sinus floor elevation is the standard procedure that allows dental implant insertion in the atrophic posterior maxilla. Instead of autogenous bone, tissue-engineered bone grafts can be used, but clear comparative clinical studies also assessing the influence of the biomaterial are missing. In six patients, tissue-engineered bone grafts were used in eight sinus floor elevations. After culturing osteoblast-like cells from biopsies of the maxilla, they were seeded on scaffolds made either from demineralised bovine bone matrix (DBBM) or from solvent-dehydrated mineralised bone (SDBB), and grafted.

[Endovascular and surgical associated treatment of post-traumatic epistaxis].

We report on two cases of patients, in whom endovascular embolization of the maxillary artery and next surgical ligation of the anterior ethmoidal artery was performed due to posttraumatic intracrable epistaxis. In those patients, conservative treatment failed, hence endovascular embolization was made. Due to persisted bleeding, right anterior ethmoidal artery were surgically ligated in both patients. After the procedures, bleeding completely stopped.

A literature review on biomaterials in sinus augmentation procedures.

BACKGROUND: Sinus augmentation is a common procedure to increase bone volume and allow for proper implant placement in the atrophic posterior maxilla. Although the patient's own bone is considered the best grafting material, various synthetic or bovine-derived alternatives are used to simplify the grafting procedure. PURPOSE: The overall objective of this review was to assess the efficacy of different graft materials used in sinus augmentation procedures as demonstrated in animal studies. MATERIALS AND METHODS: A specific and sensitive database was initially created via PUBMED, focusing on s

Endoscopic endonasal-transantral surgery for an isolated orbital floor blow-out fracture in a pediatric patient.

Orbital floor blow-out fractures occur due to blunt trauma causing displacement of orbital contents to the maxillary sinus while the orbital rim is intact. Surgical repair of these fractures includes transantral, transorbital, or endoscopic endonasal approaches with or without implant use. We report a 12-year-old boy who presented with diplopia after blunt trauma to the head while playing football. Computed tomography revealed a left isolated orbital blow-out fracture. The patient was treated by a combined endoscopic endonasal-transantral approach and stability was restored with a urethral bal…

Neonatal nasal obstruction and a single maxillary central incisor.

We present two neonates with nasal obstruction because of anterior choanal stenosis (congenital nasal pyriform aperture stenosis). An associated single maxillary central incisor was also shown on computed tomography imaging in the neonatal period. These midline anomalies are recognized minimal manifestations (microforms) of holoprosencephaly. We describe their neonatal investigation, multidisciplinary management and clinical course.

Controlling acute bacterial maxillary sinusitis.

In acute maxillary sinusitis, after an initial viral episode, bacterial infection can be demonstrated using specific investigations and bacterial isolation. In the vast majority of cases, however, the diagnosis is presumptive. Symptomatic treatments tend to reduce pain and inflammation for easier pus drainage. The decision for or against antibiotic therapy continues to be a matter of debate. Many antibiotics have been used but modern guidelines have established recommendations for the choice and duration of treatments, based on (i) knowledge of pathogens and of their resistance profiles; (ii) …

Carotid thrombosis following neck irradiation.

Therapeutic irradiation may accelerate atherosclerosis, increasing the risk of vascular stenosis or occlusion several to many years following radiation. However, intimal damage following irradiation may result earlier in thrombosis without stenosis. This report discusses three cases of carotid occlusion that occurred within 3 years of moderate dose irradiation. Angiographic studies showed that occlusion occurred in the absence of atherosclerotic stenosis. A review of the literature supports the conclusion that people who receive neck irradiation are at risk not only for the delayed development

Bacteriology of chronic maxillary sinusitis in adults.

Aspirates of 72 chronically inflamed maxillary sinuses were processed for aerobic and anaerobic bacteria. Bacterial growth was present in 66 of the 72 specimens (92%). Anaerobic bacteria were isolated in 58 of the 66 culture-positive specimens (88%). Anaerobes only were recovered in 37 cases (56%) and in 21 (32%) they were mixed with aerobic or facultative bacteria. Aerobic or facultative bacteria were present in eight cases (12%). A total of 185 isolates (2.8 per specimen)--131 (2.0 per specimen) anaerobes and 54 (0.8 per specimen) aerobes or facultatives--were isolated. The predominant anaer…

Effects of phenylpropanolamine on ostial and nasal patency in patients treated for chronic maxillary sinusitis.

Ostial and nasal patency were measured in different body positions and after phenylpropanolamine (Ppa) treatment in 12 patients earlier treated for chronic unilateral maxillary sinusitis of varying severity. Six of the sinuses were of normal appearance on sinoscopy. Phenylpropanolamine in a single dose of 100 mg was found to increase the equivalent ostial diameter significantly in the sitting and recumbent positions. The drug also reduced the nasal airway resistance. The Ppa effect might be of benefit in patients treated for chronic maxillary sinusitis.

Bidirectional trigeminal perineural spread of adenoid cystic carcinoma via the pterygopalatine fossa.

Adenoid cystic carcinoma (ACC) is characterized by a strong propensity for perineural invasion (PNI), which complicates skull base evaluation and treatment planning. We report a case of buccal mucosal ACC demonstrating simultaneous proximal extension to the cavernous sinus and distal orbital involvement following infiltration of the pterygopalatine fossa (PPF). Contrast-enhanced magnetic resonance imaging (MRI) demonstrated continuous tumor propagation from the posterior superior alveolar nerve to the maxillary nerve (V2), with bidirectional spread originating from the PPF. Positron emission t

Case report of segmental odontomaxillary dysplasia with cutaneous manifestations.

BACKGROUND: Segmental odontomaxillary dysplasia is an uncommon nonhereditary growth disorder that affects the maxilla, gums and ipsilateral dentition. The disorder is diagnosed mainly based on dental (over-retention of primary teeth, dental agenesis and diastemas) and bone findings (bone sclerosis, irregular trabeculation of immature bone and reduced maxillary sinus). This paper provides a case report. CASE REPORT: A 5-year-old child with skin manifestations including hypertrichosis, facial erythema and pigmented nevus was diagnosed with type II segmental odontomaxillary dysplasia based on cl…
