Halitosis--a common medical and social problem. A review on pathology, diagnosis and treatment.

Bad breath is a condition that has health and social implications. This paper provides a comprehensive review of the classification of halitosis, it's etiology, it's prevalence, diagnosis and treatment strategies for the condition. Halitosis is affecting about 25-30% of world's population. It includes categories of genuine halitosis, pseudo-halitosis and halitophobia. It is believed that in 80-90% of cases halitosis origins in the oral cavity and the most common causes are: gingival pathologies, caries and poor oral hygiene. Extraoral sources of halitosis are responsible for 10-20% of all case

Reproducibility of the Dutch version of the Western Ontario rotator cuff Index.

BACKGROUND: The Western Ontario Rotator Cuff Index (WORC) is a disease-specific shoulder questionnaire, originally developed at the University of Western Ontario, to measure quality of life in patients with rotator cuff disease (RCD). The aim of the present study was to cross-culturally adapt the WORC for use in the Netherlands and to evaluate the reproducibility in patients with RCD. MATERIALS AND METHODS: The WORC was translated into Dutch according to leading guidelines in the literature, and 52 patients with RCD completed the questionnaire twice within 2 weeks. The Cronbachs α and the int

Thirst in heart failure: a systematic literature review.

AIMS: Although patients with heart failure (HF) may suffer from severe thirst, this has received little attention in scientific studies. A systematic literature review was conducted to identify and analyse data on thirst in HF. METHODS AND RESULTS: Pubmed, Cochrane, Cinahl, and Medline databases were searched for original studies on patients with HF with thirst as an outcome measure. Of 174 screened citations, nine articles were included, in which a total of 4375 HF patients had been studied. Four studies comprising 181 patients provided visual analogue scale (VAS) scores. Median thirst inten

Long-term follow-up and pregnancy in a patient with neurenteric cyst: case report.

BACKGROUND: Neurenteric cysts (NC) are rare congenital anomalies. Cysts are secretory with an epithelial lining and features that resemble those of gastrointestinal and/or respiratory mucosa. They originate embryologically and result from an incomplete separation of the neurenteric canal from the foregut. CLINICAL CASE: We describe the 21-year follow-up of a female patient with a history of partial control of urinary and anal sphincters. The patient had a 6-month course of pain associated with saddle hypoesthesia, limited gait, chronic constipation, plus inferior paresthesia of the limbs. The

Maternal folate supply and sex influence gene-specific DNA methylation in the fetal gut.

SCOPE: Epidemiological evidence supports the developmental origins of health and disease hypothesis that developmental under/over-nutrition increases adulthood disease risk. Epigenetic markings are one potential mechanism mediating these effects. Altered folate supply may influence methyl group availability for DNA methylation. We reported low folate supply in utero was associated with reduced global DNA methylation in the murine small intestine of adult offspring. We hypothesised that aberrant methylation would be observed during early development. METHODS AND RESULTS: Female C57BL/6J mice w

["Malnutrition-inflammation complex syndrome" in chronic hemodialysis].

Protein-energy wasting (PEW) and inflammation are usually common and concurrent conditions in maintenance dialysis patients and associated with poor prognosis. Low appetite and hypercatabolic states are common features. In dialysis patients, the former has been suggested to be secondary to inflammation; however, the evidence is not conclusive. Hence, the term malnutrition-inflammation complex syndrome (MICS) was coined to include this clinical entity, regardless the original causes. Possible causes of MICS include comorbid illnesses, oxidative stress, nutrient loss through dialysis, hyporexia,

Youth depression and early childrearing: stress generation and intergenerational transmission of depression.

OBJECTIVE: Broadening the concept of stress generation beyond acute life events, the current study explores predictors of the creation of stressful environments-specifically, selection into early childrearing by age 20. It was predicted that youth with early onset depressive disorders would be at higher risk for early childrearing accompanied by greater depression and parenting maladjustment. Additional analyses tested hypotheses about the roles of interpersonal vulnerability and intergenerational transmission of depression and examined gender differences. METHOD: A community sample of 706 ad

Cross-cultural development of an item list for computer-adaptive testing of fatigue in oncological patients.

IntroductionWithin an ongoing project of the EORTC Quality of Life Group, we are developing computerized adaptive test (CAT) measures for the QLQ-C30 scales. These new CAT measures are conceptualised to reflect the same constructs as the QLQ-C30 scales. Accordingly, the Fatigue-CAT is intended to capture physical and general fatigue.MethodsThe EORTC approach to CAT development comprises four phases (literature search, operationalisation, pre-testing, and field testing). Phases I-III are described in detail in this paper. A literature search for fatigue items was performed in major medical data

Sudden massive neck swelling due to hemorrhage of a thyroid adenoma: a case report.

INTRODUCTION: Sudden swelling of the neck is an emergency situation that can be life-threatening for the patient. Therefore, an understanding of the possible underlying pathology is of great importance. Sudden massive swelling of the neck because of intralesional bleeding of a thyroid adenoma is seldom encountered but must be considered. Such massive swelling caused by spontaneous bleeding of a thyroid adenoma has not yet been described in the literature. CASE PRESENTATION: We report the case of a 71-year-old Caucasian man with sudden massive neck swelling due to intralesional bleeding of a t

Penetrating head injury from angle grinder: A cautionary tale.

Penetrating cranial injury is a potentially life-threatening condition. Injuries resulting from the use of angle grinders are numerous and cause high-velocity penetrating cranial injuries. We present a series of two penetrating head injuries associated with improper use of angle grinder, which resulted in shattering of disc into high velocity missiles with reference to management and prevention. One of those hit on the forehead of the operator and the other on the occipital region of the co-worker at a distance of five meters. The pathophysiological consequence of penetrating head injuries dep

Minimally invasive surgery of humeral metastasis using flexible nails and cement in high-risk patients with advanced cancer.

This study was conducted to evaluate the preliminary outcome of palliative minimally invasive surgery for humeral metastasis in patients who have multiple advanced cancers with short life expectancy. Percutaneous Ender nailing and direct transcortical intramedullary cementing were performed on a total of 15 patients with metastatic disease of the humerus. The origins of the cancers were the lung (n=9), breast (n=3), colon (n=2) and liver (n=1). Each patient had multiple unresectable organic metastases and proved to be at high risk for anesthesia and bloody surgery. All procedures were performe

Airway remodelling and its relationship to inflammation in cystic fibrosis.

Pulmonary disease is the most important cause of morbidity and mortality in cystic fibrosis (CF). Most patients with CF die from respiratory failure with extensive airway destruction. Airway remodelling, defined as structural airway wall changes, begins early in life in CF but the sequence of remodelling events in the disease process is poorly understood. Airway remodelling in CF has traditionally been thought to be solely the consequence of repeated cycles of inflammation and infection. However, new evidence obtained from developmental, physiological and histopathological studies suggests tha

Mutation and ADAMTS13-dependent modulation of disease severity in a mouse model for von Willebrand disease type 2B.

Von Willebrand disease (VWD)-type 2B originates from a gain-of-function mutation in von Willebrand factor (VWF), resulting in enhanced platelet binding. Clinical manifestations include increased bleeding tendency, loss of large multimers, thrombocytopenia, and circulating platelet aggregates. We developed a mouse model to study phenotypic consequences of VWD-type 2B mutations in murine VWF: mVWF/R1306Q and mVWF/V1316M. Both mutations allow normal multimerization but are associated with enhanced ristocetin-induced platelet aggregation, typical for VWD-type 2B. In vivo expression resulted in thr

What role does the blood brain barrier play in acute mountain sickness?

As high altitude travel increases, acute mountain sickness (AMS) and life threatening high altitude cerebral oedema (HACE) are becoming more prevalent. Acute mountain sickness occurs in 45% of lowlanders above 4250 m. Predisposing factors are still unknown and its development is more complex than the original "tight fit" hypothesis. This review examines evidence relating to a possible role of the blood brain barrier in AMS as suggested by MRI studies. Underlying mechanisms may involve vascular endothelial growth factor and free radicals in addition to increases in hydrostatic pressure. An incr

A zinc-binding site by negative selection induces metallodrug susceptibility in an essential chaperonin.

GroES is an indispensable chaperonin virtually found throughout all life forms. Consequently, mutations of this protein must be critically scrutinized by natural selection. Nevertheless, the homolog from a potentially virulent gastric pathogen, Helicobacter pylori, strikingly features a histidine/cysteine-rich C terminus that shares no significant homology with other family members. Additionally, three more (H45, C51, and C53) are uniquely present in its apical domain. The statistical analyses show that these residues may have originated from negative selection, presumably driven by either dep
