Incidence and risk of postherpetic neuralgia after varicella zoster virus infection in hematopoietic cell transplantation recipients: Hokkaido Hematology Study Group.

To assess the incidence of and risk factors associated with postherpetic neuralgia (PHN) after hematopoietic cell transplantation (HCT) varicella zoster virus (VZV) infection, we conducted a retrospective chart review of 418 consecutive patients who underwent HCT between April 2005 and March 2007. The male/female ratio was 221/197, median age at HCT was 47 years (range: 0-69 years), and autologous/allogeneic/syngeneic HCT ratio was 154/263/1. Seventy-eight patients developed VZV infection after HCT. Sixty-two patients had localized zoster, 11 patients had disseminated zoster (rash like chicken

Nonablative allogeneic stem-cell transplantation for advanced/recurrent mantle-cell lymphoma.

PURPOSE: Patients with relapsed mantle-cell lymphoma have poor prognosis and short survival. Our aim was to determine the efficacy of nonablative allogeneic stem-cell transplantation in patients with relapsed mantle-cell lymphoma. PATIENTS AND METHODS: Eighteen patients were treated in one of two consecutive trials. Thirteen patients underwent a conditioning regimen of fludarabine (30 mg/m2 daily for 3 days), cyclophosphamide (750 mg/m2 daily for 3 days), and high-dose rituximab. For the remaining five patients, the conditioning regimen consisted of cisplatin (25 mg/m2 daily for 4 days), flud

Donor CMV serostatus has no impact on CMV viremia or disease when prophylactic granulocyte transfusions are given following allogeneic peripheral blood stem cell transplantation.

We studied the impact of donor cytomegalovirus (CMV) serologic status on CMV viremia and disease when prophylactic granulocyte colony-stimulating factor (G-CSF)-mobilized granulocyte transfusions (GTs) were given following allogeneic peripheral blood stem cell (AlloPBSC) transplantation. A cohort of 83 patients who received 2 prophylactic GTs from ABO-compatible stem cell donors following AlloPBSC transplantation was compared with a cohort of 142 patients who did not. AlloPBSC donors were eligible for granulocyte donation irrespective of their CMV serostatus. Recipients received no prophylacti

Antithrombin-III for the treatment of chemotherapy-induced organ dysfunction following bone marrow transplantation.

A hypercoaguable state has been shown to follow high-dose chemotherapy for bone marrow transplantation (BMT). Deficiency of the natural anticoagulants, antithrombin III (AT-III), protein C and protein S correlate with organ dysfunction following BMT. We treated 10 patients with severe post-BMT organ dysfunction with AT-III concentrate. Indications for treatment included AT-III anticoagulant level less than 88% and life-threatening single or multiorgan dysfunction. All patients were loaded with 50 units/kg AT-III every 8 h for three doses followed by 50 units/kg/day each day for 3-12 days. Clin

Biologic properties of transplantation immune sera. IV. Influence of the course of immunization, dilution and complexing to antigen on enhancing activity of Ig classes.

The influence of the course of immunization on the facilitating-enhancing activity of antibody classes has been studied by passive enhancement of growth of A/JAX sarcomas in CBA and IC mice and of C57BL/6 EL 4 leukemia in BALB/c mice. The influence of dilution of antibodies and complexing to antigens was also studied. During immunization (with several boosters), the enhancing capacity of sera increased together with 7S IgG antibody activity, but showed no correlation with 19S IgM antibody activity. It also was mercaptoethanol resistant. IgG1 to be more enhancing than an equal number of hemaggl

Carcinoma-associated fibroblasts derive from mesothelial cells via mesothelial-to-mesenchymal transition in peritoneal metastasis.

Peritoneal dissemination is a frequent metastatic route for cancers of the ovary and gastrointestinal tract. Tumour cells metastasize by attaching to and invading through the mesothelial cell (MC) monolayer that lines the peritoneal cavity. Metastases are influenced by carcinoma-associated fibroblasts (CAFs), a cell population that derives from different sources. Hence, we investigated whether MCs, through mesothelial-mesenchymal transition (MMT), were a source of CAFs during peritoneal carcinomatosis and whether MMT affected the adhesion and invasion of tumour cells. Biopsies from patients wi

The potential of stem cells in breast cancer therapy: A review.

Breast cancer remains the most prevalent malignancy among women worldwide, underscoring the urgent need for innovative therapeutic strategies. Stem cell-based anticancer approaches have attracted substantial attention because of their ability to generate immune-related cells and selectively home to tumor sites, among which mesenchymal stem cells (MSCs) are considered particularly promising owing to their strong tumor-tropic capacity and low immunogenicity. In this review, current evidence on the biological functions and therapeutic potential of MSCs and MSC-derived exosomes in breast cancer wa

Killing cancer takes guts: lessons learned from the manipulation of gut microbiome and immunotherapy for the future of urothelial carcinoma.

Urothelial carcinoma (UC) remains a common cancer with significant morbidity and mortality worldwide. Immune checkpoint inhibitors (ICIs) have helped revolutionize the treatment of UC, and there is growing evidence suggesting the crucial role of the gut microbiome in immune system function influences immunotherapy outcomes in this disease. Herein, we review the preclinical basis for how manipulation of the gut microbiome may alter the efficacy of immunotherapy for patients with cancer, highlight interventions optimizing gut microbiome diversity currently in use, review recent and ongoing clini

Bridging the Gap Between General and Transplant Infectious Disease Practice.

The population of iatrogenic immunocompromised individuals is steadily rising as we continue to improve in our ability to manage diseases, including cancer, autoimmune disease, and end-organ dysfunction, with increasing access to novel agents and transplantation. Enhanced survivorship allows these patients to return to their home communities and lives, leading to an increased number of immunocompromised patients across all healthcare systems. However, expertise in immunocompromised host infectious diseases or transplant ID (TID) is concentrated in urban centers and often in academic settings.

Liver transplantation for Type I Abernethy malformation with liver tumors in children: a report of two cases with contrasting portal vein reconstruction strategies.

BACKGROUND: Type I Abernethy malformation is a rare congenital portosystemic shunt with an absent intrahepatic portal vein (PV). It often presents with liver tumors or metabolic disorders. Liver transplantation (LT) is the only definitive treatment, but it poses unique technical challenges due to the absent native PV. Detailed surgical guidance for variable anatomies remains scarce. CASE PRESENTATION: We report two children with Type I Abernethy malformation who underwent LT in 2023. Both presented with benign liver tumors, hyperammonemia and abnormal liver function. The cancer-microbiome axis: Mechanisms and emerging therapeutic strategies.

The human microbiome has emerged as a critical modulator of cancer development, progression, and therapeutic response. Advances in sequencing and functional profiling have revealed that commensal microorganisms—particularly bacteria—interact closely with host immune and metabolic pathways, influencing tumor immunity across multiple cancer types. Dysbiosis of the gut microbiome has been associated with tumorigenesis, immune evasion, and resistance to therapy, while specific microbial taxa and metabolites have been shown to enhance antitumor immune responses. These discoveries have catalyzed the

Adapting CAR-T and CAR-Treg cancer therapies for autoimmunity: innovations and challenges.

The application of Chimeric Antigen Receptor T-cell (CAR-T), CAR-engineered regulatory T cell (CAR-Treg) and hematopoietic stem cell (HSC) therapies has grown in recent years, driven by an increasing demand for robust, antigen-specific T lymphocytes for the treatment of both cancer and autoimmune diseases. This review begins by examining existing cell-based therapies, its biological principles and mechanisms, that have helped achieve notable success in treating cancer. The review then discusses the applicability of these approaches to autoimmune diseases such as progressive and relapsing Multi

Clinical spectrum and survival outcomes of malignancies in pediatric patients with inborn errors of immunity.

BACKGROUND: Inborn errors of immunity (IEI) predispose patients to malignancy, particularly lymphoma, but data on diagnostic sequence and outcomes are limited. METHODS: We retrospectively analyzed IEI-associated malignancies diagnosed at a single center between January 2004 and December 2025. RESULTS: Among 1,668 patients with IEI, 67 (4.0%) developed malignancy. Median ages at IEI and malignancy diagnosis were 8 and 10 years, respectively. Malignancy-first presentation occurred in 45 patients (67%), especially adolescents, and was associated with advanced-stage disease (p = 0.007). Lymphoid

The gut reservoir of carbapenem-resistant Enterobacterales: from dysbiosis and colonization to infection and decolonization, with a focus on patients with hematologic malignancies - a narrative review.

Carbapenem-resistant Enterobacterales (CRE) remain among the highest-priority antimicrobial-resistant pathogens worldwide, and intestinal colonization is increasingly recognized as the key precursor of invasive infections, particularly in patients with hematological malignancies. Increasing evidence indicates that disruption of the gut microbial ecosystem, reflected in reduced diversity, depletion of beneficial anaerobic taxa, intestinal barrier dysfunction, immune dysregulation, and expansion of Enterobacterales, plays a central role in the transition from colonization to infection. Consequen

The Proximal Fibula Bone Flap for Complex Maxillary Reconstruction: Anatomic Evaluation and a Case Report.

BACKGROUND: Total maxillectomy defects that involve both the supra- and infra-structure pose a reconstructive challenge. Accurate reconstruction of the orbital rim and alveolus is needed for orbital floor reconstruction and dental implant placement. The fibula is the workhorse flap for osseous craniofacial reconstruction. The distal fibula is harvested as vascularized bone while the proximal fibula is typically discarded after separating it from the pedicle. We evaluated the vascular anatomy of the proximal fibular segment for potential use as a separate bone flap. METHODS: The anatomy of the

Auxiliary Liver Transplantation With Delayed Total Hepatectomy: Evolution From 2-stage APOLT to RAPID Following the ALPPS Principles.

The resection and partial liver segment 2/3 transplantation with delayed total hepatectomy (RAPID) represents an innovative fusion of 2 of the most challenging procedures of modern liver surgery: auxiliary partial orthotopic liver transplantation and associating liver partition and portal vein ligation for staged hepatectomy. By combining the principles of auxiliary partial orthotopic liver transplantation of partial graft implantation with staged parenchymal modulation of associating liver partition and portal vein ligation for staged hepatectomy, RAPID enables sequential hypertrophy of the t

MASH to cirrhosis: bridging the gaps in MASLD management.

Metabolic dysfunction-associated steatohepatitis (MASH) represents a critical stage in the progression of metabolic dysfunction-associated steatotic liver disease (MASLD), significantly increasing the risk of cirrhosis, hepatocellular carcinoma (HCC), and liver-related mortality. Despite the rising global prevalence of MASLD, gaps in understanding the pathophysiological mechanisms driving MASH to cirrhosis persist, leading to challenges in early diagnosis, prevention, and treatment. This review explores the current knowledge on MASH, focusing on its pathophysiology, clinical management, and tr

Pan-urethral condyloma acuminatum in a bladder cancer patient.

This case describes a male renal transplant recipient in his 70s with a history of non-muscle invasive bladder cancer who presented with a urethral mass initially concerning for malignancy. Histopathological analysis revealed condyloma acuminatum due to low-risk human papillomavirus (HPV), which rarely occurs in the urethra. The patient underwent transurethral resection with close cystoscopic surveillance. This report highlights the importance of considering benign HPV-related lesions in the differential diagnosis of urethral masses in high-risk populations and emphasises the need for individu
