Ossification of the mandible: A regional composite model of intramembranous, parachondral, and endochondral mechanisms.

OBJECTIVE: To catalogue the ossification mechanisms operating along the proximal-distal axis of the human mandible and integrate them into a unified regional framework. DESIGN: Narrative review of embryological, molecular, transcriptomic, and clinical genetics evidence identified through structured searches of PubMed, Scopus, and Web of Science. The type of evidence (human, animal, in vitro, or combined) underpinning each major finding was tracked. RESULTS: Five non-overlapping mechanisms operate across ten mandibular regions: (i) intramembranous ossification of the body, ramus, and coronoid

Comparison of the long-term survival rates of dental implants in onlay versus interpositional bone grafts for augmentation of the atrophic mandibular ridge.

This retrospective cohort study was performed to compare the long-term survival rates of implants placed following onlay versus interpositional bone grafting in the atrophic mandible. Forty-eight patients with an adequate bone width who required ridge heightening were included. Twenty-six patients received an interpositional bone graft by sandwich technique (sandwich group) and 22 received an onlay graft (onlay group). The results showed a significant difference in implant failure: 17 of 52 implants (32.7%) placed in the onlay group failed, compared with 1 of 63 implants (1.6%) placed in the s

Stem cells from human exfoliated deciduous teeth as an alternative cell source for cartilage regeneration and repair.

PURPOSE: Stem cells from human exfoliated deciduous teeth (SHED) hold promise for cartilage regeneration due to their chondrogenic potential and minimally invasive procurement. However, the in vivo cartilage regenerative capacity of SHED remains insufficiently explored. This study aims to compare the proliferative, migratory, and chondrogenic capacity of SHED and dental pulp stem cells (DPSCs) in vitro, and to investigate the therapeutic efficacy of SHED delivered via alginate hydrogel in a rat model of knee cartilage defect. METHODS: SHED was isolated from human exfoliated deciduous teeth, a

Uncertainty-Driven Parallel Transformer-Based Segmentation for Oral Disease Dataset.

Accurate oral disease segmentation is a challenging task, for three major reasons: 1) The same type of oral disease has a diversity of size, color and texture; 2) The boundary between oral lesions and their surrounding mucosa is not sharp; 3) There is a lack of public large-scale oral disease segmentation datasets. To address these issues, we first report an oral disease segmentation network termed Oralformer, which enables to tackle multiple oral diseases. Specifically, we use a parallel design to combine local-window self-attention (LWSA) with channel-wise convolution (CWC), modeling cross-w

Stimuli-activable hollow Cu2O@C/N doped paste for the prevention of white spot lesions in orthodontic treatments.

White spot lesions (WSLs) are the most common complication of orthodontic treatment, compromising dental health and significantly affecting aesthetics. To address this clinical challenge, this study aims to develop a dual-functional therapeutic strategy that simultaneously promotes the remineralization of demineralized enamel and inhibits the activity of cariogenic bacteria, thereby achieving effective prevention and treatment of WSLs. A hollow double-shell structured Cu2O@N/C nanozyme (H-Cu2O@N/C) was synthesized using a one-step hydrothermal method. The thin nitrogen-doped carbon shell signi

Dual-targeting nanotherapy disrupts fungal-bacterial synergy to reprogram inflammatory microenvironments in periodontitis.

Conventional anti-biofilm strategies for periodontitis predominantly focus on bacterial populations, often inadvertently facilitating the resurgence of fungi and aggravating the persistence of biofilms. Here, we report a pH-responsive copper-gallic acid core-shell nanoplatform (CGC@HSAF NP) that eradicates drug-resistant fungal-bacterial biofilms through cross-kingdom synergy, marking a significant advancement over traditional species-specific methodologies. The calcium carbonate shell facilitates an acid-triggered release of the antifungal agent HSAF, which selectively disrupts fungal membran

Role of Injectable Platelet-Rich Fibrin in the Management of Soft and Hard Tissue Periodontal Regeneration in Dentistry: Protocol for a Systematic Review.

BACKGROUND: Injectable platelet-rich fibrin (i-PRF) has the capacity to release great amounts of several growth factors, as well as to stimulate increased fibroblast migration and the expression of collagen, transforming growth factor β, and platelet-derived growth factor. Consequently, i-PRF can be used as a bioactive agent to promote periodontal tissue regeneration. OBJECTIVE: We aim to compare and evaluate the effectiveness of i-PRF in periodontal tissue regeneration. METHODS: We will conduct an electronic search in the following databases: PubMed, Cochrane Library, Google Scholar, Semant

Long-Term Occlusal Outcomes for Conservative Management in Patients With Robin Sequence.

INTRODUCTION: Robin sequence (RS), the triad of micrognathia, glossoptosis, and airway obstruction, is treated with conservative management in milder cases with the expectation that mandibular growth will normalize over time. Mandibular distraction osteogenesis (MDO) is a surgical intervention utilized in more severe cases. We aim to identify the long-term outcomes in conservatively managed RS patients to identify differences in mandibular growth and structural airway pathology. METHODS: A retrospective review of patients with RS at a tertiary pediatric hospital was conducted from 2002 to 202

α-Adrenoreceptor blocker phentolamine inhibits voltage-gated sodium channels via the local anaesthetic binding site.

BACKGROUND AND PURPOSE: Phentolamine is a non-selective α-adrenoreceptor antagonist used to reverse local anaesthesia, for example, during dental procedures when a vasoconstrictor is co-applied. Phentolamine-mediated vasodilation leads to faster clearance of injected drugs. Previous electrophysiological studies hypothesized that phentolamine acts as a modulator of voltage-gated sodium channels, which could conflict with its indication as local anaesthetic reversal agent. EXPERIMENTAL APPROACH: We performed manual and high throughput patch-clamp recordings on HEK and CHO cells expressing NaV1.

Compare the accuracy and performance of two different types of CAD-CAM surgical guides for maxillary positioning in orthognathic surgery.

Three-dimensional computer-aided design/manufacturing (CAD/CAM) maxillary guides have been developed to enhance surgical predictability, reducing mandibular interference and improving planning accuracy, especially in asymmetrical cases. This study aimed to compare the accuracy of two types of CAD/CAM guides used for maxillary positioning in orthognathic surgery. In this retrospective cohort study, 38 patients who underwent bimaxillary orthognathic surgery by the same surgeon were divided into two groups. Group 1 (n = 21) used a tooth-bone-supported guide with dual functionality for maxillary p

Single-cell sequencing systematically analyzed the mechanism of Emdogain on the restoration of delayed replantation periodontal membrane.

The repair of the periodontal membrane is essential for the successful management of periodontal disease and dental trauma. Emdogain® (EMD) is widely used in periodontal therapy due to its ability to promote repair. Despite substantial research, the cellular and molecular mechanisms underlying EMD's effects, particularly at the single-cell resolution, remain incompletely understood. This study established a delayed tooth replantation model in rats to investigate these aspects. Tooth loss rate and degree of loosening were evaluated at 4 and 8 weeks. Micro-CT, HE staining, TRAP staining, and imm

Mdivi-1 alleviates aseptic craniofacial osteolysis via inhibition of PINK1/Parkin-dependent mitophagy.

Aseptic craniofacial osteolysis around the implant-bone interface, induced by wear particles, leads to the loosening and failure of dental implants, temporomandibular joint prostheses and internal fixation during maxillofacial reconstruction. Osteoclasts, as terminally differentiated multinucleated giant cells and the exclusive bone resorptive cells, play an important role in this pathological process. The PINK1/Parkin pathway is involved in mitochondrial quality control; however, its effects on osteoclast-mediated physiological bone homeostasis and the therapeutic potential on craniofacial os

Odanacatib Coating Supports Osseointegration of Implants: A Preclinical Study.

INTRODUCTION: Odanacatib (ODN), a cathepsin K inhibitor, is a drug that reduces bone resorption while preserving bone formation. ODN was initially developed for the treatment of postmenopausal osteoporosis, but further development as a systemic medication has been discontinued. Here, we propose ODN as a topical treatment, the coating of dental implants, to achieve an anabolic shift of early osseointegration. MATERIAL AND METHODS: To this aim, we have coated double acid etching titanium implants (SLA) with and without ODN dispersed in a simulated body fluid (SBF). The implants were inserted in

Enhanced Sutural Protraction: An Innovative Orthopedic Protocol for Midfacial Advancement in Growing Patients.

Midfacial hypoplasia is a common condition in clinical practice. It is characteristic of various craniofacial syndromes but can also occur as an isolated form of dento-skeletal dysmorphosis. Managing this condition in growing patients is challenging. Surgical approaches have high relapse rates, whereas orthodontic treatments yield variable results in midface advancement and stability. This study evaluates the effectiveness of the Meazzini-Mazzoleni Enhanced Sutural Protraction (MMESP) protocol for correcting severe midfacial hypoplasia in growing patients. Ten patients, with or without associa

Reconstructing Critical-Sized Mandibular Defects in a Rabbit Model: Enhancing Angiogenesis and Facilitating Bone Regeneration via a Cell-Loaded 3D-Printed Hydrogel-Ceramic Scaffold Application.

In this study, we propose a spatially patterned 3D-printed nanohydroxyapatite (nHA)/beta-tricalcium phosphate (β-TCP)/collagen composite scaffold incorporating human dental pulp-derived mesenchymal stem cells (hDP-MSCs) for bone regeneration in critical-sized defects. We investigated angiogenesis and osteogenesis in a rabbit critical-sized mandibular defect model treated with this engineered construct. The critical and synergistic role of collagen coating and incorporation of stem cells in the regeneration process was confirmed by including a cell-free uncoated 3D-printed nHA/β-TCP scaffold, a

Effectiveness of keratinized mucosa augmentation procedures around dental implants based on risk assessment: A 5-year retrospective cohort study.

PURPOSE: To compare the effectiveness of keratinized mucosa (KM) augmentation with different techniques for the treatment of dental implants based on risk assessment. METHODS: Thirty-nine patients who underwent KM augmentation at implant sites in the posterior mandible were included. Three techniques were used based on anatomy-guided risk assessment: an apically positioned flap (APF) alone, an APF plus a free gingival graft (APF plus FGG), and an APF plus a collagen matrix (APF plus CM). Clinically effective KM augmentation was defined as remaining KM ≥ 2 mm after the intervention. The effect
