The role of the laryngeal mask airway in pre-hospital care.

Since its introduction into clinical practice in 1988, the laryngeal mask airway (LMA) has fundamentally changed the airway management of patients undergoing routine anaesthesia. Currently in the UK, the LMA is used in > 50% of surgical procedures where an endotracheal tube (ETT) would formerly have been used. It seems timely to review the role of this device in resuscitation and its potential role in the pre-hospital arena.

Cardiorespiratory collapse and pulmonary oedema due to intravascular absorption of prostaglandin F2 alpha administered extraamniotically for midtrimester termination of pregnancy.

A case of severe reaction to extraamniotically administered prostaglandin F2 alpha, with cardiorespiratory collapse and pulmonary oedema necessitating transfer to an intensive care unit, is presented. Attention is drawn to the profound haemodynamic effects of systemically administered prostaglandin, and the need for caution and ready availability of facilities for resuscitation when this potent substance is administered. Treatment for the effects of intravascular absorption of prostaglandin F2 alpha is discussed.

Necrotizing fasciitis as a complication of chickenpox.

Necrotizing fasciitis is a rapid, potentially fatal soft tissue infection. Chickenpox is a common childhood illness not usually associated with severe complications. We present the case of an 8-year-old girl with necrotizing fasciitis of the upper back arising from superinfection of varicella skin lesions. Necrotizing fasciitis may have devastating sequelae, including septic shock, which mandate vigorous fluid resuscitation, appropriate antimicrobial therapy, and early aggressive surgical debridement.

Blunt abdominal trauma in children. Special considerations in evaluation and management.

Blunt abdominal trauma, common in the pediatric patient, often presents diagnostic difficulties. Following proper resuscitation of the patient, the abdomen must be carefully evaluated. Adequate physical examination is often impossible. Computed tomography of the abdomen is preferable in the stable patient, but peritoneal lavage is occasionally useful. Nonoperative therapy is often used in management of solid visceral injuries in children, but this type of therapy necessitates frequent, careful monitoring.

Anaphylactic reaction to lidocaine.

Lidocaine hydrochloride is an amide-type local anesthetic, commonly used for facial plastic as well as other surgical procedures. In an unusual case of reproducible anaphylactic reaction to lidocaine, early treatment with epinephrine, oxygenation, and intravenous fluid resuscitation resulted in relief of the symptoms. This case demonstrates the importance of sensitivity testing and choosing an appropriate alternative local anesthetic in a patient with a history of possible allergy to lidocaine.

[Carotid aneurysm following apparent complications in catheterization of the internal jugular vein].

During resuscitation of a patient wih ruptured abdominal aortic aneurysm his right internal jugular vein was cannulated with a 12 gauge catheter-through-cannula assembly (Cavafix 458, Braun-Melsungen), using a high central approach, without any apparent complications. Five weeks later a dissecting aneurysm of the right common carotid artery developed. From clinical, angiographic and histological data it is very likely that the aneurysm resulted from the catheterization procedure.

Neurological normality after extreme birth asphyxia. Case report.

A case of extremely severe birth asphyxia is reported. In spite of successful resuscitation the arterial pH was still only 6.6 at the age of 1 h. Assessment at 3 years 10 months revealed that the child was not only completely normal, but was of superior intelligence. This case demonstrates the advisability of maintaining an optimistic approach even when very severe fetal acidosis is detected around the time of birth.

Near-miss SIDS due to Brugada syndrome.

A 19 day old infant was successfully resuscitated from ventricular fibrillation. The 12 lead ECG was normal, with a normal QT interval, and remains so over three years follow up. DNA analysis revealed a missense mutation (R1193Q) in the SCN5A gene, previously linked with familial sudden unexpected nocturnal death syndrome, also known as Brugada syndrome.

Infant with high arched palate, bell-shaped chest, joint contractures, and intrauterine fractures.

A case is presented of a female newborn infant delivered with an Apgar Score of 1, who could not be resuscitated. There was a high arched palate, bell-shaped chest, contractures of writes inflexion, ankles and knees in extension, and intrauterine fractures. Clinical discussion led to a diagnosis of arthrogryposis secondary to fetal akinesia syndrome caused by nemaline myopathy. Pathology and pathologic discussion confirmed this diagnosis.

Experience with severe extensive blistering skin disease in a paediatric burns unit.

Vesiculobullous disorders are thankfully uncommon. A brief overview of the literature is presented together with our experience of managing these cases on a paediatric burns unit in the six years 1992 1998. The advantages of management on such a unit are outlined and include a facility for major dressings and frequent changes and a familiarity with fluid resuscitation in the child with major skin loss. The financial cost of treatment and rehabilitation of these children is high.

Management of the severely injured animal.

Initial management of the seriously injured animal involves the immediate recognition of life-threatening abnormalities and prompt resuscitation based on priorities of care. Of equal importance to the ultimate survival of the animal is an aggressive approach to the diagnosis and control of the complications and sequelae of injury. This approach to patient care requires constant attention to details, but offers the most efficient management of these critically ill animals.

[In situ simulation training of medical-surgical teams in the management of severe trauma patients].

In the management of severe trauma, the aim is to assess the patient's clinical stability as quickly as possible, enabling referral to imaging (whole-body CT scan, embolization if necessary) or the operating room, or even the decision to perform in situ surgery (resuscitation thoracotomy). To cope with these critical situations, team training is essential, with the aim of ensuring the reproducibility of the difficulties encountered. High-fidelity in situ simulation is the ideal tool for meeting this training challenge.

Predictors of cardiac arrest in severe accidental hypothermia.

STUDY OBJECTIVE: To indicate predictors of witnessed hypothermic cardiac arrest. METHODS: We conducted a retrospective analysis of 182 patients with severe accidental hypothermia (i.e., with core body temperature of ≤28 °C) who presented with preserved spontaneous circulation at first contact with medical services. We divided the study population into two groups: patients who suffered hypothermic cardiac arrest (HCA) at any time between encounter with medical service and restoration of normothermia, and those who did not sustain HCA. The analyzed outcome was the occurrence of cardiac arrest p

Cardiac Arrest in Special Populations.

Best practices in cardiac arrest depend on continuous high-quality chest compressions, appropriate ventilatory management, early defibrillation of shockable rhythms, and identification and treatment of reversible causes. Although most patients can be treated according to highly vetted treatment guidelines, some special situations in cardiac arrest arise where additional skills and preparation can improve outcomes. Situations covered in this section involve cardiac arrest in context of electrical injuries, asthma, allergic reactions, pregnancy, trauma, electrolyte imbalances, toxic exposures, h

Fulminant sepsis secondary to severe lung contusion from the Lund University Cardiopulmonary Assist System device.

INTRODUCTION: Portable mechanical chest compression devices have been developed to improve upon many problems of manual compression, increase patient survival, and improve neurologic outcomes. However, the use of these devices is not without risk of harm to the patient. CASE REPORT: We describe a patient who received chest compressions from a mechanical compression device after cardiac arrest and subsequently developed fulminant sepsis secondary to lung contusions and a necrotizing pulmonary infection. DISCUSSION: Although injuries from the LUCAS have been reported, we believe this is the fi
