Phosphorylated alpha-synuclein skin biopsy alters diagnosis in two out of three patients: Initial findings in a tertiary center.

BACKGROUND: Skin biopsy testing for phosphorylated alpha-synuclein (P-syn) is an emerging diagnostic tool for synucleinopathies, but its clinical utility remains to be properly defined. We thus aimed to assess its impact on diagnostic clarification and clinical management in parkinsonian patients in a tertiary movement disorders center. METHODS: We included all patients referred for synuclein skin biopsy at the Cleveland Clinic between January and December 2024. Demographic and clinical data, including biopsy results and their effects on diagnosis and management, were extracted from electroni

Alzheimer's Disease Continuum: Evaluating the Relationship between Fluid Biomarkers and Patients' Phenotype and Profile.

The measurement of fluid biomarkers such as phosphorylated tau at threonine 181 (pTAU181), amyloid-β 40 (Aβ40), and Aβ42 is a routine medical examination that contributes to achieving an accurate diagnosis within the Alzheimer's disease (AD) clinical continuum. The aim of this study is to compare the concentration of these biomarkers in CSF and plasma and determine their relationship with the patients' clinical variant and profile. Patients were diagnosed following the NIA-AA criteria. Plasma and CSF were obtained from healthy controls (HC), mild cognitive impairment (MCI non-AD), MCI displayi

A systematic review of the clinical correlates of delusions in Parkinson's disease.

INTRODUCTION: Research within Parkinson's disease (PD) psychosis has mostly focused on hallucinations due to their higher prevalence, limiting our understanding of the clinical correlates of delusions. This review aims to systematically review the content and clinical correlates of delusions in PD, including demographic, motor and non-motor symptoms and medication. METHOD: Following PRISMA Guidelines, a search on Ovid Medline, PsycINFO, Web of Science and Embase was performed to include English language studies evaluating delusions in PD. Data extraction included study and patient characteris

EASDnet: Empowering human-centered evidence-based medicine through an evidence and attention-based spatial disparity network for discriminative colorectal cancer histopathological screening and attribution.

PURPOSE: Accurate preoperative staging of colorectal cancer is critical to guide treatment decisions, including eligibility for R0 resection, to reduce recurrence and improve patient survival. However, conventional imaging evaluation depends on human experience, leading to subjectivity and diagnostic uncertainty in clinical interpretation. Therefore, an objective, evidence-based quantitative analysis tool is essential to assist clinicians. This study introduces and validates an evidence-based medicine (EBM) deep learning model that addresses these limitations by providing reliable, automated p

Explainable AI for chest radiographs: sex-stratified fairness auditing in CNN-based pneumonia detection.

Artificial intelligence (AI) systems for chest X-ray (CXR) interpretation can achieve high standalone diagnostic accuracy, yet limited transparency raises concerns about subgroup disparities. We audited sex-stratified performance and attention patterns in CNN-based pneumonia detection using the RSNA Pneumonia Detection Challenge dataset (26,684 frontal adult CXRs). ImageNet-pretrained InceptionV3 models were trained across five prespecified random seeds and combined as a seed-ensemble. Analyses used a prespecified, fixed, patient-level sex- and label-balanced test set (n = 1,000; 250 pneumonia

Development and Validation of a Computed Tomography-Based Convolutional Neural Network Model for Assisting Donor Lung Acquisition in Lung Transplantation.

Lung transplantation remains the only effective treatment for end-stage pulmonary disease. However, transplantation rates are significantly constrained by the ongoing shortage of donor lungs and the reliance on subjective assessments of donor lung quality. Although computed tomography (CT) is essential for donor lung evaluation, there is currently limited evidence supporting the use of automated prediction algorithms to determine donor lung suitability. This retrospective study included potential donors evaluated between May 2017 and July 2023 at the First Affiliated Hospital of Guangzhou Medi

From recruitment to regret: a crime script analysis of illegal kidney donation in Pakistan.

Organ transplantation is regarded as a lifesaving procedure, and a worldwide organ shortage has catalysed the market of illegal organ trade, especially for kidneys, due to high demand and need in low and middle-income countries like Pakistan. This process is often facilitated by sophisticated, organised crime networks acting within or parallel with licit healthcare systems, exploiting the vulnerable communities. Therefore, this study aims to explore the lived experiences of individuals who illegally donate their kidneys in Pakistan, with special focus on the whole process to map the findings o

Birth rates and migration: an AI and One Health approach within the Italian National Health System.

This study examines the future stability of Italy's health and social systems in the context of demographic decline. The Italian healthcare system, grounded in a universalistic and public model, founded on the principle of equity, is undergoing a phase of instability. The COVID-19 pandemic represented a critical rupture, exposing systemic weaknesses, territorial disparities, infrastructural inadequacies, the rigidity of financing mechanisms and the lack of reform regarding citizenship rights for foreign residents. An inductive, interpretive approach was adopted, starting with data analysis fro

The Face of Aidōs: From Helena's Beauty to Penthesilea's Dignity-When Achilles Learned to See the Other.

BACKGROUND: The human face has traditionally been studied as an anatomic structure and an aesthetic ideal. Ancient Greek mythology, however, offers a deeper ethical perspective in which the face becomes a medium of human recognition. This article examines the mythological encounter between Achilles and Penthesilea through the concept of aidōs (αἰδώς) and explores its relevance to contemporary craniofacial surgery and medical humanities. METHODS: Classic literary sources, including The Iliad and Posthomerica, were examined alongside studies of Greek ethics and Attic red-figure vase painting. T

ADLM Guidance Document on Laboratory Testing for Drugs of Misuse to Support the Emergency Department.

Toxicology testing for drugs associated with scenarios such as recreational use or substance use disorder can be performed in support of the emergency department (ED) for specific patient populations such as pediatrics and trauma. These compounds were historically referred to as drugs of abuse (DOA); although the word "abuse" is recognized as potentially stigmatizing, no replacement terminology for DOA has emerged in current guidelines. This document refers to these compounds as drugs or substances of misuse, and acknowledges the need for less-stigmatizing language that more fully encompasses

Beyond violence exposure: gender-specific psychological responses to violence in Ecuador - a propensity score analysis.

The consequences of violence differ depending on gender. This study explores the associations between exposure to violence and mental health outcomes in Ecuador, a country with high levels of violence yet limited research on its psychological effects. Using a nationally representative dataset and Propensity Score Matching (PSM) to reduce confounding bias, we examine gender-specific patterns of psychological outcomes. The analysis reveals that men exposed to violence are significantly more likely to display externalizing behaviors, including a near doubling of alcohol use (+93.6%) and heightene
