Considerations in the Diagnosis and Management of Lower-Extremity Infections in Injection Heroin Users: A Case Series.

BACKGROUND: On a national level, heroin-related hospital admissions have reached an all-time high. With the foot being the fourth most common injection site, heroin-related lower-extremity infections have become more prevalent owing to many factors, including drug preparation, injection practices, and unknown additives. METHODS: We present a 16-month case series in which eight patients with lower-extremity infections secondary to heroin abuse presented to The Jewish Hospital in Cincinnati, Ohio. RESULTS: Three cases of osteomyelitis were seen. All of the infections were cultured and yielded

Hand Infections.

Infections are common in hand surgery and proper management is important to achieve optimal outcomes. Although most cases are not urgent, less common, severe infections such as flexor tenosynovitis and necrotizing fasciitis require urgent identification with both medical and surgical management. It is common for diagnoses to be missed or delayed because clinical and laboratory indicators are often variably present. Delayed identification and management can result in poor outcomes with permanent deficits. This article will provide a review of hand infections with a focus on identifying serious

Evaluating the Laboratory Risk Indicator to Differentiate Cellulitis from Necrotizing Fasciitis in the Emergency Department.

INTRODUCTION: Necrotizing fasciitis (NF) is an uncommon but rapidly progressive infection that results in gross morbidity and mortality if not treated in its early stages. The Laboratory Risk Indicator for Necrotizing Fasciitis (LRINEC) score is used to distinguish NF from other soft tissue infections such as cellulitis or abscess. This study analyzed the ability of the LRINEC score to accurately rule out NF in patients who were confirmed to have cellulitis, as well as the capability to differentiate cellulitis from NF. METHODS: This was a 10-year retrospective chart-review study that include…

Limb salvage for an aorto-bifemoral vascular graft infection using multilayer negative pressure wound therapy: a case report.

INTRODUCTION: Exposure and infection of aorto-bifemoral grafts are severe complications associated with a high risk of catastrophic hemorrhage and limb loss. CASE PRESENTATION: A 71-year-old man presented with rupture of a femoral anastomotic pseudoaneurysm after previous aorto-bifemoral bypass. Following emergency vascular reconstruction, extensive debridement resulted in a large groin defect with exposure of the vascular graft and distal anastomotic sites, complicated by infection and multi-organ failure. In the absence of viable reconstructive options, low-pressure negative-pressure wound

Evaluation of Clinical Safety of 4% Articaine in Inferior Alveolar Nerve Block for Non-surgical Mandibular Third Molar Extractions.

Group A streptococcus (GAS) causes diseases ranging from benign to severe infections such as necrotizing fasciitis (NF). The reasons for the differences in severity of streptococcal infections are unexplained. We developed the polymorphic-tag-lengths-transposon-mutagenesis (PTTM) method to identify virulence genes in vivo. We applied PTTM on an emm14 strain isolated from a patient with NF and screened for mutants of decreased virulence, using a mouse model of human soft-tissue infection. A mutant that survived in the skin but was attenuated in its ability to reach the spleen and to cause a let

Ocular and periocular Usutu virus-associated lesions in naturally infected Eurasian blackbirds (Turdus merula).

Usutu virus (USUV) is a zoonotic neurotropic arbovirus related to the West Nile virus that causes mortality in birds and sporadic neurologic human disease. Current research on natural USUV-associated disease lacks data on ocular involvement. This study investigated ocular and periocular tissue involvement in natural USUV infections and associated disease in Eurasian blackbirds (Turdus merula). Twenty-two found-dead Eurasian blackbirds were examined. USUV reverse transcriptase quantitative polymerase chain reaction (RT-qPCR) detected 12/22 infected blackbirds. Histology and immunohistochemistry

Successfully treating hand primary tuberculous synovitis by synovectomy combined antituberculous therapy: A case report.

RATIONALE: Primary tuberculous infection in hand and wrist is a rare disease. Few articles reported on hand primary tuberculous synovitis. PATIENT CONCERNS: A 68-year-old Chinese male, without history of tuberculosis (TB), had complained of pain and swelling in right palm and little finger for 3 months. Patient came to our hospital on 9th Oct 2016. X-ray just showed soft tissue swelling in little finger. Magnetic resonance imaging (MRI) showed synovitis around flexor tendon of little finger, volar palm, and carpal tunnel. Notably, it also implied nodular images in little finger sizing 5 mm ×

Daptomycin: from the mountain to the clinic, with essential help from Francis Tally, MD.

Daptomycin has been approved and successfully launched for the treatment of complicated skin and skin-structure infections caused by gram-positive pathogens [1] and bacteremia and right-sided endocarditis due to Staphylococcus aureus, including strains that are resistant to methicillin or other antibiotics [2]. The development of the drug, however, was not straightforward; it involved a cast of characters, including scientists at Eli Lilly and at Cubist Pharmaceuticals. Of most importance, the development of daptomycin involved the tenacious leadership of Dr. Francis Tally. As a tribute to Dr.

Do number and location of plates impact infection rates after definitive fixation of high energy tibial plateau fractures?

PURPOSE: Surgical trauma may confer additional infectious risk after operative fixation for high energy tibial plateau fractures. This study aims to determine the impact of plate number and location on infection rates after these injuries. METHODS: This retrospective cohort study completed at two level one trauma centers included patients who underwent staged fixation for a tibial plateau fracture between 2015 and 2019. Plate number and location (lateral, medial, posteromedial, and anterior quadrants) used in the definitive fixation construct were collected from post-operative radiographs. De

Unusual Freshwater-Related Infections Caused by Haematospirillum jordaniae.

Freshwater-related infections can be caused by a broad range of pathogens, potentially leading to skin and soft tissue, pulmonary, gastrointestinal, or even systemic diseases. Haematospirillum jordaniae (H. jordaniae),a gram-negative, aerobic organism previously regarded solely as an environmental microbe, has been classified as a pathogen capable of causing human infection in the United States. There has been only one other case reported in the literature of H. jordaniae infection, and little is known about the pathogenesis. The presentation and progression of clinical symptoms in our cases in

State-of-the-art treatment of coccidioidomycosis skeletal infections.

Coccidioidomycosis is a fungal infection endemic to the southwestern United States. Typically a respiratory illness, coccidioidomycosis can rarely present as extrapulmonary infection. Skeletal coccidioidomycosis occurs in 20% to 50% of disseminated infections. Skeletal coccidioidomycosis is a chronic and progressive infection that eventually results in bone destruction and loss of function and often involves adjacent structures, such as joints, muscles, and tendons and other soft tissues. Sinus tract formation may occur. This infection may be multifocal. Although radiographs, white blood cell

Non-tuberculous mycobacterial infections.

Non-tuberculous mycobacteria (NTM) are ubiquitous environmental organisms in contrast to those belonging to the M. tuberculosi complex (MTB). NTM infects and causes disease only in hosts with local or general predisposing factors. Lung infection following inhalation of NTM is the most common NTM disease but soft tissue infections may occur in connection with contaminated trauma or surgery. Microbiological diagnosis is obtained by microscopy for acid-fast bacteria (AFB) on secretions or biopsies, and by culture on special media. With the high specificity of MTB- polymerase chain reaction (PCR),…

Comparative Evaluation of Functional and Quality of Life Outcomes in Conventional and Soft-Liner Relined Obturators for Patients With Maxillofacial Defects.

Background and objective Oral cancer is the sixth most common cancer worldwide. The prevalence of mucormycosis, a progressive fungal infection affecting the nasal and paranasal sinuses, is also on the rise, especially in patients after COVID-19 treatment. Surgical resection frequently employed in these cases results in maxillary defects which lead to functional and aesthetic impairments. Obturator prostheses are used for the rehabilitation of these maxillofacial defects. However, the choice of material and design can have an impact on patient outcomes. The present study aims to compare the max

Comparative evaluation of tissue adhesives and conventional sutures in periodontal flap surgery: A randomized clinical study.

Post-surgical discomfort, delayed soft tissue healing and risk of infection remain significant challenges associated with conventional suturing methods in periodontal flap surgeries. Therefore, it is of interest to evaluate four different groups (n=10 each); Group 1 - silk sutures, Group 2 - vicryl sutures, Group 3 - fibrin sealant (Tisseel) and Group 4 - isoepoxy 2-cyanoacrylate adhesives (Amcrylate) and compared effects on wound healing, postoperative discomfort and clinical outcomes in periodontitis patients. Phase I therapy was followed by full-thickness mucoperiosteal flaps elevation, deb

Radiological examination of the "Braided Lady": A CT-based case study of an Egyptian mummy.

The Braided Lady, an ancient Egyptian mummy previously examined through radiography, was subjected to a high-resolution multidetector CT with 0.625-mm slice thickness and multiplanar and 3-dimensional reconstructions to assess mummification techniques and explore potential causes of death within a cultural and medical context. CT analysis confirmed prior findings of well-preserved osseous and soft tissue anatomy, incomplete evisceration, and the absence of excerebration, consistent with mummification practices reserved for nonroyal elites of the New Kingdom. Dental wear and pathology were obse

Use of a 660-nm Laser to Aid in the Healing of Necrotic Alveolar Mucosa Caused by Extruded Sodium Hypochlorite: A Case Report.

The extrusion of sodium hypochlorite through the apical foramen is an accident that can occur during the flushing procedure in endodontic treatment. The symptomatology is immediate and intense, and there is a long period before the tissues return to normal. Low-level laser therapy might be useful as an adjunctive treatment for damaged soft tissues because of its anti-inflammatory and analgesic effects, which may reduce edema and prevent infection. In this clinical case, the accidental leakage of 1% sodium hypochlorite during the preparation of the root canal of a maxillary right central inciso
