Successful therapy with ABLC, surgery and posaconazole for Rhizopus microsporus var. rhizopodiformis liver eumycetoma in a child with acute leukaemia.

Invasive fungal infection negatively influences the morbidity and mortality in heavily immuno-incompetent patients. Diagnosis of non-Aspergillus mould infections remains challenging despite application of a wide spectrum of non-culture-based microbiological techniques. Invasive diagnostic procedures are often essential. In this article, we present the case of a 15-month-old boy diagnosed with Rhizopus microsporus var. rhizopodiformis liver mycetoma during induction chemotherapy for acute promyelocytic leukaemia. Following surgery, he was effectively treated with a combination of ABLC and posac

Majocchi's granuloma of the scrotum.

Majocchi's granuloma is a well known but uncommon folliculitic and perifolliculitic dermatophyte infection of the dermal and subcutaneous tissue by fungal organism, usually limited to the superficial dermis. We report a rare case of superficial perifollicular form of Majocchi's granuloma, caused by Trichophyton rubrum on the scrotal skin of a healthy man. Histological examination of the biopsied nodule revealed perifolliculitis with fungal elements and the periodic acid schiff staining was positive to fungal elements. His lesion cleared completely with the use of a topical antifungal agent.

Isolation of Cryptococcus neoformans from hollows of living trees in the city of Alfenas, MG, Brazil.

Cryptococcus neoformans is an encapsulated yeast, aetiological agent of cryptococcosis, commonly associated with pigeon droppings and plant materials. The species has also been associated with tree hollows. The aim of the present work was to verify the presence of the yeast in hollows of living trees and identify the isolates obtained in varieties and serotypes. Three samples were collected from 18 trees of five different species totalling 54 samples. Wood samples were collected by scraping the surface of the trunks and the inner face of the hollows. Samples were inoculated on to agar Niger me

Primary zygomycosis of the pericardium in an immunocompetent host.

The fungi in the order Mucorales commonly target immunocompromised hosts, especially diabetics and produce fulminating respiratory and/or central nervous system infections, which are often fatal. We report primary zygomycotic pericarditis accompanied by osteomyelitis of the sternum in an immunocompetent female: a rare site and a rare setting. The disease was clinically thought to be of tuberculous aetiology and had evaded diagnosis for a year.

Oral submucous fibrosis and oral yeast carriage - a case control study in Sri Lankan patients.

Oral submucous fibrosis (OSMF) is a well-known precancerous condition. Epithelial atrophy is one of the key features in OSMF. Presence of Candida in the mouth together with epithelial changes may predispose to candidal infection. Candidal infection together with other co-factors may also induce epithelial atypia and dysplasia leading to malignant change. The purpose of this study was to determine the prevalence of oral yeast carriage in patients with OSMF and to compare the carriage with the normal individuals. Thirty patients with histologically proven OSMF and healthy subjects were used as t

Scedosporium apiospermum infection after near-drowning.

Scedosporium apiospermum and its teleomorph (sexual form) Pseudallescheria boydii are ubiquitous saprophytic fungi, which under specific conditions, such as near-drowning, may cause therapy-refractory and life-threatening infections. We reviewed 22 cases (eight children and 14 adults) of S. apiospermum infection after near-drowning reported in the literature including an additional paediatric case from our institution. Scedosporiosis after near-drowning was associated with high mortality (16/23, 70%) even in immunocompetent hosts. It affected mainly young (mean age 24 years) and immunocompeten

Invasive fungal infections among inpatients with acquired immune deficiency syndrome at a Chinese university hospital.

Invasive fungal infections (IFIs) have become a major cause of morbidity and mortality among people with acquired immune deficiency syndrome (AIDS), however, little is known about the clinical features and prognosis of IFI in AIDS in China. This study aimed to characterise the clinical features and prognosis of IFI in AIDS patients in China. We retrospectively reviewed the records of all HIV-infected patients at a Chinese university hospital between December 2004 and May 2006. We identified 35 patients with IFI. IFIs included thrush, oesophageal candidiasis, fungal pneumonia, cryptococcosis, p

Use of diagnostic tests by dermatologists, podiatrists and family practitioners in the United States: pilot data from a cross-sectional survey.

Before treating onychomycosis, it is important to exclude other conditions such as lichen planus and psoriasis. The purpose of this study was to evaluate physician preferences and uses of diagnostic tests for toenail onychomycosis (TO) by surveying dermatologists (D), podiatrists (P) and family practitioners (FP) in the United States. Surveys were mailed to approximately 1000 randomly sampled physicians from each of the three specialities. The questionnaire consisted of 15 items regarding physician and practice characteristics, number of patients with TO seen and treated, tests used to diagnos

Isolation of Sporothrix schenckii from the environmental sources of cutaneous sporotrichosis patients in Himachal Pradesh, India: results of a pilot study.

Himachal Pradesh, India is a known endemic area for cutaneous sporotrichosis. No attempt has been made to isolate Sporothrix schenckii, the causative fungus, from environmental sources in this region or in India as such. This prospective study was carried out to isolate Sporothrix schenckii from different environmental samples collected from the vicinity of cutaneous sporotrichosis patients. All patients of cutaneous sporotrichosis diagnosed during March 2005-February 2006 were studied. Twenty-one biopsy specimens and 62 environmental samples of soil, various thorns, corn-stalk, grass-blades a

Diagnosis of paracoccidioidomycosis by a dot blot assay using a recombinant Paracoccidioides brasiliensis p27 protein.

A variety of immunological methods have proven useful for Paracoccidioidomycosis (PCM) diagnosis; however, they are often time consuming and many lack sensitivity and specificity, partially attributed to the use of crude antigens, which give cross reactivity. Until now, attempts to clone and express Paracoccidioides brasiliensis immunodominant antigens have presented difficulties of process and problems of cost. In an attempt to obtain a more rapid, sensitive, and specific test for PCM diagnosis, we subcloned the P. brasiliensis p27 gene and used the recombinant protein as the antigen in dot b

Mycological evaluation of bronchoalveolar lavage in cats with respiratory signs from Rio de Janeiro, Brazil.

Twenty-three cats with respiratory signs who had domiciliary contact with cats with sporotrichosis were studied. Sneezing was the predominant extracutaneous sign. Twelve cats had no skin lesions and 11 had ulcerated skin lesions. Mycological culture of material obtained from the nasal cavity, oral cavity, bronchoalveolar lavage (BAL) and skin lesions, when present, was performed for all cats. In the case of autopsy, lung fragments were cultured. Sporothrix schenckii was isolated from four of the 12 cats without skin lesions: BAL (one cat) and oral and/or nasal cavity (three cats). The latter t

Pneumocystis jiroveci thyroiditis: report of 15 cases in the literature.

The authors review the epidemiology, clinical manifestations, diagnosis and treatment of Pneumocystis jiroveci thyroiditis of 15 cases reported in the medical literature. Patients with acquired immunodeficiency disease syndrome were particularly at risk. P. jiroveci thyroiditis was diagnosed at autopsy as a part of disseminated infection in a substantial number of patients without clinical manifestations and laboratory evidence of thyroid dysfunction. Local signs and symptoms of infection were indistinguishable from other infectious thyroiditis and included neck enlargement with or without cer

Candida lusitaniae arthritis in an intravenous drug user.

A case of arthritis of the right knee caused by Candida lusitaniae in a 29-year-old intravenous drug abuser is described. The diagnosis was based on the isolation of C. lusitaniae from synovial fluid and was supported by the presence of C. lusitaniae-specific DNA and high levels of (1-3)-beta-d-glucan (122 pg ml-1) in the same specimen. While the isolate was susceptible to amphotericin B and fluconazole in vitro, treatment with amphotericin B was not very effective. The patient achieved complete cure with fluconazole therapy only after undergoing synovectomy. To the best of our knowledge, this

Dose-finding comparative study of 2 weeks of luliconazole cream treatment for tinea pedis--comparison between three groups (1%, 0.5%, 0.1%) by a multi-center randomised double-blind study.

Luliconazole is a newly developed imidazolyl antifungal agent. A randomised double-blind comparative study was designed to assess the efficacy and safety of 1% luliconazole cream (group A), 0.5% cream (group B) and 0.1% cream (group C), in tinea pedis (interdigital type and plantar type), when used once daily for 2 weeks. Follow-ups were performed at 4 weeks after the end of topical treatment. A total of 241 patients were enrolled and 213 patients were evaluated for efficacy. Rates of improvement of skin lesions in the A, B and C groups assessed at week 4 were 90.5%, 91.0% and 95.8%, respectiv

A case of tinea corporis purpurica.

We present a case of tinea corporis purpurica localized to the legs of a 47-year-old woman. Trichophyton rubrum was the aetiological agent. So far, only a single report has been made on tinea corporis purpurica. Although the exact mechanism of purpura formation is unknown, we postulate the virulence of the fungal strain and host immune response to play a role.

Widespread tinea corporis caused by Trichophyton rubrum with non-typical cultural characteristics--diagnosis via PCR.

From the epidemiological point of view, Trichophyton rubrum is the most important dermatophyte in the Western world. Mostly, the lesions of fungal infections of the skin are restricted and circumscribed. Immunodeficiency, diabetes or treatment with steroids, however, favour widespread disease involving the entire integument. In the present study, we describe a patient without immunodeficiency or diabetes presenting with widespread tinea corporis caused by T. rubrum. The fungal isolate showed atypical morphological features. Moreover, the typical metabolic features (negative urease test, produc

Atypical manifestations of tinea corporis.

Tinea corporis classically presents as an erythematous annular plaque with a scaly, centrifugally advancing border. However, sometimes vesicles and pustules are observed. Occasionally, even frank bullae appear secondary to severe inflammation. Diagnostic difficulties arise when atypical manifestations mimic other inflammatory skin diseases, including atopic or seborrheic dermatitis, subacute cutaneous lupus erythematosus, or vesicular diseases. We report five cases of atypical tinea corporis, where the initial clinical diagnosis was different from dermatophytosis. The differential diagnoses an

Chromoblastomycosis with underlying osteolytic lesion.

In chromoblastomycosis, the lesions develop at the site of inoculation and usually restrict themselves to cutaneous and subcutaneous tissues. Extracutaneous spread occurs rarely secondary to haematogenic and lymphatic dissemination. This report presents a case of chromoblastomycosis due to Fonsecaea pedrosoi with contiguous spread to the underlying bone in the form of an osteolytic lesion.

Malassezia species isolated from lesional and non-lesional skin in patients with pityriasis versicolor.

Pityriasis versicolor (PV) is a superficial fungal infection where Malassezia species play a definite causative role, but the clinical significance of each of these species is not fully understood. The aim of our study was to analyse the prevalence of Malassezia species in PV lesions and to examine if the range of species varies with patient sex, age, direct microscopy findings and some clinical data. Ninety patients with PV completed the study. The samples were obtained by scraping the skin surface, both from lesional and non-lesional skin and then incubated on Sabouraud dextrose agar and mod
