Cryptogenic Acute Fibrinous and Organizing Pneumonia: A Case Report.

We describe the case of a middle-aged male who presented with incidental findings of a left retrocardiac opacity seen on a chest radiograph. Subsequent computed tomography (CT) of the thorax showed multiple subpleural lesions with central necrosis. A diagnosis of acute fibrinous organizing pneumonia was made histologically upon biopsy of the subpleural lesions. The patient responded well to steroids, with full resolution of the subpleural lesions on repeat CT of the thorax. This report illustrates a case of acute fibrinous organizing pneumonia wherein the patient responded well to corticostero

Anaesthetic challenges in a patient with acromegaly and multinodular goitre undergoing endoscopic pituitary surgery.

Acromegaly is a progressive systemic disorder which is common among middle-aged women. A functioning growth hormone-secreting pituitary adenoma is the most common cause. Anaesthesia for pituitary surgery in patients with acromegaly is challenging. Rarely, these patients may develop thyroid lesions that may compromise the airway. We present the case of a young man with newly diagnosed acromegaly caused by a pituitary macroadenoma complicated by a large multinodular goitre. The aim of this report is to discuss the perianaesthetic approach in patients with acromegaly with a high risk of airway co

Laryngeal dystonia (LD) is a rare neurological disorder emerging in middle-aged persons as a chronic and disabling voice disorder. It is a focal dystonia affecting intrinsic laryngeal muscle control only during speech, resulting in voice breaks, effortful phonation, and strangled voice. Due to lack of awareness and lack of well-defined diagnostic criteria, it can be difficult for patients to be diagnosed and treated. This review, the first Danish publication on the subject of LD, presents the latest terminology, a brief history, treatment options and the psychosocial consequences of LD.

An Aberrant Seminoma as a Complication in an Undescended Testis: A Case Report and Review of Literature.

Testicular germ cell tumors are testicular neoplasms in young and middle-aged men. Undescended testis dramatically increases the risk of testicular germ cell tumors. We report the case of a 33-year-old male who complained of swelling and pain in his lower abdomen. The patient also had an undescended left testis. An intrabdominal mass was detected on ultrasound that was further characterized using contrast-enhanced CT. Imaging findings suggested testicular germ cell tumor, developing as a complication in the undescended testis. The patient was operated and the diagnosis was confirmed on histopa

Postoperative Purulent Chondritis of Thyroid Cartilage in a Diabetic Patient: A Case Report.

Purulent chondritis of the thyroid cartilage is a rare affliction that is not frequently observed in patients undergoing laryngeal surgery. The formation of abscesses between the inner and outer perichondria can cause constriction of the laryngeal lumen, leading to hoarseness and difficulty breathing. This report details the case of a middle-aged diabetic woman who experienced swelling in the tissue of her laryngeal cavity and destruction of her thyroid cartilage plate after undergoing laryngectomy. The pathological examination revealed evidence of inflammation, and the patient was ultimately

Congenital coronary anomalies present with a frequency of 0.7% in the general population. Most coronary anomalies are benign, but some may be linked to ischaemia and sudden cardiac death. In this case report, we describe the findings of a middle-aged man who was seen for unspecific cardiac symptoms. Echocardiography revealed a vascular abnormality recently recognised as an indicator of coronary anomaly, the retroaortic anomalous coronary sign. With this case, we wish to spread awareness of this sign to highlight what it indicates and what its potential implications are.

When dyspnea is a Hickam's Dictum.

Synchronous opportunistic infections are luckily rare in people living with HIV (PLWH) in the era of highly effective antiretroviral medications. We describe the case of a middle-aged man who presented with diarrhea and shortness of breath and was found to have pneumocystis pneumonia, disseminated histoplasmosis and disseminated mycobacterium avium complex infection along with a new diagnosis of human immunodeficiency virus (HIV) infection. This case highlights that individuals who remain undiagnosed with HIV infection for a long time can still present with concurrent infections and clinicians

Hemophagocytic Lymphohistiocytosis: A Rare Complication of COVID-19 in a Patient With Sickle Cell Anemia.

Hemophagocytic lymphohistiocytosis (HLH) is an uncommon condition that can be fatal due to overwhelming macrophage activation and cytokine production. It can be primary (familial/genetic) or secondary. It is associated with infections, malignancies, and rheumatologic and immunodeficiency disorders. We report a middle-aged female patient with sickle cell anemia who presented with COVID-19 infection that triggered a vaso-occlusive crisis and resulted in HLH. She had preexisting high ferritin levels and cytopenias, making the diagnosis more challenging. A high index of suspicion and timely treatm

Fibrosarcoma of the gingiva: A rare entity.

Fibrosarcoma is a rare malignancy of fibroblasts that seldom occurs in the oral cavity. Although rare, fibrosarcoma is an aggressive lesion associated with a poor survival rate. The occurrence of such a highly aggressive lesion on the gingiva is a diagnostic challenge for the clinician. This is because the gingiva is a common site for various inflammatory and nonspecific enlargements, which are commonly benign and the chances to overlook aggressive lesions are high. This case report describes an atypical case of fibrosarcoma of gingiva in a middle-aged individual that mimicked a nonspecific gi

[A particular presentation of a T cell large granular lymphocytic leukaemia].

T cell prolymphocytic leukemia (T-PLL) is a rare, aggressive neoplasm derived from post-thymic T cells. Patients are typically middle-aged with a slight male predominance who present with a high white blood cell count, hepatosplenomegaly, lymphadenopathy, and other symptoms typically associated with leukemia. Although cutaneous involvement has been reported in up to 30% of cases of T-PLL, to our knowledge, none have presented with a presentation resembling livedoid vasculopathy. In the correct clinical context, an underlying hematolymphoid neoplasm should be included in the differential diagno

Giant vegetation of the tricuspid valve in a healthy patient without risk factors for right-sided endocarditis.

We present the case of a middle-aged man who developed infective endocarditis of the tricuspid valve caused by methicillin-sensitive Staphylococcus aureus and associated with very large vegetation (∼5 cm). Besides the quite unusual size of the vegetation, this report highlights that severe right-sided endocarditis can occur in the absence of classical risk factors (intravenous drug abuse, presence of a cardiac implantable electronic device or other intravascular devices, and underlying right-sided cardiac anomaly) and that some cases of severe tricuspid endocarditis can be successfully treated

Case report: Cutaneous pseudolymphoma caused by a Leishmania infantum infection in a patient treated with anti-TNF antibody for plaque psoriasis.

For psoriasis, which affects up to 2% of the population and adalimumab is approved from the age of 4 years. Here, we present a middle-aged Italian man with long-term history of plaque psoriasis and psoriasis arthropathica and adalimumab therapy. He developed ulcers or nodules within the psoriatic plaques, resembling cutaneous infection with Leishmania infantum. TNF and other cytokines such as IL-12 and IFN-γ are central in the early control of the infection. Discontinuation of the anti-TNF-treatment resolved the infection without specific therapy.

Clinical Characteristics of 9 Adult Patients with Granulomatous Periorificial Dermatitis and Comparison with Childhood Granulomatous Periorificial Dermatitis.

To our knowledge, there have been no previous reports of granulomatous periorificial dermatitis (GPD) in adult patients in contrast to childhood GPD (CGPD). We report cases of 9 adult patients with GPD with regards to the clinical and histopathological characteristics and their management. GPD in adults may be an entity that is actually underdiagnosed, especially in middle-aged females. It is a benign disorder, albeit requiring a relatively long-term treatment. Unlike CGPD, GPD in adults is frequently accompanied by pruritus with predilection for the eyelid and should be treated initially with

Dissociative Aggression Triggered by Headache.

Headache is generally perceived as a negative symptom focused on oneself. However, there are reports suggesting that patients suffering from pain, especially headache, can be aggressive. The precise nature of the link between headache and aggression is not known. Here, we describe a homicidal attack, triggered by headache, in a middle-aged man. The patient's background and the characteristics of the attack suggested a dissociative behavior. The case shows that headache may be a trigger for homicidal behavior. Case-control studies are needed to determine the prevalence of aggressive tendencies

Pancreatic tuberculosis: An unusual presentation.

Tuberculosis (TB) of pancreas is a rare presentation in both immune-competent and immune-suppressed patients. Its presenting clinical features are usually vague and non-specific, while radiological features mimic other common pancreatic conditions such as malignancy, so it is often misdiagnosed. It commonly involves the head and the uncinate process of the pancreas. We report a middle-aged immune-compromised man who presented with left-sided tubercular pleural effusion and later was diagnosed as pancreatitis by clinical presentation and TB of pancreas on computed tomography of the abdomen.

Spontaneous dissection of bilateral internal carotid and vertebral arteries.

Spontaneous dissection of the major arteries of the neck is known to increase the risk of stroke or transient ischaemic attack in young and middle-aged adults. Most of the reported cases of arterial dissections in the neck involve one or both paired extracranial carotid or vertebral arteries. Spontaneous dissection of the bilateral internal carotid and vertebral arteries is extremely rare. We report a case of spontaneous bilateral internal carotid artery and vertebral artery dissection while using a prescribed pill for weight loss which contained amphetamine derivative. A review of literature

A case of cholesterol embolization syndrome mimicking rapidly progressive renal failure causing end-stage kidney disease.

Cholesterol embolization syndrome is an uncommon complication of coronary angioplasty. Its clinical manifestations are nonspecific and may be ascribed to other causes mimicking vasculitis syndrome. In an appropriate clinical setting, the diagnosis can be confirmed by tissue biopsy. In this case report, we present a middle-aged male who presented with cutaneous and renal manifestations within two weeks of primary angioplasty. The patient had progressive clinical deterioration in the form of dry gangrene of toes and end-stage renal disease requiring surgical amputation and maintenance hemodialys

Challenges in electromyographic endotracheal tube (EMG ETT) placement via awake fibreoptic intubation during emergency thyroidectomy.

Recurrent laryngeal nerve (RLN) injury is one of the main complications of total thyroidectomy. If the injury is bilateral, total airway obstruction, aphonia and hoarseness of voice could be precipitated. Hence, it is wise for the operating surgeon to be guided by neural monitoring during thyroidectomy. We present a valuable experience handling a middle-aged man with a huge papillary thyroid carcinoma . He needed an urgent thyroidectomy due to obstructive symptoms. We highlight our intraoperative dexterity in handling his surgery in the context of continuous monitoring of RLN using electromyog

Short-Term Resolution of Herniated Disk Mimicking Tumor.

We report a middle-aged man with a history of back pain and right-sided sciatica. Magnetic resonance imaging revealed an atypical disk fragment with radiologic characteristics of a spinal tumor. Follow-up imaging shows resolution of the lesion. Clinical symptoms resolved simultaneously. Clinical and radiologic characteristics cannot distinguish atypical disk herniation from tumors. While no imaging method allows distinguishing between these 2 entities, resolution of a disk herniation is not uncommon. In the absence of muscle weakness, follow-up imaging rather than surgery is advised in order t
