We report the case of a patient with a congenital aortic valve stenosis associated with a long-term undiagnosed intralobar pulmonary sequestration. The important blood flow through the aberrant artery led to progressive congestive heart failure and severe hemoptysis at the age of 25 years. We demonstrate the regression of cardiac symptoms and left ventricular diameter after surgical resection of the sequestration.

A 3-year-old girl with congenital mitral stenosis associated with severe pulmonary hypertension was successfully operated upon. The mitral stenosis was a commissural fusion type with severe infravalvular changes. First attempt to fix regurgitation with mitral commissurotomy was unsuccessful, which was followed by prosthetic valve replacement. Her postoperative course was uneventful. A review of Japanese literatures is reviewed as well.

We report a very rare case of hugh aortic arch aneurysm associated with aortic restenosis, which caused severe hypoxia due to ventilation-perfusion mismatch by compression of the left main bronchus and the right pulmonary artery. Aortic arch replacement was performed under circulatory arrest and deep hypothermia in addition to aortic valve replacement as a redo operation, and consequently such hypoxia was relieved.

Aortic stenosis is uncommon during pregnancy. Five cases are described in which clinical management was facilitated by the use of Doppler echocardiography to assess the severity of disease and pulmonary artery catheterization to manage maternal hemodynamics. Regional anesthesia was used without complication. In patients with severe stenosis, significant morbidity and mortality were experienced when aortic valve replacement was delayed beyond the postpartum period.

INTRODUCTION: Pulmonary artery dissection associated with patent ductus arteriosus is usually seen in patients with pulmonary hypertension and Eisenmenger's syndrome. This paper presents a case with pre-Eisenmenger patent ductus arteriosus complicated by pulmonary artery dissection, and explains how she was surgically treated.
PRESENTATION OF CASE: The transthoracic echocardiography of a 21-year-old woman complaining of effort dyspnea revealed patent ductus arteriosus. Contrast-enhanced thoracic computed tomography scan showed patent ductus arteriosus and dissection in the main pulmonary arte

OBJECTIVE: Carcinoid heart disease is characterized by tricuspid valve regurgitation and varying degrees of pulmonary valve regurgitation or stenosis. Valve replacement procedures may be complicated by systemic effects of carcinoid syndrome, as well as hepatic dysfunction and right heart failure. This study was performed to identify factors that might be associated with improving early mortality rates and late outcomes.
METHODS: Between November 1985 and January 2018, 240 adult patients underwent surgery for carcinoid heart disease at the Mayo Clinic. We analyzed the association of multiple c

Periurethral injections of pyrolyzed teflon are used for control of urinary incontinence. Here a case is reported in which teflon-particles have spread from periurethrally injected teflon to pulmonary tissue. Teflon embolization to the lungs from artificial heart valves has previously been reported. The differences between these two forms of teflon-embolization are discussed. The risk of carcinogenicity is discussed.

A 3-week-old Thoroughbred colt was presented for weakness and cyanosis. A pansystolic regurgitant murmur and other physical findings suggested that the foal developed pulmonary oedema as a consequence of congenital heart disease. A large atrial septal defect, a high ventricular septal defect and dysplasia of the atrioventricular valves were visualised echocardiographically. A persistent common atrioventricular canal was observed at necropsy.

In about 50% of the cases, the carcinoid syndrome is complicated with cardiac lesions. These predominate in the right heart and consist of tricuspid and/or pulmonary valve disease, and endocarditis of the right atrium or ventricle. Two-dimensional echocardiography provides and early and accurate diagnosis of these lesions, so that surgical correction, if possible, can be performed before right cardiac failure with its high mortality rate develops.

A new method for correction of right ventricular flow tract obstruction is described. The method combines the advantages of patch-plasty-procedure and external valved conduit by conduit integration into the right ventricular and pulmonary arterial wall. Good results had been achieved in 3 cases (2 cases of Fallot's tetralogy and one case of double outlet right ventricle).

This report details a rare case of left-sided carcinoid heart disease (CHD). In CHD, vasoactive substances released from carcinoid tumors cause fibrous tissue formation on the right side of the heart. These substances are usually inactivated by monoamine oxidase A in the lungs, safeguarding the left side of the heart. Exceptions include the presence of a patent foramen ovale (PFO), pulmonary metastasis, or elevated serotonin levels. Intriguingly, our patient exhibited significant left-sided involvement without these factors, ultimately requiring a quadruple valve replacement surgery. After eig

Ebstein's anomaly is a rare and complexed heart defect that affects the tricuspid valve and is accountable for around 1% of congenital cardiac abnormalities. It is one of the most common congenital causes of tricuspid valve regurgitation. Ebstein's anomaly is often diagnosed prenatally due to its severe cardiomegaly. Some individuals with this anomaly do not experience any complications until adulthood and even then its mostly minor complaints like exercise intolerance. Atrial septal defect is most commonly (70-90%) associated with Ebstein's anomaly. However, ventricular septal defect (VSD) c

Shone's anomaly is a very rare congenital cardiac malformation characterized by four serial obstructive lesions of the left side of the heart (i) Supravalvular mitral membrane (ii) parachute mitral valve (iii) muscular or membranous subaortic stenosis and (iv) coarctation of aorta. We report a unique presentation of Shone's complex in a 14-year-old adolescent male. In addition to the four characteristic lesions the patient had bicuspid aortic valve, aneurysm of sinus of valsalva, patent ductus arteriosus, ventricular septal defect, persistent left superior vena cava opening into coronary sinus

A 51-year-old woman was admitted with intractable congestive heart failure and progressive anemia. She had undergone mitral valve replacement for mitral regurgitation at age 23 years. Subsequently, her mitral prosthesis was replaced twice due to thrombotic stack and valve insufficiency. Signs of congestive heart failure became evident at age 46 years. Gastrointestinal endoscopy revealed esophageal varices, which were treated by endoscopic variceal ligation. Cardiac catheterization disclosed elevated pulmonary capillary wedge pressure (mean 16 mmHg), right atrial pressure (mean 15 mmHg), and he

This case-report describes an 11-month-old infant, who had been diagnosed with Noonan syndrome with a variety of associated anomalies. Multiple cardiac anomalies were present, consisting of dysplastic pulmonary valve, symmetric biventricular hypertrophy, atrial septal defect and right ventricular outflow tract (RVOT) obstruction, in which the pressure gradient measured 73 mmHg, and anomalous coronary artery. Systolic anterior motion of the mitral valve (SAM) was present, without remarkable clinical significance. RVOT transannular repair with non-cusped xenograft along with resection of hypertr

BACKGROUND: Although calcific aortic stenosis is common, calcification of the other three heart valves is not. The aortic valve interstitial cell (VIC) has been implicated in the pathogenesis of aortic stenosis. Proinflammatory stimulation of aortic VICs induces an osteogenic and inflammatory phenotypic change. We hypothesized that the VICs of the other heart valves do not undergo these changes. Using isolated human VICs from normal aortic, mitral, pulmonary, and tricuspid valves, our purpose was to compare the osteogenic response to proinflammatory stimulation via toll-like receptor 4 (TLR-4)

INTRODUCTION: Chronic low-dose cabergoline treatment for microprolactinoma may cause cardiac valve pathology, but the evidence is contradictory. We investigated whether the expectation of the echocardiographer could influence the report.
METHODS: Transthoracic echocardiograms from 40 patients aged 49·3 ± 9·6 (mean ± SD) years (Men:Women 7:33) on long-term cabergoline and bromocriptine therapy (duration 9·94 ± 4·5 years) were randomly assigned to two groups of echocardiographers so that each echocardiogram was reported twice. One group was told that 'the patients were control subjects' (Group

BACKGROUND: Few studies have explored the long-term function of cryopreserved homograft valves used for reconstruction of the right ventricular tract (RVOT) in patients with congenital heart disease.
METHODS AND RESULTS: Among 205 patients receiving cryopreserved homografts for reconstruction of the RVOT between November 1985 and April 1999, the outcome of 220 homografts in 183 operative survivors was analyzed. There were 150 pulmonary and 70 aortic homografts used. Median age at implantation was 4.4 years (mean 6.9+/-7.6 years, range 3 days to 48 years). End points included (1) patient survi

Superior-inferior ventricular arrangement is an exceedingly rare form of congenital cardiac malposition, representing fewer than 0.1% of all congenital heart diseases, and is almost invariably associated with complex intracardiac anomalies. We report a 3-year-old male who presented with persistent cyanosis, exertional dyspnea, and failure to thrive. Prospective ECG-gated CT angiography demonstrated situs solitus with levocardia. A superior-inferior ventricular arrangement was present, with the morphologic left ventricle positioned superior to the morphologic right ventricle. The great arteries
