Enhancing Clinical Confidence: Effects of Medical Screening and Differential Diagnosis Training for Low Back Pain.

PURPOSE: Direct access to physical therapy (PT) requires PTs to act as primary care providers, making differential diagnosis a critical component of patient assessment. We investigated how participation in postgraduate training in differential diagnosis and medical screening influences PTs' confidence, self-efficacy, attitudes and beliefs about treating patients with low back pain (LBP). PATIENTS AND METHODS: This prospective, two-arm study involved 49 PTs in an intervention group and 70 in a control group. The intervention included 40-hours of training on medical screening and differential d

Self-reported cognitive dysfunction and memory impairment in Systemic Autoimmune Rheumatic Diseases (SARDs): a mixed methods analysis of the INSPIRE cohort.

OBJECTIVES: To explore self-reported cognitive dysfunction, including memory impairment, across systemic autoimmune rheumatic diseases (SARDs) and examine its impact and associations with demographic, clinical and psychosocial factors. METHODS: A mixed-methods approach was employed, surveying 1853 SARD patients and 463 controls using validated instruments including the Everyday Memory Questionnaire-Revised (EMQ-R). Kruskal-Wallis tests and Spearman's rank correlations were used to compare the groups. Additionally, 67 in-depth interviews were conducted for qualitative thematic analysis. RESUL

Change in Healthcare Use After a Self-Management Supportive Intervention for Low Back Pain-A Quasi-Experimental Study.

BACKGROUND: Individuals with low back pain (LBP) have high healthcare use (HCU). It is currently unclear whether self-management supportive interventions can decrease HCU among patients with LBP. The aim of this study was to investigate changes in visits to primary care and redeemed prescriptions of analgesics after enrolment in a self-management supportive programme compared to usual care. METHODS: This quasi-experimental study included adults with LBP who enrolled in the Danish GLA:D Back programme between 2018 and 2022. GLA:D Back is a structured 10-week programme of group-based patient ed

Single fixed-dose intranasal racemic ketamine treatment for the treatment of acute suicidality in a transdiagnostic patient population: Results of a pilot study.

AIMS: Ketamine has been shown to decrease suicidality in patients with depression. However, little is known about tolerability and efficacy in heterogenous, acutely suicidal patients in emergency settings. This pilot study aimed to assess the feasibility, acceptability, and safety of generic intranasal racemic ketamine (75 mg) in the treatment of acute suicidality regardless of the underlying diagnosis, as preparation for a subsequent double-blind randomized placebo-controlled trial (RCT). METHOD: From September 2021 to June 2022, 12 patients with acute suicidality were recruited. All receive

Clinical Characteristics of Gait Disturbance in Charcot-Marie-Tooth Disease and Future Directions in Physical Therapy.

Charcot-Marie-Tooth disease (CMT) is a hereditary and progressive peripheral neuropathy affecting both motor and sensory nerves. It is among the most common inherited neuropathies and is primarily classified into demyelinating (type 1) and axonal (type 2) forms based on motor nerve conduction velocity, with over 100 genetic subtypes identified. Due to this genetic and clinical heterogeneity, the onset, severity, and degree of motor and sensory impairments vary widely among individuals.  One of the hallmark manifestations of CMT is gait disturbance. As the disease progresses, individuals often

The evolution of intractable Ménière's disease: attacks resolve over time.

INTRODUCTION: Knowledge of the natural and temporal course of a disease is important when deciding if an intervention is appropriate. In the case of Ménière's disease (MD), there is some evidence that attacks diminish over time, but the topic remains controversial. A conservative approach to surgery is usually followed in northern Europe, and leads to strict patient selection before considering surgery. Here, we describe the evolution of vertigo attacks among a group of intractable MD patients in whom surgery was considered. METHODS: Retrospective cohort study in a Ménière's disease expert ce

Interventions to improve system-level coproduction in the Cystic Fibrosis Learning Network.

BACKGROUND: Coproduction is defined as patients and clinicians collaborating equally and reciprocally in healthcare and is a crucial concept for quality improvement (QI) of health services. Learning Health Networks (LHNs) provide insights to integrate coproduction with QI efforts from programmes from various health systems. OBJECTIVE: We describe interventions to develop and maintain patient and family partner (PFP) coproduction, measured by PFP-reported and programme-reported scales. We aim to increase percentage of programmes with PFPs reporting active QI work within their programme, while

Full esophageal mobilization during hiatal hernia repair with concomitant sleeve gastrectomy improves postoperative reflux symptoms for patients with preexisting reflux.

BACKGROUND: Hiatal hernia repair (HHR) performed concurrently with vertical sleeve gastrectomy (VSG) has been shown to improve postoperative gastroesophageal reflux disease (GERD). However, data on the optimal extent of esophageal mobilization during repair are lacking. Mobilization techniques for HHR during VSG include partial (PM) or full (FM) mobilization of the esophagus. We hypothesize that patients who undergo full mobilization will be less likely to develop postoperative reflux. METHODS: A single-institution retrospective review of all patients who underwent a VSG with a HHR between 20

Factors associated with frequent buprenorphine / naloxone initiation in a national survey of Canadian emergency physicians.

OBJECTIVE: To identify individual and site-related factors associated with frequent emergency department (ED) buprenorphine/naloxone (BUP) initiation. BUP initiation, an effective opioid use disorder (OUD) intervention, varies widely across Canadian EDs. METHODS: We surveyed emergency physicians in 6 Canadian provinces from 2018 to 2019 using bilingual paper and web-based questionnaires. Survey domains included BUP-related practice, demographics, attitudes toward BUP, and site characteristics. We defined frequent BUP initiation (the primary outcome) as at least once per month, high OUD preval

Invasiveness of previous treatment for peripheral arterial disease and risk of adverse cardiac events after coronary stenting.

Patients with peripheral arterial disease (PADs), undergoing percutaneous coronary intervention (PCI), have higher adverse event risks. The effect of invasiveness of PADs treatment on PCI outcome is unknown. This study assessed the impact of the invasiveness of previous PADs treatment (invasive or non-invasive) on event risks after PCI with contemporary drug-eluting stents. This post-hoc analysis pooled 3-year patient-level data of PCI all-comer patients living in the eastern Netherlands, previously treated for PADs. PADs included symptomatic atherosclerotic lesion in the lower or upper extrem
