Retrosigmoid approach for resection of petrous apex meningioma.

We present the case of a 50-year-old female with a 1-year history of right-side facial numbness, as well as an electric shock-like sensation on the right-side of the face and tongue. She was previously diagnosed with vertigo and trigeminal neuralgia. MRI was obtained showing a large right cerebellopontine angle mass. A retrosigmoid approach was performed and total removal was achieved after dissection of tumor from brainstem and cranial nerves IV, V, VI, VII and VIII. Pathology confirmed the diagnosis of a meningioma (WHO Grade I). The patient was discharged neurologically intact on the third …

MRI characteristics of trigeminal nerve involvement in patients with multiple sclerosis.

BACKGROUND: Trigeminal neuralgia and central sensory disturbances are common in patients with multiple sclerosis. The anatomic correlation to lesions in the trigeminal nuclei in the brainstem is not well studied. OBJECTIVE: We studied the anatomical characteristics of demyelinating lesions of the trigeminal complex in the brainstem on MRI in patients with MS and Clinically Isolated syndrome (CIS). MATERIALS AND METHODS: 43 Patients with MS or CIS and MRI lesions in the trigeminal complex in the brainstem were selected from a large database of patients referred for MRI because of trigeminal s…

Avoiding abducens nerve palsy during the percutaneous balloon compression procedure.

AIMS: The main purpose of this study is to underline the factors which are likely to increase the risk of a sixth nerve injury after percutaneous balloon compression of the gasserian ganglion. METHODS: Medical records and intraoperative radiological images of 79 consecutive patients, who underwent percutaneous balloon compression for treating drug-resistant trigeminal neuralgia, were analyzed. RESULTS: Post-operatively, transient palsy of the sixth nerve developed in 2 patients. According to their intraoperative radiographs, the side effect resulted from irregular balloon shape and inflation…

[Elongated styloid process (Eagle's syndrome): literature review and a case report].

Eagle's syndrome occurs when an elongated styloid process or calcified stylohyoid ligament causes recurrent throat pain or foreign body sensation, dysphagia, or facial pain. Additional symptoms may include neck or throat pain with radiation to the ipsilateral ear. The symptoms related to this condition can be confused with those attributed to a wide variety of facial neuralgias. Diagnosis can usually be made on physical examination by digital palpation of the styloid process in the tonsillar fossa. The treatment of Eagle's syndrome is primarily surgical. The styloid process can be shortened th…

[A surgical case of hemifacial spasm caused by a tortuous, enlarged, and calcified vertebral artery].

Microvascular decompression (MVD) is an established procedure to treat hemifacial spasm and trigeminal neuralgia. However, this cannot be done in some cases in which ecstatic vertebrobasilar arteries are involved. In these instances, alternative techniques must be used. We encountered a patient with hemifacial spasm caused by a calcified, enlarged, and tortuous vertebral artery. To obtain safe and certain neurovascular decompression of the facial nerve, the tortuous and calcified vertebral artery was surgically repositioned by placing layers of Teflon felt at two regions between the brain stem…

[Neurovascular compression within posterior cranial cavity].

30 years ago Janetta proposed neurovascular compression as an etiology factor of trigeminal neuralgia. Neuroanatomical, neurophysiological studies and observations during neurosurgical operations seems to confirm Janetta's suggestions. The conception of neurovascular compression was accepted also as an etiology factor in hemifacial spasm. There are more diseases of unknown etiology described in literature in which neurovascular compression as an etiological factor is suspected. The results of initial treatment of these diseases performed by neurovascular decompression were presented. We are de…

Severe herpes zoster ophthalmicus in young African adults: a marker for HTLV-III seropositivity.

This report proves the relationship between herpes zoster ophthalmicus and seropositivity for HTLV-III in young and often apparently healthy African patients. The ophthalmologist should screen patients with herpes zoster ophthalmicus for antibodies against HTLV-III in areas where this virus is endemic or if the patient belongs to a known risk group. If the test is positive, the patient should be instructed about the infectious nature of his condition to prevent spread of this sexually transmitted disease. As the rate of corneal involvement and postherpetic neuralgia are very high in these pati…

Diagnosis of temporomandibular joint pain in patients seen at a pain clinic.

A retrospective survey of a pain clinic population found 43 patients (2.6% of the total population) who had pain in the temporomandibular joint region. The clinical presentation of these patients did not differ greatly from that observed with typical responsive temporomandibular joint pain; however, important discriminating factors were duration of the pain, a strong history of pain elsewhere in the body, and major psychiatric disturbance. The diagnosis was atypical facial pain in 39 (90%) of the cases; other diagnoses included glossopharyngeal neuralgia and perineural invasion by adenocystic …

Differential effect of antiepileptic and non-antiepileptic drugs on the reticular formation.

The effect of the antiepileptic drugs carbamazepine and phenytoin, and of the non-antiepileptic drug baclofen, was compared on various inhib … mechanisms. Baclofen also resembles carbamazepine and phenytoin in its effectiveness in trigeminal neuralgia, but baclofen is not a clinically effective antiepileptic agent. Our experiments indicate that the ability to depress the reticular formation of the diencephalon and midbrain is an important characteristic of antiepileptic drugs. This suggests that …

[Intimal fibrosis of cerebellar arteries--a contribution to the pathogenesis of neurovascular syndromes].

Cross-compression of cranial nerves at the brain stem by an arterial loop appears to be responsible for several dysfunction syndromes like trigeminal neuralgia and hemifacial spasm. The pathogenesis of these syndromes still remains obscure; some authors call atherosclerosis of the cross-compressing vessel a local pathogenetic factor. Histological examination of the lower cerebellar arteries, obtained at autopsies of humans without cranial nerve dysfunction, was carried out. There was no evidence of significant atherosclerotic changes of distant cerebellar arteries even in places predisposing t…

Corneal sensitivity after percutaneous radiofrequency trigeminal rhizotomy: a quantitative study.

Using the Cochet-Bonnet esthesiometer, we evaluated the corneal sensation in 13 patients with trigeminal neuralgia. We compared the preoperative and postoperative results after the trigeminal rhizotomy in a quantitative study. Of 13 patients who underwent operation, a decrease in corneal sensation developed compared with the eye that was not operated on in five patients. This decrease in sensitivity was not suggested by clinical symptoms or signs, and up to 23 months of follow-up failed to reveal any clinical symptoms. The failure to manifested clinical corneal symptoms in the follow-up period…

Polyvinylpyrrolidone (povidone) storage in RX-damaged skin following procaine-povidone intramuscular injections.

A man, aged 67, has been treated in 1968 by X-rays for a basal-cell carcinoma of the back. The treatment was followed by an ulcerative radiodermatitis. 3 years later, the patient was treated by ten intramuscular injections of procaine-polyvinylpyrrolidone (or procaine-povidone) for sciatic neuralgia. An orange infiltration appeared progressively around the radiodermatitis. The area was excised in 1979 and polyvinylpyrrolidone (povidone) storage, responsible for the ring-shaped orange hue, could be demonstrated in the dermis and subcutaneous fat. Special stainings confirmed the presumed chemica…

[Surgery of obturator hernia by closure of the obturator canal with the round ligament].

Almost 2/3rds of obturator hernias occur after 70 years. Favoured by changes in pelvic and spinal position, this hernia is uncommon in women and exceptional in men. Revealed by strangulation, it is in one third to one quarter of cases, unrecognised before operation, often due to failure to recognise the cause of crural neuralgia in a patient with acute intestinal obstruction. Even when the diagnosis is made, laparotomy is the best approach. It makes intestinal resection easier, this is necessary in 50% of cases, and above all, it is easier to cure the hernia for which the authors propose the u…

Neuropathic Mechanisms in Phantom Pain: A Review of Pain Pathways and Peripheral Localization.

PURPOSE OF REVIEW: Phantom limb pain (PLP), the perception of pain in a limb no longer present, is a phenomenon affecting over 80% of amputees. Its prevalence induces severe pain and psychological distress causing a significant decline in the quality of life of amputees. PLP's complex interplay of peripheral and central mechanisms remains the leading cause for limited understanding and management of the condition. This article aims to examine PLP within the framework of pain localization by reviewing previous studies and investigating underexplored dimensions of PLP. RECENT FINDINGS: With the

Aging and neuropathic pain: Mitochondria-to-glia cascade, system mechanisms, and therapeutic strategies.

Neuropathic pain (NP) creates a severe pathological condition that primarily affects elderly people because of their accumulated neurobiological changes that make them more susceptible to persistent pain. The aging process leads to multiple mechanisms that combine neurodegeneration with immunosenescence and mitochondrial dysfunction with impaired autophagy and glial priming, and ion channel dysregulation to create a nociceptive environment. This review examines how mitochondrial breakdown and dysfunctional autophagy, and ion channel disturbances with glial cell activation form a interconnected

Targeting neurovascular dysfunction in diabetic neuropathic pain: a review of mechanism-based pharmacotherapy beyond gabapentinoids.

BACKGROUND: Diabetic neuropathic pain (DNP) is a condition that affects about 20-30% of people with diabetes, significantly reducing health-related quality of life and having a significant economic impact on healthcare economics. Although conventional "gold standard" treatments such as gabapentinoids, serotonin-norepinephrine reuptake inhibitors (SNRIs), and tricyclic antidepressants (TCAs) are widely used in the clinic, a substantial number of patients have poor results with these medications in terms of the quality of their analgesia or their systemic side effects. OBJECTIVE: In order to ov

Uncommon Dermatological Reaction to Spinal Cord Stimulation-Prurigo Nodularis.

BACKGROUND: Spinal cord stimulation (SCS) is a widely used treatment for chronic neuropathic pain, demonstrating effectiveness in managing resistant symptoms in some cases. However, dermatological complications, though rare, can occur. CASE REPORT: This case report examines a patient who developed diffuse itching and skin rash with features of prurigo nodularis after undergoing surgical revisions and stimulation adjustments. The rash improved upon deactivating the pulse generator and reappeared upon reactivation, suggesting a direct connection between the neurostimulation therapy and dermatol
