Examining the temporal relationship between criminal justice involvement and sexual risk behaviors among drug-involved men.

Although criminal justice involvement has repeatedly been associated with human immunodeficiency virus (HIV)/sexually transmitted infection prevalence and sexual risk behaviors, few studies have examined whether arrest or incarceration uniquely contributes to sexually risky behavior. We examined the temporal relationship between criminal justice involvement and subsequent sexual HIV risk among men in methadone maintenance treatment in New York City. A random sample of 356 men was interviewed at baseline (time 1), 6-month (time 2), and 12-month (time 3) follow-ups. Propensity score matching, ne

Cervical dysplasia and associated risk factors in a juvenile detainee population.

STUDY OBJECTIVE: Canada has licensed a human papilloma virus (HPV) vaccine for adolescent females, with the goal of decreasing the incidence of HPV infection and associated cervical cancer. This study identifies the juvenile detainee population as a high-risk group for HPV infection and therefore an important target for primary prevention. DESIGN: A retrospective chart review. SETTING: Sundance Juvenile Detention Center, Kingston, Ontario, Canada. PARTICIPANTS: Female detainees admitted between 2003 and 2006. MAIN OUTCOME MEASURES: Papanicolaou (Pap) test results, sexually transmitted infe

[Is disulfiram (Antabus) indicated in the treatment of offenders with alcohol use disorders?].

BACKGROUND: Research studies and clinical experience provided us with valuable information about the efficacy of disulfiram. Although supervised disulfiram treatment has been shown to improve outcome in court-referred patients too it is obviously rarely used in German forensic hospitals. METHOD: Two cases illustrated the application of disulfiram to constitute an adjunct to treatment of forensic inpatients. RESULTS: Disulfiram can be used as an adjunct for maintenance of abstinence when the relapse risk is due to insufficient impulse control. DISCUSSION: The decision to use disulfiram to as

Hostility and recidivism in sexual offenders.

In this study, we examined the association of hostility, as measured by the Buss-Durkee Hostility Inventory (BDHI), with offence characteristics and recidivism in 656 adult male sexual offenders. Hostility was significantly associated with having prior violent charges, the use of violence in the index sexual offence, sexual recidivism, and violent recidivism. After controlling for risk level, as measured by a modified version of the Rapid Risk Assessment for Sexual Offence Recidivism (RRASOR-mod), the significant association between hostility and sexual and violent recidivism remained. When ex

Practice-based outcomes of dialectical behaviour therapy (DBT) targeting anger and violence, with male forensic patients: a pragmatic and non-contemporaneous comparison.

OBJECTIVE: To examine the effectiveness of an eighteen-month treatment based on dialectical behaviour therapy (DBT) targeting anger and violence, on a group of male forensic patients. METHOD: Eight male forensic patients in a high security hospital who met the criteria for borderline personality disorder measured by the Personality Assessment Inventory underwent 18 months of treatment. They completed three psychometric tests at pre-, mid- and post-treatment and at a six-month follow up. A comparison group (TAU) of nine patients, assessed as having similar personality disorders, received the u

Group therapy for incarcerated women who experienced interpersonal violence: a pilot study.

This study evaluated effectiveness of group therapy for incarcerated women with histories of childhood sexual and/or physical abuse. The intervention was based on a two-stage model of trauma treatment and included Dialectical Behavior Therapy skills and writing assignments. We randomly assigned 24 participants to group treatment (13 completed) and 25 to a no-contact comparison condition (18 completed). We evaluated treatment effects, using the Beck Depression Inventory, Inventory of Interpersonal Problems, and Trauma Symptom Inventory. The data demonstrate significant reductions in PTSD, mood,

The development of the New South Wales Service for the Treatment and Rehabilitation of Torture and Trauma Survivors (STARTTS): the first year.

Many immigrants to Australia are refugees, some of whom have experienced acute stress and trauma, including torture, prior to or during their escape from their home countries. In response to a growing recognition that the health care services may not be meeting the needs of these people the NSW Department of Health funded the establishment of a community-based rehabilitation service for traumatised refugees. This paper provides an overview of the recent history of the service, some of the organisational and staffing issues faced during its first year, some characteristics of the first 200 clie

Management of foreign body ingestion.

Management of ingested foreign bodies (FB) is a common clinical problem. A 10-year experience of 101 foreign body ingestions is reported. The experience suggests that endoscopic removal of foreign bodies is curative for objects located in the cricopharynx or upper esophagus. Foreign bodies which pass into the stomach can be observed for development of symptoms, as 80% of FB which reach the stomach spontaneously pass. Only 12% of patients required operation, which was done without mortality.

Projecting severe sequelae of injection-related hepatitis C virus epidemic in the UK. Part 1: Critical hepatitis C and injector data.

BACKGROUND: Hepatitis C is transmitted by transfusion of unscreened blood, through injecting drugs, from mother-to-child and, on occasion, sexually. Transmission generally requires that the infector is hepatitis C virus (HCV) RNA positive, a 'carrier'. About three-quarters of injectors who are hepatitis C antibody positive are HCV-RNA positive and so infectious to others. Incubation periods from HCV infection to cirrhosis and hepatocellular carcinoma are even longer than from HIV infection to AIDS, being counted in decades; they depend on age, gender, alcohol consumption and co-infection with

Non-fatal opioid overdose and unmet need for medications for opioid use disorder among recently incarcerated people who inject drugs.

BACKGROUND: Medications for opioid use disorder (MOUD) are key to preventing opioid overdose. Despite the high risk of opioid overdose among recently incarcerated people who use drugs, missed opportunities for engagement in MOUD treatment persist in this population. We examined the association between unmet need for MOUD and non-fatal opioid overdose among recently incarcerated people who inject drugs (PWID) and assessed prevalence of non-fatal opioid overdose by selected characteristics. METHODS: We analyzed 2022 data from the National HIV Behavioral Surveillance (NHBS) system among PWID fro

Ophthalmic care at an academic medical centre for patients who were incarcerated or in immigration detention.

BACKGROUND/OBJECTIVES: Ophthalmic care for patients who are incarcerated or in immigration detention presents complex challenges for patients and providers. There remains a paucity of literature investigating ophthalmic care for these marginalised populations. We investigated the delivery of ophthalmic care in an academic health system to patients who were incarcerated or in immigration detention. SUBJECTS/METHODS: We conducted a retrospective observational study and secondary analysis of electronic health record data for inpatient, outpatient, and surgical visits between 1 November 2018 and

Developing a conceptual framework for post-incarceration care for sexual and gender minoritized adults: a concept mapping study.

Mass incarceration in the United States is a social-structural driver of health for sexual and gender minoritized adults (SGMA). The aim of this study was to develop a conceptual framework for post-release healthcare to use in adapting a mobile health unit (MHU) for SGMA returning from jail. We used concept mapping to answer a focal prompt regarding health-related needs for SGMA post-release. We recruited SGMA with recent incarceration experience and relevant community partners to participate in 4 concept mapping sessions. Multidimensional scaling produced a two-dimensional point map of all st

Addressing COVID-19 and Health Literacy Disparities Among Correctional Facility Residents Through Dialogue-Based Education.

Carceral communities face heightened COVID-19-related risks while simultaneously experiencing medical mistrust and limited access to health information and services. Health education programs that incorporate dialogue-based, participatory learning models have been shown to motivate health behavior and increase health knowledge in carceral settings. To increase health literacy and change COVID-19-related health behavior among jail residents in the United States, a local health department developed a dialogue-based education program centered around COVID-19 prevention, misinformation, and naviga

Characteristics of recently incarcerated primary care patients with and without a positive posttraumatic stress disorder screening upon clinic intake.

OBJECTIVE: Posttraumatic stress disorder (PTSD) is prevalent among people who have been incarcerated. Here, we examined whether screening positive for PTSD was associated with other indicators of poor health, acute healthcare utilization, and poverty among primary care patients upon release from incarceration. METHOD: We conducted a cross-sectional survey in a national network of primary care clinics serving people recently released from incarceration. Participants were 416 patients who completed the Primary Care PTSD screen (PC-PTSD) and other questions about mental and physical health, acut

Race-Related and Mental Health Factors of Powder Cocaine Use Among Black Incarcerated Men.

Incarceration disproportionately impacts Black men in the United States, which can have compounding effects on mental health and substance use among this population. Cocaine use, in particular, carries higher severity of dependence and overdose risks among Black Americans, though research examining cocaine use correlates among Black incarcerated men is limited. The present study examines race-related and mental health correlates of powder cocaine use among a sample of incarcerated Black men (n = 208) using the General Strain Theory. Specifically, the present study was a secondary analysis exam

Electroconvulsive therapy use in the adult U.S. correctional setting: A case report and literature review.

Mr. C is a 45-year-old male inmate who was found in his cell unresponsive and mute. He had poor food and fluid intake for the last four days and was later found standing in place, frozen, and resistant to movement when encouraged by a corrections officer to rest in his bed. His symptoms were consistent with catatonia, a severe motor syndrome that can be life-threatening. The patient had a psychiatric history of bipolar I disorder with multiple past episodes of catatonia. Lorazepam was ineffective at reversing his catatonic symptoms, and his serum creatinine kinase level eventually began to ris

Are Liaison and Diversion interventions in policing delivering the planned impact: A longitudinal evaluation in two constabularies?

Liaison and Diversion (L&D) has twin objectives: improving mental health outcomes and reducing re-offending. Early diversion from police custody seems promising, but evidence of benefit is required to sustain such programmes. To test the hypothesis that contact with L&D services while in police custody would lead to improved mental health outcomes and a reduction in type and level of offending, we used a pre-post service use design. National Health Service (NHS) records in two counties were searched for evidence that patients had been involved with L&D services while in police custody during t
