Treatment of Irritable Bowel Syndrome.

Irritable bowel syndrome (IBS) is an extremely common cause of consultation, and at present is diagnosed on the basis of symptoms and a few simple exclusion tests. Exclusion diets can be successful, but many patients have already attempted and failed such treatments before consulting. Anxiety and somatization may be an important driver of consultation. Patients' concerns should be understood and addressed. Those with prominent psychiatric disease may benefit from psychotherapy. Hypnotherapy benefits symptoms in those without psychologic disturbance, but its availability is limited. Antidepress

Narcotic withdrawal syndrome following gastric bypass--a difficult diagnosis.

BACKGROUND: It is common for bariatric patients to experience postoperative nausea, depression and remorse for several months following surgery. Difficulty exists for the surgeon in delineating the physical from the psychological in treating these patients. Preoperative education, evaluation and preparation, although essential, will not identify nor eliminate all potential problems. METHODS: We report the case history of a patient who ultimately underwent reversal of her gastric bypass. Her symptoms required multiple procedures and hospital admissions for what appeared to be anatomical proble

Pelvic pain. A SAFE approach.

The efficacy of a multidisciplinary approach for the treatment of chronic pain has been well documented. A recent randomized prospective trial by Peters and co-workers confirmed that multidisciplinary management of chronic pelvic pain resulted in greater improvement in pain severity, increased employment, and improved functional health scores when compared with a traditional medical approach. Unfortunately, many gynecologists do not have the luxury of working in these types of settings. The SAFE approach allows the busy practitioner to evaluate the nonorganic and organic nature of pelvic pain

Functional bowel disorders and functional abdominal pain.

The Rome diagnostic criteria for the functional bowel disorders and functional abdominal pain are used widely in research and practice. A committee consensus approach, including criticism from multinational expert reviewers, was used to revise the diagnostic criteria and update diagnosis and treatment recommendations, based on research results. The terminology was clarified and the diagnostic criteria and management recommendations were revised. A functional bowel disorder (FBD) is diagnosed by characteristic symptoms for at least 12 weeks during the preceding 12 months in the absence of a str

Suicide and antidepressants.

Suicidal thoughts and suicide attempts are an integral part of various depressions. Suicide attempts are common in major depression but even more common in recurrent brief depression, and the rate is further increased when these depressions occur comorbidly. Combined depression, where recurrent brief depression and major depression coexist, is the strongest clinical predictor of suicide attempts in the literature. There have been very few controlled studies of treatments for depression in high-risk groups of suicide attempters. Psychotherapy has been found to significantly raise the suicide at

Quality and use of trauma histories obtained from psychiatric outpatients through mandated inquiry.

OBJECTIVE: A growing awareness of the sequelae of trauma has prompted clinical and administrative mandates that psychiatric assessments must include inquiry about whether patients have experienced trauma or abuse. This study was designed to assess how a trauma history obtained through mandated inquiry is recorded and how it is used in diagnostic formulation and treatment planning. METHODS: Histories of physical and sexual abuse and other trauma were reviewed in the charts of 180 outpatients receiving psychotherapy at an urban teaching hospital clinic. The extent and variety of traumatic exper

Quality of life in upper gastrointestinal disorders.

BACKGROUND: There is increased interest in the concept 'quality of life'. Available instruments are defined and discussed, with special emphasis on reliability and validity. Research on quality of life in patients with upper gastrointestinal disorders is reviewed, and our own results presented. METHODS: The Psychosocial Adjustment to Illness Scale was used to compare 100 patients with functional dyspepsia and 100 patients with duodenal ulcer in a cross-sectional study, and to monitor the quality of life of 74 duodenal ulcer patients in a longitudinal phase. RESULTS: The quality of life of pa

[Rehabilitation of cancer patients including creative therapies].

When sufficient experience had been accumulated to support the view that intensive aftercare of the consequences of cancer will have a favourable impact on the individual's prognosis, the German social insurance institutions extended coverage to aids for vocational reintegration, and a medical indication of cancer rehabilitation. Own experience gathered since in the longterm care of cancer patients, which lasted up to three years, almost invariably brought about improvements in the quality of life, often a prolongation of survival time that was worth living, and occasionally a shorter incapaci

Possible sequelae of trauma and somatic disorder in early life.

All children experience trauma. The age, state of development and constitutional factors will determine whether some children will have a traumatic effect. Trauma occurring before the age of three, at a time when the ego has not developed its synthetic and integrative functions, may be reproduced in later life as an isolated symptom, by selected sensations involved in a sensory imprint or screen sensation of the trauma as a simple recording. After the age of three, under the influence of a more mature ego, excessive traumatic stimuli will be integrated and elaborated in symptom formations as p

[Chronic tinnitus in children and adolescents].

BACKGROUND: The problem of tinnitus in adults is reviewed systematically in nearly all standard otolaryngology reference works, whereas textbooks and monographs that focus on pediatric otorhinolaryngology or audiology and hearing in children and adolescents provide only little information concerning the epidemiology, etiology and therapy of tinnitus. The purpose of this study was to evaluate the psychosomatic aspects of chronic tinnitus in this younger age group. A rational diagnostic approach is discussed as to which diagnostic measures are necessary in the pediatric group for deciding which

Alleviating Job-Related Burnout in Medical Professionals through Guided Imagery Hypnotherapy.

This pilot study examines the impact of guided imagery hypnotherapy (GIH) on symptoms of job-related burnout in medical professionals. Nine adults (eight female) involved in direct medical care in the region of San Diego, California participated in eight consecutive weekly GIH sessions. Subjective measures of burn-out, compassion, and self-compassion were recorded roughly one week before the start of the hypnotherapy series (baseline session zero) and after the first, fourth, and final GIH sessions. Resting state electroencephalographic data (EEG) was also recorded at each of these measurement

Managing anxiety-related disorders from pregnancy to parenthood.

PURPOSE: Anxiety-related disorders (ARD), including posttraumatic stress disorder (PTSD) and obsessive-compulsive disorder (OCD), are highly prevalent mental health conditions. The peak prevalence of ARD in women coincides with the critical period of family planning, pregnancy, and child-rearing, affecting 1 in 5 women. This poses several challenges, including fears of passing anxiety on to children, worsening of symptoms during pregnancy or postpartum, and concerns about how therapy affects pregnancy outcomes. Clinicians frequently lack the expertise to adequately address these concerns. This

Psychological Treatment for Pediatric Feeding Disorder (PFD) and Avoidant/Restrictive Food Intake Disorder (ARFID).

OBJECTIVE: To produce a consensus statement on the psychological treatment of feeding/eating aversions seen in pediatric feeding disorder (PFD) and avoidant/restrictive food intake disorder (ARFID), diagnoses that share common symptoms and psychological mechanisms but have historically been addressed separately in the literature. METHOD: To help bridge the gaps between ARFID and PFD treatment and research, we convened a group of experts to clarify and describe the psychological interventions for treating learned food avoidance. The current manuscript summarizes the conclusions of the meeting.

Effectiveness Comparisons of Various Psychological Therapies for Fear of Cancer Recurrence in Cancer Patients: A Systematic Review and Network Meta-Analysis.

BACKGROUND: Fear of cancer recurrence (FCR) is one of the most prevalent psychological burdens among cancer patients and exerts a substantial negative impact on their quality of life and overall well-being. Although various psychological interventions have demonstrated promise in alleviating FCR, their comparative effectiveness remains unclear. OBJECTIVE: To conduct an up-to-date systematic review and network meta-analysis to compare the relative efficacy of psychological interventions for alleviating FCR in cancer patients. METHODS: Six electronic databases were systematically searched from

Behavioral Practice and Homework Completion as Predictors of Outcome in a Transdiagnostic Brief Behavioral Therapy for Anxiety and Depression in Youth.

Exposure and behavioral activation have been identified as key therapeutic components in evidence-based treatment for youths with anxiety and depression, respectively. The current study expanded this work to transdiagnostic treatment for youths with anxiety and depression and examined links between (a) amount of in-session exposure/behavioral activation, (b) out-of-session homework completion, and (c) anxiety, depression, and functioning outcomes of youths following a brief behavioral therapy. Therapy data were examined from 95 youths (ages 8-16; 56.8% female; 74.7% non-Hispanic White) with an

Testing child STEPS in school-based mental health care: Cluster-randomized controlled effectiveness trial in five school districts.

OBJECTIVE: More youth mental health care in the United States is provided in schools than any other setting. So, there is a need for evidence-based psychotherapies that can work well with students in those sometimes complex environments. Transdiagnostic modular therapies may help, given their flexibility and capacity to address diverse youth disorders and problems. METHOD: We tested this possibility by conducting a school-based cluster-randomized controlled trial of Child STEPs, an intervention that combines the Modular Approach to Therapy for Children with consultation and measurement-based

Emotional labour in therapeutic work - the case of physiotherapists and psychotherapists in Poland.

This article presents a comparative analysis of emotional labour among physiotherapists and psychotherapists in Poland, based on 26 in-depth interviews conducted separately with practitioners working primarily in private practice. Despite differences in therapeutic modality - physiotherapists intervening through the body, psychotherapists through the psyche - the analysis reveals structural convergences in their emotional experiences. Drawing on Hochschild's concept of emotional labour and Bolton's typology of emotion management, the article examines how practitioners employ both surface and d

Training Communication Skills and Mentalization in Group Schema Therapy for Patients with Borderline Personality Disorder.

BACKGROUND: This article introduces a structured method for communication skills training within group schema therapy (GST) specifically tailored for individuals with borderline personality disorder (BPD). Patients with BPD often face relational difficulties due to intense emotional responses and maladaptive schema modes, thus creating a need for targeted communication training to promote healthier interpersonal relationships. METHODS: This training approach was developed based on interviews with six experienced schema therapists specialising in GST for BPD. The therapists shared practical in

Dispositional factors affecting the analgesic response to imagery and mindfulness suggestions during hypnosis.

Medical hypnosis combined with mindfulness suggestions may improve pain management, but individual benefits are difficult to predict. This study compared the effects of hypnotic induction suggestions and different hypnotic suggestions for analgesia on acute experimental pain, considering hypnotic susceptibility (SHSS:A) and five mindfulness facets (FFMQ). Thirty-four healthy participants were administered phasic electrical stimulations during a Baseline condition, a Hypnotic induction alone condition, and experimental hypnosis conditions involving Imagery and Mindfulness-like suggestions of an
