[Progressive anarthria: one case without lingual apraxia].

Progressive anarthria is a focal cortical degenerative disorder characterized by a profound, progressive alteration in speech without impairment in other cognitive domains. The first symptoms consist of an alteration in the articulation of speech producing telegraphic speech. The disorder invariably progress towards anarthria by deprogramming of the phonation and orolingual movements (bucophonetic apraxia). This type of apraxia is usually associated with orolinguofacial apraxia and it has been anatomofunctionally correlated with frontal opercular involvement of left predominance. The patient p

[Three Japanese children with X-linked alpha-thalassemia/mental retardation syndrome (ATR-X)].

X-linked alpha-thalassemia/mental retardation syndrome (ATR-X), which was first reported by Wilkie, et al. in 1991, is a disorder with severe mental retardation, characteristic facial appearance, genital abnormalities, and mild form of alpha-thalassemia. At present, about 50 cases have been reported in the world, but few in Japan. We report 3 cases of this disorder in 2 families. All cases prefer a peculiar posture and show unique movements, such as self-induced vomiting or self-hanging, which can be diagnostic. ATR-X should be considered as a differential diagnosis in all male patients with s

Pharmacologic characteristics of Meige dystonia: differentiation from tardive dyskinesia.

Idiopathic orofacial dyskinesia, also called Brueghel's syndrome, blepharospasm-oromandibular dystonia, and Meige dystonia, is characterized by involuntary facial movements. Since this disorder can be difficult to distinguish from tardive dyskinesia, we have generated a neuropharmacologic profile of Meige dystonia. Symptoms were improved by antagonists of both dopamine and acetylcholine and worsened by the cholinergic agonist physostigmine, consistent with a hypothesis of relative excess in both dopamine and acetylcholine neuronal activities. Since tardive dyskinesia is hypothesized to be char

Tourette's syndrome: a treatable tic.

Tourette's syndrome, or Gilles de la Tourette's disease, is a disorder characterized by involuntary tic-like muscular movements, compulsive behaviour and involuntary vocalization of sounds, words or profanities. It begins in childhood and may persist for life, with a varied pattern and course. Recent studies indicate an organic basis for the disorder, and an abnormality of dopamine or purine metabolism has been suggested. The treatment of choice is haloperidol administration; most patients do well with low or moderate doses for long periods. Because these patients are often mistakenly regarded

Depression mediated sleep disturbances and cognitive decline in Parkinson's disease patient.

OBJECTIVE: This study investigated the distinct mechanistic pathways linking depression, sleep disturbances, and cognitive decline in Parkinson's disease, with emphasis on differential roles of daytime sleepiness versus rapid eye movement sleep disorder. PARTICIPANTS: The study included 450 Parkinson's disease patients enrolled in the Parkinson's Progression Markers Initiative (PPMI) between 2010 and 2024, leveraging longitudinal clinical and biomarker data. MEASUREMENTS: Depressive symptoms (Geriatric Depression Scale, GDS), sleep disturbances (Epworth Sleepiness Scale, ESS for daytime slee

CSF1R-Related Disorder: Prevalence of CSF1R Variants and Their Clinical Significance in the UK Population.

BACKGROUND AND OBJECTIVES: CSF1R-related disorder (CSF1R-RD) is a devastating neurodegenerative disorder caused by variants in the colony stimulating factor-1 receptor (CSF1R) gene. CSF1R-RD leads to a variable combination of cognitive impairment, movement disorders, upper motor neuron signs, and spasticity with associated imaging abnormalities in brain white matter. Although increasingly recognized, there is evidence that it is significantly underdiagnosed or misdiagnosed, and its true prevalence is unknown. We leveraged the large data set of the UK Biobank to determine the prevalence of CSF1…

New insights from a multi-ethnic Asian progressive supranuclear palsy cohort.

BACKGROUND: Progressive supranuclear palsy (PSP) is a rare, disabling, neurodegenerative disease, with few studies done in Asian populations. METHODS: We prospectively characterized the clinical features and disease burden in a consecutively-recruited multi-ethnic Asian PSP cohort. Patients were extensively phenotyped using the Movement Disorder Society (MDS-PSP) clinical diagnostic criteria and the PSP-Clinical Deficits Scale (PSP-CDS). Caregiver burden was measured using the modified Zarit Burden Interview (ZBI). Investigations (neuroimaging and genetic tests) were reviewed. RESULTS: There

Current Guidelines for Classifying and Diagnosing Cervical Dystonia: Empirical Evidence and Recommendations.

BACKGROUND: The dystonias are phenotypically and etiologically heterogenous disorders. Many proposals and a consensus recommendation have been provided for the diagnosis and classification of the dystonias, but these recommendations serve only as general guidelines. Current diagnosis and classification may still depend on clinical judgment causing different opinions. OBJECTIVE: To delineate clinical features used by movement disorder specialists in the diagnosis and classification of isolated focal cervical dystonia, and to develop recommendations for a more consistent approach to classificat

Repetitive electric stimulation elicits enduring improvement of sensorimotor performance in seniors.

Age-related changes occur on all stages of the human somatosensory pathway, thereby deteriorating tactile, haptic, and sensorimotor performance. However, recent studies show that age-related changes are not irreversible but treatable through peripheral stimulation paradigms based on neuroplasticity mechanisms. We here applied repetitive electric stimulation (rES) to the fingers on a bi-weekly basis for 4 weeks to induce enduring amelioration of age-related changes in healthy individuals aged 60-85 years. Tactile, haptic, and motor performance gradually improved over time of intervention. After

Sedentary behavior, physical activity, psychological well-being, and gynecologic and obstetric disorders: A multivariable Mendelian randomization and mediation analysis.

The independent causal effects of interrelated lifestyle behaviors and psychological states on women's gynecologic and obstetric health remain incompletely understood. We conducted a comprehensive Mendelian randomization (MR) study to disentangle the effects of leisure screen time, moderate-to-vigorous physical activity (MVPA), experiencing mood swings, and the well-being spectrum on a wide range of these disorders. We performed 2-sample MR analyses using data from the UK Biobank and FinnGen cohorts for 20 outcomes, with causal estimates combined via meta-analysis. The analytical framework inc

Unique sleep disorders profile of a population-based sample of 747 Hmong immigrants in Wisconsin.

Concerns regarding sleep disorders in Hmong immigrants in the US emerged when an astonishingly high mortality rate of Sudden Unexplained Nocturnal Death Syndrome (SUNDS) was documented in Hmong men. Stress, genetics, and cardiac abnormalities interacting with disordered sleep were hypothesized as contributing factors to SUNDS. Most recently, sleep apnea has been implicated in nighttime deaths of Brugada Syndrome. This syndrome is thought to comprise a spectrum of sudden cardiac death disorders, including SUNDS. However, little research since has placed SUNDS in its context of Hmong cultural be

[Non-epileptic motor paroxysmal phenomena in wakefulness in childhood].

Paroxysmal events in childhood are a challenge for pediatric neurologists, given its highly heterogeneous clinical manifestations, often difficult to distinguish between phenomena of epileptic seizure or not. The non-epileptic paroxysmal episodes are neurological phenomena, with motor, sensory symptoms, and/or sensory impairments, with or without involvement of consciousness, epileptic phenomena unrelated, so no electroencephalographic correlative expression between or during episodes. From the clinical point of view can be classified into four groups: motor phenomena, syncope, migraine (and a

Illness Anxiety Disorder Manifesting as Huntington's Disease: A Rare Case Report.

Illness anxiety disorder (IAD) is a psychological disorder characterized by an exaggerated preoccupation with the possibility of having a serious illness even in the absence of somatic symptoms. A 38-year-old woman presented to the clinic complaining of rhythmic and involuntary movements of her head and body. She had a history of chronic stress and recurrent significant loss, and she grew up in a highly emotional family. She remained preoccupied with the possibility of Huntington's disease, despite unremarkable neurological and laboratory findings. In the past, there have been some reports abo

[Diagnostics and treatment of suicidality; a matter of customization].

This article illustrates the importance of conducting a comprehensive analysis of suicidality through the case study of an adolescent patient dealing with both depressive disorder and obsessive-compulsive disorder. The aim of treating suicidality is to address the underlying psychiatric conditions and factors contributing to the disorder. This necessitates a thorough evaluation of the treatment environment, the establishment of continuous care, and ensuring safety. By utilizing a new model to distinguish various forms of suicidal behavior and examining suicidality as a distinct phenomenon, it

Early onset epileptic and developmental encephalopathy and MOGS variants: a new diagnosis in the whole exome sequencing (WES) ERA : Report of a new patient and review of the literature.

Mannosyl-oligosaccharide glucosidase - congenital disorder of glycosylation (MOGS-CDG) is determined by biallelic mutations in the mannosyl-oligosaccharide glucosidase (glucosidase I) gene. MOGS-CDG is a rare disorder affecting the processing of N-Glycans (CDG type II) and is characterized by prominent neurological involvement including hypotonia, developmental delay, seizures and movement disorders. To the best of our knowledge, 30 patients with MOGS-CDG have been published so far. We described a child who is compound heterozygous for two novel variants in the MOGS gene. He presented Early In…

Excessive fragmentary myoclonus (EFM) is a polysomnographic finding registered by the surface electromyography (EMG) and characterized as a result of the muscle activity consisting of sudden, isolated, arrhythmic, asynchronous and asymmetric brief twitches. The EMG potentials are defined by the exact criteria in The International Classification of the Sleep Disorders, 3rd edition and they appear with high intensity in all sleep stages. Clinical significance of EFM is unclear. It was observed in combination with other diseases and features such as obstructive and central sleep apnea, narcolepsy
